PARASITIC DISEASES IN THE CARIBBEAN, 1982 Intestinal Parasites Since 1978, the Caribbean Epidemiology Centre (CAREC), at the request of various ministries o f health, has been conducting surveys for enteric parasites m Caribbean schoolchildren. These surveys were stratified random samples on S- and 9-year-old children. A measured quantity (500 mg) of faecal sample o f each selected child was preserved in merthiolate-formalin solution and alter MIF-ether concentration, the entire sediment was examined at CAREC. All helminth eggs were counted and estimates o f protozoan densities were made. From 1977 to 1981, surveys were conducted in 9 countries, and in 1982 Grenada and Saint Christopher and Nevis were added to the list In Grenada, a total o f344 children was selected from 7 schools. Of these, 94% were positive for 1 or more helminth or protozoan. Trichiuris trichiura was the most common helminth found and 85% of the children were infected. Next was Ascaris, with a pre­ valence rate o f 33%. However, at one school 70% (47 examined) of the children were infected with Ascaris eggs. Hookworm had a scattered distribution and while a 6% overall infection rate was noted, 50% of the cases originated from one school. O f the parasitic protozoans, Entamoeba histolytica was found in 25% of the children, the highest rate thus far encountered o f all the Caribbean countries surveyed. The other parasitic protozoan, Giardia, was present in 16% o f the children and varied from 11% to 23% in the various schools. Almost all the infections were o f low intensity. Only 1 out o f 294 children infected with T. tnchiura had an egg count > 2 500/500 mg faeces. None o f 115 children infected with A. lumbricoides had an egg count above 10 000/500 mg faeces. In the Saint Christopher and Nevis survey o f 356 children se­ lected, 288 produced samples giving a response rate of 81%. O f these, 80% were positive for 1 or more parasites. T. trichiura was the most common helminth encountered, followed by Ascaris. Only 1 person was found positive for hookworm. Intensity of infection—as measured by egg count—was mild for both parasites, with only 7 out of 149 children on Saint Christopher having Trichiuris egg counts > 2 500/500 mg faeces, and none o f 49 on Nevis. None of 46 children on Saint Christopher nor of 8 on Nevis infected with Ascaris had infections > 10 000/500 mg faeces. MALADIES PARASITAIRES AUX CARAÏBES, 1982 Parasites intestinaux Depuis 1978, le Centre épidémiologique des Caraïbes (CAREC), à la demande de divers ministères de la santé, fait des enquêtes sur les para­ sites intestinaux chez les écoliers des Caraïbes. Ces enquêtes sont menées sur la base d’échantillons aléatoires stratifiés d’enfants de 5 et 9 ans. Une quantité mesurée (500 mg) de selles prélevées sur chaque enfant sélec­ tionné est conservée dans une solution de merthiolate-formol et le sédi­ ment total est ensuite examiné au CAREC, après concentration au MIFéther. Tous les œufs d’helminthes sont comptés et l’on évalue les densités de protozoaires. Entre 1977 et 1981, des enquêtes ont été faites dans 9 pays, auxquels sont venus s’ajouter la Grenade et Saint-Chnstophe-etNevis en 1982. A la Grenade, 344 enfants ont été sélectionnés dans 7 écoles. Sur ce total, 94% présentaient 1 ou plusieurs types d’helminthes ou de proto­ zoaires. Trichiuris trichiura était l’helminthe le plus couramment mis en évidence, puisque 85% des enfants étaient infestés. Venait ensuite Ascaris avec un taux de prévalence de 33%. Toutefois, dans une école, 70% des enfants (47 ont été examinés) étaient infestés par des œufs d 'Ascaris. L’ankylostomiase était irrégulièrement répartie et, si l’pn a noté un taux général d’infestation de 6%, 50% des cas provenaient d’une seule école. Quant aux protozoaires parasites, Entamoeba histolytica a été mise en évidence chez 25% des enfknts, taux le plus élevé jusqu’ici pour tous les pays des Caraïbes ayant fait l'objet d’une enquête. Un autre protozoaire parasite, Giardia, était présent chez 16% des enfants, le taux variant de 11% à 23% selon les