‫تقرير مرحلي حول توسيع نطاق العمل في مجال رعاية الصحة النفسية‬ ‫مقدمة‬ ‫أصدرت اللجنة اإلقليمية ملنظمة الصحة العاملية لشرق املتوسط يف دورهتا الثانية والستني عام ‪ 2015‬القرار‬ ‫ش م‪/‬ل إ ‪/62‬ق‪ 5-‬الذي اعتمدت فيه اإلطار اإلقليمي لتوسيع نطاق العمل يف جمال رعاية الصحة النفسية يف‬ ‫إقليم شرق املتوسط‪ .‬واهلدف من هذا اإلطار هو تفعيل خطة العمل الشاملة العاملية اخلاصة بالصحة النفسية‬ ‫(‪ )2020-2013‬للمنظمة‪ ،‬وأن يكون مبثابة خارطة طريق إقليمية تعطي األولوية جملموعة من التدخالت‬ ‫كمة يف جمال الصحة النفسية‪ ،‬والرعاية الصحية‪ ،‬وتعزيز الصحة والوقاية‪،‬‬ ‫االسرتاتيجية يف أربعة جماالت هي‪ :‬الحوح‬ ‫والرتصد والرصد والبحوث‪.‬‬ ‫ومبوجب القرار‪ ،‬طلبت الدول األعضاء من املدير اإلقليمي رفع تقرير للجنة اإلقليمية كل سنتني عن حالة تنفيذ‬ ‫اإلطار اإلقليمي لتوسيع نطاق العمل يف جمال رعاية الصحة النفسية‪.‬‬ ‫أحرزته البلدان يف تنفيذ التدخالت االسرتاتيجية الواردة يف‬ ‫ويقد‬ ‫ِّم هذا التقرير املرحلي ملخصا حول التقدم الذي ح‬ ‫دما ملواصلة توسيع نطاق العمل يف هذا اجملال‬ ‫سبُل املضي قُ ُ‬ ‫إطار العمل يف ضوء املؤشرات املتفق عليها‪ ،‬ويناقش ُ‬ ‫باإلقليم‪.‬‬ ‫‪1‬‬

‫تفيد سبعة عشر بلدا حاليا بأن لديها سياسة للصحة النفسية‪ ،‬غري أن أيا منها ال ينفذ سياسته تنفيذا كامال‪.‬‬

‫وخالل الفرتة من ‪ 2015‬إىل ‪ ،2017‬عززت املنظمة دعمها إلعداد سياسات واسرتاتيجيات وخطط عمل الصحة‬ ‫النفسية وتنفيذها يف األردن ولبنان وقطر والصومال واإلمارات العربية املتحدة‪ .‬ومت حتديث السياسات أو اخلطة‬ ‫الوطنية وفقا للمؤشرات واألهداف املتفق عليها يف إطار خطة العمل الشاملة العاملية للصحة النفسية ‪-2013‬‬ ‫‪ 2020‬وإطار العمل اإلقليمي لشرق املتوسط‪.‬‬ ‫رف وفقا لبنود خطة العمل‬ ‫ويدعم املكتب اإلقليمي األردن وقطر وتونس لوضع خطط عمل وطنية ملعاجلة اخلحح‬ ‫رف اليت اعتمدهتا مجعية الصحة العاملية السبعون يف عام‬ ‫العاملية للمنظمة بشأن االستجابة الصحية العمومية ح‬ ‫للخح‬ ‫‪.2017‬‬

‫وتفيد ستة عشر بلدا بأن لديها تشريعات للصحة النفسية‪ ،‬غري أن حنو ثلث تلك التشريعات فقط هي اليت تتفق‬ ‫متاما مع صكوك حقوق اإلنسان الدولية األساسية‪.‬‬ ‫وقدم املكتب اإلقليمي الدعم ألفغانستان ولبنان واملغرب وقطر واإلمارات العربية املتحدة يف مراجعة وصياغة‬ ‫التشريعات واللوائح اليت تنظم الصحة النفسية وفقا لبنود اتفاقية األمم املتحدة لقوق األشخاص ذوي اإلعاقة‪.‬‬ ‫ومت إعداد دورة تدريبية إقليمية عن القيادة يف جمال الصحة النفسية بالتعاون مع خرباء دوليني من ذوي اخلربة يف‬ ‫برامج مماثلة يف أسرتاليا واهلند ونيجرييا‪ .‬وتستهدف الدورة املديرين من املستويات اإلدارية املتوسطة من بلدان‬ ‫اإلقليم لتمكينهم من توسيع نطاق خدمات الصحة النفسية يف بلداهنم‪ ،‬وتنظِّم املنظمة النسخة الثالثة من الدورة‬ ‫التدريبية يف عام ‪ ،2017‬وتعتزم تنظيمها سنويا‪.‬‬

