Introduction

In 1989, the W orld H ealth Assembly (WHA) adopted the goal o f neonatal tetanus (N T) elimination by the year 1995. T he Expanded Programme on Im m unization (EPI) has defined the elimination target as a reduction in the incidence of N T to less than 1 case per 1 000 live births in every district of every country. T o achieve this goal in developing countries, E PI has prom oted a three-pronged strategy: immunization o f all women of childbearing age with at least 2 doses of tetanus toxoid (T T 2+), provision o f clean deliveries, and strength ening of N T surveillance to identify high-risk areas. In 1990, 90 developing countries were considered endemic for N T , with m ortality rates higher than 1 N T death per 1 000 live births. T en countries accounted for m ore than 70% o f all N T deaths worldwide and were targeted for focused activi ties (Table I). M ost N T cases are no t brought to medical attention because they occur at home, usually in rural settings with poor access to health facilities. T he num ber o f cases report ed thus grossly underestim ates the true burden of this dis ease. Community-based N T m ortality surveys indicate that routine surveillance systems in developing countries detect only 2% to 8% of cases. Therefore, while supporting the improvement of reporting systems, the EPI uses an estimate of the global num ber of N T deaths to m onitor progress towards the elimination of this disease. T his paper presents a brief description o f the EPI m ethods used for estimating global N T deaths and progress towards the 1995 goal o f elimination. Progress is m easured by the im plem entation of th e 3 strategies outlined above and the estim ated reduction in N T deaths in developing coun tries. 277
En 1989, l’Assemblée mondiale de la Santé a adopté le b u t de l'élim ination du tétanos néonatal d’ici 1995. Le program m e élargi de vaccination (PEV) a défini la cible de l’élim ination comme la réduction de l’incidence du tétanos néonatal à moins d’un cas pour 1 000 naissances vivantes dans tous les districts de tous les pays. Pour atteindre ce bu t dans les pays en développement, le PEV a encouragé la mise en œuvre d’une stratégie com prenant 3 volets: vaccination de toutes les femmes en âge de procréer p ar au moins 2 doses d ’anatoxine tétanique (A T2+), hygiène de l’accouchem ent, et renforcem ent de la surveillance du tétanos néonatal pour déterm i ner les zones à haut risque. En 1990, le tétanos néonatal était considéré comme endém ique dans 90 pays en développement, où les taux de m ortalité étaient supérieurs à 1 décès par tétanos néonatal pour 1 000 naissances vivantes. Dix pays totalisant plus de 70% de tous les décès par tétanos néonatal dans le m onde ont été choisis pour des activités focalisées {Tableau 1). La plupart des cas de tétanos néonatal ne sont pas portés à l’attention des m édecins car ils surviennent à domicile, générale m ent dans un cadre rural où l’accès aux services de santé est limité. Le nom bre de cas signalés est donc largement en deçà du véritable fardeau que représente cette maladie. Des enquêtes com m unautai res sur la m ortalité par tétanos néonatal m ontrent que les systèmes de surveillance systématique eh place dans les pays en développe m ent ne dépistent que de 2% à 8% des cas. Aussi, tout en encoura geant l’am élioration des systèmes de notification, le PEV s’appuie sur une estim ation du chiffre m ondial de décès par tétanos néonatal pour suivre les progrès vers l’élim ination de cette maladie. Le présent article décrit brièvement les m éthodes utilisées pour évaluer le nom bre de décès par tétanos néonatal dans le m onde et les progrès réalisés vers le but de l’élim ination en 1995. Les progrès sont mesurés d’après la mise en œ uvre des 3 stratégies décrites cidessus et la baisse estimée du nom bre de décès par tétanos néonatal dans les pays en développement.

EPI estimates the num ber of N T deaths occurring annually in every developing country (n = 158). If a country has a T T 2+ immunization coverage of more than 90% or a clean delivery rate of more than 80%, the num ber of reported N T cases in a given year is taken as the num ber of N T deaths occurring. In all other countries, the num ber of N T deaths is calculated on the basis of the num ber of live births (LB), the baseline N T mortality rate (N TM R), the level of immuniza tion coverage with at least 2 doses of tetanus toxoid (T T 2+), and the vaccine efficacy (VE = 95%) such that:

