Q OUTBREAK NEWS Crimean-Congo haemorrhagic fever (CCHF), Kosovo (update).1 As of 26 June, the WHO office in Kosovo has reported 69 suspected cases, of which 6 have died. Of these, 18 are confirmed CCHF (15 laboratory and 3 on clinical/epidemiological grounds). Thirty-four are laboratory-negative and 17 are still under investigation. Most of the primary infections have occurred in areas of Kosovo that have had CCHF activity in previous years. CCHF infections are expected to continue throughout the summer and autumn months while ticks are still active. Ⅲ 1

Q LE POINT SUR LES ÉPIDÉMIES Fièvre hémorragique Crimée-Congo (FHCC), Kosovo (mise à jour).1 Au 26 juin, le bureau OMS au Kosovo avait signalé 69 cas présumés, dont 6 sont décédés. On a confirmé la FHCC pour 18 d’entre eux (15 en laboratoire et 3 sur des bases cliniques/épidémiologiques). Trente-quatre cas se sont avérés négatifs en laboratoire, et 17 sont encore en cours d’étude. La plupart des infections primaires se sont produites dans des zones du Kosovo qui ont connu une activité de FHCC les années précédentes. Onchocerciasis (river blindness) Report from the tenth InterAmerican conference on onchocerciasis, Guayaquil, Ecuador The Onchocerciasis Elimination Program for the Americas (OEPA) is a regional initiative working towards the goal of eliminating morbidity and transmission of the filarial parasitic disease known as “river blindness” or “Robles’ disease”. The OEPA strategy is to support ministries of health to provide sustained mass treatment every 6 months with safe and effective microfilaricide ivermectin (Mectizan®, donated by Merck & Co.) to populations in the region residing in communities known to be endemic for onchocerciasis. OEPA and the Pan American Health Organization (PAHO) have convened to the InterAmerican conferences on onchocerciasis (IACOs) representatives of the 6 countries of the Americas endemic for onchocer-

Onchocercose (cécité des rivières) Rapport de la dixième conférence interaméricaine sur l’onchocercose, Guayaquil, Equateur Le Programme pour l’élimination de l’onchocercose dans les Amériques (OEPA) est une initiative régionale qui vise à interrompre la transmission de la filariose parasitaire appelée «cécité des rivières» ou «onchocercose oculaire» et à éliminer la morbidité dont elle est la cause. ciasis, annually since 1991.1 OEPA and the IACO meetings receive financial support from the Carter Center/Lions Clubs International Foundation, the Inter American Development Bank and PAHO. The tenth annual conference (IACO’2000) was held in Guayaquil, Ecuador on 6-9 November 2000. In addition to representatives of the national programmes, the meeting was attended by representatives of WHO/PAHO, nongovernmental organizations (the Carter Center, the Lions Clubs, the Mectizan® Donation Program, and ChristoffelBlindenmission), the Centers for Disease Control and Prevention (United States), the African Programme for Onchocerciasis Control and other interested parties. The theme of IACO’2000 was New challenges for the regional initiative, and the topics addressed included the need for new and alternative diagnostic techniques for monitoring incidence of disease in human populations, the logistics needed to apply PCR in the field to measure infection rates in Simulium blackflies, the need to better monitor the impact of the programme on ocular morbidity, and the critical role of good data collection, timely data analysis and information exchange. Member countries accepted these challenges, and reaffirmed their commitment to delivering 2 treatment rounds per year, eliminating new ocular morbidity due to onchocerciasis by the year 2002, and suppressing transmission by the year 2005. Key recommendations of the meeting were that: (1) all programmes should heighten their efforts to provide 2 treatments per year (with at least 85% coverage of the eligible populations in each round in all communities in the region known to be endemic); and (2) programmes should report treatment data promptly (by treatment round and by community) to OEPA headquarters in Guatemala City. To monitor progress towards these goals, IACO’2000 adopted a new reporting index, the UTG(2), as the primary single indicator for measuring progress. The UTG(2) is defined as the number of individuals in the region who require ivermectin treatment (the Ultimate Treatment Goal) multiplied by 2 (since each individual should be treated twice during the course of a calendar year). IACO’2000 recommended the UTG(2) to better monitor the success of programmes in providing 2 treatments per year to all eligible individuals at risk. Treatment and assessment advances in the 6 endemic countries in the region for the year 2000 (supplemented by reports since the meeting), and other key conclusions and recommendations of IACO’2000, are summarized below. Treatment coverage. In previous reports, coverage has been reported as a percentage of the Ultimate Treatment Goal (UTG) which is defined as the total number of persons eligible for treatment in the Region of the Americas (429 920 persons). In the first treatment round of 2000, 367 619 persons were treated, representing a UTG coverage of 86%. This reflects a 34% increase over the number treated during the first round of 1999 (compared to a 1

