C anada. — Pertussis morbidity and mortality was declining in Canada even before the advent of vaccines in the early 1940’s. From a high of 19 878 cases in 1940, a low of 1 232 cases was reported in 1969. The high and low in pertussis fatalities was 1 374 in 1923 and 6 in 1966.

Figure 1 shows that the general decline in cases and deaths has been interrupted by epidemic peaks at two to five-year intervals. A plateau is evident in the morbidity trend between 1945 and 1955, but a sharp drop occurred subsequently. Figure 2 from the 1968 D.B.S. Annual Report of Notifiable Diseases shows widely differing case rates among the provinces and is probably largely due to reporting practices rather than variation in immunization programmes or prevalence of B. pertussis in the population. In the reports with age and sex breakdowns specified, 86.3% were in the 0 to 9 age group and another 10.8% were in the 10 to 14 age group.

It is understood that pertussis vaccines produced in Canada by Connaught Laboratories, Toronto, and the Institute of Micro­ biology and Hygiene, Montreal, contain all serotypes. (Epidemiological Bulletin, E ditorial N ote : The efficacy of current preparations of whoop­ ing-cough vaccines 1 needs to be assessed by continuing surveillance of the disease. The presence of cases in an apparently well pro­ tected community may be explained by a long interval since vacci­ nation, particularly if this was performed in early infancy, but it may also be due to a change in the prevailing serotypes of B. per­ tussis and B. parapertussis.

C anada. — La morbidité et la mortalité dues à la coqueluche ont diminué au Canada, même avant l’emploi de vaccins, depuis le début des années 1940. On avait enregistré 19 878 cas en 1940, contre 1 232 seulement en 1969. Les chiffres maximum et minimum de mortalité par coqueluche ont été respectivement de 1 374 en 1923 et de 6 en 1966. La figure 1 montre que la baisse générale des taux de morbidité et de mortalité a été interrompue, à des intervalles de deux à cinq ans, par des poussées épidémiques. Entre 1945 et 1955, la courbe de morbidité est demeurée stationnaire, mais une nette régression s’est manifestée ultérieurement. La figure 2 (Rapport annuel sur les maladies à déclaration obligatoire, 1968, du Bureau fédéral de la Statistique) reflète des différences considérables entre les chiffres de morbidité selon les provinces; ces écarts sont probablement dus en grande partie au mode de déclaration, plutôt qu’à des variations dans les pro­ grammes de vaccination ou dans la prévalence réelle des infections à B. pertussis parmi la population. On note, dans les rapports qui spécifient pour chaque cas le sexe et l’âge, que la répartition des malades est la suivante: 86,3% dans le groupe d’âge de 0 à 9 ans et 10,8% dans le groupe d ’âge de 10 à 14 ans. On sait que les vaccins anti-coquelucheux fabriqués au Canada par les Laboratoires Connaught, à Toronto, et par l’Institut de Microbiologie et d’Hygiène de Montréal, contiennent tous les sérotypes. SHIGELLA U nited K ingdom . — During the first week of April 1970 a number of cases of diarrhoea occurred in a male and a female ward of a large psychiatric hospital in Scotland. These wards are situated almost half a mile apart, and do not share any staff; the only link between them is that both receive food from the main hospital kitchen.

INFLUENZA Fui (situation on 18 April 1970). — 1 In Rewa Province, an outbreak of influenza-like illness in the general population started on 16 April. In Tailevu Province, an outbreak of influenza occurred in a boarding school from 26 March to 2 April (19 cases); serolo­ gical evidence of infection with virus A was obtained. DIPHTHERIA U n it e d K in g d o m . — During 1969, a total of 17 cases of diphthe­ ria, with no deaths, was recorded in the Registrar General’s Weekly Returns for England and Wales, compared with 17 in 1968 and 9 in 1967; there were no cases notified in Scotland in 1969, 7 in 1968 and 12 in 1967. Only 2 cases of diphtheria in which virulent strains of Corynebacterium diphtheriae mitis were isolated, were reported by laboratories in 1969.1 Laboratories also reported 7 cases in which Corynebacterium ulcerans were detected.2 No case of diphtheria was recorded by the Registrar General for England and Wales or Scotland in the first quarter of this year. Recently a London laboratory reported the isolation of Coryne­ bacterium diphtheriae mitis, virulent type, from the throat of a 39-year-old woman who attended a hospital outpatient depart­ ment with a complaint of recurrent sore throats. Her sevenyear-old son was carrying the organism; extensive swabbing (520 persons) revealed six other members of the family or school contacts of the child who were also carriers. None had clinical features of diphtheria. All child contacts were immunized.

Un laboratoire londonien a récemment annoncé l’isolement d ’un type virulent de Corynebacterium diphthe­ riae mitis dans un prélèvement fait chez une femme de 39 ans qui s’était présentée à la consultation externe d’un hôpital en se plai­ gnant de maux de gorge répétés. Son fils, âgé de sept ans, était également infecté. Des prélèvements opérés sur un nombre élevé de personnes (520) ont permis de constater que six autres membres de la famille ainsi que des écoliers ayant été en contact avec l’en­ fant étaient porteurs du germe. Aucun des sujets examinés ne manifestait de symptômes cliniques de la diphtérie.
