MALADIES QUARANTENAIRES — QUARANTINABLE DISEASES Territoires infectés au 6 février 1969 — Infected areas as on 6 February 1969 Notifications reçues aux termes du Règlement sanitaire international concernant les circonscriptions infectées ou les territoires où la présence de maladies quarantenaires a été signalée (voir page 96). Pour les autres territoires, la présence de maladies quarantenaires a été notifiée aux termes des articles 4, 5 et 9 a) pour la période se terminant à la date indiquée. Notifications received under the International Sanitary Regulations relating to infected local areas and to areas in which the presence o f quarantinable diseases was reported (see page 96). For the other areas, the presence o f quarantinable diseases was notified under Articles 4, 5 and 9 (a) for the period ending on the date indicated.

Amérique Bolivie. ■ 1 Une épidémie de peste a été signalée dans quatre localités des Provinces de Belisario Boeto et Tomina (Département de Chuquisaca). Pasteurella pestis a été identifié par le laboratoire. Trente cas (14 décès) ont été observés entre le 22 novembre et le 19 décembre 1968. Depuis cette date, grâce aux mesures d’endiguement qui ont été prises, aucun cas n ’a été signalé. Cette poussée de peste n ’implique aucun risque pour les personnes effectuant des voyages internationaux. 1 Voir page 105,

America Bolivia. — 1 An outbreak of plague, with laboratory confirmation of Pasteurella pestis, has been reported from four localities in Belisario Boeto and Tomina Provinces (Chuquisaca Department). There were 30 cases with 14 deaths in the period 22 November to 19 December 1968. As a result of successful control measures there have been no further cases since the latter date. The outbreak does not represent any hazard to international travel. '■ j.

CHOLÉRA Asie Malaisie. — Un cas de choléra El Tor confirmé par le laboratoire, a été rapporté dans le District sanitaire de Kulim, dans l’Etat de Kedah (Malaisie occidentale) le 6 février. Une enquête est en cours afin d’établir la source de l’infection. L’Etat de Kedah était indemne de choléra depuis l’épidémie de 1963-64, à l’exception toutefois de cinq cas sporadiques observés entre le 26 mai et le 3 août 1968.

CHOLERA Asia Malaysia. — One case of choiera El Tor confirmed in the laboratory has been reported from Kulim Health District, Kedah State (West Malaysia) on 6 February. The source of infection is being investigated. Since the epidemic of 1963-64 Kedah State has been free of cholera except for five sporadic cases occurring between 26 May and 3 August 1968.

VARIOLE Afrique Soudan. — 1 Trois nouveaux cas ont été signalés dans la Province du Nil bleu. Il s’agit d’une personne venue de la Province du HautNil et de deux contacts des cas précédemment notifiés. Un rapport concernant cette poussée de variole sera publié prochainement. 1 Voir N° 5, p. 97.

America Canada (information dated 24 January 1969). — 2 Since December 1968, scattered influenza outbreaks associated with virus A2/Hong Kong/68 have been reported in most provinces. The incidence reported from 20 to 24 January appears unchanged when compared with that of the period 13 to 17 January. Absenteism does not appear excessive for this time of year, except in a few instances. Virus A2/Hong Kong/68 has been isolated in all provinces except New Brunswick.

Europe Denmark (information dated 25 January 1969). — Some sporadic cases of influenza-like disease have been observed in various parts of the country. Serological evidence of infection with virus A was obtained in five patients. Finland (information for the weekending 1 February 1969. — 3 The influenza epidemic is developing in the general population in the south and the south-west of the country. The weekly absen­ teism in factories in Helsinki has increased in three weeks from 1 % to 4.5 % (week of 19 to 25 January). In middle and eastern Finland, influenza has begun to spread in the population generally. In northern Finland, influenza has been confirmed in military units. Adults are more often attacked than children. The disease is generally mild. Fifteen strains of virus A2/Hong Kong/68 (and many adeno­ viruses) were isolated in the week ending 1 February. Serological evidence of infection with virus A2/Hong Kong/68 has been obtained in many patients.

