HEPATITIS Acute viral hepatitis B: laboratory reports 1980-1984 U n i t e d K i n g d o m . — Reports o f patients with acute viral hepa titis B have been received from laboratories in England, Wales and Ireland since 1972. The annual number of reports, which had remained approxi mately 1 000 in each of the years 1975-1979, increased during the following 5-year period from about 1 000 in 1980 to more than 1 200 in each of the years 1981,1982 and 1983, with a sharp rise to almost 2 000 in 1984 (Table 1). Acute viral hepatitis B remained uncommon among children and the elderly. Incidence rates for adults of 15-64 years are 6 per 100 000 for men and 2 per 100 000 for women. Compared with the corresponding rates in 1975-1979, there was a 50% rise in inci dence for men but no change in the rate for women.

Le nombre annuel de rapports, qui est resté d’environ 1 000 au cours des années 1975-1979, a augmenté au cours des années qui ont suivi, passant d’environ 1 000 en 1980 à plus de 1 200 en 1981, 1982 et 1983, puis brutalement à près de 2 000 en 1984 (Tableau I).
L’hépaute virale B aiguë est restée rare chez les enfants et les personnes âgées. Chez les adultes (15-64 ans) les taux d’incidence étaient de 6 pour 100000 chez les hommes et 2 pour 100000 chez les femmes. Si l’on compare ces chiffres avec ceux de 1975-1979, on observe une augmen tation de 50% de l’incidence chez les hommes, mais pas de changement chez les femmes.

Drug abuse was, asm 1975-1979, the most common cause. The increase in the annual total of patients in 1981 and again in 1984 was directly associated with an increase in the number o f patients with a history ° f drug abuse. O f the 1 566 patients with a history of drug abuse reported during the 5 years, only 319 (20%) were women. Male homosexuals formed 8% of all patients during the period. From 1982 there was a sustained increase in this number so that in 1984 there were twice as many as in 1980. Hepatitis B infection appeared to remain endemic in many institutions for the mentally handicapped. The infections of 55 patients and 43 staff were usually sporadic and were associated with carriers rather than other acute infections. There were no reports of outbreaks in renal units. Hepatitis was the cause of 30 of 34 deaths reported. Among adults aged 15-64 years case-fatality rates were higher for women (0.6%) than for men (0.3%). Age-specific rates were as follows: 0.3% for the age group 15-34 years, 1.2% for 35-64 years, and 2% for > 65 years. There were 2 deaths among the 72 children. Acute hepatitis B infection in health service staff During the 5 years, 364 staff with acute viral hepatitis B were reported, considerably more than the total o f 287 reported in 1975-1979. However, in 1980-1984, 80 (22%) acquired the infec tion while working abroad whereas only 9% had done so in 19751979. Although specific immunoglobulin continued to be available throughout the period, most ofthe infected health service staffhad either failed to report or were unaware o f their exposure: 3 (0.8%) of the infections were o f staff who developed hepatitis despite prophylaxis. There were only 2 reports of staffo f renal units—both were physicians. One had taken repeated blood samples from a transplanted patient, who was a known highly infective carrier, but she could not recall any accidental inoculation or contamination. The other worked in a unit in which there were no carrier patients : the time of the onset suggested that the infection had been acquired before beginning work in the unit. None o f the health service staff infections resulted in death.

En général, les cas notifiés (55 malades et 43 membres du,personnel) étaient sporadiques et étaient plutôt asso ciés à des porteurs qu'à d’autres cas d’infection aiguë. Aucune flambée d’hépatite B n’a été signalée dans les services de néphrologie. Trente des 34 décès notifiés étaient directement dus à l’hépatite. Parmi les adultes de 15 à 64 ans, les taux de létalité étaient plus élevés chez les femmes (0,6%) que chez les hommes (0,3%). Les taux selon l’âge étaient de 0,3% pour les 15-34 ans, 1,2% pour les 35-64 ans et 2,0% pour les 65 ans et plus. Deux décès sont survenus parmi les 72 enfants atteints. Hépatite B aiguë chez les personnels de santé En 1980-1984, 364 cas d'hepatite virale B aiguë survenus chez des membres des personnels de santé ont été notifiés, soit nettement plus qu’en 1975-1979 (287 cas). Toutefois, en 1980-1984,80 de ces malades (22%) ont contracté l’infection en travaillant à l’étranger, contre 9% en 1975-1979. Bien que pendant la période 1980-1984 des immunoglobulines spéci fiques aient en tout temps été à la disposition des personnels de santé, la plupart des malades ne s’étalent pas aperçus qu’ils avaient été exposés à l'hépatite B, ou ne l’avaient pas signalé : 3 cas (0,8) sont toutefois survenus malgré cette prophylaxie. Deux cas seulement (2 médecins) ont été noti fiés parmi le personnel des services de néphrologie. L’un des médecins, une femme, avait effectué plusieurs fois des prises de sang sur un trans planté, que l’on savait être porteur et fortement infectieux, mais elle ne se souvenait d'aucun incident ayant pu entraîner une inoculation ou une contamination accidentelle. L’autre médecin travaillait dans un service où il n’existait pas de porteur parmi les malades: d’après le moment d’appantion de la maladie, il semblerait que ce malade ait contracté l’infecnon avant de commencer à travailler dans le service. Aucune des infections notifiées parmi les personnels de santé n’a entraîné de décès.