écoles. Presque toutes les infestations étaient de faible intensité. Chez un seul des 294 enfants infestés par T. tnchiura, le nombre d’œufs était supérieur à 2 500/500 mg de selles. Sur 115 enfants infestés par A. lumbricoides. aucun ne présentait un nombre d’œufs supérieur à 10 000/500 mg de selles. Dans l’enquête faite à Saint-Christophe-et-Nevis, on a obtenu des échantillons pour 288 des 356 enfants sélectionnés, soit un taux de réponse de 81%. Sur ce total, 80% présentaient 1 ou plusieurs types de parasites. Trichiuris tnchiura était l’helminthe le plus courant, suivi par Ascaris. On n’a trouvé d’ankylostomes que chez 1 enfant. L’intensité de l’infestation — mesurée par le nombre d ’œufs — était faible pour les 2 types de parasite, puisque seuls 7 enfants sur 149 à Saint-Christophe présentaient un nombre d’œufs de Trichiuris supérieur à 2 500/500 mgde selles et aucun des 49 enfants à Nevis. Aucun des 46 enfants de SaintChristophe ou des 8 de Nevis qui étaient infestés par Ascaris ne présentait un nombre d’œufs supérieur à 10 000/500 mg de selles. from a patient who worked in Antigua and Saint Christopher. Subsequently, in the Caribbean Islands the parasite has been reported from the Dominican Republic, Puerto Rico, Vieques, Samt Martin, Saint Christopher, Antigua, Montserrat, Guade­ loupe, Martinique and Saint Lucia. In addition, there are recent reports o f the introduction of S. mansoni and S. haematobium into Cuba. The disease is now believed to have been eliminated from Saint Christopher and Vieques. While clinical disease has not been recognized as such over the last 50 years in Montserrat and Antigua, there have been occa­ sional reports of 5 mansoni eggs in stool specimens from indi­ viduals in these 2 countries. Thus, the chance finding of 5 indi­ viduals passing S mansoni eggs in Montserrat in 1978 prompted closer scrutiny of residents in Trants and Farm River villages. A house-to-house survey showed that 14% of 137 faecal samples examined were positive while 16% of 132 persons tested were positive by serology. Since it was learnt that subsequent to the 1978 study some o f the residents o f T rants were resettled at Bethel, Bramble and Tuitis in the early 1960s, another survey was under­ taken in these 3 villages in 1981. During a routine school survey for intestinal parasites in Antigua, 2 individuals from 1 village were found positive for S. mansoni and this finding prompted a house-to-house survey in 1981. The results of the faecal examination of samples from these 2 surveys were given in the 1981 reports, but the serological tests were completed in 1982. For the sake of comparison and com­ pleteness, the results o f both techniques are given in this report. Attempts were made to collect faecal and blood samples from all persons 5 years and over. In the field, approximately 1 g of faeces was mixed with 10.0 ml of MIF solution. In the laboratory, after ether concentration all o f the sediment was examined. Only 1 specimen was examined from each individual. For serological studies, blood spots were obtained on chroma­ tography No. 3 paper. The ELISA tests were performed at the London School of Hygiene and Tropical Medicine by the method of McLaren et al (1979). In Montserrat, it was estimated that a total o f 314 persons resided m the 3 villages. From these, 247 blood spots and 189 stool specimens were obtained. Thus the response rate for blood was 84% whereas for stools it was 60%. By stool examination 10% were positive whereas the ELISA test showed 25% to be positive. High prevalence rates by stool examination were noted in the age group 15-24 years. More males were positive in the 25-44-year group. No one under 15 was positive. By serology, prevalence of

