INTRODUCTION In this report we examine the decline in infection rates since vector control activities started as measured by: (i) the number of infections anpng children born since control started; (ii) changes in the prevalence of infection over time in villages in which 3 or more epidemio'logical surveys have been conducted; (iii) changes in the prevalence of infection between the first and subsequent surveys in all villages in which 2 or more surveys have been conductedraccording to the time since vector control activities started; (iv) changes in age and sex specific prevalences and intensities of infection related to the duration of control activities. A preliminary analysis was presented and discussed at a meeting in Geneva, August l5-]9, 1983 with Drs Prost, l,lalsh, Remme, Phillipon, Karam and Grunewald. Various suggestions were made for revised and additional ana'lyses and these have been incorporated into the present report. 2. DATA ANALYSED The analyses have been based on data available by mid-August .l983 from villages 'in phase areas I, II, IIIE and IIIW in which two or more "91oba1" Surveys had been conducted. V'il1ages in which treatment -2- substudies have taken place at any stage were excluded*. Villages first surveyed 4 or more years after control started were also excluded (villages 124, 125, 126), One village was included in the analyses even though the skin snip examinations at the first survey were not made using the standard techniques. It was considered, however, that these results from this vi1'lage (no.63) would be compatible with other surveys (A Prost). 3. GEOGRAPHICAL DIVISION OF STUDY VILLAGES The villages included in the analyses have been grouped into l0 clusters within each of which the vector control situation is thought to have been fairly homogeneous. These clusters were defined by Drs Walsh and Prost from a consideration of the results from entomo'logical surveys only. The cluster definitions are given in Table I and are shown also in Figure 1. Ivory Coast phase IIIl,l was excluded from al1 analyses. A descriptionof each cluster based on the entomological results, due to Dr }.lalsh, is given in Appendix l. Data from a total of I43 vi'l'lages, 66 with 3 or more surveys' were analysed. Thein locations within the clusters are shown in Figure 1. Details of the village names, the dates of the surveys in each and the prevalence of infection found at each survey are given in Appendix 2. * These were villages 22,24,?B (3 surveys) and villages 6,29 (2 surveys). Villages 370 and 37.I (2 surveys) were also excluded since approximately 25% of the population were recorded as hav'ing received treatment at the second survey although these vi1'lages were not coded aS "TR" substudy vi11ages. The categories used for this division were defined according to the age-sex standardised prevalence at the first g1obal survey. Villages with a prevalence of less than 35% were class'ified as "hypoendemic", those with a preva'lence of 35% to 59.9% as "rndsoendemic" and thcse with a prevalence of 6A% or more as "hyperendemic". 4. RESULTS 4..I New infections among those born after the start of vector control activities - Infection rates have been examined separateiy for the fo'llowing groups of children (i) those born between surveys 1 and 2 and examined at survey 2 (ii) those born between surveys I and 2 and e)(amined at survey 3 (iii) those born between surveys 2 and 3 and examined at survey 3. -4- Note that children in group (ii) are a'lso included in group (i) and so the results should not be conbined. They have been presented separately since children found infected at survey 2 wil'l not necessarily be found so againatsurvey 3 and vice-versa. In order to combine these groups without simply excluding the second survey results in villages with 3 surveys it would be necessary to have data on the age and sex specific incidence of both acquiring and losing infections under pre-control conditions in order to calculate expected rates of infection. Such data are not available. (i) Children born between survegs 7 and 2 examined at sutveg 2 4158 children were registered as "newborns" at the second survey and examined for microfilariae. The age of each registered newborn was checked against the interval between surveys for the village. Four "newborns" were found whose age exceeded the interva'l between surveys by more than one year and these were excluded from further analyses*. There were a further ll0 children whose recorded age was suctr that they may have been born up to a year before the first survey. As ages are not recorded precisely in the field surveys (A Prost) it is possible that these children were truely born after the first surveys. Analyses are presented both inc'luding and excluding these l'10 children. * The 4 excluded were a 4 and a 49 year old female. village 139) was positive. 7 year old and 28 28 year old male year old male ( ID l43l 1"4, year oId, 0nIy the -5- l0 of the 4.l54 "newborn" children examined at the second surveys in the villages were found to be infected (Table 2(a)). It is noteworthy that 7 of these children are from villages in cluster I0, an area in which vector control has been relatively poor. Five of these children were from one village (no.l39). Two of the 5 were recorded as 4 years old whereas the interval since the previous survey was on'ly 3 years 3 months and so there is some doubt as to whether or not these were really "newborns". The numbers of children who would have been expected to be infected, if those seen at the second survey had had the same prevalence rates as children of the same age and sex at the first survey, are also shown in Table 2(a). These have been calculated separately for each village cluster. Overall about 107.5 of the newborn children would have been expected to be infected had there been no change in the prevalence rate whereas only l0 were found to be so. Results are shown in Table 2(b) excluding the II0 children whose recorded age at the second survey was inconsistent with them having been born after the first survey. Among the remaining 4044 children 8 were found to be infected against 98.5 expected. Individual details of the "newborn" children found to be infected at survey 2 are given in Table 3(a). ChiTd.ren born between survegs 7 and 2 examined at sur -6- 1494 of those registered aS newborns at the second survey were later examined in a third survey. Two of these children were found to be infected at the second survey but only for one of these 2 was evidence of infection found again at the third survey. Micro- filariae were detected at the third survey in a further two children who had not been found to be infected at the second survey. Details of these children are shown in Table 3(b). 