Netherlands (information dated 28 January 1969). — 4 The influenza epidemic associated with virus A2/Hong Kong/68 is declining in the west and the centre of the country. In the north, the east and the south, the incidence is also beginning to decrease, in spite of the persistence of a few foci. The disease is mild or of moderate severity. There are fewer complications than in the preceding influenza season. Portugal (information dated 1 February 1969). — Sporadic cases of influenza-like disease were observed in the last two weeks of January in several parts of the country. Jusqu’ici, 64 848 cas ont été signalés à l’OMS pour l’année 1968. On pense que les renseignements non encore parvenus accroîtront le total de 2 à 3000 unités. Même ainsi, il restera très inférieur à celui de 1967 (122 098 cas) et sera le plus faible qui ait jamais été enregistré, exception faite de 1960 (65 186 cas). Dans les pays d’Afrique occidentale et centrale, 5 327 cas ont été notés en 1968 contre 10 813 en 1967. Comme le montre la figure 1, les chitfres ont tendu à baisser régulièrement depuis le début de l’année. En septembre et en octobre, une légère remontée a coïncidé avec le début d’une campagne de surveillance et de dépis­ tage intensif; elle a été suivie en novembre d’une forte diminution. To date, 64 848 cases have been notified to WHO for the year 1968. Delayed reports are expected to increase this provisional total by 2 000 to 3 000 cases. The total, however, will be far less than the 122 098 cases recorded in 1967 and, with the exception of 1960 (65 186 cases), should be the lowest ever recorded. In the countries of Western and Central Africa, 5 327 cases have been recorded during 1968 compared to 10 813 cases in 1967. As observed in Figure 1, the number of cases has fallen steadily since the beginning of the year. In September and October, the number of cases increased slightly coincident with the beginning of an inten­ sive surveillance and case investigation programme but again declined sharply in November. The ensuing several months will be of particular interest for, as may be observed in the graph, there is normally a significant seasonal increase in the first half of the year.

La figure 2 a trait aux zones d ’Afrique occidentale et centrale où des cas de variole ont été notifiés au cours de trois mois distincts de l’année 1968. L’aire d’infection s’est rétrécie à mesure que l’année s’avançait, à tel point qu’en novembre, il ne subsistait que trois foyers d’étendue limitée: 1) Nigéria - Niger, 2) Nigéria - Togo Dahomey, et 3) Guinée - Sierra Leone. Une action intensive de dépistage et d’endiguement est menée partout pour interrompre complètement la transmission. On a constaté que la maladie se pro­ page souvent à travers les frontières, ce qui montre bien la nécessité de concevoir l’éradication à l’échelle régionale et d’assurer une coordination aussi poussée que possible entre pays limitrophes.

Areas of Western and Central Africa reporting cases of smallpox during three different months of 1968 are depicted in Figure 2. The infected areas have become progressively less extensive as the year has progressed to the extent that, in November, cases were detected only in three limited foci: 1) Nigeria - Niger, 2) Nigeria - Togo Dahomey and 3) Guinea - Sierra Leone. Intensive case finding and containment activities are in progress in all areas in an effort to interrupt completely smallpox transmission. It has been noted that transfer of infection across national boundaries occurs fre­ quently, illustrating the need for smallpox eradication to be under­ taken as a regional effort with the maximum of coordination bet­ ween the adjacent countries.

