ANTE-NATAL URINARY INFECTIONS U nited K in g d o m . — The diagnosis and treatment o f urinary infections during pregnancy is especially important in view of the increased frequency o f subsequent pyelonephritis.
INFECTIONS URINAIRES ANTE-NATALES R ovaume -U n l — Le diagnostic et le traitement des infections urinaires de la grossesse sont particulièrement importants car ces infections peuvent entraîner par la suite une augmentation de fré quence des pyélonéphrites. De juin à décembre 1974, le Central H ospital Laboratory de l ’Ayrshire (Ecosse) en a diagnostiqué 164 chez 154 femmes enceintes qui présentaient une bactériurie (> 50 000 organismes/ml). Cer taines de ces infections étaient symptomatiques, d ’autres asympto matiques; dans huit cas, il s’agissait d ’une infection double par des organismes différents. Des bacilles Gram-négatifs (Escherichia coli, Klebsiella aerogenes, le groupe Proteus ou les « Coliformes ») étaient les agents étiologi ques responsables de 128 (78 %) de ces infections, dont 49 % étaient dues à E. coli. Des cocci Gram-positifs étaient à l’origine de 36 infec tions (22%); Micrococcus, sous-groupe 3, dont on sait maintenant qu’il est un pathogène important dans les infections urinaires aiguës chez la femme enceinte, n ’a pas été identifié séparément pendant cette période, mais classé comme Staphylococcus albus (Tableau 1),
At Ayrshire Central Hospital Laboratory, Scotland, from JuneDecember 1974, 164 urinary infections were diagnosed in 154 pregnant women with bacteriuria (> 5 0 000 organisms/ml). Some of the women were symptomatic and some asymptomatic, eight having two separate infections with different organisms. Gram-negative bacilli comprising Escherichia coli, Klebsiella aerogenes, Proteus group or “Coliforms" were causal agents in 128 (78%) of the infections, E. coli accounting for 49%. Gram positive cocci were responsible for 36 (22%); Micrococcus sub group 3, which is now recognized as an important pathogen in acute urinary infections in pregnant women, was not separately identified during this period but was classified as Staphylococcus albus (Table 1 ).

Each isolate was tested by disc-diffusion procedure against ampicillin, co-trimoxazole, nalidixic acid, nitrofurantoin, sulphonamide and tetracycline. For the four most effective of these antibiotics, the percentage sensitivity over all isolates was highest for nitrofurantoin (84 %) and lowest for ampicillin (64%). For the total 128 Gram-negative bacilli the sensitivity figures were highest for nalidixic acid (89%) followed by nitrofurantoin (84%); ampi cillin was least effective, only 60% being sensitive. On the other hand, for the Gram-positive cocci ampicillin was most effective (76% sensitive) and nalidixic acid least effective (6% sensitive). However, since Gram-negative bacilli are the predominant pathogens in such infections, the preferred treatment before the results of sensitivity tests are known would at present be nalidixic acid or nitrofurantoin. Also, these two antibiotics seem to be free of toxic effects on the fœtus.
Chaque isolat a été testé par la méthode des disques en présence des antibiotiques suivants; ampicilline, co-trimoxazole, acide nalidixique, nitrofurantoîne, sulfanilamide et tétracycline. Pour les quatre plus efficaces de ces antibiotiques le pourcentage de sensi bilité des isolats a été maximal avec la nitrofurantoîne (84 %) et minimal avec l’ampicilline (64%). Les 128 bacilles Gram-négatifs étaient sensibles surtout à l’acide nalidixique (89%) puis à la nitrofurantolne (84%) et beaucoup moins à l’ampicilline (60%). Avec les cocci Gram-positifs, c’est au contraire l’ampicilline qui été l’anti biotique le plus efficace (76 %) et l’acide nalidixique le moins actif (6%). Toutefois, comme les bacilles Gram-négatifs sont les patho gènes prédominants dans ces infections, il faut donner la préférence au traitement par l'acide nalidixique ou par la nitrofurantoîne tant que l’on ne connaît pas les résultats des épreuves de sensibilité. En outre, ces deux antibiotiques paraissent ne pas avoir d ’effets toxiques sur le fœtus.