Par la suite, le parasite a été signalé dans diverses îles des Caraïbes : République dominicaine, Porto Rico, Vieques, Saint-Martin, Saint» Christophe, Antigua, Montserrat, Guadeloupe, Martinique et SainteLucie. De plus, des indications récentes font état de l'introduction dé S. mansoni et S. haematobium à Cuba. On pense que la maladie a mainte­ nant été éliminée de Saint-Chnstophe et de Vieques. Si l’infection clinique n’a pas été décelée en tant que telle ces 50 der­ nières années à Montserrat et Antigua, on a épisodiquement signalé la présence d’œufs de S. mansoni dans des échantillons de selles provenant d’habitants de ces 2 îles. Aussi en 1978 la découverte fortuite de 5 sujets excrétant des œufs de S. mansoni à Montserrat a-t-elle incité à examiner de plus près les habitants des villages de Trants et Farm River. Une enquête à domicile a montré que 14% des 137 échantillons de selles examinés étaient positifs tandis que 16% des 132 sujets soumis à une analyse sérologique étaient positifs. Après l’étude de 1978, certains des habitants de Trants ont été réinstallés à Bethel, Bramble et Tuitts au début des années 60. C'est pourquoi une autre enquête a été faite dans ces 3 villages en 1981. Au cours d’une enquête systématique dans les écoles d’Antigua des.- , tinée à rechercher la présence de parasites intestinaux, 5. mansoni a été. décelée chez 2 habitants d’un village, d’où l'organisation d'une enquête a domicile en 1981. Les résultats de l’examen des échantillons de selles pratiqué dans le cadre de ces 2 enquêtes ont été présentés dans les rapports de 1981 mais les épreuves sérologiques ont été faites en 1982. A des fins de comparaison et d’exhaustivité, les résultats des 2 techniques sont données ici. On s’est efforcé de recueillir des échantillons de selles et de sang chez tous les individus de plus de 5 ans. Sur place, on a mélangé environ 1 g de selles à 10,0 ml de solution MIF. Au laboratoire, après concentration à l’éther, on a examiné la totalité du sédiment Seul 1échantillon par sujet a été examiné. Pour les études sérologiques, on a obtenu des taches de sang sur papier chromatographique N° 3. Les épreuves ELISA ont été pratiquées à l’Ecole d’hygiène et de médecine tropicale de Londres au moyen de la méthode de McLaren et al (1979). A Montserrat, on a évalué à 314 le nombre d ’habitants des 3 villages, chez lesquels on a prélevé 247 taches de sang et 189 échantillons de selles. Le taux de réponse était donc de 84% pour les échantillons sanguins et 60% pour les échantillons de selles. L’examen des selles a donné un pourcentage de résultats positifs de 10%, alors que pour l’épreuve ELISA il était de 25%. L’examen des selles a révélé de forts taux de prévalence dans le groupe d’âge 15-24 ans. Les cas positifs étaient plus nombreux chez les hommes du groupe d’âge 25-44 ans. antibodies was consistently higher in males than in females. There were only 4 persons positive (of 67 examined) in the 5-14-year group. Two of these were 9-year-olds, a boy and a girl. Figure l compares the overall results obtained from both tests. The stool examination shows decreasing prevalence rates with increasing age while serology shows increasing rates with increasing age. A total of 182 samples was examined by both tests and it was noted that the 18 found positive by stool examination were also positive by ELISA, giving a sensitivity rate o f 100%. O f the 164 persons negative, 136 were negative by ELISA, thus giving a specificity rate o f 83% (assuming that the stool test is defini­ tive). The locale o f Schistosoma activity in Montserrat is on the eastern side o f the island near Trants and Farm River villages where the initial surveys were undertaken in 1978. During the early days o f resettlement, the people from the villages of Bramble, Bethel and Tuitts continued to obtain their water from Farm River where the snail Biomphalaria glabrata was known to be present, and where they presumably became infected. However, with the introduction o f a good pipe-borae water supply, there was no need to use the nver for washing and bathing. Since no one under 15 years was found to be excreting S. mansom eggs, presumably transmission is no longer occurring. Alternatively, it is possible that teenagers and young men and women find the nearby river and dam a source of recreation and frequent this area, with the consequence that they become infected.