‫الرعاية الصحية‬ ‫‪ُِ .10‬‬ ‫اُتذت خطوات واعدة إلعادة توجيه النُظُم الصحية اإلقليمية وتعزيزها يف بعض البلدان‪ ،‬إال أن النموذج السائد‬ ‫للرعاية ال يزال قائما على الرعاية املؤسسية يف ظل وجود عدد قليل نسبيا من اخلدمات اجملتمعية‪ ،‬وعدم اكتمال‬ ‫إدماج مكونات الصحة النفسية يف الرعاية الصحية األولية وحمدودية التغطية باخلدمات‪.‬‬ ‫وسعيا إىل سد الفجوة العالجية وتعزيز تقدمي الرعاية املتكاملة اجملتمعية‪ ،‬قدمت املنظمة الدعم لتنفيذ برنامج رأب‬ ‫الفجوة يف الصحة النفسية يف العديد من بلدان اإلقليم‪ ،‬إذ قامت خالل الفرتة ‪ 2017-2015‬بتدشني الربنامج‬ ‫يف باكستان ومصر وتونس‪ ،‬ودعمت مواصلة تنفيذه يف أفغانستان واألردن والكويت ولبنان وعُمان وفلسطني‬ ‫وقطر‪.‬‬ ‫وتقوم مجهورية إيران اإلسالمية حاليا بتوسيع نطاق مناذج اخلدمات املتكاملة يف املراكز الضرية‪ ،‬بينما تعكف‬ ‫البحرين ومجهورية إيران اإلسالمية وقطر على إنشاء خدمات جمتمعية إيصالية‪.‬‬ ‫ومت إعداد مسودة للتوجيها ت بشأن إدماج الصحة النفسية يف الرعاية الصحية األولية هبدف دعم البلدان يف‬ ‫عملية توسيع نطاق العمل يف جمال رعاية الصحة النفسية من خالل إدماجها يف الرعاية الصحية األولية‪.‬‬ ‫ويتزايد االحتياج خلدمات الصحة النفسية والدعم النفسي االجتماعي بسبب تفاقم الصراعات وأزمة الالجئني‬ ‫املرتبطة هبا يف عدد من بلدان اإلقليم‪ .‬وحتتاج اجملتمعات اليت تعيش يف ظل هذه الظروف إىل جمموعة من‬ ‫خدمات الدعم النفسي االجتماعي‪ .‬وال خيفى أن تلبية احتياجات الصحة النفسية هو أمر بالغ األمهية يف‬ ‫أوقات األزمات والتعايف منها‪ ،‬فضال عن التنمية املستدامة‪.‬‬ ‫كاء الدوليني‬ ‫كاالت األمم املتحدة‪ ،‬والشر‬ ‫وقد اضطلع املكتب اإلقليمي بدور فعال يف التنسيق مع سائر و‬ ‫والوطنيني ألجل توفري خدمات الصحة النفسية والدعم النفسي االجتماعي يف البلدان املتضررة من األزمات‬ ‫اإلنسانية‪ ،‬مبا فيها العراق واألردن ولبنان وليبيا وسوريا واليمن‪ .‬وقد مشل الدعم املقدم تيسري الدورات التدريبية‬ ‫على اإلسعافات األولية النفسية لبناء قُدرات اجملتمعات وضمان مرونتها‪ ،‬وتقدمي حزمة من التدخالت النفسية‬ ‫االجتماعية من خالل العاملني اجملتمعيني أثناء الطوار‪ ،،‬وتدريب العاملني يف جمال الرعاية الصحية األولية على‬ ‫التعرف املبكر على مشاكل الصحة النفسية ذات األولوية والتدبري العالجي هلا‪ ،‬باالستعانة بدليل التدخالت‬ ‫اإلنسانية لربنامج رأب الفجوة يف الصحة النفسية‪ ،‬وجتديد مرافق الصحة النفسية‪ ،‬وإنشاء خدمات الصحة‬ ‫‪.11‬‬