0verall 123..l of the 1494 children born between the first two surveys would have been expdcted to be infected at the third survey' but only 3 infected children were found (Table 2(c))' (iii) ChiTdren born between-Egr 2 and 3 examined at s None of the l25I children born between the second and third surveys and examined for microfilariae at the third survey were found to be infected compared to an expected number of 36.5 (Tab'le 2(d)). 4.2 Changes in prevalence and intensity between surveys (i) vilTages with 3 or npre survegs The prevalences at each survey for the 66 villages with 3 or npre surveys are shown, separately for each cluster of villages, in Figures 2(a) to (h). (There were no villages with 3 or more surveys in clusters 7 and 9). Except in cluster 8, there was a larger decline -7* between surveys 2 and 3 than between surveys I and 2, this difference being rpst marked in clusters 1, ?,4 and 5. The average annual decreases in these two intervals are given in the figures and summarised on Figure 3 which shows an average comparison of the clusters. In this figure the average prevalences and dates of each of the three surveys are p'lotted for each cluster. (ii) Prevafence changes between the first and subse$uent surveys in viLTages with 2 or rpre survegs. , Changes in the prevalence of infection between the first and subsequent surveys in each village have been related to the'duration of control' between the surveys. The 'duration of control' has been defined as fol'lows. For villages surveyed lefore the start of control activities the change'in prevalence was related to the time between the date control activities started and the subsequent survey. For vil'lages first surveyed aftsr the start of control activities the change in prevalence was related to the time between the two surveys. The results are shown for each of the clusters in Figures a(a) to (i). Villages with three or four surveys contribute more than one point to this analysis. For example village 4 in cluster'l contributes 3 points showing the changes between surveys I and 2, I and 3rand I and 4 respectively. '(' -9- compared to the other results. The linked lines in Appendix 3(b) however show similar rates of decline in these v'i'llages as in the others. These villages were aII seen about two years before control started, which is an unusually iong interval. As trans- mission was thought to have already started to decrease in cluster 2 (Upper Volta II + III) due to climatic conditions and the effect of control to the l,lest in phase I, it might be considered more appropriate to relate the change in prevalence to the tinre since the first survey rather than the start of control in these villages. The resulting graph is shown in Figure 4(b.-2) and the'moved,points in this figure do seem to be more in accord with the other results. It should be stressed that such'ad hoc' changes to data on the basis of resu'lts should be made with extreme caution. For this neason, this adjustment to the duration of control has not been made in the analysis of age-sex specific changes (see beior). (iii) Prevaience changes bg age and sex between the first and subsequent survegs Tlrc changes in the age-sex specific prevalences between the first and subsequent surveys are related to the duration of control between the surveys in Figure 5. The calculations were done separately for each sex and for each of the 3 cluster groups representing differing control conditions. Each graph shows the baseline curve, that is the first - i0 - S,ir'leys af i;tr 0-2 3'*ars, 3-4 years , 5-6 years and 7+ i/e&rs duration of cu;rtrol. They i,ave been adj us i,ed for c'i:;rpariscn ui ih the overall baseline level using. the specific baseline values in their respective vil'tages by calculating "adjusted" age-sex prevalences, for all villages resurveyed in a particular time interval (eg 5-6' years duration of control), as below ; "adjusted" prevalence at resurvey = prevalence at resurvey initial prevalence in these villages x, i ni ti aI preval ence i n a'l I v'i I l ages It can be seen that there is a fairly clear trend of decrease in prevalence in all age-sex groups with increasing duration of control nob only in the areas of good contro'l but also in those of reinvasion and poor control, an exception being among adult males in the last group. These comparisons have been repeated for the areas of good control subdividing vi1'lages according to their initial endemicity. The results for the hyper- and meso-endemic vi'l'lages are shown in Figures 6(a) and (b). The numbers of persons in the hypo-endemic villages were too small to allow this comparison. Intensity has been measured by the geometric nean microfi'larial count among those infected. The age-sex intensities at the different durations of control have been adiusted in a similar way to the prevalences to allow for the fact that they each are based on different villages by calculating adjusted intensity at resurvey = intensity at resurvey x initial intensity in all vil16ges initial intensitY in these villages The picture is less clear thalr that for preva'lence. In the areas of good contro'I, there is a tendency among those aged more than about 25 years for a decrease in intensity with increasing duration of control, but this is not so apparent among younger persons. This might be expected hovever, dS, in the pre-control situation, persons acquiring new infections, which would tend to be relatively light, formed a significant proportion of the younger age-groups. Thus as the inunediate effect of control would be to eliminate new infections, the average leve'l of infection arnong young positives would tend to be increased, with no light nelJ infections to di'lute it. As noted above the comparisons at different durations of control are each based on different viliages and adjustments have beenmadeto al'low for this. A clearer picture is obtained if the comparisons are restricted to the sane vi'l1ages. In Appendix 4 the results are shown at each survey for villages with 3 surveys. It should be noted that the intervals between the surveys are not the sane for all the villages and that c'lusters have been combined. Nevertheless there is a clear reduction in preva'lence rate from survey to survey over al1 age groups and in both sexes. Among those aged less than about 20 there is a tendency for the intensity to have increased with each survey.
