Pharmacovigilance: halofantrine Modification des recommandations d'emploi

H alofantrine is a phenantrenem ethanol antim alarial which is effective against the asexual erythrocytic stage of malaria parasites. It is indicated for the treatm ent of acute malaria caused by single or mixed infections of Plasmodium falci­ parum or P. vivax. It is adm inistered in a total dosage of 24 mg/kg given as 8 mg/kg 3 times at 6-hourly intervals. The majority of patients who have been treated with halofantrine have been infected with P. falciparum in areas where chloro­ quine or m ultidrug resistant strains are common. Recent research reports have alerted that the adm inistra­ tion of halofantrine can result in prolongation of the Q -T intervals and ventricular dysrythmias in susceptible indi­ viduals.1 ,213 T here have also been some spontaneous reports of serious ventricular dysrythmias, rarely associated with death. In total, 8 cardiac arrests have been reported to the pharm aceutical company, leading to 6 deaths, some of which may have been associated with ventricular dysryrhmias. These cases have occurred particularly under certain conditions which include the use o f doses higher than recom m ended, recent or concom itant treatm ent with mefloquine, the presence of pre-existing prolongation of Q -T interval or in patients with thiam ine deficiency. An analysis of available ECG data in patients with P. falciparum malaria and in healthy volunteers indicates that: (i) halofantrine causes an increase in the Q -T interval at recom m ended doses; (ii) the absorption of halofantrine is increased approxi­ mately six-fold when taken with a fatty meal, w ith additional increases in the Q -T interval.

L ’halofantrine est un antipaludique dérivé du phénanthrylcarbinol qui est efficace contre le stade asexué érythrocytaire des plasmodies. U est indiqué dans le traitem ent des accès aigus de paludisme à Plasmodium falciparum, P. vivax, ou les deux à la fois. La dose totale est de 24 mg/kg administrée en 3 fois à raison de 8 mg/kg toutes les 6 heures. La majorité des patients traités par l’halofantrine avaient contracté une infection à P. falciparum dans des zones où les souches chloroquino- ou polychimiorésistantes sont courantes. Selon de récents rapports de recherche, l’adm inistration d ’halofantrine peut provoquer un allongem ent de l’intervalle Q -T ainsi que des arythmies ventriculaires chez les sujets réceptifs.1,2,3 On a également signalé spontaném ent des arythmies ventriculaires gra­ ves, mais rarem ent fatales. Le laboratoire qui produit l’halofantrine a reçu en tout 8 rapports faisant état d’arrêts cardiaques, entraînant 6 décès, dont certains pourraient être liés à une arythmie ventri­ culaire. H s’agit d ’accidents survenus dans des conditions particu­ lières: administration de doses supérieures aux doses recomman­ dées, traitem ent récent ou simultané par la méfloquine, allongement préexistant de l’intervalle Q -T ou carence en thiam ine. L ’analyse de l’EC G des m alades atteints d ’un paludism e à falciparum par comparaison avec celui de volontaires en bonne santé m ontre que; i) aux doses recommandées, l’halofantrine provoque u n allon­ gem ent de l’intervalle Q -T; ii) l’absorption de l’halofantrine est environ 6 fois plus im por­ tante lorsque le médicament est pns avec un repas riche en graisses, d ’où un allongem ent supplém entaire de l’intervalle Q -T .

T he pharm aceutical company is consequently revising the data sheet for this product. Jh advance of this revision W HO has been advised by die pharm aceutical company that halofantrine: — is contraindicated in patients with a family history of congenital Q -T prolongation; — is no t recommended for usage in combination with drugs or clinical conditions known to prolong the Q -T interval or in patients who may suffer from thiamine deficiency; — should not be adm inistered to patients w ith severe elec­ trolyte imbalance; — treatm ent should not exceed the recommended total dosage of 24 m ^kg given as 8 mg/kg 3 times at 6-hourly intervals; — should be adm inistered on an empty stomach (i.e. not given in association with food); — should only be used as an emergency self-medication for presumptive therapy in those patients known to have norm al Q -T intervals. T he pharm aceutical company recommends a second therapeutic course 1 week following the initial treatm ent of patients who have no previous exposure to malaria, such as travellers from non-endem ic areas.

La compagnie pharm aceutique a donc été amenée à revoir la notice du produit. En attendant la nouvelle nonce, elle a commu­ niqué à l’OMS les renseignem ents suivants concernant l’halofantrine: — le m édicament est contre-indiqué chez les malades ayant des antécédents familiaux d ’allongem ent congénital de l’intervalle Q -T; — le médicam ent n ’est pas recom m andé en association avec des m édicaments ou en présence d’u n état clinique susceptibles d ’entraîner un allongem ent de l’intervalle Q -T , ni chez les malades qui pourraient présenter une carence en thiam ine; — le médicam ent ne doit pas être administré aux patients souffrant d ’un déséquilibre électrolytique grave; — la cure ne doit pas dépasser la dose totale recommandée de 24 mg/kg fractionnée en 3 doses de 8 mg/kg toutes les 6 heures; —- le produit doit être pris à jeun (c’est-à-dire qu’il ne doit pas être administré en même tem ps que des aliments); — il ne doit être utilisé pour un autotraitem ent présom ptif d ’urgence que par des malades dont l’intervalle Q -T est normal. La compagnie pharm aceutique recommande une deuxième cure une semaine après la prem ière chez les malades qui n ’ont encore jamais été exposés au paludisme, par exemple les voyageurs en provenance de régions où la maladie n ’est pas endémique.