‫النفسية اجملتمعية‪ .‬وقد مت جتريب دورة توجيهية يف جمال الصحة النفسية والدعم النفسي االجتماعي يف اململكة‬ ‫العربية السعودية للمديرين من املستويات اإلدارية املتوسط ممن يعملون يف أوضاع الطوار‪ ،‬املعقدة‪.‬‬ ‫تعزيز الصحة والوقاية‬ ‫هناك عشرة بلدان متتلك‪ ،‬يف جمال تعزيز الصحة والوقاية من األمراض‪ ،‬برامج لتعزيز الصحة النفسية والوقاية‪،‬‬ ‫وهي حتديدا‪ :‬البحرين ومصر ومجهورية إيران اإلسالمية واملغرب وعُمان وباكستان وفلسطني وقطر واململكة‬ ‫العربية السعودية واإلمارات العربية املتحدة‪ ،‬إال أن هناك تفاوتا كبريا بني البلدان يف نطاق التغطية هبذه الربامج‪.‬‬ ‫وتفيد ثالثة بلدان فقط بأهنا وضعت اسرتاتيجية وطنية للوقاية من االنتحار‪.‬‬ ‫وتقد م املنظمة الدعم إلجراء استعراض لتنفيذ اسرتاتيجية الوقاية من االنتحار لدى مجهورية إيران اإلسالمية‪،‬‬ ‫ِّ‬ ‫ولوضع الصيغة النهائية السرتاتيجية وطنية ذات صلة باملسألة ذاهتا يف أفغانستان وتونس‪.‬‬ ‫وانتهي املكتب اإلقليمي من إعداد حزمة للصحة النفسية املدرسية عقب تنظيم تدريب جترييب يف مثانية بلدان‪،‬‬ ‫وجيري تنفيذها حاليا يف مصر ومجهورية إيران اإلسالمية واألردن وباكستان‪ ،‬يف حني أن فلسطني وقطر وسوريا‬ ‫واإلمارات العربية املتحدة مل تزل يف مرحلة التخطيط لتنفيذ تلك الزمة‪.‬‬ ‫قدرات الالزمة لتشخيص اضطرابات طيف التوحد والتدبري العالجي هلا يف عام ‪،2015‬‬ ‫وأُجري تقييم للموارد وال ُ‬ ‫بالتعاون مع أحد املعاهد اإليطالية للصحة العامة ومؤسسة أوتيزم سبيكس‪ ،‬مث تبع ذلك وضع عُمان وقطر‬ ‫واإلمارات العربية املتحدة خططا وطنية معنية بالتوحد‪.‬‬ ‫وجترى حاليا جتارب عشوائية مضبوطة يف باكستان لتدريب اآلباء واألمهات على إتقان مهارات التعامل مع‬ ‫ُ‬ ‫األطفال ذوي اإلعاقة الفكرية والتنموية‪ ،‬وال تزال تلك التجارب يف املرحلة األوىل يف مصر‪ ،‬بينما خيطط األردن‬ ‫اجلمهورية العربية السورية لتنفيذ ذلك التدريب‪.‬‬ ‫وقد أعدت املنظمة حزمة لتعليم املهارات الياتية‪ ،‬واليت ستكون جزءا من حزمة شاملة للتدخل للحفاظ على‬ ‫الصحة االنفعالية للمراهقني‪ .‬وتستهدف حزمة املهارات الياتية املراهقني الذين ترتاوح أعمارهم بني ‪ 11‬و‪15‬‬ ‫كز على جمموعة أساسية من املهارات الياتية‪ ،‬ومنها حل املشكالت‪ ،‬واُتاذ القرارات‪ ،‬وحتديد‬ ‫عاما ‪ ،‬وتر‬ ‫كيد الذات‪ ،‬والوعي الذايت‪ ،‬ومهارات التعامل مع الضغوط‪.‬‬ ‫األهداف‪ ،‬والتفكري النقدي‪ ،‬ومهارات االتصال‪ ،‬وتو‬ ‫ذت املنظمة االكتئاب موضوعا ليوم الصحة العاملي لعام ‪ .2017‬واست ِ‬ ‫خدمت املواد اإلعالمية اليت أعدهتا‬ ‫واُت ح‬ ‫ُ‬ ‫املنظمة هبذه املناسبة على نطاق واسع يف البلدان لتشجيع املعرفة مبباد‪ ،‬الصحة النفسية‪.‬‬ ‫وجيري حاليا مجع البيانات إلعداد أطلس للصحة النفسية لعام ‪ 2017‬هبدف توصيف حاالت املوارد يف بلدان‬ ‫حرز يف تنفيذ األهداف واملؤشرات املتفق عليها يف إطار العمل‪،‬‬ ‫اإلقليم وقدراهتا على اإلبالغ عن التقدم الـ ُ‬ ‫م ح‬ ‫وسوف تُنشر يف وقت الحق من هذا العام‪.‬‬ ‫وقد أعدت املنظمة أطلسا ملوارد وقدرات الصحة النفسية لألطفال واملراهقني واألمهات‪ ،‬لدعم البلدان يف تطوير‬ ‫اخلدمات اخلاصة بالصحة النفسية لألطفال واملراهقني‪.‬‬ ‫‪3‬‬ ‫‪.16‬‬

