ACQUIRED IMMUNODEFICIENCY SYNDROME (AIDS) Surveillance update to 31 December 1989 in the WHO European Region and projections up to 1991 By 31 December 1989, 31 497 cases had been reported to the WHO Collaborating Centre on AIDS by 32 countries (Table 1). This represents an increase of 65.5% (12 439 new cases) since December 19881 and of 12% (3 368 new cases) since September 1989.2 Between September and December 1989, the greatest in­ creases in the number of reported cases were noted in the fol­ lowing countries: Newly reported cases Country Number per week

Comparisons of percentage changes in cases diagnosed between 1988 and 1989., adjusted foi• reporting delays, show that homo/bisexuals now present the lowest estimated increases. Although the IV drug user (IVDU) group shows a large estimated increase, the largest percentage rise is estimated to be among the heterosexual transmission group. AIDS in this group could become at greater problem in the European Region (Fig, 1). On the other hand, a decline is estimated to have occurred in the mother-child transmission group. Although this decline is diffi­ cult to interpret, the increasing availability of HIV screening enables more women to be aware of their HIV status and thus to take informed decisions regarding pregnancy (abortion and conuacepuon). ■ S tc N o 15,1989, pp 109-112 2 See No 5, 1990, pp. 31-34.

Les comparaisons entre 1988 et 1989 des modifications en pourcen­ tage des cas diagnostiqués, redressés en fonction des délais de déclara­ tion, montrent que le groupe des homo/bisexuels présente actuellement l’accroissement estimé le plus bas. Bien que le groupe des toxicomanes IV présente un accroissement estimé non négligeable, l’accroissement le plus important est noté parmi le groupe des hétérosexuels. Ce groupe pourrait poser dans l’avenir un problème croissant pour la Région euro­ péenne (Fig: !). D’autre part, il semble qu’il y ait une décroissance estimée dans le groupe de transmission mère-enfant. So far, predictions were obtained by extrapolation from expo­ nential functions fitted to recent half-yearly incidence data adjusted for reporting delays. As this approach becomes less appropriate with the lengthening of doubling times in various populations, several alternative functions have been fined to improve the analysis of recent trends and to provide a range of short-term predictions for the European Community which cur­ rently accounts for 92% (28 920/31 497) of reported AIDS cases in Europe. Extrapolations to the end of 1991 using each of 3 functions (exponential [EXP], linear [LIN], linear with Box Cox transfor­ mation [BC-L], quadratic with Box Cox transformation [BC-Q], and linear-logistic [Lin-Log]) provide a range of forecasts (Fig. 2). This range is based solely on the best fitting curve for each model and does not take into account their confidence intervals (which can be as wide as the differences between models). With this reservation, total projected cumulated cases to the end of 1991 he in the range of 60 000-78 000 (all groups combined), with 24 00030000 projected cases among homo/bisexuals, 23 000-33.000 among IV drug users, and 6 000-8 000 in the heterosexual trans­ mission group. With the possible exception of the latter group, the lower parts of these ranges appear more probable. After 1991, the divergence between the alternative projections becomes unac­ ceptably large and extrapolauon beyond this time is not recom­ mended.

Jusqu’ici, les prévisions ont été obtenues par extrapolation de fonc­ tions exponentielles, ajustées aux données d’incidence par semestre, et redressées en fonction des délais de déclaration. Comme cette méthode devient de moins en moins applicable pour des populations chez les­ quelles le temps de doublement s’allonge, plusieurs fonctions alternati­ ves ont été appliquées pour améliorer l’analyse des tendances récentes et donner une fourchette de prévisions pour l’ensemble des pays de la Communauté européenne, qui contribue actuellement pour 92% (28 920/31497) des cas déclarés en Europe. Les extrapolations jusqu’à la fin de 1991 utilisant 5 fonctions diffé­ rentes (exponentielle [EXP], linéaire [LIN], linéaire avec transformauon Box Cox [BC-L], quadratique avec transformation Box Cox [BC-Q] et hnéaire-logistique [Lin-Logj) donnent une fourchette de prévisions (Fig. 2). Cette fourchette est basée seulement sur la courbe la mieux ajustée pour chaque modèle et ne prend pas en compte les intervalles de confiance (qui peuvent être aussi larges que les différences entre les; modèles). Avec cette réserve, le nombre total de cas cumulés projetés à la; fin de 1991 varie de 60 000 à 78 000 pour tous groupes confondus, de 24000 à 30000 pour les homo/bisexuels, de 23 000 à 33 000 pour les' toxicomanes et de 6 000 à 8 000 pour le groupe de transmission hété­ rosexuel. A l’exception possible du dernier groupe, la limite inférieure de ces fourchettes parait plus probable. Après 1991, la divergence entre les projections alternatives s’élargit de manière inacceptable et une extrapolation au-delà de cette période n’est pas recommandée.