L’analyse des selles fait apparaître des taux de prévalence décroissants avec l'âge tandis que l'examen sérologique fait apparaître des taux croissants avec l’âge. On a examiné au total 182 échantillons par les 2 méthodes et observé que les 18 cas qui s’étaient avérés positifs à l’examen des selles étaient également positifs d’après l’épreuve ELISA, soit un taux de sensibilité de 100%. Sur les 164 échantillons négatifs, 136 l’étaient par l’épreuve ELISA, soit un taux de spécificité de 83% (en supposant que l’examen de selles soit définitif). L’activité de Schistosoma à Montserrat se limite à la partie orientale de file, près des villages de Trants et Farm River où les enquêtes initiales avaient été faites en 1978. Au début de leur réinstallation, les habitants des villages de Bramble, Bethel et Tuitts ont continué d’aller chercher de l’eau à Farm River, où la présence du mollusque Biomphalaria glabrata était connue et où ils ont probablement été infestés. Toutefois, avec l'installation d’un bon réseau d’approvisionnement en eau par canalisa­ tions, il n’était plus nécessaire d’aller à la rivière pour faire la lessive et se laver. Puisqu’aucun enfant de moins de 15 ans ne s’est révélé excréter des oeufs de S. mansom, il est vraisemblable que la transmission a été inter­ rompue. Mais il se peut aussi que, pour les adolescents et les jeunes, la rivière et le barrage voisins soient un heu de loisirs où ils se rendent, ce qui serait une source d’infestation.

In Antigua, from an estimated survey population of 394 per­ sons, samples were received from 373 giving a response rate of 95%. Stool examination of 321 persons showed 17% to be positive, while 35% of 373 sera were positive by the ELISA test. There were 4 persons positive by microscopy (all males) in the 5-14 year age group. The youngest found positive was a 12-yearold boy. Highest prevalence rates were again found in the 15-24 and 25-44 year age groups. However, the rate for males in the 15-24 year group was higher than the females. By serology, the prevalences for both sexes were about equal. The combined data for both sexes are compared by the results obtained from the 2 tests (Fig. 2). By microscopy, a decline in prevalence rates was noted after the 15-24 year age group, but with the ELISA test the rates remained about the same after the 15-24 year age group.

A Antigua, sur une population enquêtée de 394 personnes, on a reçu des échantillons de 373 sujets, soit un taux de réponse de 95%. L’examen des selles pratiqué chez 321 sujets a donné un taux de positivité de 17% tandis que l’épreuve ELISA a donné un taux de positivité de 35% pour 373 sérums. Il y avait 4 cas positifs par microscopie (tous des garçons dont le plus jeune était âgé de 12 ans) dans le groupe d’âge 5-14 ans. La encore, les plus forts taux de prévalence ont été notés dans les groupes d’âge 15-24 ans et 25-44 ans. Cependant, le taux parmi les 15-24 ans était plus élevé chez les hommes que chez les femmes. Du point de vue sérologique, la pré valence était à peu près la même pour les 2 sexes. Les résultats des 2 épreuves permettent de comparer les données pour les 2 sexes (Fig. 2). A l’examen microscopique, on a note une chute des taux de prévalence au-delà du groupe d’âge 15-24 ans, mais avec l’épreuve ELISA, les taux sont restés environ les mêmes dans les groupes d’âge ultérieurs.

The ELISA test and the standard stool examination were com­ pared for sensitivity. A total of 316 samples was tested by both techniques. All 53 stool positives were also positive by ELISA giving a 100% sensitivity rate by this technique. O f the 263 nega­ tives, 198 were negative by ELISA showing a specificity rate of 75%. From past records, Swetes, Liberia, Bendals and John Hughes were considered schistosomiasis areas. However, during a school survey for enteric parasites in 1980 in these areas, only 2 persons mansom. Both individuate came were found to be excreting from John Hughes and it was these 2 positives that prompted the present survey. The finding o f such high prevalence rates in this village was surprising as the local health officials did not think the parasite was still present to this extent on the island. Most of the adults at John Hughes are engaged in subsistence farming, i.e., the cultivation of bananas, root crops, fruits and vegetables. The area o f transmission is probably at Body Pond, a series of dammed pools which was formerly the main water supply for the capital city. Young people use the pools as recreational areas and the adults may use the pools for bathing and washing following their agricultural activities. Infected persons from Montserrat are already being treated and treatment is also planned for Antigua. This should reduce the reservoir of infection and lead to possible eradication o f the para­ site. These surveys highlight: (/) the insidious nature of the disease and the need for continuing surveillance in areas where the vector is present and where there is danger o f introduction and réintro­ duction o f the disease; and (2) the value o f developing good specific serological tests, particularly for epidemiological pur­ poses.