‫رف للتجريب يف ثالثة بلدان‪ ،‬وجيري تنفيذه حاليا يف األردن وقطر وتونس‪ .‬وسوف‬ ‫رصد العاملي ح‬ ‫م ح‬ ‫وخضع الـ ح‬ ‫للخح‬ ‫تتوسع املنظمة يف نطاقه ليشمل املزيد من البلدان يف املرحلة املقبلة هبدف مجع البيانات عن جمموعة من‬ ‫رف‪ ،‬تزامنا مع إعداد خطط‬ ‫حرز بشأن خطة العمل العاملية ح‬ ‫رصد التقدم الـ ُ‬ ‫املؤشرات اليت يستفاد منها يف ح‬ ‫للخح‬ ‫م ح‬ ‫عمل تسرتشد بتلك البينات على الصعيد الوطين‪.‬‬ ‫وتشارك املنظمة يف مبادرة حبثية واسعة النطاق هتدف إىل تعزيز إعداد البيحنات والرباهني واالستفادة منها حمليا‪.‬‬ ‫كز املبادرة على عدة جماالت‪ ،‬وهي‪ :‬إرساء نُظُم للصحة النفسية قادرة على االستجابة إبان أزمة الالجئني‬ ‫وتر‬ ‫السوريني‪ ،‬وزيادة إتاحة رعاية الصحة النفسية لألشخاص الذين يواجهون الشدائد يف لبنان‪ ،‬وإنشاء شبكة لتنفيذ‬ ‫برامج الصحة املدرسية‪.‬‬ ‫الوصم املستمر املرتبط بالصحة النفسية‪ ،‬وما ينتج عنه من‬ ‫ال تزال هناك حاجة لبذل جهود منسقة للتغلب على ح‬ ‫التحديات اليت تعوق التقدم يف جمال الصحة النفسية استمرار‬ ‫متييز وفجوة يف الصول على العالج‪ .‬ومن أهم ِّ‬ ‫عدم االستقرار السياسي والصراعات واألزمات اإلنسانية يف عدد من بلدان اإلقليم‪ .‬وفيما يلي عوامل أخرى‬ ‫تسهم يف اتساع فجوة الصول على عالج األمراض النفسية‪:‬‬ ‫كيز على مناذج‬ ‫عدم كفاية املوارد البشرية واملالية‪ ،‬ويتفاقم أثر ذلك باالستغالل غري الفعال هلا‪ ،‬مع استمرار الرت‬ ‫الرعاية املؤسسية يف تقدمي اخلدمات اخلاصة باضطرابات الصحة النفسية‪.‬‬ ‫عدم كفاية الماية املتوفرة لقوق األشخاص ممن يعانون اضطرابات صحية نفسية وأسرهم‪.‬‬ ‫النقص الاد يف خدمات الرعاية النهارية اجملتمعية‪.‬‬ ‫غياب اآلليات والعمليات املؤسسية الالزمة إلدماج مكونات الصحة النفسية يف الرعاية الصحية األولية‬ ‫واخلدمات العامة للرعاية الصحية‪.‬‬ ‫التصدي‬ ‫ِّ‬ ‫عدم كفاية التنسيق مع سائر الربامج داخل القطاع الصحي‪ ،‬وبني خمتلف القطاعات هبدف‬ ‫للمحددات العامة للصحة النفسية‪.‬‬ ‫عدم املنهجية يف مجع البيانات عن املؤشرات املتعلقة باضطرابات الصحة النفسية يف إطار النظام الوطين‬ ‫للمعلومات الصحية‪ ،‬وضعف نُظُم تسجيل األحوال املدنية يف العديد من البلدان‪.‬‬ ‫‪‬‬ ‫‪‬‬ ‫‪‬‬ ‫‪‬‬ ‫‪‬‬ ‫‪‬‬

Progress report on scaling up mental health care Introduction In 2015 the 62nd session of the WHO Regional Committee for the Eastern Mediterranean issued resolution EM/RC62/R.5 in which it endorsed a framework to scale up action on mental health in the Eastern Mediterranean Region. The framework operationalizes the WHO global comprehensive action plan for mental health (2013–2020) and constitutes a regional roadmap which prioritizes a set of strategic interventions in four domains: mental health governance; health care; promotion and prevention; and surveillance, monitoring and research. In the resolution, Member States requested the Regional Director to report to the Regional Committee every two years on the status of implementation of the regional framework to scale up action on mental health. This report summarizes the progress made by countries in implementing the strategic interventions outlined in the framework along agreed indicators, and discusses ways to further scale up action in the Region.

