0dq REPORT ON ONCHOCERCIASIS EPIDEMIOLOGICAL EVALUATION ACTTVTIp$ IN, GHANA. 2002 INTRODUCTION Epidemiological evaluation activities were carried out in selected communities in seven river basins in ttre OCP original areas and in ten others in the exte,nsion areas from 29\ovember-286 Decerrber, 2002. These were in the upper East, Upper West, Northem, Brong Ahafo, Volta and Western Region. The decentralized regional health teams undertook the exercise. These health teams have already received appropriate training in the techniques of skin snipping and in microscopy. Each team consisted of 3 members with technical assistance from OCP. OBJECTTVE To evaluate the epidemiological sihration of oncho in the river basins METHODOLOGY A one-day briefing was held at the Rural Health Training School - Kintampo to update the Regional health teams on the situation in the country and plan for the exercise. Evaluation in all six regions took place simultaneously, The decentralized regional teams carried out the activities in their respective regions only. These include the following: a) community entry b) Skin snip c) microscopical diagnosis, and d) migration studies RESULTS 53 out of 56 communities were evaluated. 560 positive cases were found in 39 communities. Of the 39 communities with positive czses, 21 were in the original areas and 18 in the exte,lrsion areas. The crude prevalence in the original areas r-anged from 1.5124.2, and in the extension areas from 1.1- 24.8. REMARKS o The decentralized regional health teiuns have demonstrated their ability to carry out epidemiological evaluation of onchocercal infection through simple skin snip technique. The overall participation rate was very low (46.8%) It was also observed that; the district and sub-district health workers did not supply ivermectin to most community distributors regularly. There was poor monitoring of ivermectin dishibution in the sub-districts and community levels. The data retums at the district levels and management on iverrnectin distribution was poor. There was low commitment of health workers with respect to ivermectin distribution in the disuict and zub{isEict levels throughout the areas evaluated. a a o a I oRECOMMENDATION 1. Epidemiological evaluation activities should be carried out during the first quarter of the year. 2. Specific period should be planned for ivermectin distribution throughout the country. 3. Completeness and timeliness of ivermectin distribution retums should be encouraged. 4. The number of community distributors should be more than one in each community to resolve high atrition rate. 5. DisnicUsub-district health workers and community distributors should be rerained with regard to ivermectin distribution so as to bring the disease under control. 6. IEC activities should be regularly carried out by health workers and District Assemblies using various communication techniques, information media, and available materials to bring onchocerciasis under control by means of ivermectin distibution. CONCLUSION There has been a downwards trend regarding the prevalence of the disease in most of the river basins.
