GASTROENTERITIS C anada. — In April 1975, an outbreak of febrile gastroenteritis, which it is considered could have been due to Yersinia enterocolitica, occurred in Montreal. A questionnaire investigation revealed that 831 persons from two schools could have been exposed to a common source of infection over a period o f 18 days. The unimodal distri bution o f the epidemic curve supported this conclusion. In all 138 people (16.6%) had become ill. Diarrhoea was the dominant symptom of the illness which had an incubation period of two to five days. The acute phase lasted two to seven days with an average of five days. In addition to those clinically ill, there were a large number of mildly symptomatic cases with predominantly dyspeptic symptoms. Although laboratory analysis was carried out m a number of cases, the only aetiological agent identified was Y. enterocolitica and this in stool samples from only two children in the same family. An epidemiological investigation suggested that grossly contamin ated raw milk could have been the vehicle o f infection. Y. entero colitica was isolated from the milk but the strain 0: 6.30 was not the same serotype as that isolated from the children 0:5.27.
GASTRO-ENTÉRITE C anada. — Une épidémie de gastro-entérite fébrile dont Yersinia enterocolitica est considéré comme agent responsable possible s’est produite à Montréal en avril 1975. Une enquête par questionnaire a révélé que 831 personnes de deux écoles avaient pu être contaminées par une même source d ’infection pendant une période de 18 jours. Le caractère unimodal de la courbe épidémique étaye cette hypothèse. Au total, 138 personnes (16,6%) ont été atteintes. La diarrhée était le principal symptôme de la maladie, dont l’incubation a duré entre deux et cinq jours. La phase aiguë a duré de deux à sept jours, la moyenne étant de cinq jours. Outre les sujets cliniquement malades, on a observé de nombreux cas présentant de légers symp tômes à prédominance digestive. Bien que des analyses de laboratoire aient été faites dans un cer tain nombre de cas, le seul agent étiologique identifié est Y. enteroco litica qui n ’a d ’ailleurs été isolé que dans des spécimens de selles de deux enfants d ’une même famille. Une enquête épidémiologique a fait penser que le véhicule de l ’infection pourrait être du lait cru fortement contaminé. Y. enterocolitica a été isolé dans le lait mais la souche (0:6,30) n ’appartenait pas au même sérotype que celle qui a été mise en évidence chez les enfants (0:5,27). Si l'épidémie a été effectivement due à Y. enterocolitica, il sem blerait que ce soit le premier cas d ’épidémie impliquant cet orga nisme au Canada. La période d'incubation observée correspond à un type d ’incuba tion qui pourrait être attribué à des infections à Yersinia en suppo sant qu’il s’agisse d 'un seul agent pathogène. Il importe de noter q u ’il est difficile de culuver cette bactérie à la température de 37° C normalement utilisée pour les coprocultures, car sa tempérarature optimale de culture se situe entre 22 et 29° C. Lorsqu’il s’agit d ’une poussée d’allure infectieuse à longue période d ’incuba tion, il convient de l ’indiquer au bactériologiste afin que les cultures soient faites à la température adéquate.
If the outbreak was in fact due to Y. enterocolitica, it would appear to be the first recorded instance involving this organism in Canada. The incubation period recorded is compatible with one range for Yersinia infection assuming a point source exposure. It is important to note that this bacterium is difficult to grow at the 37° C usually used for stool cultures. Its optimum growing temperature varies between 22 and 29° C. When there is a suspected outbreak of infections origin with a long incubation period, the bacteriologist involved should be informed so that cultures will be made at favour able temperatures.