Status and progress1 Governance Seventeen countries now report having a mental health policy. However no country is fully implementing its policy. During the period of 2015–2017, WHO enhanced its support to develop and implement mental health policies, strategies and action plans in Jordan, Lebanon, Qatar, Somalia and United Arab Emirates. The national policies or plan were updated in accordance with the indicators and targets agreed upon as part of the global comprehensive mental health action plan 2013–2020 and the Eastern Mediterranean regional framework for action. Jordan, Qatar and Tunisia are being supported to develop national action plans to address dementia in accordance with the provisions of the WHO global action plan on the public health response to dementia adopted by the Seventieth World Health Assembly in 2017. Sixteen countries report that they have mental health legislation, but only approximately one third of them are fully compliant with the required international human rights instruments. Afghanistan, Lebanon, Morocco, Qatar and United Arab Emirates were supported in reviewing and drafting mental health legislation and regulation in accordance with provisions of the United Nations Convention on the Rights of Persons with Disabilities. The curriculum for a regional course on leadership in mental health has been developed in collaboration with international experts with experience in similar programmes in Australia, India and Nigeria. The course is designed for mid-level managers from countries of the Region to empower them to scale up mental health services in their respective countries WHO is organizing its third iteration in 2017 and plans to organize the course annually.

EM/RC64/INF.DOC.7 Health care While promising steps have been taken to reorient and strengthen regional health systems in some countries, the predominant model of care is still institution-based, with relatively few community-based services, patchy integration of mental health components in primary health care and limited service coverage. In order to bridge the treatment gap and promote the delivery of community-based integrated care, WHO has been supporting the implementation of the mental health gap action programme (mhGAP) in several countries of the Region. During 2015–2017, the mhGAP was initiated in Pakistan, Egypt, and Tunisia and its implementation strengthened in Afghanistan, Jordan, Kuwait, Lebanon, Oman, Palestine and Qatar. The Islamic Republic of Iran is in the process of expanding models of integrated services in urban centres, and Bahrain, Islamic Republic of Iran and Qatar are setting up outreach community services. Draft guidance on the integration of mental health in primary health care has been developed to support countries in the process of scaling up mental health care through integration in primary health care. Mental health and psychosocial support needs are amplified by the growing conflict and related refugee crisis in a number of countries of the Region. Communities living under these circumstances require a range of psychosocial support. Addressing mental health needs is critical in times of crisis and recovery, in addition to sustainable development. The Regional Office has been active in coordination with other United Nations agencies, international and national partners in the provision of mental health and psychosocial support in countries affected by humanitarian crises including Iraq, Jordan, Lebanon, Libya, Syria and Yemen. Support has included facilitating training for psychological first aid to build community capacity and resilience, delivering a psychosocial intervention package through community workers in emergencies, training primary health care staff on early recognition and management of priority mental health problems using the mhGAP humanitarian intervention guide, refurbishment of mental health facilities and the establishment of community mental health services. An orientation course on mental health and psychosocial support was piloted in Saudi Arabia for mid-level managers working in complex emergency situations. Promotion and prevention In the area of promotion of health and prevention of disease, 10 countries have more than two functional mental health promotion and prevention programmes: Bahrain, Egypt, Islamic Republic of Iran, Morocco, Oman, Pakistan, Palestine, Qatar, Saudi Arabia and United Arab Emirates. However, the range and coverage of programmes vary greatly between countries. Only three countries report having developed a national suicide prevention strategy. WHO is providing support for a review of the implementation of the suicide prevention strategy for the Islamic Republic of Iran and for the finalization of a related national strategy in Afghanistan and Tunisia. A school mental health package was finalized following pilot training for eight countries. It is currently being implemented in Egypt, Islamic Republic of Iran, Jordan and Pakistan, while Palestine, Qatar, Syria and United Arab Emirates are in the planning phase. An assessment of resources and capacities for the diagnosis and management of autism spectrum disorders was conducted in 2015 in collaboration with an Italian public health institute and Autism Speaks. Subsequently, Oman, Qatar and United Arab Emirates have developed national autism plans. Randomized controlled trials for parent skills training for children with intellectual and developmental disabilities are under way in Pakistan and in the initial phase in Egypt. Jordan and Syria are planning for implementation of the training. A life skills education package has been developed by WHO and will be part of a comprehensive intervention package for the emotional health of adolescents. The life skills package is targeted at adolescents 2