L’IACO 2000 avait pour thème De nouveaux défis pour l’initiative régionale, et les questions qui ont été traitées ont notamment porté sur la nécessité de mettre au point des techniques diagnostiques nouvelles et différentes pour surveiller l’incidence de la maladie dans les populations, les moyens logistiques à mettre en œuvre pour appliquer la PCR sur le terrain afin de mesurer les taux d’infection chez les simulies, la nécessité de surveiller plus étroitement l’impact du programme sur la morbidité oculaire et le rôle essentiel de la collecte et de l’analyse des données et des échanges d’informations. Les pays membres ont accepté ces défis et réaffirmé l’engagement qu’ils ont pris d’assurer 2 tournées de traitement chaque année, d’éliminer les cas nouveaux de maladie oculaire due à l’onchocercose d’ici à l’an 2002, et d’interrompre la transmission d’ici à l’an 2005. Les participants ont recommandé en particulier: 1) que tous les programmes intensifient leurs efforts pour assurer 2 traitements par an (avec un taux de couverture d’au moins 85% des populations visées lors de chaque tournée dans toutes les communautés de la région connues pour être endémiques); et 2) que les responsables des programmes communiquent sans tarder les données relatives aux traitements (par tournée de traitement et par communauté) au siège de l’OEPA à Guatemala City. Afin de suivre les progrès accomplis pour atteindre ces objectifs, la CIAO 2000 a adopté un nouvel indicateur, l’objectif thérapeutique final (2) ou OTF(2) comme indicateur unique des progrès accomplis. L’OTF(2) correspond au nombre d’habitants de la région qui ont besoin d’un traitement à l’ivermectine (objectif thérapeutique final) multiplié par 2 (chaque individu devant être traité 2 fois par année civile). La CIAO 2000 a recommandé cet indicateur qui devrait permettre de mieux déterminer dans quelle mesure les programmes parviennent à garantir le traitement semestriel de tous les individus exposés visés par l’initiative. Le présent article (complété par des rapports établis depuis la réunion) donne un aperçu des activités exécutées en 2000 en matière de traitement et d’évaluation dans les 6 pays où l’onchocercose est endémique et récapitule les principales conclusions et recommandations de la CIAO 2000. Couverture du traitement. Dans les rapports précédents, la couverture était indiquée sous la forme d’un pourcentage de l’objectif thérapeutique final (OTF) qui correspond au nombre total de personnes à traiter dans la Région des Amériques (429 920). 1% increase between 1998 and 1999) (Fig. 1). The improvement in treatment figures for 2000 was due to the dramatic growth in the programmes of Guatemala and Venezuela (see below). However, at most, 59.6% of eligible persons (256 385) in the region were treated twice in 2000 (256 385 persons were treated in the second half of the year), giving a total of 624 004 ivermectin treatments provided in 2000. Use of the new UTG(2) denominator of 859 840 (twice the UTG of 429 920) shows that the overall UTG(2) treatment coverage in 2000 for the region was 73%, with only Colombia and Mexico achieving over 85% of the UTG(2) (Fig. 2). It was agreed at IACO’2000 that all programmes would aim to reach at least 85% of their UTG(2) in 2001, with the exception of remote Amazon foci in southern Venezuela and Brazil, which would aim to reach their UTG(2) in 2002.

