ANTIGENIC CHARACTERISTICS OF CURRENT INFLUENZA A ISOLATES Recommended Composition of Tnflnenra Vaccines for Use in the 1974-1975 Season CARACTÉRISTIQUES ANTIGÉNIQUES DES SOUCHES DE VIRUS GRIPPAL A ACTUELLEMENT ISOLÉES Recommandations concernant la composition des vaccins antigrippaux à utiliser au cours de la saison 1974-1975 In the period 1971-1973 influenza outbreaks in several countries were associated with the A/England/42/72 (H3N2) variant and this was the prevalent influenza A virus in all areas from mid1972 to September 1973. An earlier variant A/Hong Kong/107/71 (H3N2) produced outbreaks in the Western Pacific region in 1972 but did not gain worldwide prevalence. During September and October 1973 epidemics of influenza A infection were reported in New Zealand and Western Australia.1 Virus isolates from these outbreaks were found to show antigenic differences from A/England/42/72 (H3N2) in both their haemagglutinin and neuraminidase antigens. 1 See Nos. 42, 1973, p. 406 and 44,1973, p, 421.

The results of haemagglutination-inhibition tests and neurami­ nidase-inhibition tests to compare the recent Australasian isolates with former variants of the Hong Kong virus are shown in Tables 1 and 2 respectively. A striking finding was the low degree of cross-reaction of the variants in tests with antiserum to the haemagglutinin and the neuraminidase of A/Hong Kong/1/68 virus. There was evidence of a closer relationship to A/England/42/72 but nevertheless both haemagglutinin and neuraminidase antigens of the Australasian isolates showed clear evidence of antigenic differences from those of A/England/42/72. Immuno-doublediffusion tests with antisera to purified haemagglutinin (H3) and neuraminidase (N2) provided confirmatory evidence that the sur­ face antigens of the new variants were of the same subtype (H3 and N2) as other influenza A viruses which have circulated since 1968 indicating that the antigenic variation could be attributed to “drift” . These tests, like the results of quantitative singleradial-diffusion tests shown in Table 3, also provided evidence of the antigenic differences between A/England/42/72 and the Aus­ tralasian variant. Six influenza A strains isolated from sporadic cases and one from a hospital outbreak in the United Kingdom (see A/England/954/ 73 Tables 1 and 2) during October-Deoember 1973 resembled the Australasian variant as did strains isolated during the same period in France (one from Caen), Lebanon (one from Beirut), Japan (two from Tokyo) and Hong Kong (two). These findings strongly suggest that the new variant is becoming disseminated in the north­ ern hemisphere. No cunent isolates from these areas were found to resemble A/England/42/72. The isolate A/Port Chalmers/1/73 (H3N2) is regarded as typical of the new variants.

Des tests d ’inhibition de l’hémagglutination et d ’inhibition de la neuraminidase ont été faits pour comparer les souches isolées récem­ ment en Australasie avec de précédents variants du virus Hong Kong (!Tableaux 1 et 2). Un résultat remarquable a été le faible degré de réactivité croisée des variants testés avec des sérums anti-hémagglutinine et anti-neuraminidase A/Hong Kong/1/68. Il est apparu que les souches australasiennes présentaient une plus grande ressem­ blance avec A/England/42/72, mais que leurs antigènes hémagglutinine et neuraminidase étaient nettement différents de ceux de ce variant. Les tests d ’immunodiffusion double effectués avec des sérums anti-hémagglutinine purifiée (H3) et anti-neuraminidase purifiée (N2) ont confirmé que les antigènes de surface des nouveaux variants appartenaient au même sous-type (H3 et N2) que ceux des autres virus grippaux A qui ont circulé depuis 1968, indication que la variation antigénique pourrait être attribuée à un « glissement». Les résultats de ces tests, comme ceux des tests quantitatifs de diffusion radiale unique (Tableau 3), mettent également en lumière les différences antigéniques qui existent entre A/England/ 42/72 et le variant australasien. Au cours de la période d'octobre à décembre 1973, des souches de virus grippal A ressemblant au variant australasien ont été isolées au Royaume-Uni (six souches isolées dans des cas sporadiques et une souche provenant d ’une poussée épidémique dans un hôpital) (voir A/JEngland/954/73, Tableaux 1 et 2), en France (une à Caen), au Liban (une à Beyrouth), au Japon (deux à Tokyo) et à Hong Kong (deux), ce qui donne fortement à penser que le nouveau variant est en train de se répandre dans l’hémisphère nord. Aucune des souches récemment isolées dans ces régions ne ressemble à A/England/42/72. La souche A/Port Chalmers/1/73 (H3N2) a été jugée représentative du nouveau variant.

Table 4 shows the frequency and titres of haemagglutmationinhibiting antibody to A/Port Chalmers/1/73 in recently collected sera from normal adults in the United States of America and the United Kingdom. Only one-third of individuals tested pos­ sessed antibody at titres of 1:20 and less than 20% had titres of 1:40 or greater. It seems likely that the antibody detected with A/Port Chalmers/1/73 represents cross-reactive antibody induced by infection with former variants, A/Hong Kong/1/68 or A/England/ 42/72.