EM/RC64/INF.DOC.7 aged 11–15 years and focuses on a core set of life skills that include problem-solving, decision-making, goalsetting, critical thinking, communication skills, assertiveness, self-awareness and skills for coping with stress. The theme of World Health Day 2017 was depression. Related communications materials developed by WHO were extensively used in countries to promote mental health literacy. Surveillance, monitoring and research Data collection for a mental health atlas 2017 for mapping resources and capacities in countries of the Region to report on the targets and indicators agreed upon in the framework for action is currently under way and will be published later in the year. An atlas for child, adolescent and maternal mental health resources and capacities has been developed to support countries in developing services for child and adolescent mental health. A global dementia observatory was piloted in three countries and is currently being implemented in Jordan, Qatar and Tunisia. It will be extended to cover more countries in the next phase to collect data on indicators for monitoring progress on the global action plan for dementia while guiding the development of evidence-informed action plans nationally. WHO is participating in a large-scale research initiative aimed at strengthening local generation and utilization of evidence. The initiative focuses on several areas: fostering responsive mental health systems in the Syrian refugee crisis; increasing access to mental health care for people in Lebanon living in adversity; and a school health implementation network.

Challenges and the way forward Concerted efforts are still needed to overcome the persisting stigma attached to mental health and resulting discrimination and treatment gap. Overarching challenges hindering progress in mental health include continuing political instability, conflict and humanitarian crises in a number of countries of the Region. Other factors contributing to the mental health treatment gap include the following. • Insufficient human and financial resources compounded by their inefficient use with continued stress on institutional care models of service delivery for mental health disorders • Inadequate protection of the rights of persons with mental health disorders and their families • A severe shortage of community-based and day care services • A lack of institutional mechanisms and processes for integration of mental health components in primary health care and general health care services. • Insufficient coordination with programmes within the health sector and between sectors to address the broad determinants of mental health • Lack of systematic collection of data on indicators related to mental health disorders as part of the national health information system, and weak vital registration systems in many countries In view of these challenges, WHO will continue to work with Member States and provide guidance and technical support for scaling up the implementation of the strategic interventions in the four priority areas of the regional framework to scale up action on mental health.

Rapport de situation sur l’intensification des soins de santé mentale Introduction 1. En 2015, la soixante-deuxième session du Comité régional de l’OMS pour la Méditerranée orientale a voté la résolution EM/RC62/R.5, dans laquelle a été adopté un cadre pour intensifier l’action en santé mentale dans la Région de la Méditerranée orientale. Ce cadre permet d’appliquer le plan d’action global de l’OMS sur la santé mentale (2013-2020) et constitue une feuille de route régionale qui établit l’ordre de priorité d’un ensemble d’interventions stratégiques dans quatre domaines : la gouvernance en santé mentale, les soins de santé, la promotion et la prévention, ainsi que la surveillance, le suivi et la recherche. 2. Dans la résolution, les États Membres ont demandé au Directeur régional de rapporter tous les deux ans au Comité régional le statut de la mise en œuvre de ce cadre régional pour intensifier l’action en santé mentale. 3. Le présent rapport résume les progrès accomplis par les pays dans la mise en œuvre des interventions stratégiques exposées dans le cadre avec les indicateurs convenus, et discute des moyens de renforcer l’intensification de l’action dans la Région.

Situation et progrès 1 Gouvernance 4. Dix-sept pays déclarent disposer désormais d’une politique de santé mentale. Cependant, aucun de ces pays n’applique pleinement sa politique. 5. Sur la période 2015-2017, l’OMS a renforcé son soutien à l’élaboration et à la mise en œuvre de politiques, stratégies et plans d’action sur la santé mentale dans les Émirats arabes unis, en Jordanie, au Liban, au Qatar et en Somalie. Les plans ou politiques nationaux ont été mis à jour conformément aux indicateurs et cibles convenus dans le plan d’action global sur la santé mentale 2013-2020 et dans le cadre d’action régional pour la Méditerranée orientale. 6. La Jordanie, le Qatar et la Tunisie bénéficient d’un soutien dans l’élaboration de plans d’action nationaux de lutte contre la démence, conformément aux dispositions du plan mondial d’action de santé publique contre la démence de l’OMS, adopté par la Soixante-Dixième Assemblée mondiale de la Santé en 2017. 7. Seize pays déclarent disposer d’une législation en santé mentale, mais seulement environ un tiers d’entre eux respectent pleinement les exigences en matière d’instruments internationaux relatifs aux droits humains. 8. L’Afghanistan, les Émirats arabes unis, le Liban, le Maroc et le Qatar ont bénéficié d’un soutien dans la revue et la rédaction de leurs législations et règlementations en santé mentale, conformément aux dispositions de la Convention des Nations Unies relative aux droits des personnes handicapées.