Brazil provided 7 659 ivermectin treatments to its eligible population of 6 794. Total treatments for the year 2000 represent 56% coverage of the UTG(2) for Brazil of 13 588. These treatments were provided to migrant communities in the remote jungle areas of the northern states of Roraima and Amazonas. During the first half of 2000, 5 103 persons (75% of the UTG) were treated, an 86% increase from 1999 treatments given in the first half of that year. During the second half of 2000, 2 556 persons (38% of the UTG) received treatment. The distribution strategy in Brazil utilizes health care centres situated in accessible base camps that are staffed by the Ministry of Health and by personnel from nongovernmental development organizations. Treatments took place in 15 of the 19 base camps in endemic areas, including 4 of the 5 high-risk base camps (e.g. those RELEVE EPIDEMIOLOGIQUE HEBDOMADAIRE, No 27, 6 JUILLET 2001

where the prevalence of infection was > 60%). A key need for the Brazilian programme is to reach > 85% of its UTG(2) by 2002. Colombia has a single known endemic community (Naicioná, in the municipality of López de Micay, department of Cauca). For 2000, 1 101 persons eligible for treatment were registered in the endemic area, and during the first semester 1 070 persons (97% of the UTG) were treated. This already high coverage rose to 100% of the UTG for the latter half of the year. The UTG(2) coverage for 2000 was 99% (2 171 treatments of its 2 202 UTG(2)). This was achieved despite civil unrest in and around the small Colombian focus that prevented the epidemiological impact evaluations scheduled for 2000 from being carried out. IACO’2000 commended the community and local health services for continuing ivermectin distribution activities despite the unrest. In-depth epidemiological evaluations to determine whether transmission has been suppressed in Colombia have been rescheduled to take place during 2001. Ecuador treated 16 490 persons in the first half of 2000, 85% of the country’s 19 321 eligible population. However, only an additional 2 770 treatments (14% of the UTG) were provided during the second half of the year, resulting in an overall UTG(2) coverage of only 50% (19 260 of 38 642 required treatments). Of the 119 endemic communities, 106 (89%) received treatment, including all 42 high-risk communities. The programme pledged to provide 2 treatment rounds in all endemic communities in 2001, and the Ministry of Health committed US$40 000 per year to the programme. Guatemala treated 127 978 persons during the first 6 months of 2000, or 80% of the UTG. This was a 66% increase compared to 1999, when 76 985 (48%) persons were treated. During the second half of the year, 108 350 persons received treatment (68% of the UTG). Considering both rounds, a total of 236 328 doses of ivermectin were administered in 2000, representing 74% of its UTG(2) of 320 000. Of the 552 communities which were endemic at the beginning of 2000, 501 (91%) received treatment, including 38 (84%) of 45 high-risk communities. Key needs for the programme in 2001 will be to strengthen community-based ivermectin delivery (through the use of community volunteers) and attain >85% of the UTG(2). Mexico administered a total of 290 190 treatments and achieved a coverage of 91% of its UTG(2) of 317 648. In the first 6 months of 2000, 157 291 persons were treated (99% of the UTG), a 3% increase over 1999. During the second half of the year, 132 899 persons were treated (84% of the UTG). Of the 689 endemic communities, all received treatment (including 100% of the 39 high-risk communities). In 2000, the programme was host to preparatory exercises towards certification of elimination of onchocerciasis in the foci of Oaxaca and northern Chiapas, where the status of efforts to suppress transmission was examined (results pending). A key need for the programme in 2001 will be to strengthen community-based ivermectin delivery (through the use of community volunteers) in Chiapas state, where political unrest hampers activities. 208

