SALM ONELLA W IEN OUTBREAKS I t a l y . — Among more than 30 nosocomial outbreaks with salmonella observed throughout the country in maternity wards during the last two years, two recent epidemics associated with S. wien were reported to WHO. On 3 September 1975, in a private maternity ward in Avellino, three out of some 50 babies showed diarrhoeal symptoms. S. wien was isolated during bacteriological tests carried out on 5 September. The strain was found to be sensitive to trimethoprim/sulphamethoxazole mixture (1:5), gentamycin, nalidixic acid and nitro furantoin, but resistant to chloramphenicol, ampicillin, colistin, tetracycline, streptomycin and certain sulphonamides.1 In spite of preventive measures, 32 cases occurred and mortality was high, 18 babies having died from the infection. Bacteriological tests on water, food, etc. were negative for salmonella. The clinic was overcrowded. S. wien was isolated from two nurses and 8% of asymptomatic carriers were found in the general population of Avellino. Acute gastroenteritis with S. wien was usually not associated with adults.
POUSSÉES À SALM ONELLA WIEN I t a l ie . — Parmi plus de 50 poussées nosocomiales à salmonella observées au cours des deux dernières années dans des crèches pour nouveau-nés situées dans l’ensemble du pays, deux épidémies récentes à S. wien ont été notifiées à l’OMS. Le 3 septembre 1975, dans une maternité privée à Avellino, trois bébés sur environ 50 présentèrent une symptomatologie diarrhéique. S. wien fut isolée au cours des examens bactériolo giques effectués le 5 septembre. La souche était sensible à une préparation composée de triméthoprime/sulphaméthoxazole (1:5), à la gentamycine, à l’acide naiidixique et à la nitrofurantoïne, mais résistante au chloramphénicol, à l’ampicilline, à la colistine, à la tétracycline, à la streptomycine et à certains sulfamides.1 Malgré les mesures prophylactiques, 32 cas se produisirent et la mortalité fut élevée, 18 bébés étant morts de l’infection. Les exa mens bactériologiques de l'eau, des aliments, etc. ont été négatifs pour les salmonella. La clinique était surpeuplée. Deux infirmières étaient des porteurs de S. wien et l’on trouva 8% de porteurs asymptomatiques dans la population d ’Avellino. Les gastroentérites aiguës à S. wien n ’étaient généralement pas associées avec des adultes. Une seconde poussée d ’infections à S. wien, qui s’est produite à partir du 25 septembre 1975 dans un hôpital de Bolzano, a atteint 45 nouveau-nés (dont certains étaient des prématurés). Trois décès ont été signalés. 1 Ce type de résistance fait penser que la souche est la même que celle qui fut d 'ab o rd isolée en Algérie en novem bre 1969 et se répandit p a r la suite en France et dans diveis’autres pays.
A second outbreak o f S. wien infections occurred from 25 Sep tember 1975 in a hospital in Bolzano, attacking 45 newborn babies (some of whom were premature). Three deaths have been reported. > This resistance pattern suggests th a t this strain is the same as th a t which*was first isolated in Algeria in N ovem ber 1969 an d which subsequently spread in France and various other countries.
