REGIONAL IMPLICATIONS OF THE DECISIONS AND RESOLUTIONS OF THE 54TH WORLD HEALTH ASSEMBLY AND THE 107TH AND 108TH SESSIONS OF THE WHO EXECUTIVE BOARD AND REVIEW OF THE DRAFT PROVISIONAL AGENDAS OF THE 109TH SESSION OF THE WHO EXECUTIVE BOARD AND THE FIFTY-FIFTH WORLD HEALTH ASSEMBLY

The 28th meeting of the Consultative Committee for Programme Development and Management (CCPDM) recommended that the working papers relating to resolutions of regional interest passed by the previous sessions of the Executive Board (EB) and the World Health Assembly (WHA), and a review of the draft provisional agendas of the future sessions of the Executive Board and the World Health Assembly should be combined and brought out as one document. The 37th meeting of the CCPDM proposed that, at its next meeting, all resolutions and decisions of the EB sessions and WHA should be submitted for their information, review, suggestions or comments. These proposals were endorsed by subsequent sessions of the Regional Committee. Accordingly, this document is submitted for the consideration of the Regional Committee. Part I of the document relates to all resolutions and decisions of the 107th and 108th Sessions of the EB and the Fifty-fourth WHA. Part II relates to a review of the draft provisional agendas of 109th session of the EB and the Fifty-fifth WHA. These agendas are not yet available and will be submitted separately. Part I contains 10 decisions and 16 resolutions emanating from the 107th session of the Executive Board in January 2001; and 11 decisions and 22 resolutions by the Fifty-Fourth World Health Assembly held in May 2001. The 108th session of the Executive Board adopted 11 decisions and one resolution. The salient information from the operative paragraphs of the decisions/resolutions as well as actions proposed for Member States and WHO are presented briefly in tabular form. Copies of the decisions and resolutions of the EB sessions and the WHA held in 2001 are also attached.

General programme of work (GPW)

EB107 endorsed and recommended to the WHA54 to adopt. EB107 also noted that the Board would review developments and evaluation affecting the GPW of 20022005, particularly in relation to the elaboration of PB 20042005 at its 109th session. WHA54 approved the GPW of 2002-2005, which outlined the strategic directions to address the changing context of international health. It adopted the six core functions. The Organization-wide priorities are already set as part of PB 2002-2003. The priorities for 2004-2005 will be reviewed in consultation with Member States when PB 2004-2005 is prepared.

The Regional Committee may need to comment on priorities for 20042005.

WHA54.2 and EB107.R16

Infant and young child nutrition

EB107 after reviewing the report on the global strategy on infant and young child feeding (EB107/3), recommended to WHA54 to adopt a resolution. WHA54 urged Member States to strengthen the activities and develop new approaches to protect, promote and support exclusive breastfeeding for six months, as a global public health recommendation, and to provide safe, adequate and appropriate complementary foods, with continued breast feeding for up to two years of age or beyond. WHA54 also requested the Director-General to provide support to Member States in the identification, implementation and evaluation of innovative approaches to improving infant and young child feeding, emphasizing exclusive breastfeeding for six months; and also requested to submit the global strategy for consideration to EB109 and WHA55 in 2002. WHA54 established the salaries for ungraded posts and the Director-General and decided to adjust with effect from 1 March 2001. WHA54 examined and accepted the unaudited interim financial report on the accounts of WHO for 2000.

Step by step country and region-based approach to developing the new global strategy on infant and young child feeding has to be continued with full involvement of the international health and development community, in particular UNICEF and other stake holders.

Special arrangements for settlement of arrears

WHA54 invited those member in arrears who wish to reschedule the payment of their arrears as part of an arrangement to have their voting rights restored, to address request to DG no later than 31 March 2002. WHA54 also requested the Administration, Budget and Finance Committee of the Executive Board to review and make appropriate recommendations to the Health Assembly. WHA54 authorized financing from Real Estate Fund for various projects including the future WHO/UNAIDS office development in Geneva; and requested the DG to report to WHA55. WHA54 decided the funds available from casual income to part finance the regular budget for the period 2002-2003, to be apportioned among Members in accordance with the financial incentive scheme (resolution WHA41.12) from the estimated interest earnings in 2000, the Real Estate Fund, the Working Capital Fund and to retain the balance as part of Miscellaneous Income. WHA54 decided that the scale of assessment for the Federal Republic of Yugoslavia should be at the annual rate of 0.026% for the year 2000 and 0.020% for the year 2001. WHA54, taking into consideration the report on HIV/AIDS, urged Member States to scale up their responses to HIV/AIDS, with particular emphasis on building partnerships, strengthening health care systems, nutrition, education and information programmes and developing prevention, treatment and care interventions, and availability of medicines for HIV/AIDS. It also urged Member States to support the establishment of a Global HIV/AIDS and Health Fund, and required the DG to take an active part, together with other international actors, in the development of the Fund, including promoting mechanisms for a transparent and participatory governance structure.