Venezuela provided treatment to a total of 59 687 persons (71% of its UTG of 83 880) during the first half of 2000 in the 2 endemic foci in the north of the country, and in the smaller southern Amazon focus. This represents an increase of 156% over all treatments provided in 1999. However, only 8 676 treatments (15% of the UTG) were administered during the second half of the year, resulting in a total of 68 363 treatments for 2000, the lowest UTG(2) coverage (41%) in the region. Of the 618 confirmed endemic communities in the country, all 80 high-risk communities were treated, along with 373 (69%) of the remaining 538 endemic communities. Additional funding is needed to strengthen the programme in 2001 to reach > 85% UTG(2) coverage. Editorial note. The OEPA initiative made significant progress in 2000 towards the eventual regional elimination of onchocerciasis from the western hemisphere, by increasing treatment services by over 30% compared to 1999. Additionally, a group of onchocerciasis experts was convened in September 2000 under the auspices of WHO headquarters to review and revise proposed guidelines (drafted by OEPA and ratified by IACO’99) for the certification of elimination of onchocerciasis.2 The greatest possibility for prompt suppression of transmission of onchocerciasis in the region is in Mexico and Guatemala, where the vector (Simulium ochraceum) is very

inefficient. Semi-annual treatment activities in 2000 in Guatemala and Mexico were relatively successful (as well as in Colombia, despite the unrest). In contrast, the experience of Ecuador illustrates the need to heighten regional surveillance of treatments by defining coverage based on treatments needed – UTG(2) – rather than on the eligible population at risk (UTG). Whereas 85% of the eligible population in Ecuador received treatment during the first round in 2000, relatively few (14%) of these persons received a second dose. Consequently, UTG(2) coverage in 2000 was only 50% in Ecuador. The need to sustain semi-annual treatment is particularly important in Ecuador, since onchocerciasis is transmitted there by the most efficient vector species in the Americas, Simulium exiguum. Similarly, while the programme in Venezuela increased treatments in 2000 dramatically, the greatest number of persons in the region not being reached by any ivermectin distribution services whatsoever (an estimated 15 517) are in Venezuela. Des tournées semestrielles de traitement exécutées en 2000 au Guatemala et au Mexique ont été relativement fructueuses (de même qu’en Colombie, malgré les troubles). Par contre, ce qui s’est passé en Equateur montre qu’il est nécessaire de renforcer la surveillance régionale des traitements en définissant la couverture sur la base des traitements à administrer – OTF(2) – plutôt que de la population à traiter (OTF). Alors que 85% de la population exposée en Equateur a été traitée lors de la première tournée de 2000, seul un petit pourcentage (14%) de cette population a reçu une deuxième dose. En conséquence, le taux de couverture de l’OTF(2) en 2000 n’a été que de 50% en Equateur. Il est particulièrement important d’assurer des traitements semestriels durables en Equateur où l’onchocercose est transmise par l’espèce vectrice la plus efficace présente dans les Amériques, Simulium exiguum. De même, alors que le programme exécuté au Venezuela a conduit à une augmentation spectaculaire du nombre des traitements en 2000, c’est également dans ce pays que se trouve le plus grand nombre de personnes de la région (estimé à 15 517) qui ne bénéficient d’aucune distribution d’ivermectine. Influenza Argentina (23 June 2001). A local outbreak of influenza A was detected during the third week of June, which was the first sign of influenza spread for the season. 1

Grippe Argentine (23 juin 2001).1 Une flambée locale de grippe A a été dépistée au cours de la troisième semaine de juin, ce qui représente le premier signe de propagation grippale de la saison. Chili (16 juin 2001).1 L’activité grippale s’est maintenue à un niveau général au cours de la deuxième semaine de juin et a touché les villes de Santiago, Talcahuano et Valparaíso.