On a comparé la sensibilisation de l’épreuve ELISA et de l’examen de selles classique avec un total de 316 échantillons. Les 53 cas positifs à l’examen de selles l’étaient également à l’épreuve ELISA, soit un taux de sensibilité de 100% pour cette technique. Sur les 263 échantillons négatifs, 198 l’étaient avec l’épreuve EL1SA, soit un taux de spécificité de 75%. D'après les dossiers antérieurs, Swetes, Liberta, Bendals et John Hughes étaient considérés comme des zones de schistosomiase. Or, durant une enquête scolaire laite dans ces villages en 1980 en vue de rechercher la présence de parasites intestinaux, seuls 2 sujets se sont avérés excréter S. mansoni. Ils vivaient tous 2 à John Hughes et ce sont ces résultats qui ont amené à faire la présente enquête. On a été surprisde noter des taux de prévalence aussi élevés dans ce village car les autorités sanitaires locales ne pensaient pas que l’infestation parasitaire sur Pile conservait une telle ampleur. A John Hughes, la plupart des adultes pratiquent l’agriculture de sub­ sistance, c’est-à-dire qu’ils cultivent bananes, racines vivnères, fruits et légumes. La zone de transmission se situe sans doute à Body Pond, une série de retenues d ’eau qui était autrefois la principale source d’approvi­ sionnement en eau de la capitale. Lesjeunesy vont durant leurs loisirset fi se peut que les adultes s’y rendent après le travail des champs pour se laver ou y faire la lessive. Les sujets infestés de Montserrat sont déjà en traitement et il est prévu d’appliquer un traitement à Antigua —ce qui devrait contribuera réduire le réservoir d’infestation et peut-être éliminer le parasite. Ces enquêtes font ressortir 2 points: J) la nature insidieuse de la maladie et la nécessité de maintenir la surveillance dans les zones où le vecteur est présent et où il y a un risque d’introduction ou de réintro­ duction de la m aladif 2) l’intérêt de mettre au point de bonnes épreuves sérologiques spécifiques, notamment à des fins épidémiologiques.

Using the modified Ritchie formol-ether concentration technique (MRCT) only 51 out of 73 (70%) infections were detected on first stool examination in a population in Puerto Rico with a mean infection intensity o f < 20 eggs/g faeces. In the same population it was found that, compared with multiple stool examinations by MRCT, an ELISA using soluble egg antigen was only 7 5% sensitive but 92% specific, and that sensitivity drops markedly with increased age and decreased intensity o f infection. Or, dans ce même groupe de population, il a été constaté que, par rapport à des examens multiples de selles avec la tech­ nique MRCT, l’épreuve ELISA utilisant un antigène soluble préparé à partir d’œufs de & mansoni présentait un taux de sensibilité de 75% mais un taux de spécificité de 92%, et que la sensibilité diminuait nettement avec l’âge et la diminution de l’intensité de l’infection. SURVEILLANCE OF FOODBORNE DISEASES Staphylococcal Food Poisoning on a Junior School Outing U nited Kingdom. — On 14 July 1983, 63 children aged 5-7 years, 3 teachers, 3 school domestic staff) an infant welfare assis­ tant and 17 parents from a school in Somerset set out on a hot morning for a day’s outing to a wildlife park. At about midday the party had a picnic lunch together, which had been prepared by the school kitchen staff and consisted o f various sandwiches, cakes, fruit and orange squash ; however, most of the parents took their own packed lunch. The Outbreak Between 14 h and 17 h that afternoon, 30 o f the children and 5 adults developed symptoms which included stomach pains and weakness, copious vomiting and severe diarrhoea, symptoms made worse by the high afternoon températures, estimated to be over 29 ’C. Most of the sick were transported in a fleet of ambulances and private cars to the local district hospilaL Twenty-three children and 3 adults were treated in the casualty department and, of these, 4 children and 2 adults were admitted for observation and rehy­ dration. Two children and 2 adults required intravenous fluid replacement. Seven other children were treated at a health centre and allowed home. All recovered after 24-48 hours. SURVEILLANCE DES MALADIES D’ORIGINE ALIMENTAIRE Ton-infections à staphylocoques lors d’une excursion scolaire R oyaume-U n i . — Le 14 juillet 1983, 63 enfants âgés de 5 à 7 ans, 3 enseignants, 3 membres