EXPANDED PROGRAMME ON IMMUNIZATION Immunization of Sick Children I ndonesia . - A study has been earned out in the Department of Child Health Medical School, Snwijaya University, Palembang, on the immunization o f sick children with DPT and poliovirus vac cines. The study was conducted from April to July 1980 among children under 6 years o f age attending the Child and Mother Centre, Palem bang. In all 890 children were included in the study, 209 o f whom were considered healthy and 681 sick. Most of them (72-75%) were below 24 months of age. All these children, healthy and sick, were immu nized with DPT and/or oral poliovirus vaccine according to the immunization schedule (Table 1). Sick children were seeking curative help for mild upper and lower respiratory tract infections, gastroen teritis, skin, ear or eye infections (Table 2). They were given analgetic/antipyretic or other medications according to then- disease. Healthy children were given no medication before or after immuni zations.
PROGRAMME ÉLARGI DE VACCINATION Vaccination des enfants malades I ndonésie. - Une étude sur l’administration des vaccins DTC et antipohomyélitique à des enfants malades a été réalisée au Départe ment de Pédiatrie de l’Ecole de Médecine de l’Université Snwijaya, à Palembang. Cette étude a été conduite d’avnl à juillet 1980 chez des enfants de moins de 6 ans présentés au centre de santé maternelle et infantile de Palembang. Elle a porté au total sur 890 enfants dont 209 ont été jugés bien portants et 681 malades. La plupan (72-75%) étaient âgés de moins de 24 mois. Tous ces enfants, bien portants et malades, ont été vaccinés à l’aide des vaccins DTC et/ou antipoliomyélitique buccal conformément au calendrier habituel (Tableau /). Les enfants malades présentaient des infections bénignes des voies respiratoires supérieures et inférieures, des gastro-entérites, des infections cutanées ou des infections de l’oreille ou de l’oeil (Tableau 2). On leur a admi nistré des analgésiques/antipyrétiques ou d’autres médicaments en fonction de leur maladie. Les enfants bien portants n’ont reçu aucune médication, ni avant ni après les vaccinations.

Very ill children were excluded from the study. In addition, DPT vaccine was not administered to children with a history o f convul sions, and poliovirus vaccine was not administered to children with diarrhoea. The children were examined before immunization, and then on the second, third and fourth days after immunization. They were checked for local reactions and general reactions, including malaise, anorexia, fever, convulsions through light stimulation, coughing, difficult breathing and diarrhoea. If the symptoms mentioned were present previously, any increasçjn their status was recorded. In further analy sis only those children who came for checking at least once were included; there were 563 such children: 144 healthy and 419 sick children (69% and 62% respectively o f all those immunized). No serious adverse reactions, such as convulsions or other neurological symptoms were found among healthy or sick children immunized with DPT and/or poliovirus vaccines. The signs and symptoms in sick children appeared not to increase following im munizations. Fever was more often found among healthy than sick
Les enfants très malades ont été exclus de l’étude. En outre, on s’est abstenu d’administrer le vaccin DTC aux enfants présentant des anté cédents de convulsions et le vaccin antipoliomyélitique aux enfants atteints de diarrhée. Les enfants ont été examinés avant la vaccination, puis les deuxième, troisième et quatrième jours après la vaccination. On a cherché à dépister d’éventuelles réactions locales et générales telles que malaises, anorexie, fièvre, convulsions par stimulation lumi neuse, toux, difficultés respiratoires et diarrhée. Toute intensification des symptômes présents auparavant a été relevée. Pour la poursuite de l’étude, seuls ont été pris en considération les enfants qui se sont présentés au moins une fois pour un contrôle ; ces derniers ont été 563, soit 144 enfants bien portants et 419 enfants malades (respectivement 69 et 62% de tous les enfants vaccinés). Aucune réaction sérieuse, convulsions ou autres symptômes neu rologiques par exemple, n’a été observée chez les enfants bien portants ou malades auxquels avaient été administrés les vaccins DTC et/ou antipoliomyélitique. Les signes et symptômes relevés chez les enfants malades n’ont apparemment pas augmenté en intensité après les vac-

children (Table 3). It could however have been influenced by antipy retics administered to sick but not to healthy children. Local reactions, erythema and induration were similar in both groups, but pressure pain occurred significantly more often in healthy than in sick children. This could also have been influenced by the impact of analgetics/antipyreucs given to the sick children.

Les réactions fébriles ont été plus fréquentes chez les enfants bien portants que chez les enfants malades (Tableau 3) mais il se peut que ce résultat soit dû aux antipyrétiques qui étaient admi nistrés aux enfants malades et non aux enfants bien portants. Les réactions locales, érythème et induration, ont été du même ordre dans les deux groupes, mais les douleurs à la pression ont été sensiblement plus fréquentes chez les en&nts bien portants que chez les enfants malades. Là encore, ce résultat tient peut-être aux effets des analgésiques/antipyrétiques administrés aux enfants malades.
