BOX 549 - Tel.(226) 30.23.12 - Telex 5241 BF OUAGADOUGOU 01 - BURKINA FASO The National Onchocerciasis Team Makeni and District health teams Ministry of Public Health and Social Services Sierra Leone Registry File: 08/18 ll lll Date:28llll97 Allotment: OU/ICP/CTD/426ION 197 I 834104 For the purpose of carrying out the training of village distributors by the Peripheral Health Teams for the installation of community directed treatment with ivermectin in the villages in five districts: Bombali, Kambia, Tonkolili, Koinadugu and Port Loko on Seli, Kaba, Mabole, Mongo, Kolente, Rokel Bagbe and Pampana river basins in Sierra Leone in conformity with your country specific microplan: The World Health Organisation, Onchocerciasis Control Programme, hereinafter referred to as "WHO/ONCHO" on the one hand, and The National Onchocerciasis Team Makeni and District health teams and the Ministry of Public Health and Social Services Sierra Leone hereinafter referred to as the "INSTITUTION", on the other hand; HAVE AGREED AS FOLLOWS 1. Duties to be performed Sensitization of village communities, training of village distributors in community directed treatment with ivermectin, supervision of treatrnent activities carried out by the trained village distributors and provision of a report of the community directed treatment with ivermectin by the health centre nurses of the peripheral health units in northem region, in Bombali, Koinadugu, Kambia, Port Loko, and Tonkolili districts, on Rokel, Kaba, Mabole, Kolente, Seli, Mongo, Bagbe and Pampana rivers basins. The working prograrnme for this agreement could be modified in the course of its execution by WHO/ONCHO after discussions with the National team and the health districts officers enumered above under the Ministry of Health and Social services. 22. Modalities of application (implementation terms). In charse of implementation. Ministry of Public Health and Social Services: Sierra Leone. Through The National Team for Onchocerciasis Control, Regional Health Service Officer of northern province and Port Loko, Kambia, Bombali, Koinadugu and Tonkolili District Health teams. The nurses working in the dispensaries or peripheral health units (health centers) of the districts enumered above will establish the community directed treaunent with ivermectin in the villages of Kaba, Rokel, Mabole, Mongo, Seli, Bagbe, Kolente and Pampana river basins. They will proceed to: Carry out village sensitization of communities and request them to choose their own village distributors for the ivermectin treatment. Train the village distributors in the methodology of treatment of onchocerciasis with ivermectin using height to calculate dose, the exclusion criteria for Ivermectin treatment, recording of treatment on data, and supply of ivermectin for the initial treaftnent from the health centrer. Communities will collect te next supply of ivermectin on the health centers themselves. Help the village distributors to make accurate recording of people in the villages covered by the health centre during this first session of ivermectin distribution. They will carry out the supervision in the villages to make sure that everybody has receveid the treaffnent ivermectin and to veriff if the data on treatment were correctly recorded. Each nurse will compile all the ivermectin treatment data from the villages covered by the health centre and forward it to the District Health Officer who will inturn forward it to the Regional health Officer and the national onchocerciasis coordinator. The results of community directed treatment with ivermectin should be presented as follows: Name of the dispensary or health centre (PHU) which covers the villages . Name of the Health Centre area or District Health. Date of the treatment session in the village and the name of the river basin. Exhaustive list of villages in the area with census population, coverage rate, number of ivermectin tablets distributed and remaining ivermectin tablets, and number of ivermectin tablets needed for the next treatment session. 3Community treatment activities need be integrated into other health centre activities. Supervision and the follow up of the treatment activities and monitoring should therefore be carried out by the doctor in charge of the health district or health region and should be integraded in the other supervisory activities in the district or in the region. The community directed treatment with ivermectin report in the villages should be sent by the medical officer in charge of the district to the National Onchocerciasis Control Programme coordinator (Ministry of Public Health) who will forward them to WHO/OCP Ouagadougou. The village communities should be mobilized to come to the health centre to collect their own ivermectin tablets for the next treatment. In the case where a community fails to come to collect their ivermectin tablets willin the period for the treafinent, the nurse in charge for the health centre should remind the community for the ivermectin treatment and the community should come to the health