(1) T otal deaths Using the m ethods described above, it is estim ated that about 594 000 N T deaths occurred in developing countries in 1992; nearly 1.3 million deaths would have been expected in the absence of immunization. Three W HO Regions ac counted for over 80% of all N T deaths, w ith approximately 40% of deaths in the South-East Asia Region (SEAR) and 20% of deaths in each of the African Region (AFR) and the W estern Pacific Region (WPR) (Map 1). In 1988, the year prior to the adoption of the W HA resolution to eliminate N T , there were an estim ated 787 000 N T deaths; 1.19 million deaths would have been expected w ithout an immu nization programme. Adjusting for the increase in live births between 1988 and 1992, there has thus been a 20% reduction in the num ber of N T deaths. T his represents an estim ated fall in the global N T m ortality rate from 6.5 to 4.5 N T deaths per 1 000 live births. T he estim ated fell in mortality, by W HO Region, is presented in Fig. 1. T he total num bers of N T cases reported to W HO for 1988 and 1990 (the latest year for which full data are available) were 20 506 and 24 486, respectively. Based on EPI estimates, less than 4% o f all N T cases are reported. T he num ber o f countries reporting N T cases to W HO has risen from 33 (17%) in 1974 when E PI began, to a high o f 131 (83%) by 1990 (Fig. 2). N um bers are lower from 1991 onwards because o f delayed reporting' 279

Le nom bre total de cas de tétanos néonatal signalés à l’OMS pour 1988 et 1990 (année la plus récente pour laquelle on dispose de données complètes) était de 20 506 et 24 486 respectivem ent. D ’après les estimations du PEV, moins de 4% de tous les cas de tétanos néonatal sont déclarés. Le nom bre de pays qui déclarent les cas de tétanos néonatal à l’OMS est passé de 33 (17%) en 1974, au début du PEV, à 131 (83%) en 1990 (Fig. 2). Accelerated imm unization activities in those coun tries have reduced this rate to 5.3 N T deaths per 1 000 live births in 1992 (country range: 4.00-9.44). By 1992, Nepal (9 893 deaths) and Somalia (9 384 deaths) had replaced Uganda and Viet N am among the 10 countries with the highest estim ated num ber o f N T deaths. T he num ber o f countries with an estimated rate of over 5 deaths per 1 000 live births decreased from 61 to 23 between 1988 and 1992. W hereas 10 countries had an estim ated rate of over 10 N T deaths per 1 000 live births in 1988 (6 in AFR, 3 in SEAR, and 1 in the Eastern M editerra nean Region [EMR] only 5 rem ained in 1992 (3 in AFR, 1 in SEAR, and 1 in EM R).

Implementation of strategies

(1) T T 2+ coverage As of January 1993, 117 countries had a policy targeting women of childbearing age or pregnant women for immuni zation with at least 2 doses of tetanus toxoid (111 countries in 1988). A t the Regional level, the percentage o f developing countries using T T to control N T varied from 51% in AMR to over 90% in APR, the European Region (EUR) and SEAR. In 5 Regions (A FR AM R, E M R EU R and SEAR), more than 90% of infents were bom in a country where T T is used as part of the N T elimination strategy. Overall, in 1992, 78.1% of the world’s infants were bom in a country with a T T programme; excluding China (which began phas ing in T T immunization o f pregnant women in 1993), the total was 97.7%. Prior to 1980, only 7 countries reported T T 2+ coverage. In developing countries, the estim ated global immunization coverage with T T 2+ reached 43% in 1992 (54% excluding China). T he rise in T T 2+ imm unization coverage by W HO Region is summarized in Fig. 3. Between 1988 and 1992, the percentage of the world’s children bom in countries with T T 2+ coverage of over 50% rose from 33% to 54% (from 41% to 67%, excluding China). (2) Clean delivery rates O f the 158 developing countries in which E PI m onitors N T cases, recent inform ation on clean delivery rates is available for 113 (71% ). Thirty-six o f these countries (32%) have clean delivery rates of 80% o r m ore and 30 (26%) have rates between 50% and 80%. T he remaining 47 countries have clean delivery rates below 50% (Map 2).

Parm i ces pays, 36 (32%) ont des taux d ’accouchements hygiéniques de 80% ou plus et 30 (26%) ont des taux se situant entre 50% et 80%. Les 47 pays restants ont des taux inférieurs à 50% (Carte 2). Conclusions
Significant progress has been m ade towards the goal o f N T elimination by the year 1995. T he estim ated global N T mortality rate has fallen by 31%, and 78 out of 158 targeted countries have reduced their national N T m ortality rate to less than 1 N T death per 1 000 live births. T he development of district level surveillance systems that can guide N T control activities and confirm the target of elimination is critical. M ethods for accurately calculating T T 2+ coverage will be needed to assess the progress to wards N T elimination with confidence in areas where dis trict-level surveillance is lim ited.

Le taux m ondial estimé de m ortalité par téta nos néonatal a baissé de 31% , et 78 des 158 pays cibles ont ramené leur taux national de m ortalité par tétanos néonatal à moins de 1 décès par tétanos néonatal pour 1 000 naissances vivantes. La mise en place de systèmes de surveillance dans les districts pour guider les activités de lutte contre le tétanos néonatal et confir m er la cible de l’élimination est une nécessité absolue. D es m étho des pour calculer avec précision la couverture par A T2+ seront nécessaires pour évaluer fiablem ent les progrès réalisés vers l’élimi nation du tétanos néonatal dans les régions où la surveillance au niveau du district est limitée.
