WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies v foreword Public health emergencies are a significant risk to health and well- being across the WHO South-East Asia Region. Among other risks, the Region is vulnerable to emerging and re-emerging diseases, diseases associated with climate change, and natural disasters such as floods, cyclones and earthquakes. These and other humanitarian crises can have a significant short- and long-term impact on mental health, which can be mitigated through the provision of appropriate mental health and psychosocial support services. This Regional Collaborative Framework for a coordinated response to Mental Health and Psychosocial Support in emergencies will guide the scale up of appropriate services across the Region, which is especially needed in the context of the emergence and spread of COVID-19. The framework can be integrated with existing WHO recommendations on emergency preparedness, including the Emergency Response Framework and the Strategic Framework for Emergency Preparedness. The framework identifies six domains of action through which Member States can strengthen services for mental health and psychosocial support, and documents Member State progress in each area. I urge Member States and partners in the Region to leverage this framework to ensure that the adequate provision of mental health and psychosocial support services is included in emergency preparedness and response planning. Since 2014, scaling up emergency risk management has been a regional Flagship Priority, leading to sustained progress across the Region. Rather than developing a parallel and independent framework for MHPSS, a specific aim has been to ensure that the proposed framework integrates well with the existing recommendations by WHO on emergency preparedness. Two of these key documents are the “Emergency Response Framework (ERF)” and the “Strategic Framework for Emergency Preparedness”. The purpose of ERF is to clarify WHO’s roles and responsibilities in emergencies and to provide a common approach for its response. The ERF requires WHO to act with urgency and predictability to best serve and be accountable to populations affected by emergencies. The “Strategic Framework for Emergency Preparedness” is a unifying framework developed by WHO which identifies the principles and elements of effective country health emergency preparedness. It adopts the major lessons of previous initiatives and lays out the planning and implementation process by which countries can determine their priorities and develop or strengthen their operational capacities. These two documents offer the scaffolding on which specific recommendations for a coordinated response to MHPSS in emergencies have been developed. Besides these two publications from WHO, the other published literature on the relevant themes was reviewed and referred to during preparation of this document. The draft of the WHO SEA Regional Collaborative Framework for a coordinated response to MHPSS in emergencies was presented to the group of representatives from all 11 Members States of the SEA Region during the Regional Capacity Building Workshop on Mental Health in Emergencies held at Kathmandu, Nepal during 28–29 August 2018. The recommendations of the group contributed to the final version of the document. The WHO SEA Regional Collaborative Framework for a coordinated response to MHPSS in emergencies is covered under the following sections – operational definitions, guiding principles, operationalization, domain and resources. Also, the current state of readiness and preparedness of the Members States on the various domains of the regional framework has been presented as an Annex. WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies2 operatIonal defInItIons Emergency An event or threat or difficult condition that produces or has the potential to produce a range of consequences that require urgent, coordinated action. Emergency preparedness The knowledge, capacities and organizational systems developed by governments, response and recovery organizations, communities and individuals effectively to anticipate, respond to, and recover from the impacts of likely, imminent, emerging or current emergencies. Mental health and psychosocial support The term “mental health and psychosocial support (MHPSS)” describes any type of support that aims to promote mental health, prevent or treat mental disorder, and/or protect or promote psychosocial well-being. Risk The combination of the probability of an event and its consequences. Risk results from interactions between natural and human-induced hazards, vulnerability, exposure and capacities. Risk assessment The process of determining those risks to be prioritized for risk management by a combination of risk identification, risk analysis and evaluation of risk level. A risk assessment includes a review of the technical characteristics of hazards, analysis of exposures and vulnerability, and evaluation of the effectiveness of existing coping capacities. An assessment of protective factors such as individual and community strengths, coping capacities, resources and resilience that would allow for increased social functioning and inclusion and the capacity for psychosocial recovery. Risk management Coordinated activities to direct and control risk in order to minimize potential harm. These activities include risk assessments, risk reduction, risk communication, implementing risk treatment and response measures, and evaluation, monitoring and review. WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies 3 GuIdInG prIncIples The following are the guiding principles of the WHO SEA Regional Collaborative Framework for a coordinated response to MHPSS in emergencies. These principles are in keeping with the principles that underlie WHO’s “Strategic Framework for Emergency Preparedness”. These have been adapted and modified as required, along with inclusion of other principles that guide this collaborative framework. ¤ Safeguarding, maintaining and restoring the health and mental well-being and quality of community life. ¤ Communities are critical to a coordinated response to MHPSS in an emergency. ¤ Community preparedness can significantly decrease the impact of an emergency. ¤ Preparedness for MHPSS in emergencies requires sustained advocacy to ensure a robust policy and legal framework, political commitment, partnerships and funding. ¤ Participation and inclusion of other sectors in the planning and management process is required to promote and protect mental health and psychosocial well-being in a comprehensive and coordinated manner. ¤ Achieving preparedness for MHPSS in emergencies is an investment in health, safety, security and development. ¤ Health systems and preparedness for MHPSS in emergencies reinforce one another, and along with other systems contribute to the resilience of communities and countries. ¤ Preparedness for MHPSS in emergencies should be addressed with an all-hazards approach. ¤ Social support is essential to protect and support mental health and psychosocial well-being in emergencies ¤ An integrated support system and multi-layered support should be in place. ¤ A risk management approach underpins the assessment, planning and implementation of MHPSS in emergencies including prevention and mitigation of