DIPHTHERIA 1977-1987 C anada . - Since national tabulation of diphtheria began in 1924, there has been a remarkable decline in both morbidity and mortality. An earlier report described the downward trend of the disease for the period 1924 to 1976.1 T he data presented here indicate that this trend has continued (Fig. I). A very marked decline is observed from 1980 onwards. This is primarily due to a change in case definition that was introduced in the autumn of that year to exclude carriers from the case count in all provinces. Moreover, laboratory evidence suggests thaï the number of per­ sons harbouring Corynebacterium diphtheriae, with or without symptoms, is actually decreasing. In fact, since 1983 the inter­ medium biotype appears to have disappeared in Western Canada. In a highly immunized population such as Canada’s, this may be due to the fa a that vaccine-induced immunity, while it does not prevent carriage of the organism, almost always prevents mem­ brane formation and coughing which decreases transmission of * Sec No. 42, 1978, pp. 301-302.

DIPHTÉRIE 1977-1987 C anada . - Depuis 1924, date à laquelle la mise en tableaux des données nationales relatives à la diphtérie a commencé au Canada, on a observé une baisse considérable de la morbidité et de la mortalité. Un précédent rapport avait fait état d’une tendance à la baisse pour la période allant de 1924 à 1976.1 Selon les données présentées ici, cette tendance s’est poursuivie (Fig. 1). La diminution très marquée observée depuis 1980 s’explique surtout par une modification apportée en automne 1980 à la définition du cas, qui exclut les porteurs dans toutes les provinces. En outre, les données de laboratoire indiquent qu’il y a de moins en moins de porteurs de Corynebacterium diphtheriae, symptomauques ou non. De fait, le biotype intermédiaire semble avoir disparu de l’Ouest canadien depuis 1983. Dans une population fonement immu­ nisée comme celle du Canada, cette observation peut s'expliquer par le fait que l’immunité vaccinale, si elle n ’empêche pas d’être porteur du micro-organisme, empêche presque toujours la formation de membra­ nes et la toux, réduisant ainsi la transmission. the organism. Elimination of cutaneous diphtheria notification from 1980 onwards in the United States, is the main reason for a similar marked decline in reports of diphtheria. Demographic descriptions of cases and deaths During the period 1977-1987 no cases were reported by the 4 Maritime provinces. A large proportion (56%) of the total cases were reported by Alberta. While the majority of the total cases (63%) were less than 15 years of age, persons over 30 accounted for 20% of all cases (Table 1). The proportion of cases under age 15 has remained unchanged (2/3) since 1976 and is higher than that found in the United States where only half of the reported cases are under that age. T he majority of cases occur in autumn and winter.

La plupart des cas se manifestent en automne et en hiver.

Diphtheria infection in Canada is mainly associated with poor socioeconomic conditions. T he endemicity in North American Indians and Inuit, and indigent adults is related to this factor. However, diphtheria is not restricted to these 2 groups and several episodes have been reported in city schools. Coinciding with the decline in reported cases, the number of deaths has also decreased. Five were reported during this surveil­ lance period: 4 from British Columbia and 1 from Saskatchewan. All occurred between 1977 and 1980. Four of the 5 were males; ages ranged from 3 to 92 years. Two of 3 adult cases had under­ lying malignancy and 1 was a North American Indian. The casefatality rate for the period 1977-1987 was 1%, which is lower than that reported for the previous period 1970-1977. Twenty per cent of the deaths occurred in individuals less than 10 years of age, which is similar to the proportion reported for the previous period. The case-fatality rate rose with age, reaching 6% for those over 30. Carriage, transmission and immunisation While it is believed that the number of persons carrying C. diphtheriae is declining, it is also true that very few laboratories still routinely look for the organism. This may reduce the chances of identifying both cases and carriers. Despite this, 3 investiga­ tions carried out during the past 7 years have reported relatively high carriage rates. The first one in the autumn of 1981 involved the contacts of 2 cases and 1 carrier from a family in Northern Ontario. Six children, all in frequent contact with each other and all fully immunized, were identified as carriers of the same tox­ igenic C. diphtheriae var gravis strain. During the second investigauon, 4% of the asymptomatic contacts of a case in a Toronto school in 198S were positive for the organism (2.5% carried tox­ igenic strains) despite high immunization coverage. In 1987, during the investigation of an outbreak of scarlet fever on an Indian reservation in Alberta, non-toxigenic C. diphtheriae was frequently isolated from throat swabs. These 3 investigations illustrate once more the continuing presence of C. diphtheriae even in well immunized populations. Unimmunized or inad­ equately immunized individuals, therefore, continue to be at risk of exposure and subsequent disease and, consequently, it is essen­ tial to maintain high levels of immunity. Adsorbed diphtheria toxoid vaccines were introduced in 1980 in British Columbia, Alberta, Saskatchewan, Manitoba and New Brunswick; in 1984 in Ontario, Nova Scotia, Prince Edward Island and Newfoundland; and in 1985 in Quebec. These vac­ cines have the following 2 advantages: (1) a formulation which allows safe and effective routine immunization of adults without Schick testing ; and (2) increased antigenicity which has extended the recommended period of time between booster doses from 5 to 10 years. Both advantages have contributed to improved protec­ tion of the Canadian adult population against diphtheria. When the timing of the introduction of the adsorbed vaccines is con­ sidered with respect to the sharp decrease that occurred in the incidence of the disease prior to 1981, it is unlikely that these vaccines contributed to this decline. However, they probably have contributed to the maintenance of low rates since that time.

