IN FLU EN ZA IN TH E W ORLD: PER IO D OCTOBER 1970 — SEPTEM BER 19711 LA G R IPPE DANS LE M O N D E: PÉR IO D E OCTOBRE 1970 — SEPTEM BRE 19711 Epidemiologicalfeatures During the period from October 1970 to September 1971 the incidence of influenza in the world was relatively low In the temperate countries of the northern hemisphere, where influenza is usually prevalent between October and March, most of the infections were associated with virus B. The United States o f America reported extensive epidemics associated with this virus (in New England, m the mid-Atlantic area, in Virginia and m Alaska ). Elsewhere virus B was associated with sporadic cases or moderate to small outbreaks. Virus A2/Hong Kong/68 was rarely mvolved during the winter in the temperate countries of the northern hemi­ sphere In the United States, a few localized outbreaks associated with this vu us occurred in the Pacific area and a widespread out­ break associated with viruses A and B occurred in Hawaii, Virus A2/Hong Kong/68 was epidemic in the Republic o f Korea and in many prefectures in Japan. In contrast to its pattern m the winter of 1970-71, virus A2/Hong Kong/68 was unusually active during the spring and summer of 1971 in some of the temperate countries in the northern hemisphere In Spam, there was an epidemic in a small town near Madrid. Sporadic cases were unusually frequent in England between April and July Localized outbreaks occurred m Finland, Switzerland and Romania during August and September In the tropics and sub-tropics and m countries of the southern hemisphere, where influenza usually is prevalent between April and September, virus B caused only sporadic cases or small outbreaks, but some large outbreaks associated with virus A2/Hong Kong/68 were reported, some of which probably occurred in populations previously not touched by the virus. One of these outbreaks affected miners coming for the first time from tropical areas to a mining region in the Witwatersrand District (South A frica). Others affected Las Palmas in the Canary Islands, Western Samoa, certain villages in Sarawak (M alaysia) and a remote area m Papua and New Guinea. Medium-sized outbreaks associated with virus A2/Hong Kong/68 were reported from Wellington (New Zealand), Singapore, Dakar, Puerto Rico and Trinidad In otiier countries of the tropics or the southern hemisphere, virus A2/Hong Kong/68 was associated only with small outbreaks or sporadic cases. Caractéristiques épidémiologiques Pendant la période allant d ’octobre 1970 a septembre 1971, l’incidence grippale fut relativement faible dans l’ensemble du monde Dans les pays tempères de l ’hemisphere nord, où la grippe sévit habituellement entre octobre et mars, les infections furent associées le plus souvent au virus B. Les Etats-Unis d ’Amérique signalèrent de vastes épidémies associées à ce virus (en Nouvelle-Angleterre, dans la region du Moyen-Atlantique, en Virgmic et en Alaska). Ailleurs, le virus B fut associe a des cas sporadiques ou à des poussées de moyenne ou faible importance. Le virus A2/Hong Kong/68 fut rarement en cause pendant l’hiver dans les pays tempères de l’hémisphere nord: aux Etats-Unis, on notifia des poussées localisées dans la region du Pacifique et une poussée etendue à Hawaï (cette demiere était associée aux virus A et B), des epidemics dues au virus A2/ Hong Kong/68 furent aussi observées en République de Corée ainsi que dans de nombreuses préfectures du Japon P ar contre, le virus A2/Hong Kong/68 manifesta pendant le printemps et l’ete une activité inhabituelle dans certains pays tempérés de l’hémisphere nord eptdemte en Espagne dans une petite ville proche de Madrid, fréquence inhabituelle des cas sporadiques en Angleterre entre avril et juillet, poussées locales en Finlande, en Suisse et en Roumanie pendant les mois d ’août et septembre Dans les pays chauds et dans les pays de l’hémisphère sud où la grippe sévit habituellement entre avril et septembre, le virus B ne causa guère que des cas sporadiques ou de petites poussées, mais on signala quelques épidémies importantes associées au virus A2/Hong Kong/68 parmi des populations dont certaines avaient pro­ bablement été épargnées jusque-là par le virus l’une d’elles atteignit des mineurs nouvellement amvés de régions tropicales dans une zone minière du District de Witwatersrand (Afrique du Sud), d ’autres atteignirent Las Palmas aux Iles Canaries, le Samoa-Occi­ dental, certains villages de Sarawak (Malaisie) ainsi qu’une région reculée de Papua et Nouvelle-Guinée. Virus A2/Hong Kong/68 was in general antigemcally similar to the original A2/Hong Kong/1/68 or A2/Hong Kong/8/68 strains The variant A2/England/878/69 which had been found in 1969 and 1970 in England, France, Portugal, Algeria, Singapore and New Zealand did not become generalized and, during the period under consideration, this variant was found in very few countries, in par­ ticular Hong Kong The 1971 New Zealand strains identified in London were close to the prototype A2/Hong Kong/1/68 and no more to A2/England/S78/69. In Japan, the A2/Nagasaki/l/70 and A2/Chiba/5/71 strains, which differ antigenically from one another, show certain differences from A2/Hong Kong/1/68 and A2/England/ 878/69. Of the total isolations o f virus A2 in Japan the proportion resembling A2/Chiba/5/71 increased as the epidemic progressed

Clinical features On the whole, influenza cases associated with virus B were mild, as usual, and some countries affected reported no excess mortality. A few pneumonic complications were reported, however, some o f them being of viral origin (two confirmed cases in Austria) Although influenza cases associated with virus A2/Hong Kong/68 were more severe than those associated with virus B they appear, on the whole, to have been less serious than during the influenza year 1969-1970, and pulmonary complications seem to have been, in general, relatively less frequent It is interesting that in three remote villages in Sarawak, which were apparently affected for the first tune by virus A2/Hong Kong/68, the disease was not very serious.