POUSSÉE DE VARIOLE AU SIERRA LEONE En mai 1968, une poussée inhabituelle de variole atteignant prin­ cipalement les adultes a été observée dans la Chefferie de Yawei, District de Kailahun, dans l ’est du Sierra Leone. Une enquête et des mesures d ’endiguement rapides ont limité la propagation de la maladie à une seule génération de cas. Le district intéressé est situé près des frontières de la Guinée et du Libéria; il compte 163 000 habitants et sa densité de population est de 97,0 par mille carré. A la fin de mai 1968, des cas de variole ont été signalés à un ins­ pecteur de la santé local qui a immédiatement recherché et isolé les malades et vacciné les sujets de leur entourage immédiat. Le 8 juin, le personnel du Service national de l’éradication de la variole a été alerté et une équipe a été envoyée sur place; les 14 et 15 juin, 3 421 vaccinations supplémentaires ont été pratiquées. L’enquête épidémiologique a révélé que le premier cas était une femme âgée de 40 ans, commerçante, qui était tombée malade vers le 7 mai, une semaine après être rentrée d’un district voisin infecté où elle avait passé la nuit, Elle avait d’abord souffert de céphalées, de fièvre et de douleurs dans le cou et, 5 jours plus tard, une éruption avait été constatée. Elle était décédée deux jours après, mais la variole n’avait pas été diagnostiquée. Cette femme était le responsable local d’une association féminine. De nombreuses personnes sont venues la voir lorsqu’elle était malade, et après sa mort, six femmes ont aidé à laver son corps; en outre, 243 personnes étaient, paraît-il, présentes à son enterrement, beau­ coup d’entre elles venant de villages voisins. Entre le 22 et le 29 mai, on a observé 10 autres cas parmi les sujets qui avaient été exposés. Le premier était le mari de la défunte qui est tombé malade le 22 mai et est mort. Les 23 et 24 mai, quatre nouvelles personnes du même village sont tombées malades. Toutes les quatres s’étaient rendues en visite chez la première malade et deux d’entre elles avaient lavé le corps. Les intervalles entre le début de leur maladie et le début du premier cas, étaient de 12 à 14 jours. L’intervalle entre le début de la maladie et le lavage du corps était de huit jours dans les deux cas. H est donc probable que l’infection a été contractée au cours des visités à la malade plutôt qu’au moment du lavage du corps. Quatre femmes venant d’autres villages avaient également aidé à laver le corps et toutes sont tombées malades de 11 à 14 jours plus tard. Deux d’entre elles ont rendu visite à la malade et ont pu con­ tracter l’infection à ce moment. Toutefois il est certain que l’une d’entre elles, et probable qu’une autre, n ’avaient pas rendu visite à la malade. Enfin, un enfant de 4 ans qui n ’avait eu aucun contact direct avec le cas primaire a été atteint. Sa mère avait rendu visite à la première malade mais n ’a pas été atteinte. L’infection de l’enfant pourrait donc provenir d ’un objet rapporté par la mère, avoir été contractée à l’enterrement ou à partir d ’une source inconnue. Il faut noter que le corps est resté 24 heures chez le sujet N° 4 et que sur 15 personnes vivant dans la maison, dont aucune n ’avait été vaccinée, un seul cas a été observé. La personne atteinte était une femme qui avait rendu visite à la première malade. En dépit du fait qu’un quart des adultes et une proportion plus grande des enfants habitant cette région n ’avaient pas été vaccinés, on n’a pas constaté d’autres cas. Cette poussée épidémique illustre l’importance des contacts directs dans la propagation de la maladie. La rapidité d ’action de l’inspecteur local, qui a isolé tous les sujets et vacciné les contacts de l’entourage immédiat des malades, a sans aucun doute arrêté la transmission et écarté un risque d ’épidémie grave.

SMALLPOX OUTBREAK: SIERRA LEONE During May 1968, an unusual outbreak of smallpox involving principally adults was observed in Yawei Chiefdom, Kailahun District in Eastern Sierra Leone. Prompt investigation and con­ tainment action limited the spread of the disease to a single genera­ tion of cases. The district concerned is located near the borders of Guinea and Liberia and has a population of 163 000 persons with a population density of 97.0 persons per square mile. In late May 1968, cases of smallpox were reported to a local health inspector who immediately sought out and isolated the patients and vaccinated individuals in close contact. On 8 June, the National Smallpox Eradication staff was notified and a team was despatched to the area: on 14-15 June, 3 421 additional vaccinations were done in the area of the cases. Epidemiological investigation revealed that the first case was a 40-year-old woman, a trader, who became ill about 7 May, one week after returning from an overnight visit in a neighbouring infected district. She complained initially of headache, fever and neck pain and five days later a rash was observed. She died two days later, but smallpox was not diagnosed. The woman was the local head of a female society. She had a number of visitors while ill; after she died, 6 women participated in washing her corpse; subsequently, 243 persons were said to have been present at her funeral, many of whom came from neighbouring villages. Between 22 and 29 May, 10 additional cases occurred among those who were exposed. The first case was the patient’s husband, who became ill on 22 May and died. On 23 and 24 May, four additional persons in the patient’s village became ill. All four had visited during her illness and two of the four had washed her corpse. The intervals between their onsets of illness and the onset of the patient’s illness were from 12 to 14 days. The interval between onset of illness and washing the corpse was in both cases eight days. It is therefore more likely that infection was acquired during visits to the patient than during the washing of the corpse. Four women from other villages also washed the corpse, and all of these became ill 11 to 14 days later. Two of these women had visited the woman during her illness and might have been infected then. One certainly, and another probably, had not visited the woman when she was ill. Finally, one patient, a 4-year-old child, had no direct contact with the patient. His mother had visited the patient but did not become ill. The child’s infection might have originated from something brought back by the mother, at the funeral, or from some other unrecognized source. Notably, the corpse had been kept in the house of patient No. 4 for 24 hours, but, of 15 inhabitants, none of whom had ever been vaccinated, only a single case occurred. This occurred in a woman who had visited the patient while ill. Despite the fact that one-fourth of the adults and a larger pro­ portion of children in this area were unvaccinated, no further cases occurred. The outbreak illustrates the importance of intimate contact in spread of the disease. Prompt action on the part of the local health inspector who isolated all patients and vaccinated close contacts undoubtedly deterred further transmission and prevented a poten­ tially major epidemic.
