TUBERCULOSIS Algeria - Changes in tuberculosis morbidity may be studied at ihe national level through 2 sources o f information. Firstly, the annual notifications o f tuberculosis cases collected regularly since 1975 by the medical districts of 8 witayas (depart ments) of the country are available. These results are set out m Table 1. which shows the change in the annual incidence of cases of pulmonary tuberculosis with positive smears per 100 000 inhab itants. The annual incidence declined from 78 cases of bactenologically positive pulmonary tuberculosis per 100 000 population in 1975 to 60 cases in 1981.

Algérie. - L’évolution de la morbidité tuberculeuse peut être étudiée au niveau national grâce à 2 sources d'information. En premier heu. les déclarations annuelles des cas de tuberculose faites régulièrement depuis 1975 par les secteurs sanitaires de 8 wtlayas (dépar tements) du pays sont disponibles. Ces résultats sont rapportés dans le Tableau 7. qui montre l'évolution de l’incidence annuelle des cas de tuberculose pulmonaire à frottis positif pour 100 000 habitants. L’inci dence annuelle a diminué, passant de 78 cas de tuberculose pulmonaire à bactériologie positive pour I0Û00Û habitants en 1975 à 60 cas en 1981.

Secondly, very valuable information is provided by the epi demiological data collected nationally since 1982 by the Ministry of Health, these notifications cover 95.7% o f the entire population of Algeria. Table 2 shows that the combined annual incidence of all forms of tuberculosis was 72 per 100 000 population in 1982 and 69 per 100 000 population in 1983. The annual incidence of cases of pulmonary tuberculosis with positive smears was 35 per 100 000 in 1982 and 33 per 100000 in 1983.
En second lieu, les données épidémiologiques recueillies depuis 1982 au niveau national par les structures du Ministère de la Santé apportent des renseignements précieux; 95,7% de la population totale de l’Algérie sont concernés par ces déclarations. Le Tableau 2 montre que l’incidence annuelle des cas de tuberculose, toutes formes confondues, était de 72 pour 100 000 habitants en 1982 et de 69 pour 100 000 habitants en 1983 L’incidence annuelle des cas de tuberculose pulmonaire à frottis positif ètaitde35pour 100 000 habitants en 1982 et de 33 pour 100 000 habitants en 1983.

Analysis by age and sex of bactenologically positive cases of pulmonary tuberculosis notified in 1982 and 1983 shows that at the national level men are affected more often than women. The disease affects the adult male of all ages, but above all the young adult, between 20 and 29 years ( 100 cases per 100 000 population) and the elderly male over 60 years (104 cases per 100 000), as compared with an annual incidence of cases with positive smears of 41 per 100 000 in the male general population. However, pulmonary tuberculosis is not very often diagnosed before the age of 15, and bacteriological proof is given in only about half of the cases (249 of 489 cases notified in 1983). This constitutes an annual incidence o f cases ofpulmonary tuberculosis with positive smears o f 3 cases per 100 000 for young people under 15 years.
L’analyse en fonction de l’âge et du sexe des cas de tuberculose pul monaire à bactériologie positive déclarés en 1982 et en 1983 à l’échelle nationale montre que la tuberculose atteint plus souvent l’homme que la femme. Elle atteint l’homme adulte à tous âges, mais surtout l’adulte jeune, âgé de 20 à 29 ans ( 100 cas pour 100 000 habitants) et l’homme âgé de plus de 60 ans (104 cas pour 100 000 habitants), alors que l’incidence annuelle des cas à frottis positif est de 41 pour 100 000 habitants dans la population masculine générale. Cependant, la tuberculose pulmonaire est peu souvent diagnostiquée avant l’âge de 15 ans, la preuve bactériologique n’étant apportée que dans la moitié des cas environ (249 cas déclarés sur 489 en 1983). Ceci repré sente une incidence annuelle des cas de tuberculose pulmonaire à frottis positif de 3 cas sur 100 000 habitants pour les jeunes de moins de 15 ans.
Annual risk of tuberculosis infection Many tuberculin surveys have been carried out in Algeria during 3 different epochs over a period of more than 30 years. However, these surveys dealt with the child population in differents parts of the country and were carried out by different technical teams. Consequently, they are not comparable and cannot be used for continuous assessment of the trend of the tuberculosis infection risk for the whole of the period 1938-1963, nor to make projec tions. Nevertheless, each survey (or group of surveys) does provide valuable information on the nsk of infection and on the trend for a limited period (Table 3). Sun-evs in the period 1938-1948. The annual nsk of tuberculosis infection was 5.01% m 1938 and 4.09% in 1948, and fell by 2% annually during that decade (Table 3) ; this reduction was greater in the rural environment (—3.79%) than in the urban environment (-0.42%). The results of a survey carried out in 1969 in SidiBel-Abbès and in greater Oran show that the reduction in the risk of tuberculosis infection was greater between 1951 and 1969 than between 1938 and 1948.
Risque annuel d’infection tuberculeuse De nombreuses enquêtes tuberculiniques ont été réalisées en Algérie depuis plus d’une trentaine d’années à 3 époques différentes. Elles ont cependant concerné des populations d’enfants de differentes régions du pays et ont été réalisées par des équipes techniques differentes. Elles ne sont donc pas comparables entre elles et ne permettent pas de faire une estimation continue de la tendance evolutive du risque d’infecuon tuber culeuse pour toute la pénode 1938-1963, ni d ’en faire des projections. Cependant chaque enquête (ou groupe d ’enquêtes) apporte des informa tions essentielles sur le risque d’infection et sur sa tendance pour une période limitée (Tableau 3). Enquêtes de la période 1938-1948. Le risque annuel d’infection tuber culeuse était de 5,01% en 1938 et de 4,09% en 1948etiladim inuéde2% annuellement pendant cette décennie (Tableau 3), cette diminution étant plus importante en milieu rural ( - 3,79%) qu’en milieu urbain (-0,42%). Les résultats d’une enquête réalisée en 1969 à Sidi-Bel-Abbès et dans l’agglomération oranaise montrent que la diminution du risque d’infec uon tuberculeuse a été plus importante entre 1951 et 1969 qu’entre 1938 et 1948.

