‫الوقاية من التهاب الكبد الفيروسي ومكافحته‬ ‫المقدمـة‬ ‫التهاب الكبد الفريوسي هو أحد األسباب رئيسية يف األمراض املزمنة والوفاة‪ ،‬إذ يؤثر على شخص واحد من‬ ‫بني كل اثىن عشر شخصا يف العامل‪ ،‬نتيجة ألسباب معظمها العدوى بالتهاب الكبد ‪ B‬و‪ .C‬وتؤدي اإلصابة‬ ‫املزمنة بالتهاب الكبد ‪ B‬أو ‪ C‬إىل تشمع الكبد واإلصابة بالسرطان الكبدي اخللوي‪ ،‬ومها مسؤوالن عن‬ ‫‪ 1.4‬مليون حالة وفاة سنويا‪ ،‬أي مبا يعادل ‪ %2‬من إمجايل عبء املرض على الصعيد العاملي‪ .‬ويف عام ‪،2114‬‬ ‫دعت مجعية الصحة العاملية السابعة والستون‪ ،‬يف قرارها ج ص ع‪ ،7-76‬إىل استجابة مكثفة وموسعة اللتهاب‬ ‫الكبد على الصعيد العاملي وطلبت من أمانة املنظمة حبث إمكانية القضاء على التهاب الكبد ‪ B‬و‪.C‬‬ ‫وتشري التقديرات‪ ،‬يف إقليم شرق املتوسط‪ ،‬إىل أن هناك ‪ 161‬مليون شخص مصابون بفريوس التهاب الكبد‬ ‫و‪ 16‬مليون مصابون بالتهاب الكبد ‪ .C‬ويصاب سنويا ‪ 4.3‬ماليني شخص بالتهاب الكبد ‪ ،B‬بينما يصاب‬ ‫‪ 011‬ألف شخص بالتهاب الكبد ‪ .C‬ويتباين وجود التهاب الكبد ‪ B‬و‪ C‬يف بلدان اإلقليم املختلفة؛ إذ يرتفع‬ ‫خطر اإلصابة بالتهاب الكبد ‪ B‬يف مخسة بلدان (هي أفغانستان‪ ،‬وباكستان‪ ،‬والسودان‪ ،‬والصومال‪ ،‬واليمن)‪،‬‬ ‫ويكون معتدال يف البلدان املتبقية وعددها ‪ 16‬بلدا‪ .‬وحسب التقديرات‪ ،‬يرتاوح انتشار التهاب الكبد ‪ C‬على‬ ‫املستوى الوطين من ‪ %1‬إىل ‪ %3‬يف معظم البلدان‪ ،‬ويرتفع إىل ‪ %4.0‬يف باكستان‪ 1‬و‪( %6‬الفئة العمرية ‪-11‬‬ ‫‪ 15‬سنة) يف مصر‪ .2‬ويساهم التهاب الكبد الفريوسي يف ‪ %3‬من إمجايل حاالت الوفاة يف مجيع أرجاء اإلقليم‪.‬‬ ‫ويف مصر تبلغ نسبة الوفيات النامجة عن التهاب الكبد ‪ ،%5‬بينما تبلغ هذه النسبة ‪ %3‬تقريبا يف باكستان‪.‬‬ ‫كما أن التهاب الكبد مسؤول عن ‪ %1‬من إمجايل الوفيات يف معظم البلدان األخرى‪.3‬‬ ‫‪B‬‬ ‫‪.1‬‬

‫ويف معظم بلدان اإلقليم تنتج العدوى اجلديدة بالتهاب الكبد ‪ B‬و ‪ C‬عن املمارسات غري املأمونة يف مواقع‬ ‫الرعاية الصحية‪ .‬وهو ما يؤثر على العاملني يف جمال الرعاية الصحية وطاليب الرعاية على حد سواء‪ .‬ويشكل‬ ‫متعاطو املخدرات عن طريق احلقن فئة سكانية أخرى معرضة بشدة النتقال العدوى بالتهاب الكبد ‪ B‬و‪C‬‬ ‫إليها‪ ،‬بسبب تبادل احملاقن وأدوات تعاطي املخدرات عن طريق احلقن‪ .‬وإضافة إىل ذلك‪ ،‬يهدد التهاب الكبد ‪B‬‬ ‫كيات جنسية شديدة اخلطورة ‪ ،‬مثل الرجال الذين ميارسون اجلنس مع الرجال‬ ‫الفئات السكانية اليت متارس سلو‬ ‫واملشتغلني واملشتغالت باجلنس‪.‬‬

