Expanded Programme on Immunization (EPI) Update: Mass vaccination with oral poliovirus vaccine, Asia and Europe, 1996 In 1995, a group of 18 geographically contiguous countries/areas in Europe, Central and South Asia, and the Middle East cooperated in conducting coordinated National Immunization Days (NIDs) with oral poliovirus vaccine (OPV).1 WHO has designated this effort Operation MECACAR (MEditerranean, CAucasus, and CAR for Central Asian Republics). Operation MECACAR was repeated in 1996 with the addition of the Russian Federation (Map 1). This report describes OPV coverage achieved during the mass vaccination campaign in 1996, and summarizes the impact of the NIDs on poliomyelitis incidence during 1995 and the first 6 months of 1996. To maximize the geographical area covered and the number of children targeted simultaneously for mass vaccination campaigns with OPV, adjoining countries/areas in Europe (Armenia, Azerbaijan, Bulgaria [1995 only], Georgia, Russian Federation [starting in 1996], and Turkey), Central Asia (Kazakstan, Kyrgyzstan, Tajikistan, Turkmenistan and Uzbekistan), South Asia (Afghanistan, Iran [Islamic Republic of], and Pakistan), and the Middle East (Iraq, Jordan, Lebanon, Syrian Arab Republic, United Nations Relief and Works Agency for Palestine Refugees in the Near East [UNRWA]) carried out synchronized NIDs during 1995 and 1996, and are planning to conduct NIDs in 1997. Based on the desirability of scheduling mass vaccination campaigns during the low poliomyelitis incidence season, either the first round or the second round of NIDs was scheduled between March and May. The only exception was Pakistan, which carried out NIDs in December 1995 and January 1996 to synchronize efforts with India2 and China.3 1 2 3

Le présent rapport traite de la couverture obtenue pendant cette campagne massive dadministration du VPO en 1996 et présente succinctement limpact des JNV sur lincidence de la poliomyélite en 1995 et pendant les 6 premiers mois de 1996. Pour accroître au maximum la couverture géographique et le nombre denfants visés simultanément par des campagnes de vaccination de masse avec le VPO, les pays/territoires contigus dEurope (Arménie, Azerbaïdjan, Bulgarie [1995 seulement], Fédération de Russie [à partir de 1996], Géorgie et Turquie), dAsie centrale (Kazakstan, Kirghizistan, Ouzbékistan, Tadjikistan et Turkménistan), dAsie du Sud (Afghanistan, Iran [République islamique d] et Pakistan) et du Moyen-Orient (Iraq, Jordanie, Liban, République arabe syrienne, Office de Secours et de Travaux des Nations Unies pour les Réfugiés de Palestine dans le ProcheOrient [UNRWA]) ont organisé des JNV synchronisées en 1995 et 1996 et envisagent de les répéter en 1997. A total of 62 million children were targeted to receive 2 doses each of OPV: 16 million children aged under 4 years in the WHO European Region (EURO) and 46 million children aged under 5 years in the WHO Region for the Eastern Mediterranean (EMRO). Reported coverage was generally very high and varied from 92% to 99% for each round in EURO and from 84% to 100% in EMRO (Table 1). The mass vaccination efforts have led to considerable declines among participating countries in the incidence of poliomyelitis reported to EURO and EMRO. In EURO, a total of 50 cases of poliomyelitis were reported in 1995 compared with 201 cases in 1994. For the first 6 months of 1996, only 13 cases were reported from participating countries in EURO (all from the Russian Federation and Turkey). In EMRO, a total of 647 cases of poliomyelitis were reported from participating countries/areas in 1995 compared with 691 cases in 1994. For the first 6 months of 1996, 60 cases of poliomyelitis were reported from these countries/areas (Table 1). Editorial Note: More than 8 years have elapsed since the World Health Assembly (WHA) adopted the goal of global poliomyelitis eradication by the year 2000.1 Considerable progress has been achieved since the WHA resolution, and the goal of a world free of poliovirus is rapidly becoming a reality in many parts of the world. The Americas were already certified as free of wild poliovirus by an International Certification Commission in 1994,2 while substantial progress has been achieved in the rest of the world.3 The progress is a direct result of implementing the 4 recommended strategies for poliomyelitis eradication: (1) achieving and maintaining high routine vaccination coverage with OPV; (2) instituting sensitive surveillance systems for poliomyelitis; (3) introducing supplemental immunization activities, including NIDs; and (4) conducting other supplemental vaccination activities (e.g., mopping-up operations). In 1995, approximately 300 million children aged under 5 years (or half of the worlds children in this age group) received OPV administered during NIDs.3 Similarly, by the end of 1996, all poliomyelitisendemic countries in Europe and Asia will have conducted NIDs, while 29 countries in the WHO African Region will have conducted NIDs (n=26) or Sub-National Immunization Days (SNIDs) (n=3). The collaboration among the 18 countries/areas for poliomyelitis eradication is remarkable since it has brought together countries with very different political systems, economic organization, racial and ethnic groups, and religions. In 1996, the Russian Federation joined the coalition of countries comprising Operation MECACAR as a result of a large outbreak of 154 cases of poliomyelitis in Chechenia in 1995, and suboptimal routine immunization coverage in at least some oblasts. Bulgaria, after conducting very successful NIDs in 1995, accomplished the objective of raising coverage among travelling people and did not participate in Operation MECACAR in 1996. Albania