MENINGOCOCCAL MENINGITIS IN AFRICA African countries reported a marked increase in cases of meningitis in 1988 and 1989 (Table 1). In Benin, Ethiopia, Kenya, Niger, Togo and the United Republic of Tanzania, case levels m 1989 exceeded those of 1988. Laboratory confirmation, when available, indicated serogroups A and C of Neisseria menin­ gitidis as the most common organisms. Meningococcal meningitis epidemics have been documented to recur in 8-12 year cycles during the dry season in the subSahara “meningitis belt” although intervals have shortened in several countries. Disease rates are often elevated 1-2 years after an epidemic. Ethiopia continues to experience elevated rates fol­ lowing last year’s epidemic : from 100-200 cases per month dur­ ing September-December 1989 to 305 cases in January 1990 and 702 in February. Epidemics occurring outside the meningitis belt (in Kenya and the United Republic of Tanzania in 1989 and in Uganda this year) are considered rare.

MÉNINGITE A MÉNINGOCOQUES EN AFRIQUE Les pays africains ont fait état d’une augmentationsensible du nombre de cas de méningite en 1988 et 1989 (Tableau IJ. En 1989, le nombre des cas rapportés au Bénin, en Ethiopie, au Kenya, au Niger, en Républi­ que-Urne de Tanzanie et au Togo a été plus important qu’en 1988. Lorsqu’elle a été possible, la confirmation au laboratoire a indiqué que les germes les plus communément en cause appartenaient aux sérogrou­ pes A et C de Neisseria meningitidis. On sait que les épidémies de méningite ménigococcique reviennent de façon cyclique tous les 8 à 12 ans au cours de la saison sèche dans la «ceinture de la mémngite» subsaharienne, cet intervalle ayant d’ailleurs été raccourci dans plusieurs pays. Les taux de morbidité sont souvent élevés 1 à 2 ans après une épidémie. Par exemple, l’Ethiopie continue à enregistrer des taux élevés depuis l’épidémie de l’année dernière; de 100 à 200 cas par mois, de septembre à décembre 1989, à 305 cas en janvier 1990 et 702 cas en février. On estime que les épidémies survenant en dehors de la ceinture de la méningite (au Kenya et en République-Unie de Tanzanie en 1989, en Ouganda cette année) sont rares. Vaccination On dispose d’un vaccin antiméningococcique contre les sérogroupes A et C, qui n’occasionne que de rares réactions indésirables, toujours bénignes. La vaccination est efficace 10 à 14 jours après administration et l’on pense que l’immunité protectrice qu’elle confère dure de 3 à 5 ans. Il est conseillé aux touristes de se faire vacciner avant de se Tendre dans des régions d’hyperendémicité ou dans des pays où sévissent des épidémies. Lutte contre les épidémies Bien que l’on ait une bonne connaissance de l’épidémiologie de la méningite à méningocoques, il peut s’avérer difficile de définir les seuils épidémiques. L’analyse de l’augmentation des taux de morbidité locaux doit tenir compte de l’épidémiologie de la maladie dans la région, notamment de ses aspects saisonniers, de la survenue de poussées récen­ tes, de son importance dans les pays avoisinants et des caractéristiques de chaque malade. Une fois qu’on a identifié l’épidémie, déterminé l’agent étiologique et pris la décision de vacciner, la campagne de vaccination doit commencer sans retard. Dans les .pays en développement où les épidémies de méningite sévissent fréquemment, on a testé diverses approches de vacdnauon sélective dont la vaccination de groupes de population restreints, ou celle de l’ensemble de la population de zones limitées dans lesquelles une flambée s’est produite ou pour lesquelles les données de la surveillance indiquent un risque immment d’épidémie.

Immunization A serogroup A and C meningococcal vaccine is available with only mild and infrequent adverse reactions. The immunization is valid 10-14 days after administration and protective immunity is estimated to last for 3 to 5 years. Tourists are advised to be immunized before travelling to hyper-endemic areas and to those countries experiencing outbreaks. Outbreak control Although much is knownof the epidemiology of meningococ­ cal meningitis, defining thresholds for outbreaks can be difficult. Analysis of increased local rates of disease needs to take into consideration the epidemiology of the disease in the region including seasonality, occurrence of recent outbreaks, disease in neighbouring countries and charactensucs of the pauent. Once an outbreak has been identified, the etiological agent determined and the decision made to immunize, there should be no delay in starting the immunization campaign. In developing countries where epidemics of meningococcal disease occur frequently, a number of approaches to selective vaccination have been tried. These include vaccination of restricted population groups or vaccination of the entire population in restricted areas where an outbreak has occurred or where surveillance data indicate an imminent risk of outbreaks.