Assessment of the Federal Republic of Yugoslavia Scaling up the response to HIV/AIDS

WHA54.10

Within the UNAIDS programme framework, WHO should take a leadership role in health sector response to HIV/AIDS, including surveillance, HIV prevention, care and support. WHO should continue to carry out advocacy through various policy and technical forums. Equity and access to medicines should be emphasized in work with countries. Advice on sustainable drug financing mechanisms and information on drug prices should be provided. Advice on public health provisions in national legislation on Intellectual Property should be provided.

WHO medicines strategy

WHA54, taking into consideration the report on the revised drug strategy (A54/17), urged Member States to make every effort to promote equitable access to medicines; to undertake necessary action within their national health policies, and to cooperate constructively in strengthening pharmaceutical policy and practices and intellectual property regimes. WHA also requested the DG to explore the feasibility and effectiveness of implementing systems for voluntary monitoring of drug prices and reporting global drug prices; and to continue and enhance efforts to study and report on existing and future health implications of international trade agreements; to continue WHO’s work in the field of traditional medicine; and to report to the WHA55.

WHA54.12 and EB107.R2

Strengthening nursing and midwifery

WHA54, having reviewed the report (A54/11), urged Member States to further the development of their health systems; to pursue health sector reforms by involving nurses and midwives in the framing, planning, and implementation of health policies at all levels and to establish comprehensive programmes for the development of human resources for nurses and midwives. WHA also urged the DG to provide support to Member States for enquiring into the global shortage of nursing and midwifery personnel; to develop an action plan for strengthening of nursing and midwifery, and to report to WHA56 in 2003.

Advisory group on management of nursing and midwifery work force is constituted to address the problem of continuing shortage of nursing and midwifery personnel. WHO Guidelines for effective management of nursing and midwifery work force should be developed and widely disseminated to promote their use. Core group of nurse/ midwife leaders in countries should be developed to enable them to effectively participate in national health development.

WHA54.13

Strengthening health systems in developing countries

WHA54 reaffirmed its commitment to the objectives of health-for-all strategy, in particular the achievement of equitable, affordable, accessible and sustainable health systems based on primary health care in all Member States. It urged Member States to reaffirm the importance of health as an indispensable resource for sustainable development and to advance such development through actions which promote and maintain equity and equality, including men and women; to continue to support research in the area of human genetics and biotechnology to accepted scientific and ethical standards and to the potential benefit of all; and to refrain from all measures contrary to international law, including international conventions, which hinder health services delivery and deny care to those in greatest need. WHA also urged the DG to support the efforts of Member States to meet their health needs; to strengthen the capacity of health sector, to participate effectively in multisectoral efforts which seek to address the root causes of ill health; and report progress to the WHA56 in 2003.

WHO should improve and update on how countries and WHO will work on HSD. Each Member State has been requested to select at least one district to field test the concept of healthy district.

Global health security: epidemic alert and response

WHA54, having considered the report (A54/9), expressed its support for ongoing work on the revision of the International Health regulations, including criteria to define what constitutes a health emergency of international concern and the development of a global strategy for containment and where possible prevention of antimicrobial drug resistance. It urged Member States to participate actively in the verification and validation of surveillance data and information concerning health emergency of international concern, together with WHO and other technical partners, and to develop and update national preparation and response plans. WHA54 requested the DG to make appropriate arrangements for the development of regional preparedness and response plans and to provide technical support to Member States.

Rapid Response Teams (RRT) were established to respond effectively to disease outbreaks in India, Indonesia, Myanmar, Nepal, Sri Lanka and Thailand, at the central level and are gradually being set up at the district level. Regional guidelines for training RRT were developed. Stock piling and provision of diagnostic reagents/test-kits for control of outbreaks. (cholera, dengue, leptospirosis, J.E. and meningitis) “Early Warning Systems” were developed to detect impending outbreaks. A series of actions on regional capacity building in epidemiology surveillance and response were organized and will continue expanding further. Coordination will be strengthened with Global Outbreak Alert Systems.

WHA 54.15

Health conditions of, and assistance to, the Arab population in the occupied Arab countries, including Palestine International Decade of the World’s Indigenous People

WHA54 urged Member States to support the health development of the Palestinian people.

WHA54.16

WHA54, having deep concern about the disparities in health conditions of indigenous people in comparison to the overall population, urged Member States to make adequate provisions for indigenous health needs in their national health systems. It also requested WHO Regional Committees to give urgent attention to the adoption of regional plans of action on indigenous health. The Health Assembly urged the DG to complete a framework of global plan of action to improve the health of indigenous people for submission to the WHA55 in 2002.

Regional Committee should consider this area as a priority and provide guidance to develop regional plan of action.