du personnel domestique de l’école, une assis­ tante sociale d’école maternelle et 17 parents d’élèves d’une école du Somerset partirent, par une chaude matinée, passer la journée dans un parc naturel. Vers midi, le groupe consomma un pique-nique préparé par le personnel de la cantine scolaire et composé de diverses sortes de sandwiches, de pâtisserie, de fruits et d’une orangeade. La plupart des parents avaient toutefois apporté leur propre panier-repas. La poussée Dans l’après-midi, entre 14 h et 17 h, 30 enfants et 5 adultes présentè­ rent divers symptômes — douleurs gastriques, faiblesses, vomissements abondants, diarrhées sévères — encore aggravés par la chaleur ambiante, évaluée à plus de 29 'C. La plupart des malades furent transportés par ambulances et par voi­ tures particulières vers l’hôpital de district le plus proche. Vingt-trois enfants et 3 adultes reçurent un traitement au service des urgences, sur ce nombre, 4 enfants et 2 adultes furent hospitalisés pour observation et réhydratation. Deux enfants et 2 adultes durent être réhydratés par per­ fusion. Sept autres enfants furent traités dans un centre de santé et ren­ voyés chez eux. Tous se rétablirent en 24 à 48 heures. Enquête et analyse préliminaire Une enquête sur la poussée fut entreprise le jour même en début de soirée, peu après l'amvée des premiers enfants au service des urgences.

food poisoning was clearly the most likely diagnosis. A number of ill and well children and adults were interviewed while in the casualty department and food histones showed that many of the children had supplemented their picnic lunch with ice-cream and drinks purchased from a shop at the wildlife park. One of the schoolteachers reported that of her group o f 8 or 9 children, all had been affected after eating the picnic lunch and none had purchased ice-creams or drinks. She had taken her own sandwiches and had not been affected. O f the other people questioned, all those who were unaffected had not eaten the school-prepared picnic lunch, but of those who were affected, most had eaten the school lunch. The following day, food histories were taken from all the children and adults from the party who were at school that day. The headmaster and staff assisted by obtaining similar histones from all other children and parents as they returned to school or by visiting them at home. Microbiology A schoolteacher who had not been affected but had helped to transport the children to hospital, returned to the wildlife park to obtain samples of the picnic lunch which had been discarded in a rubbish bin. The samples o f food, but no orange squash, were retrieved and sent to the Public Health Laboratory for investiga­ tion the following morning after overnight refrigeration. Four samples of faeces and 1 of vomitus were also collected and sent to the laboratory. Inspection of School Kitchen An inspection was made of the school kitchens in conjunction with an environmental health officer. The 3 kitchen staff were examined and questioned about the preparation of the picnic lunch. A bottle of salad cream which had been used to mix with hard boiled eggs for some o f the sandwiches was collected for analysis. Results Microscopy of samples of egg mayonnaise and luncheon meat sandwiches showed large numbers of Gram-positive cocci On culture, Staphylococcus aureus was isolated in large numbers, namely 3.5 X 109organisms/gand6.5 X 107organisms/g respec­ tively for egg and meat sandwiches, and from 4 separate samples of faeces and 1 sample of vomit. The salad cream was found to be sterile. All the isolates o f S. aureus had the same phage typing pattern 29/52/80 and produced enterotoxin A. Attack rates were calculated from the food histones o f 83 of the 85 individuals on the school trip. One well child and 1 ill adult were not interviewed. The results showed that the illness was confined to those who ate the school packed lunch, with 32 out of 76 (42%) affected compared with none o f the 7 who did not eat the school lunch (p = 0.03). A more specific analysis o f the 76 who ate the school lunch showed that luncheon meat sandwiches and egg sandwiches pro­ duced the highest differential attack rates. Attack rates for those who ate both meat and egg sandwiches, only 1 sort of sandwich or neither confirms that illness was asso­ ciated with both meat and egg sandwiches.