conducted NIDs during April and May 1996. In the autumn of 1996, the Republic of Moldova and Ukraine will conduct NIDs, while Romania and the former Yugoslavia will conduct SNIDs. The primary purpose of mass vaccination campaigns with OPV is to interrupt the transmission of wild poliovirus. These mass vaccination efforts have interrupted wild poliovirus transmission in many areas of Europe, as reflected by the rapid decrease in cases in EURO following the efforts conducted in 1995 and 1996; of the 50 countries in EURO, only 4 (Albania,1 the Russian Federation, Turkey, and Ukraine) reported poliomyelitis cases in 1996. Despite some considerable successes in EMRO (Iran [Islamic Republic of], Jordan, Lebanon, and UNRWA), other countries (Afghanistan, Iraq, Pakistan, and the Syrian Arab Republic) reported more limited progress in 1995. For example, in 1995, Pakistan reported 64% of the regional total number of poliomyelitis cases in EMRO, and in 1995, the Syrian Arab Republic detected 4 cases associated with isolation of wild poliovirus. However, preliminary data for January-June 1996 suggest a substantial decline in the number of reported cases in both EURO and EMRO countries/areas ( Table 1). The technical basis for achieving worldwide poliomyelitis eradication already exists; however, insufficient political will, inadequate funding and other barriers will need to be addressed to ensure that the eradication objective will be achieved by the year 2000. 1

Influenza France (25 October 1996). Six members of a family in Lyons presented with influenza-like illness in mid-October shortly after a visit to Réunion. The first case was the father who had onset of illness on 13 October, 24 hours after their return. Specimens from 2 adolescents in the family taken on 14 October were positive for influenza A. Japan (25 October 1996). The first influenza virus isolated this season was an influenza A(H3N2) virus. It was isolated from a sporadic case in the northern part of the country in the first week of October. New Zealand (25 October 1996).1 The epidemic was over by August and in September only 2 cases of influenza A(H3N2) were confirmed by isolation of the virus. United States of America (24 October 1996). Influenza A and B viruses continued to be isolated throughout the summer and sporadic activity continued in September and October. All influenza A viruses further characterized were of H3N2 subtype. Some were associated with outbreaks in nursing homes in June and July. 1
Grippe France (25 octobre 1996). Six membres dune famille de Lyon ont présenté un syndrome grippal à la mi-octobre, peu après un voyage à la Réunion. Le premier patient a été le père, tombé malade le 13 octobre, 24 heures après leur retour. Des échantillons prélevés sur 2 adolescents de la famille le 14 octobre étaient positifs pour le virus grippal A. Japon (25 octobre 1996). Le premier virus grippal isolé cette saison a été un virus A(H3N2), provenant dun cas sporadique dans le nord du pays au cours de la première semaine doctobre.
Nouvelle-Zélande (25 octobre 1996).1 Lépidémie était terminée en août, et en septembre seuls 2 cas de grippe A(H3N2) ont été confirmés par isolement du virus. Etats-Unis dAmérique (24 octobre 1996). Des virus grippaux A et B ont continué à être isolés pendant tout lété, et une activité sporadique sest poursuivie en septembre et octobre. Tous les virus grippaux A étudiés de manière plus approfondie étaient du soustype H3N2. Quelques-uns étaient associés à des flambées survenues dans des maisons de retraite en juin et en juillet. 1

Dengue and dengue haemorrhagic fever India. The National Institute of Communicable Diseases (NICD), Delhi reported 7 427 cases of dengue and dengue haemorrhagic fever, of which 297 were fatal, as at 29 October 1996. These numbers were based on reports received from hospitals in Delhi. Over half of 783 serum samples tested had antibodies to dengue virus in MACELISA/haemagglutination inhibition tests. Dengue virus serotype 2 has been isolated by the All-India Institute of Medical Sciences, New Delhi and the National Institute of Virology, Pune.

Electronic publication of the Weekly Epidemiological Record As of January 1996, the Weekly Epidemiological Record (WER) has been available free of charge in electronic format on the Internet. To access the electronic edition users must have Internet access and software that retrieves files by file transfer protocol (FTP) or provides access to the World Wide Web (WWW). Issues of the WER are available in Adobe Acrobat portable document format (.pdf). To view the WER, the program Acrobat Reader 1 is required. Different versions of this program are available free of charge for most operating systems. Each .pdf file represents a single issue of the WER and is named according to the volume and issue number. For example, the file wer7120.pdf contains the WER volume 71, number 20.
Publication électronique du Relevé épidémiologique hebdomadaire Depuis janvier 1996, la version électronique du Relevé épidémiologique hebdomadaire (REH) est disponible gratuitement sur Internet. Pour accéder à cette version électronique du REH, il suffit de disposer dun accès au réseau Internet permettant un transfert de fichiers via le protocole FTP ou un accès au World Wide Web (WWW). Chaque numéro du REH est disponible au format .pdf (portable document format) de Adobe Acrobat. Pour accéder au REH, il faudra disposer du programme Acrobat Reader.1 Ce programme, distribué gratuitement, est disponible pour la plupart des systèmes dexploitation. Chaque fichier .pdf correspond à un numéro complet du REH et est nommé en conséquence. Ainsi, le fichier contenant le numéro 20, volume 71, du REH sera nommé wer7120.pdf.
Where to obtain the WER through Internet (1) WHO WWW SERVER: Use WWW navigation software to connect to the WER pages at the following address: http://www.who.ch/wer/wer_home.htm. (2) WHO FTP SERVER: Use FTP to connect to WHOs file server ftp.who.ch. At the user name prompt enter anonymous, and in response to the prompt for password users should enter their E-Mail address. Select the directory pub, then subdirectory wer. From the listing, files of interest can be downloaded. (3) E-MAIL LIST : An automatic service is available for receiving a weekly notification of the contents of the WER and short epidemiological bulletins. To subscribe, send an E-Mail message to majordomo@who.ch . The subject field should be left blank and the body of the message should contain only the line subscribe wer-reh. Subscribers will be sent a copy of the table of contents of the WER automatically each week, together with other items of interest. 1