Chemoprophylaxis No form of chemoprophylaxis is recommended against epi­ demic meningitis in Africa, Sulfonamides and rifampicin, which are being used elsewhere, are not suitable. Most N. meningitidis strains isolated in Africa are sulfonamide-resistant. The efficacy of rifampicin against epidemic meningococcal meningitis has not been proven and widespread use of the drug could lead to the development of resistant mycobacteria jeopardizing the treat­ ment of leprosy and tuberculosis. Treatment of cases and contacts The efficacy of a single injection of an oily suspension of chloramphenicol in the treatment of patients of all ages with meningococcal meningitis has been demonstrated. This anti­ biotic is therefore the recommended form of treatment for the disease under epidemic conditions. However, it is noi adequate for treatment of patients with meningitis caused by Streptococcus pneumoniae or Haemophilus influenzae.

Chimioprophylaxie On ne recommande aucune forme de chimioprophlyaxie contre la méningite épidémique en Afrique. Les sulfamides et la rifampidne utilisés ailleurs ne conviennent pas ici: la plupart des souches de N. meningitidis isolées en Afrique sont résistantes aux sulfamides, l’effica­ cité de la rifampidne contre la méningite à méningocoques épidémique n ’a pas été prouvée, et l’emploi généralisé de ce médicament pourrait conduire à l’émergence de mycobactéries résistantes, compromettant ainsi le traitement de la lèpre et de la tuberculose. Traitement des cas et des contacts On a démontré l’efficacité d’une injection unique d’une suspension huileuse de chloramphénicol pour le traitement des malades de tous âges. Cet antibiotique est par conséquent la forme de traitement recom­ mandée en cas d’épidémie. Toutefois, elle ne convient pas pour traiter les malades atteints de méningite à Streptococcus pneumoniae ou à Haemophilus influenzae.

Close contacts of patients with menigococcal disease are at a high risk of infection, especially during the first few days after, onset of illness of the index case. Such contacts should be warned about their need to obtain immediate medical attenuon at the first sign of any illness and immunized with an appropriate vaccine if the serotype of the index case or cases in the community is known.

Les contacts, proches des sujets atteints de méningite présentent un risque élevé de contracter l ’infection, en particulier les premiers jours sunrant l’apparition des symptômes chez le cas initial. Il faut donc les avertir de la nécessité de se faire immédiatement examiner au premier signe de maladie, et de se faire vacciner si l’on a pu déterminer le sérotype du cas initial ou des cas décelés dans la communauté.

This topic is being repeated m order to study the possible variations in incidence, the risk groups, the methods used and the seriousness of the anempt. An additional questionnaire is used to obtain information on the outcome and the circumstances that might account for the attempt. The first results of the reporting of cancers of the breast, cervix, colon /rectum, prostate and skin indicated that the importance of early detection of these cancers is not yet well appreciated bythe medical profession and the public. It seems important to continue this recording in order to evaluate the prevention campaigns. Information on the risk factors and treatment is requested in an additional questionnaire. Requests for H IV tests aie still being recorded in order to study possible variations in the frequency of requesting and in the profile of the patients asking for the test. Measles continues to be part of the recording programme in order to follow the impact of immunization on the incidence of this disease.

SURVEILLANCE ÉPIDÉMIOLOGIQUE Nouveau programme d’enregistrement par les médecins vigies (sentinelles)1 Belgique. - Un nouveau programme d’enregistrement par les méde­ cins vigies, auquel participent 176 médecins généralistes, a débuté en novembre 1989. Les thèmes suivants ont été choisis: tentative de suicide, cancer, demande de test VIH et rougeole. Les tentatives de suicide ont déjà été enregistrées en 1982-1983. Ce thème a été repris afin d’émdier les variations possibles dans l’mcidence, les groupes à risque, les moyens utilisés et la gravité de la tentative. Un questionnaire complémentaire permet d’obtenir des renseignementssur l’issue et sur les circonstances pouvant éventuellement expliquer le passage à l’acte. Les premiers résultats de l’enregistrement des cancers du sein, du col, du colon/rectum, delà prostate et de lapeau indiquaient que l’importance d’un dépistage précoce de ces cancers ne paraît pas encore bien être perçue par le corps médical et la population. La continuité de cet enregistrement paraît importante pour l’évaluation des campagnes de prévention. Des renseignements relaufs aux facteurs de risque et au traitement sont demandés dans un questionnaire complémentaire. Les demandes de tests VIH sont toujours enregistrées afin d’étudier des variations possibles dans la fréquence de la demande ainsi que dans le profil des patients demandant le test. La rougeole fait toujours partie du programme d’enregistrement afin de suivre l’impact de la vaccination sur l’inddence de cette maladie.

Sentinel health information systems, in which a limited number of general practitioners report on a defined list of carefully chosen topics (which change from time to time), are being increasingly used to supplement the information collected routinely or by registries in a number of countries. A sentinel network of general practitioners can be defined as one that keeps a watchful eye on a sample of the population by supplying regular and standardized reports on specific diseases and procedures in primary health care. These systems operate in various European countries including Belgium, France,, th e Netherlands, Portugal, Spain, Switzerland, and the United King­ dom. Population coverage is about 1% in most systems. The computerized French network includes on-line data bases and electronic mail for communication between the various part­ ners.1 In all the networks, there is regular feedbackof information in the form of articles and reports. . ...