‫ويف إطار مواجهة خطر فريوس الكبد الوبائي املتزايد يف اإلقليم‪ ،‬اعتمدت اللجنة اإلقليمية لشرق املتوسط‪ ،‬يف‬ ‫دورهتا السادسة واخلمسني اليت عقدت يف عام ‪ ،2115‬قرارا يدعو الدول األعضاء ومنظمة الصحة العاملية إىل‬ ‫اختاذ إجراءات عاجلة للوقاية من التهاب الكبد ‪ B‬و‪ C‬ومكافحته‪.‬‬ ‫أدخلت ‪ 17‬بلدا من بلدان اإلقليم جرعة لقاح التهاب الكبد ‪ B‬اليت تعطى عند الوالدة‪ ،‬غري أن التغطية ال تزال‬ ‫منخفضة على املستوى اإلقليمي (حيث بلغت ‪ %24‬يف عام ‪ .)2114‬ويف مقابل ذلك‪ ،‬وصل متوسط التغطية‬ ‫اإلقليمية بتطعيم األطفال بثالث جرعات من لقاح التهاب الكبد ‪ B‬إىل ‪ ،%03‬حيث تراوحت من ‪ %42‬يف‬ ‫الصومال إىل ‪ %55‬يف البحرين ومجهورية إيران اإلسالمية واملغرب‪.‬‬ ‫وال يزال ضمان مأمونية نقل الدم واحلقن يشكل حتديا لكثري من البلدان يف اإلقليم‪ .‬ورغم ورود تقارير من مجيع‬ ‫البلدان تفيد تبنيها لسياسات تتعلق بسالمة احلقن‪ ،‬ورغم أن عشرين بلدا تستخدم احملاقن الذاتية التعطل يف‬ ‫خدمات التطعيم‪ ،‬فإن ثالثة بلدان فقط توصي صراحة باستخدام هذه احملاقن بشكل عام‪ .‬ويتم إجراء اختبارات‬ ‫على عمليات نقل الدم للتحقق من خلو الدم املنقول من فريوس التهاب الكبد من النمطني ‪ C‬و‪ ،B‬لكن ثلثي‬ ‫البلدان فقط هي اليت أفادت باستمرار اإلمدادات مبستلزمات االختبار لديها دون انقطاع‪.‬‬ ‫ولدى مجيع البلدان مقرر عالجي واحد على األقل لعالج التهاب الكبد ‪ B‬و‪ ،C‬باستثناء جيبويت‪ ،‬والصومال‪،‬‬ ‫واليمن‪ .‬وال يزال اإلنرتفريون املمتد املفعول يستخدم على نطاق واسع‪ ،‬غري أنه جيري حاليا‪ ،‬من جانب بلدان‬ ‫عدة‪ ،‬تسجيل ووصف مضادات فريوسات جديدة ذات مفعول مباشر لعالج التهاب الكبد ‪ .C‬وقد حققت‬ ‫كة جيليد) بعد مفاوضات على‬ ‫مصر إجنازا كبريا باحلصول على املنتج األصلي من سوفوسبوفري (من إنتاج شر‬ ‫سعره‪ ،‬حيث حصلت عليه مقابل ‪ 011‬دوالر أمريكي تقريبا لكل مقرر عالجي مدته اثىن عشر شهرا يف‬ ‫املستشفيات العامة‪ .‬ونتيجة لذلك‪ ،‬تتوخى مصر السرعة يف توسيع نطاق عالج التهاب الكبد ‪ C‬وتستهدف‬ ‫عالج مليون شخص حبلول عام ‪ .2110‬أما يف باكستان‪ ،‬فقد جنح القطاع اخلاص يف خفض سعر حزمة الرعاية‬ ‫السريرية اليت تقدم ملدة ‪ 12‬أسبوعا مبا فيها املنتج األصلي من سوفوسبوفري واختبار احلمل الفريوسي حىت وصل‬ ‫إىل حنو ‪ 1611‬دوالر أمريكي تقريبا‪ .‬وحبلول منتصف عام ‪ ،2111‬سيمتد اتفاق الرتخيص الطوعي املوقع بني‬ ‫كائها املنتجني للدواء اجلنيس لسوفوسبوفري لكي يشمل معظم بلدان اإلقليم ذات الدخل‬ ‫كة جيليد وشر‬ ‫شر‬ ‫املنخفض وذات الشرحية املنخفضة من الدخل املتوسط‪ .‬وسعيا إىل حتقيق مزيد من اخلفض يف التكلفة‪ ،‬تفضل‬ ‫بلدان عديدة منها مصر‪ ،‬واملغرب‪ ،‬وباكستان إنتاج األدوية اجلنيسة حمليا‪.‬‬ ‫تلبية لدعوة مجعية الصحة العاملية يف قرارها الصادر يف أيار‪/‬مايو ‪ 2114‬بشأن التهاب الكبد (ج ص ع ‪،)7-76‬‬ ‫يعكف الربنامج العاملي ملكافحة التهاب الكبد حاليا على وضع اسرتاتيجية عاملية حول الوقاية من العدوى‬ ‫بالتهاب الكبد الفريوسي ومكافحتها بالتشاور مع األطراف املعنية على املستوى الوطين واإلقليمي والعاملي‪.‬‬ ‫وتضع االسرتاتيجية استجابة القطاع الصحي اللتهاب الكبد الفريوسي ضمن سياق التغطية الصحية الشاملة‬ ‫والغايات املتصلة بالصحة يف خطة التنمية ملا بعد عام ‪ .2111‬وتتماشى تلك االسرتاتيجية مع االسرتاتيجيات‬ ‫‪-2-‬‬