Un programme de cours régional sur le leadership en santé mentale a été mis au point en collaboration avec des experts internationaux ayant déjà travaillé sur des programmes similaires en Australie, en Inde et au Nigeria. Ce cours est conçu pour les administrateurs de niveau intermédiaire des pays de la Région afin de leur donner les moyens d’intensifier les services de santé mentale dans leurs pays respectifs. En 2017, l’OMS coordonnera la troisième édition de ce programme et prévoit de le reconduire tous les ans. Soins de santé 10. Tandis que des mesures prometteuses ont été prises pour réorienter et renforcer les systèmes de santé régionaux dans certains pays, le modèle de soins prédominant continue de reposer sur les institutions, avec relativement peu de services à base communautaire, une intégration inégale des composantes de santé mentale aux soins de santé primaires et une couverture des services limitée. 11. Afin de combler les lacunes en matière de traitement et de favoriser la prestation de soins communautaires intégrés, l’OMS offre son soutien pour la mise en œuvre du programme d’action intitulé Combler les lacunes en santé mentale (mhGAP) dans plusieurs pays de la Région. Sur la période 2015-2017, le mhGAP a été lancé au Pakistan, en Égypte et en Tunisie, et sa mise en œuvre a été renforcée en Afghanistan, en Jordanie, au Koweït, au Liban, à Oman, en Palestine et au Qatar. 12. La République islamique d’Iran travaille actuellement à l’élargissement des modèles de services intégrés en milieu urbain ; et Bahreïn, la République islamique d’Iran et le Qatar mettent en place des services de sensibilisation au niveau communautaire. 13. Un projet d’orientations sur l’intégration de la santé mentale aux soins de santé primaires a été mis au point pour soutenir les pays lors du processus d’intensification des soins de santé mentale via leur intégration aux soins de santé primaires. 14. Les besoins en santé mentale et en soutien psychosocial sont amplifiés par le conflit grandissant et la crise des réfugiés qui en résulte dans de nombreux pays de la Région. Les communautés qui vivent dans ces conditions nécessitent divers soutiens psychosociaux. Répondre aux besoins en santé mentale est crucial en période de crise et de relèvement, en plus d’un développement durable. 15. Le Bureau régional a agi pour la coordination avec les autres institutions des Nations Unies ainsi que les partenaires nationaux et internationaux pour apporter un soutien psychosocial et en santé mentale dans les pays touchés par les crises humanitaires, notamment l’Iraq, la Jordanie, le Liban, la Libye, la République arabe syrienne et le Yémen. Ce soutien a consisté à faciliter la formation sur les premiers secours psychologiques pour le renforcement de la résilience et des capacités communautaires, à dispenser un ensemble d’interventions psychosociales dans les situations d’urgence via les agents communautaires, à former les personnels des soins de santé primaires à l’identification et la gestion des problèmes de santé mentale prioritaires en appliquant le guide d’intervention humanitaire mhGAP, à rénover les établissements de santé mentale, ainsi qu’à mettre en place des services communautaires de santé mentale. Un cours d’orientation en santé mentale et en soutien psychosocial a été testé en Arabie Saoudite pour les administrateurs de niveau intermédiaire qui travaillent au cœur de situations d’urgence complexes. Promotion et prévention 16. Dans le domaine de la promotion de la santé et de la prévention des maladies, 10 pays disposent de plus de deux programmes fonctionnels de promotion et de prévention en santé mentale : l’Arabie Saoudite, Bahreïn, l’Égypte, les Émirats arabes unis, la République islamique d’Iran, le Maroc, Oman, le Pakistan, la Palestine et le Qatar. Cependant, l’étendue et la couverture de ces programmes présentent de grandes variations entre les pays. 17. Seuls trois pays déclarent avoir mis au point une stratégie nationale de prévention du suicide.