Une toxi-infecuon aiguë était manifestement le diagnostic le plus pro­ bable. Plusieurs enfants et adultes, tant malades que bien portants, furent interrogés au service des urgences; il apparut que de nombreux enfants avaient complété leur pique-nique par des glaces et des boissons achetées dans une échoppe du parc naturel. L’une des institutrices indiqua que sur les 8 ou 9 enfants qui composaient son groupe, tous avaient été atteints après le pique-nique mais aucun n’avait acheté de glaces ou de boissons. Elle-même avait apporté ses propres sandwiches et ne ressentait aucun symptôme. Parmi les autres personnes interrogées, aucune de celles qui n’étaient pas malades n’avait consommé le déjeuner préparé a l’école, contrairement à la plupart des personnes atteintes. Le lendemain, on interrogea tous les enfants et adultes ayant participé à l’excursion qui étaient présents à l’école ce jour-là. Le directeur et son personnel contribuèrent à l’enquête en recueillant également les témoi­ gnages de tous les autres enfants et parents à leur retour à l’école, ou à leur domicile. Etude microbiologique Une institutrice qui n’avait pas été malade mais avait aidé à transporter les enfants à l’hôpital retourna dans le parc naturel pour recueillir des échantillons du pique-nique jetés dans une poubelle. Les échantillons des aliments — mais non de l’orangeade — furent récupérés et envoyés le lendemain matin pour analyse au laboratoire de santé publique, après avoir été conservés sous réfrigération pendant la nu it Quatre échantillons de matières fécales et un échantillon de vomissures furent également recueillis et envoyés au laboratoire. Inspection des cuisines scolaires Les cuisines scolaires furent inspectées avec un responsable de l’hygiène de l’environnement. Les 3 membres du personnel des cuisines furent examinés et questionnés sur la préparation du pique-nique. Une bouteille de mayonnaise de conserve dont le contenu avait été utilisé, avec des oeufs durs, pour fournir un certain nombre de sandwiches fut recueillie pour analyse. Résultats L’examen microscopique d’échantillons des sandwiches œuf-mayon­ naise et des sandwiches au pâté de viande de conserve fit apparaître de très nombreux cocci à Gram positif. Après culture, Staphylococcus aureus fut isolé en grand nombre dans les sandwiches — 3,5 X 109organismes/g et 6,5 X 107 organismes/g respectivement pour les sandwiches à l’œuf et les sandwiches à la viande — ainsi que dims 4 échantillons distincts de m a tiè r e s fécales et 1 échantillon de vomissures. La mayonnaise en bou­ teille se révéla stérile. Tous les isolements de 5 aureus présentaient le même lysotype 29/52/80 et produisaient des entérotoxines de type A. Les taux d ’atteinte furent calculés à partir des interrogatoires de 83 des 85 individus ayant participé à l’excursion. Un enfant non atteint et un adulte malade ne furent pas questionnés. D apparut que tous les malades avaient consommé le pique-nique préparé à l’école. Sur les 76 personnes qui avaient ingéré ce repas, 32 (soit 42%) frirent atteintes; aucune des 7 personnes qui avaient apporté leur propre panier-repas ne fut malade (p = 0,03). Une analyse plus précise concernant les 76 individus ayant consommé le déjeuna' préparé à l’école a montré que les taux d’atteinte différentiels les plus élevés correspondaient à l’ingestion des sandwiches à la viande en conserve et de sandwiches à l’œuf. Les taux d’atteinte des personnes ayant consommé à la fois des sand­ wiches à la viande et des sandwiches à l’œuf, ou seulement une sorte de sandwich ou aucune, confirme que l’affection était associée tant aux sandwiches à la viande qu’aux sandwiches à l’œuf Préparation du pique-nique Le pique-nique avait été confectionné dans la cuisine scolaire par 3 membres du personnel à partir d’œufs durs, de mayonnaise en bouteille, de pâté de viande de conserve et de fromage râpé pour des sandwiches de pain blanc en tranches. Des biscuits sablés, des petits gâteaux de ménage et des pommes figuraient également au menu. L’orangeade avait été diluée dans la cuisine et conservée dans de gros récipients en matière plastique. Les sandwiches avaient été confectionnés vers 14h451e 13 juillet, soit la veille du pique-nique. Ils avaient été placés dans des boîtes métalliques qu’on avait enveloppées de Cellophane; on avait ensuite replacé les cou­ vercles et scellé le tout à l’aide de ruban adhésif. Une boîte de sandwiches à l’œ uf et une autre de sandwiches au fromage furent placées jusqu’au lendemain matin sur l’étagère la plus fraîche d’un garde-manger. Une boîte contenant du pâté de viande en boîte et la quatrième boîte, conte­ nant à la fois du fromage et du pâté de viande frirent mis au réfrigérateur pour la nuit. On a estimé que la température dans le garde-manger pendant que les sandwiches s’y trouvaient avait fluctué entre 15'C et 27 ’C Elans le compartiment du réfrigérateur où avaient été placés les