Prevention and control of viral hepatitis Introduction 1. Viral hepatitis is a leading cause of chronic disease and mortality, affecting one in 12 people in the world, mostly as a result of infection with hepatitis B virus (HBV) and hepatitis C virus (HCV). Chronic infection with HBV or HCV leads to hepatic cirrhosis and hepatocellular carcinoma, which are responsible for 1.4 million deaths annually, i.e. 2% of all global burden of disease. In 2014, the Sixtyseventh World Health Assembly in resolution WHA67.6 called for an intensified and expanded global hepatitis response and requested the WHO Secretariat to examine the feasibility of elimination of hepatitis B and C. 2. In the Eastern Mediterranean Region, it is estimated that 170 million people are infected with HBV and 17 million with HCV. Annually, 4.3 million and 800 000 people, respectively, become infected with HBV and HCV. The epidemic of HBV and HCV in the Region presents in different ways in different countries of the Region. The risk of HBV infection is high in five countries (Afghanistan, Pakistan, Somalia, Sudan and Yemen), and moderate in the remaining 17 countries. The national prevalence of hepatitis C is estimated to range between 1% and 3% in the majority of countries and up to 4.8% in Pakistan1 and 7% (age group 15–59 years) in Egypt2. Region-wide, viral hepatitis contributes to 3% of all mortality. In Egypt, 9% of mortality is attributable to hepatitis; in Pakistan, this proportion is about 3% and in most other countries hepatitis accounts for around 1% of total mortality.3 3. In the majority of the countries of the Region, new infections with HBV and HCV result from unsafe practices in health care settings. This affects both health care workers and those seeking care. People who inject drugs constitute another population group at very high risk of HBV and HCV transmission due to the sharing of injections and drug injection equipment. Additionally, HBV threatens population groups with high-risk sexual behaviour, such as men who have sex with men and sex workers. 4. Facing the growing threat of viral hepatitis in the Region, in 2009 the 56th session of the WHO Regional Committee for the Eastern Mediterranean endorsed a resolution calling for urgent action by Member States and WHO to prevent and control HBV and HCV.

Progress and challenges 5. Hepatitis B birth-dose immunization has been introduced in 16 countries of the Region; however, coverage remains low at regional level (24% in 2014). In contrast, the average regional coverage of childhood vaccination with three doses of hepatitis B vaccine has reached 83%, ranging from 42% in Somalia to 99% in Bahrain, Islamic Republic of Iran and Morocco. 6. Ensuring safe blood transfusions and injections remains a challenge to many countries in the Region. Although all countries report having injection safety policies and 20 countries use auto-disable syringes in vaccination services, only three countries explicitly recommend auto-disable syringes for injections in general. Blood transfusions are tested for hepatitis C and B but only two-thirds of countries report uninterrupted supply of test kits. 1

7. All countries have at least one treatment regimen available for HBV and HCV, except Djibouti, Somalia and Yemen. Pegylated interferon is still widely in use. However, new direct-acting antivirals for HCV treatment are getting registered and prescribed in several countries. Egypt has made a breakthrough in obtaining originator sofosbuvir (Gilead) for a negotiated price of approximately US$ 800 per 12-week treatment course in the public sector. As a result, Egypt is rapidly scaling up treatment of HCV infection with the target of treating 2 million people by 2018. In Pakistan, the private sector has succeeded in reducing the price for a 12-week clinical care package including originator sofosbuvir and viral load testing to approximately US$ 1700. By mid-2015, a voluntary licensing agreement between Gilead and its generic manufacturing partners for sofosbuvir includes the majority of low and lower–middle income countries of the Region. Aiming at further cost reduction, several countries including Egypt, Morocco and Pakistan are opting for local generic production.