risks, preparedness activities, coordinated response, recovery and reconstruction. ¤ Every emergency has a unique political, physical and socioeconomic environment that will influence the response. ¤ Preparedness for MHPSS in emergencies – through community planning, training and mitigation – can significantly lessen the damage and loss. ¤ Cultural competence and family support are crucial to a coordinated response to MHPSS in emergencies. WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies4 ¤ Ensuring equality and diversity issues are addressed with a human-rights approach. ¤ Tackling stigma and discrimination and preventing harmful mental health practices such as generalizing post-traumatic stress disorder (PTSD) and psychological debriefing. ¤ Ensuring acceptance to cultural practices of the local communities. WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies 5 operatIonalIzatIon The following are the key actions for operationalization of the WHO SEA Regional Collaborative Framework for a coordinated response to MHPSS in emergencies: ¤ Make an assessment of risk and capacity for MHPSS in emergencies. ¤ Plan for a coordinated response to MHPSS in emergencies. ¤ Prepare for prevention and mitigation of threats or minimizing mental health and psychosocial consequences before an emergency occurs. ¤ Implement the plan for a coordinated response to MHPSS in emergencies. ¤ Establish a coordination mechanism among all stakeholders and partners. ¤ Ensure financing of all steps of preparedness planning for a coordinated response to MHPSS in emergencies. ¤ Collaborate with the existing coordination mechanism for delivery of MHPSS in response to an emergency. ¤ Evaluate and take corrective actions based on the experience after implementation in view of the changing needs and demands. ¤ Ensure that MHPSS services are provided within the existing health structure with multisectoral coordination during the response and recovery period. Emergency Response Framework (ERF) The purpose of the ERF is to clarify WHO’s roles and responsibilities for emergency response and to provide a common approach for its work during emergencies. Sendai Framework for Disaster Risk Reduction The Sendai Framework is a 15-year, voluntary, non-binding agreement which recognizes that the State has the primary role to reduce disaster risk but that responsibility should be shared with other stakeholders including the local government, the private sector and other stakeholders. It is the outcome of stakeholder consultations initiated in March 2012 and inter-governmental negotiations held from July 2014 to March 2015, which were supported by the United Nations Office for Disaster Risk Reduction (UNISDR) upon the request of the UN General Assembly. Build Back Better (sustainable mental health care after emergencies) This WHO publication shows how emergencies in 10 diverse areas were used as opportunities to build better mental health systems for all people in need. This is important because mental health is crucial to the overall well-being, functioning and resilience of individuals, societies and countries recovering from emergencies. WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies 11 Inter-Agency Standing Committee (IASC) guidelines for mental health and psychosocial support in emergency settings Initiated by WHO, the IASC Guidelines for mental health and psychosocial support in emergency settings reflect the insights of numerous agencies and practitioners worldwide and provide valuable information to organizations and individuals on how to respond appropriately during humanitarian emergencies. Specific action sheets offer useful guidance on MHPSS, and cover the following areas: coordination, assessment, monitoring and evaluation, protection and human rights standards, human resources, community mobilization and support, health services, education, dissemination of information, food security and nutrition, shelter and site planning, and water and sanitation. Psychological first aid: guide for field workers This manual was developed to have a set of psychological first-aid materials for use in low- and middle-income countries. This guide – endorsed by many international agencies – reflects the emerging science and international consensus on how to support people in the immediate aftermath of extremely stressful events. Problem Management Plus (PM+) This manual describes a scalable psychological intervention called Problem Management Plus (PM+) for adults impaired by distress in communities who are exposed to adversity. The intervention is aimed at offering individual psychological help for adults impaired by distress in communities exposed to adversity. mhGAP Humanitarian Intervention Guide (mhGAP-HIG) The mhGAP Humanitarian Intervention Guide (mhGAP-HIG) contains first-line management recommendations for mental, neurological and substance use conditions for non-specialist health-care providers in humanitarian emergencies where access to specialists and treatment options is limited. It is a simple, practical tool that aims to support general health facilities in areas affected by humanitarian emergencies in assessing and managing acute stress, grief, depression, PTSD, psychosis, epilepsy, intellectual disability, harmful substance use and risk of suicide. mhGAP Intervention Guide (mhGAP-IG) for mental, neurological and substance use disorders in non-specialized health settings The mhGAP Intervention Guide (mhGAP-IG) for mental, neurological and substance use disorders for non-specialist health settings is a technical tool developed by WHO to assist in the implementation of mhGAP. The mhGAP-IG describes integrated management of priority conditions using protocols for clinical decision-making. The priority conditions included are: depression, psychosis, bipolar disorders, epilepsy, developmental and behavioural disorders WHO SEA Regional Collaborative Framework for coordinated response to MHPSS in emergencies12 in children and adolescents, dementia, alcohol use disorders, drug use disorders, self-harm/ suicide and other significant emotional or medically unexplained complaints. These guidelines are developed by the WHO Regional Office for South-East Asia for use by mental health and psychosocial service providers in disaster settings. This document aims to add to disaster management preparedness. Addressing mental health of disaster management professionals This document has been prepared by the WHO Regional Office for South-East Asia to serve as a reference document on assessment of disaster management personnel for adverse mental health conditions and management of these adverse mental health conditions among these personnel. The document summarizes various adverse health consequences that these persons can experience. It lists various instruments and tools that can be used to identify and follow-up these issues. The document also describes various approaches and interventions to address psychological issues in disaster management personnel. Guidance on Identification and Management of Psychosocial Distress for Disaster Management Personnel This guide is developed by the WHO Regional Office for South-East Asia. This self-help guide directly addresses disaster management personnel and helps them understand various facets of psychological impact of disaster on them..