L’OMS fournit un appui à l’examen de la mise en œuvre de la stratégie de prévention du suicide en République islamique d’Iran, ainsi qu’à la finalisation d’une stratégie nationale connexe en Afghanistan et en Tunisie. 19. Un ensemble de documents sur la santé mentale en milieu scolaire a été finalisé suite à la formation pilote pour huit pays. Il est actuellement utilisé en Égypte, en République islamique d’Iran, en Jordanie et au Pakistan, tandis que les Émirats arabes unis, la Palestine, le Qatar et la République arabe syrienne envisagent de s’en servir prochainement. 20. Une évaluation des ressources et des capacités pour le diagnostic et la prise en charge des troubles du spectre autistique a été menée en 2015 en collaboration avec un institut italien de santé publique et l’organisation Autism Speaks. Par la suite, les Émirats arabes unis, Oman et le Qatar ont mis au point des plans nationaux pour l’autisme. 21. Des essais cliniques contrôlés randomisés, portant sur le développement des compétences parentérales pour les parents dont les enfants présentent des déficiences mentales et des troubles développementaux, sont en cours au Pakistan et en phase initiale en Égypte. La Jordanie et la République arabe syrienne planifient actuellement la mise en œuvre de la formation. 22. Une formation aux compétences essentielles a été mise au point par l’OMS et fera partie d’un ensemble d’interventions complet sur la santé émotionnelle des adolescents. Cette formation aux compétences essentielles cible les adolescents âgés de 11 à 15 ans. Elle se focalise sur un ensemble de base de compétences essentielles qui regroupent la résolution de problèmes, la prise de décision, la fixation d’objectifs, la pensée critique, les aptitudes de communication, la confiance en soi, la conscience de soi et des outils de gestion du stress. 23. La dépression était le thème de la Journée mondiale de la Santé 2017. Les supports de communication connexes élaborés par l’OMS ont été largement utilisés dans les pays pour promouvoir les connaissances en santé mentale. Surveillance, suivi et recherche 24. Le recueil des données pour l’atlas 2017 de la santé mentale, visant à cartographier les ressources et les capacités dans les pays de la Région afin de rendre compte des cibles et indicateurs convenus dans le cadre d’action, est en cours d’élaboration et sera publié dans le courant de l’année. 25. Un atlas des ressources et capacités en santé mentale chez la mère, l’enfant et l’adolescent a été mis au point pour aider les pays à concevoir des services pour la santé mentale de l’enfant et de l’adolescent. 26. Un observatoire mondial de la démence a été expérimenté dans trois pays et est en cours de mise en œuvre en Jordanie, au Qatar et en Tunisie. Il sera étendu afin de couvrir davantage de pays lors de la phase suivante et permettra de recueillir des données sur les indicateurs, pour suivre les progrès du plan d’action mondial de lutte contre la démence tout en orientant l’élaboration de plans d’action nationaux reposant sur des données factuelles. 27. L’OMS participe à une initiative de recherche à large échelle visant à renforcer la génération et l’utilisation de données probantes au niveau local. Cette initiative se focalise sur différents aspects : favoriser des systèmes de santé mentale réactifs dans le contexte de la crise des réfugiés en République arabe syrienne ; augmenter l’accès aux soins de santé mentale pour les personnes vivant dans l'adversité au Liban ; et un réseau de mise en œuvre de la santé scolaire.

Défis et orientations futures 28. Des efforts concertés demeurent nécessaires pour vaincre la stigmatisation persistante liée à la santé mentale, ainsi que les discriminations et les lacunes de traitement qui en résultent. Les enjeux prioritaires qui freinent les progrès en matière de santé mentale incluent la persistance de l’instabilité politique, des conflits et des crises humanitaires pour de nombreux pays de la Région. Les autres facteurs contribuant aux lacunes de traitement en santé mentale sont listés ci-dessous : • Insuffisance des ressources financières et humaines, accentuée par une utilisation inefficace de celles-ci découlant du maintien des modèles de soins institutionnels de prestation de services – par opposition aux services à base communautaire – pour les troubles de santé mentale • Protection inadéquate des droits des personnes atteintes de troubles de santé mentale et de leurs familles • Grave pénurie de services communautaires et de garde de jour • Manque de mécanismes et de processus institutionnels pour l’intégration des composantes de santé mentale aux soins de santé primaires et aux services de santé généraux • Coordination insuffisante avec les programmes du secteur de la santé et entre les secteurs pour agir sur les déterminants de la santé mentale au sens large • Absence de recueil systématique des données sur les indicateurs liés aux troubles de santé mentale, dans le cadre de systèmes d’information sanitaire nationaux, et faiblesse des systèmes d’enregistrement de l’état civil dans de nombreux pays 29. Compte tenu de l’ensemble de ces enjeux, l’OMS va poursuivre son travail auprès des États Membres afin d’apporter des conseils et un soutien technique en vue d’accélérer la mise en œuvre d’interventions stratégiques dans les quatre domaines prioritaires du cadre régional pour l’intensification de l’action en santé mentale.