Future directions 8. Responding to the call of the World Health Assembly (WHA) resolution on hepatitis in May 2014 (WHA67.6), the WHO Global Hepatitis Programme is currently developing a global strategy on the prevention and control of viral hepatitis infection in consultation with national, regional and global stakeholders. The strategy positions the health sector response to viral hepatitis within the context of universal health coverage and the post-2015 health and development agenda and targets. It is closely aligned with related global health strategies and plans, including those for HIV, sexually transmitted infections, blood safety and noncommunicable diseases. 9. The current draft strategy sets out an ambitious set of targets for 2030 that pave the way for the elimination of viral hepatitis as a public health problem and identifies the national action required to reach those targets. Specifically the draft strategy aims to achieve by 2030: • 90% reduction in new cases of chronic hepatitis B and C; • 65% reduction in hepatitis B and C deaths; • 80% of treatment eligible persons with chronic hepatitis B and C infections treated. 10. A regional consultation of hepatitis focal points was facilitated by the WHO Regional Office in June 2015 involving national hepatitis focal points, experts and civil society organizations. In this meeting the participants discussed the strategy structure, targets and strategic directions and provided their input into the draft strategy. 11. The global strategy on hepatitis will be presented to the Sixty-ninth World Health Assembly in May 2016 for endorsement. Once it is endorsed, the Regional Office will develop in consultation with Member States a regional action plan for the implementation of the strategy in the Region. Within this framework, WHO will focus its support to countries on establishing national leadership and coordination in hepatitis control, collecting and analysing strategic information that guides the response, developing or revising policies and strengthening services for prevention and treatment of hepatitis, increasing access to affordable medicines and monitoring and evaluating the response.

L’hépatite virale est une cause principale de maladie chronique et de mortalité, qui touche une personne sur 12 dans le monde, principalement du fait d’une infection par les virus de l’hépatite B (VHB) et de l’hépatite C (VHC). L’infection chronique par le VHB ou le VHC entraîne la cirrhose du foie et le carcinome hépatocellulaire, qui sont responsables d’1,4 million de décès chaque année, c’est-à-dire 2 % de la charge de morbidité mondiale totale. En 2014, la SoixanteSeptième Assemblée mondiale de la Santé dans la résolution WHA67.6 a appelé à une action mondiale intensifiée et élargie contre l’hépatite et a demandé au Secrétariat de l’OMS d’étudier la faisabilité de l’élimination de l’hépatite B et C. Dans la Région de la Méditerranée orientale, selon les estimations, 170 millions de personnes sont infectées par le VHB et 17 millions par le VHC. Chaque année, 4,3 millions et 800 000 personnes contractent une infection par le VHB et le VHC respectivement. L’épidémie de VHB et de VHC dans la Région se présente de diverses manières dans les différents pays de la Région. Le risque d’infection à VHB est élevé dans cinq pays (Afghanistan, Pakistan, Somalie, Soudan et Yémen), et modéré dans les 17 autres pays. La prévalence nationale de l’hépatite C est comprise, selon les estimations, entre 1 % et 3 % dans la majorité des pays et atteint 4,8 % au Pakistan1 et 7 % (groupe d’âge des 15-59 ans) en Égypte2. Au plan régional, l’hépatite virale contribue à 3 % de l’ensemble de la mortalité. En Égypte, 9 % de la mortalité est attribuable à l’hépatite ; au Pakistan, cette proportion s’élève environ à 3 % et dans la plupart des autres pays, l’hépatite compte pour environ 1 % de la mortalité totale.3 Dans la majorité des pays de la Région, les nouvelles infections par le VHB et le VHC résultent de pratiques dangereuses dans les établissements de soins. Ces pratiques affectent les agents de soins de santé tout comme les demandeurs de soins. Les personnes qui s'injectent des drogues constituent un autre groupe de population ayant un risque très élevé de transmission du VHB et de VHC du fait du partage d’injection et de matériel d'injection de drogue. En outre, le VHB menace les groupes de population qui ont un comportement sexuel à haut risque, tels les hommes qui ont des rapports sexuels avec des hommes et les travailleurs du sexe. Face à la menace croissante de l'hépatite virale dans la Région, la cinquante-sixième session du Comité de l'OMS pour la Méditerranée orientale en 2009 a adopté une résolution appelant à une prise de mesures rapide par les États Membres et l'OMS pour prévenir et lutter contre le VHB et le VHC.

