The new edition was developed in collaboration with the US National Cancer Institute (NCI) and the WHO International Agency for Research on Cancer (IARC). The classification, which has been finalized following extensive field testing throughout the world, permits the coding of all neoplasms as malignant, benign, in situ, of uncertain behaviour, or metastatic. As in the previous edition, a separate code for histologic grading and differentiation has been included. The following features are new: -adoption of the ICD-10 classification for tumour topography with its alphanumeric coding system; -inclusion of several new histologic types in the morphology section; -complete remodelling of the section on non- Hodgkin's lymphoma; and -expansion of the sixth-digit code for histologic grading and differentiation in order to identify T- and B-cell involvement for lymphomas and leukaemias. ICD-O, which comes into force before ICD- 10, can be used for both topographic sites and histologic types of neoplasms by those who wish to code greater detail about a neoplasm than is provided for in the neoplasms chapter of ICD-10. Since the great majority of users are from tumour registries, this edition features specific instructions for tumour reg- istrars as well as for pathologists. These include intense concern about the environmental impact of chemicals, growing appreciation for the necessity of an integrated approach to control, and a correspond- ing move towards such non-chemical measures as environmental management and the use of biological agents. Noting the continuing need for pesticides, particularly in the emergency response to epidemics, the book places particular emphasis on the demand for equipment that is both suitable for use by com- munities and capable of delivering accurate and judi- cious quantities of pesticides. The book also addresses safety issues and dis- cusses the crucial roles of information dissemination, training, and research. ISBN 92 4 154289 6 Price: Sw.fr. 22.-/US$ 17.60 Formaldehyde, a flammable, colourless and readily polymerized gas present in the environment from both natural and man-made sources, is produced indus- trially in large quantities for a wide range of ap- plications. These include the production of glue or coatings for particle boards and plywood, the manu- facturing of sterilizing and disinfecting agents, medi- cines, cosmetics, and a number of consumer goods. Other major man-made sources are automobile and aircraft exhaust emissions, tobacco smoke, natural gas, fossil fuels, waste incineration, and oil refineries. The book makes a clear distinction between outdoor and occupational exposures, and exposures arising from the emission of formaldehyde into indoor air environ- ments. Particular attention is given to hospitals and scientific facilities, where formaldehyde is used as a sterilizing and preserving agent, and living spaces, such as schools, kindergartens, and mobile homes or apart- ments, where uncontrolled emissions of formaldehyde from tobacco smoking, building materials, and furni- ture may pose a particular health hazard. Concern about the toxicological effects of formal- dehyde is addressed in a review of data from experi- mental animals and in vitro test systems. The section on health effects in man draws upon clinical and epidemiological studies to evaluate sensory, toxic, respiratory, dermal, genotoxic effects and effects on reproduction. The evaluation cites evidence of a possible cancer risk for humans from exposure to formaldehyde, but concludes that the carcinogenic potential is not high, that only nasal or nasopharyn- geal tumours are likely to be causally related to formaldehyde exposure, and that formaldehyde is not teratogenic. Because of its strong repellent properties and effectiveness as a stomach and contact insecticide, permethrin is widely used in the protection of several agricultural crops, in the control of insects in house- holds and on cattle, in aerial application for forest pest control, as a fog in mushroom houses, and as a wood preservative. Public health applications include the disinsection of aircraft, treatment of mosquito nets, and human lice control. One section describes the large number of studies investigating residues in fruits, vegetables, dairy milk, and grains. Other sections review findings on the environmental behaviour of permethrin, its metabolic pathway,s in mammals, and its effects on non-target organisms. The most extensive section presents the findings of experimental studies conducted to assess toxicity, primary irritation, sensitization, long-term toxicity, carcinogenicity, mutagenicity, teratogenicity, neurotoxicity, and behavioural effects. On the basis of this review, the book concludes that most toxic effects are transitory, that the likelihood of carcino- genic effects in humans is extremely low or non- existent, and that permethrin, when used as recom- mended, is not likely to present a hazard to the general public, exposed workers, or the environment.b Fenvaleratea Geneva, 1990, 121 pages (Environmental Health Criteria, No. 95). The opening section of the book describes the properties of fenvalerate and identifies appropriate methods for environmental sampling and the analysis of residues. A brief discussion of sources of human exposure, concentrating on studies of dietary residues, concludes that residues in crops grown by good agricultural practice are generally low. Other sections outline metabolic pathways and summarize studies of effects on aquatic and terrestrial organisms, including investigations of uptake, loss, and bioaccumulation. Fol- lowing information on the properties and methods of sampling and analysis, the available data on world- wide industrial production and use are reviewed. One section on investigations of photodegradation and transport concludes that photodegradation under outdoor conditions is rapid, but that d-phenothrin remains virtually intact for up to 12 months on grains stored in the dark. The most extensive chapter evalu- ates toxicological data from studies in experimental animals and in in vitro test systems, concentrating on investigations performed to assess embryotoxicity, teratogenicity, and neurotoxicity. The conclusions con- cerning levels of exposure and the associated risks to health are as follows: the general public is determined to be at low risk of methylmercury poisoning, but populations consuming high quantities of fish may attain a blood methylmercury level associated with a low risk of neurological damage to adults. In view of the particular sensitivity of the fetus to methylmercury, exposure during pregnancy is judged to carry a high risk of neurological disorder in the offspring. By documenting a progression of refinements in methodology and data requirements, it also provides a view of both the strict criteria used in JMPR assessments and the responsiveness of these criteria to advances in scientific knowledge. The most extensive chapter provides a step-by-step guide through the battery of test methodologies currently used in the safety assessment of pesticide residues. Both classical experiments and very new tests in the fields of immunotoxicity and behavioural toxicity are critically reviewed in terms of their capacity to con- tribute to the safety assessment. Evaluations of carcinogenic risk concentrate on epidemiological evidence of carcinogenicity at the oral and pharyngeal, oesophagus and stomach, nasal cavity, larynx, lung and bladder sites. In view of the strength of the findings of bladder cancer among dyers and among weavers and of cancer of the nasal cavity among weavers and other textile workers, the mono- graph concludes that working in the textile manufac- turing industry entails exposures that are possibly carcinogenic to humans. In the first section of each Monograph a number of synonyms and trade names have been included for each chemical or complex mixture and thus, progressively it has become more and more difficult for the reader to determine whether a chemical or a complex mixture has been evaluated by an IARC working group. The first report on that studyC provided findings on a sample of respondents aged 60-90 years and covered their health and functional ability, living conditions, way of life and use of services. These findings were pre- liminary and purely descriptive, and much more information was generated by the study than could have been included in the first report. The present analysis permits a deeper insight into health and functioning in old age, into the living conditions of the elderly, and into the use of services by different groups. The data presented should con- tribute to a better understanding of the health and social problems of elderly people and their needs in the last decade of this century. The main part of the book presents and explains the CIOMS procedure for the international reporting of adverse drug reactions. The procedure, which was worked out and agreed upon by representatives of six regulatory authorities and seven manufacturers em- ploys a uniform set of definitions and procedures, a fixed time-frame for submission of the report, and a reporting form with standardized elements and for- mat. The second half of the book presents evaluations of the scheme prepared by each of the participating regulatory authorities and manufacturers. At present, the CIOMS scheme has been adopted for use in the United Kingdom, the USA, the Federal Republic of Germany, and Italy.