The 1966-1967 tuberculin surveys. The numerous surveys undertaken in Algeria in the years 1966 and 1967 have allowed an assessment of the mean annual risk of tuberculosis infection : it ranged, depending on the area, from 2.5% to 4% in 1966. The Bhda survevs and the permanent national surveys, 19761983 The most recent tuberculin surveys have been carried out since 1976. These surveys were conducted by the same team of experienced technicians and the WHO standard tuberculin test was the method used in all of them. The 2 Blida surveys, earned out 5 years apart in 1976 and 1981 among children aged between 6 and 10 years, gave the following results : the annual risk of tuberculosis infection was 1.42% in 1976 and 0.75% in 1981. The annual decline of this risk may be esti mated at -12.4% in this area over the last 10 years. The permanent national tuberculin survey, which has been under way since 1980, has provided for the testing every year of about 4 000 children 8 years of age representing the population living in regions with different socioeconomic levels. Table 3 shows the annual mean risk o f infection of these areas, which ranges from 0.27% at Bejaïa in 1982 to 0.68% in the Guelma area. This survey is still under way. An investigation m the same areas after a 5-year interval will allow the evolution of the nsk of infec tion to be studied. Comments The risk of infection and the tuberculosis morbidity show that the problem of tuberculosis is clearly on the decline in Algeria. The tuberculin survey of the Bhda wilaya, carried out with a 5-year interval revealed that the risk had fallen by 12.4% per annum between 1976 and 1981. At the same time the annual incidence per 100 000 population o f cases of pulmonary tuberculosis with posi tive smears in 8 mlayas, includmg Blida, fell from 78 cases in 197 5 to 60 in 1981. which constitutes a reduction of roughly 25% in 6 years. If this favourable situation is confirmed for the country as a whole and maintained over the next few years, the annual risk of infection will be 0.1% in the year 2000. It will then be possible to consider that the great struggle against the scourge o f tuberculosis is on the way to being won.
Les enquêtes tuberculiniques 1966-1967, Les nombreuses enquêtes entreprises en Algérie lors des années 1966 et 1967 ont permis d’estimer le nsque annuel moyen d’infection tuberculeuse: il variait de 2,5% à 4% selon les régions en 1966. Les enquêtes de Blida et l'enquête nationale permanente 1976-1983. Les enquêtes tuberculiniques les plus récentes ont été réalisées depuis 1976. La même équipe de techniciens expérimentés a réalisé ces enquêtes qui ont toutes utilisé la méthode du test tuberculinique standard de l’OMS. Les 2 enquêtes de Blida, réalisées à 5 ans d’intervalle en 1976 et 1981 parmi des enfants âgés de 6 à 10 ans, ont donné les résultats suivants: le risque annuel d’infection tuberculeuse était de l,42%en 1976 et de 0,75% en 1981. La diminution annuelle de ce nsque peut être estiméeà -12,4% dans cette région au cours de ces 10 dernières années. L'enquête tuberculinique nationale permanente entreprise depuis 1980 a permis de tester chaque année environ 4 000 enfants âgés de 8 ans, représentant des populations vivant dans les régions à niveau socio économique différent. Le risque annuel moyen d’infection de ces régions est représenté au Tableau 3 .11 varie de 0,27% à Béjaïa en 1982 à 0,68% dans la région de Guelma. Cette enquête se poursuit Le passage dans les mêmes régions à 5 ans d’intervalle permettra d’étudier la tendance évolutive du risque d’infection. Commentaires Le risque d’infection et la morbidité tuberculeuse montrent une nette tendance à la diminution du problème de la tuberculose en Algérie. L’enquête tuberculinique de la wilaya de Blida, réalisée à 5 ans d’inter valle, a permis de constater que ce risque a diminué de 12,4% par an de 1976 à 1981. En même temps, l’incidence annuelle pour 100 000 habi tants des cas de tuberculose pulmonaire à frottis positif dans 8 mlayas. dont Blida, est passée de 78 cas en 1975à60casen 1981, ce qui représente une diminution de 25% environ en 6 années. Si cette situation favorable se confirme pour tout le pays et se maintient lors des prochaines années, le risque annuel d'infection sera égal à 0,1% en l’an 2000. Il sera alors possible de considérer que la grande lutte engagée contre le fléau tuberculeux est en voie d’être gagnée.
