CHOLERA SURVEILLANCE Appearance of Classical Biotype of Vibrio cholerae B a n g l a d e s h . - The classical biotype of V. cholerae gradually dis­ appeared from the South-East Asian region during the 1960s to early 1970s with the advent of the seventh cholera pandemic, which was caused mainly by the eltor biotype of V. cholerae 01. Though the last few cases of cholera due to the classical biotype were seen in Calcutta during 1968-1969, this biotype persisted in Dhaka up to August 1973 when the eltor biotype completely replaced the classical biotype. Since then no classical biotype was isolated from clinical cases or the envi­ ronment until October 1979, when 5 cases of cholera due to classical biotype were detected. more isolation ofclassicalbiotype in 1979 and 3 in 1980. Two classical strains were isolated in 1981 by the Institute o f Public Health, Dhaka from the Mymensingh district, 130 km from Dhaka. Each year all isolates of V. cholerae are routinely checked for chicken cell aggluti­ nation, sensitivity to polymixin B (50IU disc) and Mukherjee’s clas­ sical phage group IV and eltor phage group 5. From May 1980 to September 1982 over 3 500 isolates o f V. cholerae were checked; all belonged to the eltor biotype. In 1982 the first classical biotype of V. cholerae was isolated on 4 September. The patient gave no history of travel. Table 1 gives the weekly isolation of V cholerae at ICDDR.B from 4 September to 22 October 1982.

A representative sample o f these recent isolates was phage-typed with Mukherjee’s classical cholera phages and was found to belong to type 3. The classical strains isolated in the 1982 outbreaks were found to be indistinguishable from those isolated in 1969 and 1979 by the usual biochemical and other tests. The search for the classical biotype was continued in the V. cholerae 01 isolated from ICDDR, B hospitals in Matlab (Comilla) and Teknaf (Chittagong) as well as among isolates from other cholera affected districts of Bangladesh such as Mymensingh and Rangpur. As of 22 October 1982 none of the isolates were of classical biotype.1 Unlike the sporadic cases in previous years, in 1982 the classical biotype has appeared in some areas along with the eltor biotype. Bangladesh (ICDDR,B) a de nouveau isolé le biotype classique, une fois en 1979 et 3 fois en 1980. L’Institut de Santé publique de Dhaka a isolé 2 souches classiques a i 1981 dans des échantillons provenant du district de Mymensingh, à environ 130 km de Dhaka. Chaque année, tous les isolements de V, cholerae sont soumis en routine à diverses épreuves: agglutination des érythrocytes de poulet, sensibilité à la polymixine B (disque de 50 UI), et appartenance au lysotype IV du vibrion classique ou au lysotype 5 du vibrion eltor, suivant la classi­ fication de Mukheqee, Entre mai 1980 et septembre 1982, plus de 3 500 isolements de V. cholerae ont été testés; ils appartenaient tous au biotype eltor. En 1982, le biotype classiquede V. cholerae a été isolé pour la première fois le 4 septembre. Le patient n’a pas signalé avoir voyagé. On trouvera au Tableau l la ventilation des isolements heb­ domadaires de T cholerae à 1TCDDR.B, du 4 septembre au 22 octo­ bre 1982. Le typage d’un échantillon représentatif de ces isolements récents a été fait avec les phages classiques de Mukherjee et l’on a constaté que les souches appartenaient au lysotype 3. Des épreuves biochimiques de routine et autres n’ont pas permis de distinguer les souches clas­ siques isolées lors de la poussée épidémique de 1982 de celles isolées en 1969 et en 1979. On a continué à rechercher le biotype classique dans les isolements de V, cholerae 01 en provenance des hôpitaux de l’ICDDR,B de Matlab (Comilla) et de Teknaf (Chittagong), ainsi que dans les isole­ ments provenant d’autres districts du Bangladesh touchés par le cho­ léra, comme Mymensingh et Rangpur. Au 22 octobre 1982, aucun de ces isolements n’avait été décelé comme appartenant au biotype clas­ sique, 1 En 1982, contrairement aux années précédentes où il n’etait que sporadique, le biotype classique est apparu dans certaines zones con­ jointement avec le biotype eltor.