Preparation of the Picnic Lunch The picnic lunch was prepared in the kitchen by 3 kitchen staff from boiled eggs, salad cream, luncheon meat and grated cheese as fillings for sliced white bread sandwiches; shortbread biscuits, home made rock buns and apples were also provided. Orange squash was diluted in the kitchen and stored in gallon plastic containers. The sandwiches were made at about 14 h 45 on 13 J uly, the day before the picnic. They were put m large tins which were covered with clingfilm, the lids applied and sealed with sticky tape. One tin of egg sandwiches and another o f cheese sandwiches were placed on a larder cold slab where they remained overnight One tin containing luncheon meat and the fourth containing a mixture of cheese and luncheon meat were placed in the refrigerator over­ night It is estimated that the temperature fluctuated between 15 and 27 ‘C in the kitchen larder during the time the sandwiches were stored. The temperature in the refrigerator compartment where the meat and cheese sandwiches were stored was 9 'C. All

the food was loaded on to the coaches about 9 h on the morning of 14 July, where it remained until midday. There was no evidence o f skin infection or boils on the hands, forearms or laces of the 3 kitchen staff and none had a recent history of nose or throat infections. No swabs were taken during this investigation. The standard of cleanliness in the kitchen was excellent throughout although there were a small number of struc­ tural defects in need of attention.' Conclusion Thirty children and 3 adults suffered acute food poisoning after a picnic lunch. The illness, although short lived, was distressing and required a sudden and concentrated use of ambulance, hos­ pital and GP services. Six people were admitted to hospital and 4 required intravenous fluid therapy. The subsequent investigation provided evidence that egg and meat sandwiches had been con­ taminated with 5. aureus which had been allowed to multiply and produce sufficient toxin to cause severe symptoms. The picnic lunch was prepared too far in advance of the time of consumption and the storage procedure employed was inad­ equate. In order to prevent similar outbreaks, prepacked school lunches should be prepared on the day of consumption. If this is not possible, then prepared food should be wrapped in clingfilm before being placed in bulk containers. The containers should then be refrigerated at 1-4 ‘C without lids, to aid rapid refrigeration of contents. The lids should be replaced after removing from the refrigerator and not removed again until distribution of the food.

Aucun des 3 employés des cuisines ne présentait d'infection cutanée ou de furoncle sur les mains, les avant-bras ou le visage, et aucun n'avait récemment souffert d'infections des voies nasales ou de la gorge. Aucun prélèvement ne fut pratiqué au cours de cette enquête. Malgré l’existence de quelques défauts structurels appelant des réparations, la cuisine se trouvait dans un excellent état de propreté. Conclusion Trente enfants et 3 adultes ont été atteints d’une toxi-infection aiguë à la suite d’un pique-nique. L’affection, bien que brève, a été très pénible et a nécessité l’intervention rapide et intensive d’ambulances, de services hospitaliers et de médecins. Six personnes ont été hospitalisées, et 4 ont eu besoin d’un traitement réhydratant par voie intraveineuse. L’enquête a révélé que les sandwiches à l’œuf et à la viande avaient été contaminés par S. aureus, qui avait pu se multiplier et produire suffisamment de toxines pour provoquer des symptômes sévères. Le pique-nique avait été préparé trop longtemps à l’avance, et la méthode de conservation employée était inadéquate. Afin d'éviter les poussées de ce genre, les déjeuners scolaires préemballés devraient être confectionnés le jour même de leur consommation. Si ce n’est pas pos­ sible, les aliments préparés devraient être emballés dans de la Cellophane avant d’être placés dans les récipients. Ceux-ci devraient ensuite être réfrigérés à 1-4‘C sans couvercle, afin de faciliter le refroidissement rapide de leur contenu. Les couvercles devraient être replacés au sortir des récipients du réfrigérateur et laissés en place jusqu’à la distribution des aliments.