Au Canada, l'infection diphtérique est principalement associée à des conditions socio-économiques médiocres. Chez les Inuit et les Indiens d’Amérique du Nord, ainsi que chez les adultes indigents, l'endémicité est liée à ce facteur. La diphtérie ne se limite toutefois pas à ces 2 groupes et plusieurs épisodes ont été enregistrés dans des écoles en milieu urbain. En même temps que la baisse du nombre de cas notifiés, on a observé une diminution des décès. Au cours de la période considérée, 5 décès ont été signalés: 4 par la Colombie britannique et 1 par le Saskatchewan. Tous ont été recensés entre 1977 et 1980; les sujets étaient âgés de 3 à 92 ans et 4 d’entre eux étaient de sexe masculin. Deux des 3 adultes en cause présentaient une affection maligne sous-jacente, et 1 était amé­ rindien. De 1977 à 1987, le taux de létalité a été de 1%, soit plus faible qu’au cours de la période précédente (1970-1977). Vingt pour cent des décès sont survenus chez des moins de 10 ans, chiffre qui se rapproche du pourcentage signalé pour la période précédente. Le taux de létalité a augmenté avec l’âge, atteignant 6% chez les plus de 30 ans.

Portage, transmission et vaccination Si le nombre de porteurs de C. diphtheriae semble en diminution, il est vrai aussi que très peu de laboratoires pratiquent encore la recherche systématique du micro-organisme, ce qui peut réduire les chances d’identifier tant les cas que les porteurs. Malgré cela, 3 enquêtes effec­ tuées au cours des 7 dernières années ont signalé des taux de portage relativement élevés. La première, en automne 1981, portait sur les contacts de 2 cas et d’un porteur appartenant à une famille du nord de l’Ontario. Elle a permis d’identifier comme porteurs de la même souche toxigène de C. diphtheriae type gravis 6 enfants qui avaient tous de fréquents contacts les uns avec les autres et avaient tous reçu la série vaccinale complète. La deuxième enquête a révélé, malgré une couver­ ture vaccinale élevée, la positivité à l’égard du micro-organisme de 4% des contacts asymptomatiques d’un cas enregistré en 1985 dans une école de Toronto (2,5% étaient porteurs de souches toxigènes). En 1987, dans le cadre d’une enquête sur une poussée de scarlatine survenue dans une réserve indienne de l’Alberta, C. diphtheriae non toxigène a été isolé fréquemment à partir d’écouvillonnages de gorge. Ces 3 enquêtes met­ tent encore une fois en évidence la présence de C. diphtheriae même chez des populations correctement vaccinées. Pour les sujets non immu­ nisés ou mal immunisés le risque d’exposition et de maladie persiste donc ; c’est pourquoi il est essentiel de maintenir l’immunité à un niveau élevé. Les vaccins à base d’anatoxine diphtérique adsorbée ont été introduits en 1980 en Colombie britannique, en Alberta, en Saskatchewan, au Manitoba et au Nouveau Brunswick; en 1984 en Ontario, en NouvelleEcosse, dans rîle du Prince-Edward et à Terre-Neuve; et en 1985 au Québec. Ils ont 2 avantages: 1) la formulation permet de vacciner sys­ tématiquement les adultes, de façon sûre et efficace, sans réaction de Schick, et 2) l’antigénicité accrue a permis de faire passer de 5 à 10 ans l’écart recommandé entre les rappels. Ces 2 avantages ont contribué à améliorer la protection de la population canadienne adulte contre la diphtérie. Si l’on considère le moment où les vaccins adsorbés ont été introduits et la baisse sensible de l’incidence de la maladie observée avant 1981, on s’aperçoit qu’il est peu probable que ces vaccins aient contribué à cette diminution. Cependant ils sont probablement pour quelque chose dans le maintien des faibles taux d’incidence depuis cette date.