On trouvera dans le Tableau 4 la fréquence et le titre des anti­ corps d ’inhibition de l’hémagglutination vis-à-vis de A/Port Chalmers/1/73 dans des sérums récemment prélevés sur des adultes nor­ maux aux Etats-Unis d ’Amérique et au Royaume-Uni. Un tiers seulement des sujets soumis à l’enquête sérologique présentaient des titres égaux à 1:20, et moins de 20% des titres égaux ou supé­ rieurs à 1:40. Les anticorps décelés avec A/Port Chalmers/1/73 cor­ respondent sans doute à des anticorps à réactivité croisée engendrés par une infection antérieure due à de précédents variants, A/Hong Kong/1/68 ou A/England/42/72.

Tables 5 and 6 show the antibody responses in individuals who received inactivated monovalent influenza vaccine containing 600 international units of A/Hong Kong/1/68 and A/England/42/72 virus respectively in autumn 1972. Whilst A/Hong Kong/1/68 vaccine induced high antibody titres to the homologous virus (85 % of vaccinées showed four-fold rise), the response to A/Port Chalmers/1/73 was poor (20% with four-fold rises). The anti­ body responses to vaccine containing A/England/42/72 are shown in Table 6. The proportion of individuals showing a four-fold antibody response to A/Port Chalmers/1/73 was the same as for the homologous virus (89%) whilst the geometric mean litres to the homologous virus in the post-vaccination sera were approxi­ mately four-fold greater than for A/Port Chalmers/1/73 virus (1:980 and 1:240 respectively). The results o f studies with A/Eng­ land/42/72 vaccine carried out in the United States of America in autumn 1973 were similar to the above.1 It appears that A/Port Chalmers/l/73-like strains are becoming disseminated in several areas and are likely to become the prevalent influenza A virus in the northern hemisphere. The populations in the United Kingdom and in the United States of America, and probably other areas, appear to be largely susceptible to infection with this variant. Although vaccines containing A/England/42/72 virus should provide some protection against infection with A/Port Chalmers/1/73, it would be expected that their efficacy would be less than optimal. Vaccines containing A/Hong Kong/1/68 virus would be expected to be ineffective. National authorities may therefore wish to review the composition of influenza A vaccines SO as to include an A/Port Chalmers/l/73-like strain. The recom­ mended composition o f bivalent vaccines for use in the 1974-1975 season would be thus as follows:2* A/Port Chalmersj 1173-like strain BjHong Kong/5172-like strain A high yielding recombinant (MRC11) suitable for production of inactivated vaccines is available from the World Influenza Centre. This strain has been produced from A/Port Chalmers/1/73 (isola­ tion and passage history only in embryonated eggs) and A/PR8/34 (HON1). Les Tableaux 5 et 6 indiquent les réponses en anticorps observées chez des sujets vaccinés en automne 1972 avec un vaccin antigrippal inactivé monovalent contenant 600 unités internationales de virus A/Hong Kong/1/68 ou A/England/42/72. La réponse en anticorps suscitée par le vaccin A/Hong Kong/1/68 a été très élevée pour le virus homologue (85% des vaccines ont vu leur titre quadrupler) mais assez médiocre pour A/Port Chalmers/1/73 (titre quadruplé chez 20% seulement des vaccinés). Les réponses en anticorps au vaccin A/England/42/72 sont indiquées dans le Tableau 6. La pro­ portion des sujets dont la réponse en anticorps vis-à-vis de A/Port Chalmers/1/73 a quadruplé a été la même que pour le virus homo­ logue (89%), mais la moyenne géométrique des titres d’anticorps contre le virus homologue dans le sérum post-vaccinal a été environ quatre fois plus grande que pour A/Port Chalmers/1/73 (1:980 et 1:240 respectivement). Les études faites aux Etats-Unis d ’Amérique en automne 1973 avec le vaccin A/England/42/72 ont donné des résultats similaires.1 Il semble que des souches ressemblant à A/Port Chalmers/1/73 commencent à se répandre dans plusieurs régions et nsquent de devenir les souches grippales A prévalant dans l’hémisphère nord. Les populations du Royaume-Um et des Etats-Unis d ’Amérique, ainsi sans doute que d ’autres régions, semblent être dans l’ensemble assez réceptives à l’infection par ce variant. Les vaccins contenant A/England/42/72 devraient conférer une certaine protection contre A/Port Chalmers/1/73, mais il ne faut pas s’attendre à ce qu’ils soient d’une efficacité optimale. Quant aux vaccins à base de A/Hong Kong/1/68, on peut penser qu’ils seraient inefficaces. Les autorités nationales tiendront sans doute à modifier la composition des vaccins antigrippaux A pour y faire entrer une souche ressemblant à A/Port Chalmers/1/73. Il est donc recommandé de préparer pour ta saison 1974-1975 des vaccins bivalents contenant à la fois une souche ressemblant à A/Port Chalmersj 1/73 et une souche ressemblant à BjHong Kong!5172.2 Le Centre mondial de la Grippe peut fournir un recombinant à haute productivité (MRC11) qui convient pour la fabrication de vaccins inactivés. Cette souche a été obtenue à partir de virus A/Port Chalmers/1/73 (uniquement par isolement et passage sur œuf embryonné) et de virus A/PR8/34 (HON1).
