T he Austrian Salmonella Reference Laboratory receives Salmonella s tr a in s o f .h u m a n and non-hum an sources from the whole country fo r typing and epidemio­ logical interpretation. Since the beginning of the interna­ tional S. enteritidis epidemic in the late 1980s, Austria, like many other European countries, has mainly registered 5. ententidis phage type (PT) 4 (Colindale scheme), whereas P T 21 rem ained rare and scattered all over the country. Le Tableau 1 donne le nom bre de cas de S. enteritidis notifiés en 1994.

Since D ecem ber 1994, however, the reference labora­ tory has registered an increasing num ber o f sporadic hu­ m an cases of 5. enteritidis P T 21 infections in a circum­ scribed region in W estern A ustria, w ith a cum ulative total o f 219 cases (Fig. 1). In April 1995, P T 21 was identified for the first tim e from a cloacal swab isolate of a herd a t the end o f lay in a single egg-producing form. T he location of the farm and its trade area correlated well w ith the hom e addresses o f patients, as did individual histories of egg244

D epuis décembre 1994, toutefois, le laboratoire de référence a enregistré un nom bre croissant de cas hum ains sporadiques d ’infection à S. enteritidis L T 21 dans une région circonscrite de l’ouest de l’Autriche, avec un total cumulé de 219 cas (Fig. T). En avril 1995, le L T 21 a été isolé pour la prem ière fois à partir d ’écouvillonnages closeaux de poules en fin de ponte dans une exploitation productrice d ’œufs. La situation de l’exploitation et sa spécialisation correspondaient parfaitem ent avec les adresses des patients, ceux-ci ayant soit acheté des œufs, soit consommé des

buying, and consum ption of undercooked or raw egg dish­ es. T he local health authorities carried out bacteriological exam inations of eggs at the farm . O ut o f 1 200 eggs inves­ tigated, 13 pools of 5 eggs were found to be positive for S. enteritidis P T 21 (5.4% ). T he farm is the m ajor egg producer in th e region, with a daily output of about 40 000 eggs, which could represent several hundred infected eggs a day. plats à base d'œufs crus ou mal cuits. Les autorités sanitaires locales ont effectué des analyses bactériologiques sur des œufs de cette exploitation. Sur 1 200 œufs analysés, 13 mélanges de 5 œufs se sont révélés positifs pour S. enteritidis L T 21 (5,4% ). C ette exploitation est le principal producteur d'œufs de la région, avec une production journalière d’environ 40 000 œufs, ce qui pourrait représenter plusieurs centaines d ’œufs infectés par jour.

In m id-M ay 1995, the farm was legally forbidden to m arket fresh eggs, b u t no recall action was undertaken. Since then, the incidence of 5. enteritidis P T 21 has de­ clined steadily (Fig. 2), whereas a sim ilar decline has not been observed for other phage types of S. enteritidis. D uring week 28 of 1995, no hum an cases of S. enteritidis P T 21 had been reported. A la m i-mai 1995, l’exploitation s'est vu signifier l’interdiction de com mercialiser des œufs frais, mais aucune m esure de rappel n 'a été prise. T h e episode has highlighted the following; Joint phage typing and interpretation of hum an and non-hum an SalnwneSa isolates provides a good model o f successful intervention epidemiology. • Phage-typing has proven to be a helpful analytical m ethod for trend analysis and for a better understand­ ing of the ongoing problem of 5. enteritidis. T he “key” cloacal swab isolate made it possible to trace back the presum ed source of the outbreak which, w ithout phage typing, could n o t have been identified easily. T h e possible im pact of a single egg producer on the epidemiology o f 5. enteritidis has been brought to light. T h e public is n o t aware enough of th e potential risk of infection from single shell eggs. T o be efficient, the control of S. enteritidis ip eggs m ust be established at the produc­ tion level in order to prevent the spread -of the agent from a n i m a ls to h u m a n s . Pour être efficace, la lutte contre S. enteritidis dans les œufs doit s’instaurer au niveau de la production, afin d’éviter la transm ission de l’organism e de l’anim al à l’hom m e.