centre and collect their tablets 3. Obligations of WHO/ONCHO 3.1 The cost for treating villages See annex I A total amount of Le l0 479 450 (Ten million, four hundred and seventy nine thousand four hundred fifty Leones) representing the cost for treating villages will be paid to the nurses of Peripheral health units who will be in charge: for the villages communities sensitization, the training of villages distributors, the supervision of the treatment done by the village distributors and the census of poupulation in all the villages This amount will be paid to the nurses or peripheral health workers by the National Onchocerciasis Control Programme in Sierra Leone through the district medical officers of districts listed above care of Ministry of Health and Social Services . This amount correspond the cost of treating villages to establish effectively the community directed treatrnent with ivermectin in all the villages in the districts on the river basins enumered above. 3.2 Ooeratins costs - Transoortation A total amount of Le 954000( nine hundred and fifty four thousands Leones) will be paid to the nurses for transportation of nurses of peripheral health units ( 159 nurses x Le 6000 = Le 954000) for the five districts. WHO/ ONCHO will provide the number of tablets of ivermectin needed to install the communities directed treatment . 3.3 Means of travel Vehicles and motorcycles which should be used to establish the communities directed treatment with ivermectin in the villages covered by the health centers will be prroviJedloytre government. 43.4 Office Supolies and others. The office fournitures ( registers pens, and calculators etc..) Will be provided by the institution ( government) and the village communities. Any others expenses will be provided by the institution. 4. Contribution of the Institution The Institution, in agreement with his government will provide the requisite personnel for the training for the installation of community directed treatment with ivermectin. These personnels will consist of : the nurses in charge of the peripheral health units which covers the villages, Districts health tearns ( Districts health medical officers), the national onchocerciasis control progarnme coordination c/o Ministry of Public Health and Social Services. 5. Period covered by the agreement The present agreement will cover a period of time from 15 December 1997 to 15 March 1998 for effective installation of communities directed treaunent with ivermectin in the villages in the districts on the river basins cited above. 6. Financial arrangements 6.1 The total amount of expenditure estimated by this agreement is Le 1l 433 450 (eleven million four hundred and thirty three thousand four hundred fifty Leones) 6.2 This amount will be paid in one instalment as soon as this agreement is duly signed by the two parties concerned and the document returned to the WHO/ONCHO in Ouagadougou. 6.3 At the end of each mission all accounting documents (bank statement, receipts, travel claims etc.) shall be forwarded to the following address: The Programme Director Attention: Budget and Finance Officer WHO/ONCHO 01 P.O. Box 549 Ouagadougou 01 Burkina Faso 6.5 Any unliquidated balance at the end of the period covered by this current agreement shall be paid back to WHO/ONCHO by the Instinrtion or deducted later on from the amount of the sum WHO/ONCHO will have to pay in the framework of any possible subsequent agreement. Should in case if there is no letter of agreement, then the sum should be payed by cheque to WHO/ONCHO. The funds provided to the institution under the present agreement for this implementation will be deposited into a banking account opened for this matter. WHO/ONCHO has rights to keep control over the funds at any time and supervise or evaluate the activities carried out by the nurses of the health centre (PH[I) in the villages. 7. Leealarransements It is understood that the execution of this work does not constitute any employer/employee relationship between WHO/ONCHO and the Institution. The Institution shall therefore be solely responsible for the modalities for the execution of the work. WHO/ONCHO shall not be responsible for any loss, accident, damage or injury suffered by the Institution or any person claiming under it arising in and out of the execution of this work or in any manner whatsoever. Any dispute relating to the interpretation or execution of this agreement shall, unless amicably settled, be subject to conciliation. In the event of failure of the latter, the dispute shall be settled by arbitration. The arbitration shall be conducted in accordance with the modalities to be agreed upon by the parties or, in the absence of any agreement, with the rules of arbitration of the International Chamber of Commerce. The parties shall accept the arbitral award as final. 6If your Institution accepts this proposal, we should be grateful if you would see to the signing of all the 4 copies of this agreement by two authorities responsible for the execution of the work and return three copies to us.