Progrès réalisés et défis 5. L'administration d'une dose du vaccin contre l'hépatite B à la naissance a été introduite dans 16 pays de la Région ; toutefois, la couverture demeure faible au niveau régional (24 % en 2014). En revanche, la couverture régionale moyenne pour la vaccination par les trois doses du vaccin contre l’hépatite B durant la petite enfance a atteint 83 % ; elle est comprise entre 42 % en Somalie et 99 % à Bahreïn, en République islamique d’Iran et au Maroc.

Garantir la sécurité des transfusions et des injections demeure un défi dans de nombreux pays de la Région. Bien que tous les pays signalent disposer de politiques en matière de sécurité et que 20 pays utilisent des seringues autobloquantes dans les services de vaccination, seuls trois pays recommandent explicitement les seringues autobloquantes pour les injections de manière générale. Le sang transfusé fait l’objet de tests de dépistage pour l’hépatite B et C mais deux tiers des pays seulement signalent avoir un approvisionnement ininterrompu en kits de test. Tous les pays, sauf Djibouti, la Somalie et le Yémen, ont au moins un schéma thérapeutique disponible pour l’infection à VHB et VHC. L’interféron pégylé est toujours largement utilisé. Toutefois, de nouveaux antiviraux à action directe pour le traitement de l’infection par le VHC sont en cours d’enregistrement et sont prescrits dans plusieurs pays. L’Égypte a fait une avancée remarquable en obtenant le sofobuvir princeps (Gilead) à un prix négocié à USD 800 pour un traitement sur 12 semaines dans le secteur public ; de ce fait, l’Égypte intensifie rapidement le traitement de l’infection à VHC ciblant la prise en charge de 2 millions de personnes d’ici 2018. Au Pakistan, le secteur privé a réussi à faire baisser le prix d’un ensemble de soins cliniques sur 12 semaines comprenant le sofobuvir princeps et le test de la charge virale à environ USD 1700. À la mi-2015, un accord d’homologation volontaire entre Gilead et des partenaires fabriquant des génériques du sofosbuvir inclut la majorité des pays à revenu faible et à revenu intermédiaire de la tranche inférieure de la Région. En vue de réduire davantage le coût, plusieurs pays, dont l'Égypte, le Maroc et le Pakistan, optent pour la production locale de génériques.

Orientations futures 8. Faisant suite à l’appel lancé dans la résolution de l’Assemblée mondiale de la Santé sur l’hépatite en mai 2014 (WHA67.6), le Programme mondial de l’OMS pour la lutte contre l’hépatite prépare actuellement une stratégie mondiale sur la prévention et la lutte contre l’hépatite virale en consultation avec les parties prenantes nationales, régionales et mondiales. La stratégie place la réponse du secteur de la santé face à l’hépatite virale dans le contexte de la couverture sanitaire universelle et du programme d'action et des cibles en matière de santé et de développement pour l’après-2015. Elle est fortement alignée sur les stratégies et plans sanitaires mondiaux dans le même domaine, y compris ceux qui concernent le VIH, les infections sexuellement transmissibles la sécurité transfusionnelle et les maladies non transmissibles. Le projet de stratégie actuel définit un ensemble ambitieux de cibles pour 2030 qui préparent le terrain pour l’élimination de l’hépatite virale en tant que problème de santé publique et identifie les mesures requises au plan national pour atteindre ces cibles. De manière spécifique, le projet de stratégie vise à atteindre, d’ici 2030 : une réduction de 90 % des nouveaux cas d’hépatite chronique B et C ; une réduction de 65 % des décès liés à l’hépatite B et C ; le traitement de 80 % des personnes atteintes d’hépatite B et C chroniques qui sont éligibles au traitement.

Une consultation régionale des points focaux pour l'hépatite a été animée par le Bureau régional de l’OMS en juin 2015 impliquant les points focaux nationaux pour l’hépatite, des experts et des organisations de la société civile. Durant cette réunion, les participants ont examiné la structure de la stratégie, ses cibles et ses orientations stratégiques et ont fourni une contribution au projet de stratégie. 11. La stratégie mondiale contre l’hépatite sera présentée lors de la Soixante-Neuvième Assemblée mondiale de la Santé en mai 2016 pour approbation. Une fois approuvée, le Bureau régional préparera, en collaboration avec les États Membres, un plan d’action régional pour la mise en œuvre de la stratégie dans la Région. Dans ce cadre, l’OMS orientera son soutien aux pays sur les points suivants : mise en place d’une direction et d’une coordination nationales pour la lutte contre 2

l’hépatite, collecte et analyse des informations stratégiques qui guident la réponse à apporter, élaboration ou révision des politiques et renforcement des services de prévention et de traitement de l'hépatite, augmentation de l’accès à des médicaments abordables au plan économique et suivi et évaluation de la réponse apportée.