Assessments for the financial period 2002-2003

WHA54, having considered the report of the DG (A54/49), decided that the assessed contribution of Members for 2002-2003 should be as set out in the Annex to the resolution. It further decided to review at WHA55, after review by the EB109 in 2002. WHA54 urged Member States to be aware of affiliations between tobacco industries and members of their delegations; and also WHO and Member States to be alert to any efforts by the tobacco industries to continue this practice and to assure the integrity of health policy development in any WHO meeting and in national governments.

In January 2002, the Executive Board will review the assessments for 2003 and make recommendations as appropriate. Copies of guidelines on WHO interaction with tobacco industry as well as WHA54.18 have been shared with all countries. Reports on regional analysis of the tobacco industry document on their operations in Member Countries conducted by SEARO has also been shared with relevant countries. SEARO would continue to support countries in vetting participants to WHO supported expert meetings, task forces and consultation in accordance with WHO/ HQ format. Countries are requested to approach SEARO in the event of requests from the tobacco industry on WHO’s response thereof. The Regional Committee may consider this area for priority attention.

Transparency in tobacco control process

Schistosomiasis and soiltransmitted helminth infections

WHA54 urged Member States to give high priority to control of schistosomiasis and soil-transmitted helminth infections, and to ensure access to essential drugs with the goal of attaining a minimum target of regular administration of chemotherapy to at least 75% and up to 100% of all school-aged children at risk of morbidity by 2010. It also requested the DG to continue to promote the strengthening of health systems and services as an important component of successful disease control programmes, and also to seek the resources required to support advocacy, coordination, programmes and research activities. WHA54, while commending the budget reform with the integrated presentation of PB 2002-2003, resolved to appropriate for the financial period 2002-2003 an amount of US$ 935 654 000, as per table on page 2 of the resolution.

International classification of functioning, disability and health

WHA54 endorsed the second edition of the International Classification of Impairments, Disabilities, Handicaps (ICIDH-2) with the title International Classification of Functioning, Disability and Health (ICF) and urged Member States and WHO to use the ICF.

WHO will provide support in making use of ICF in Member States for their research, surveillance and reporting. Member states may wish to get involved in the process in consultation with the current EB Members (DPR Korea, India and Myanmar). The new Financial Regulations and Rules came into effect in January 2001 following confirmation of the new Rules. The amendments to the Staff Rules are now in force.

Reform of the Executive Board

WHA54 requested the Executive Board to conduct a review of its working methods and those of its subsidiary bodies in order to ensure that they are effective, efficient and transparent, and to ensure improved participation of Member States in its proceedings; and to establish, for that purpose, an ad-hoc open-ended intergovernmental working group, and also to report to WHA55 in 2002. EB107 having considered the DG's proposal (EB107/13), confirmed the revised financial rules. These were later noted by WHA54.

Confirmation of amendments to the Staff Rules

EB107 confirmed the amendments to the WHO Staff Rules, in respect of special leave, sick leave and maternity and paternity leave as well as travel of a staff member with a nursing infant with effect from1 January 2001. EB107, having analyzed the WHR200 and the report of the DG (EB107/9) and the EB Chairman (EB107/35 Rev.1), took note with satisfaction the measures proposed by the DG such as establishment of a technical consultation process, forming a small advisory group, producing the health systems performance assessment (HSPA) report every two years and completion of next report by mid-2002. The Board also requested the DG to initiate a scientific peer review of the HSPA methodology as part of technical consultation process, to develop a multi-year plan for further research and development, and to report on the impact of HSPA reports on Member States’ policy and practice.

Health systems’ performance assessment

The DG has reported her follow-up action to the WHA54. The technical consultative process was started. WHO HQ/ SEARO has jointly organized a regional consultation and technical workshop on 18-21 June 2001. A few countries of the Region were involved in the global initiative, called EHSPA. The amendments to the Staff Rules are now in force.

EB107 confirmed the amendments made by the DirectorGeneral to the Staff Rules with respect to dependency allowances for the professional and higher categories with effect from1 January 2001, and also on the salary scale for use in conjunction with gross base salaries applicable to staff in the professional and higher categories with effect from1 March 2001. EB107 confirmed the amendments made by the DirectorGeneral to the Staff Rules in respect of assignment grant, payments and deductions, official holidays, and board of appeal with effect from 1 January 2001.

Relations with nongovernmental organizations

EB107 decided to establish official relations with the International Association for the Study of Obesity and the Global Forum for Health Research, to discontinue official relations with the International Sociological Association and International Medical Society of Paraplegia, and also to suspend official relations with the International Council of Societies of Pathology and the World Federation of United Nations Associations. EB108 confirmed the amendments made by the DirectorGeneral to the Staff Rules with respect to standards of conduct, with effect from 1 June 2001, and in respect to eduction grant entitlements with effect from the school year in progress on 1 January 2001. On the basis of the reports of collaborations (EB107/19), EB107 decided to maintain official relations with the International Association for Accident and Traffic Medicine for one year; to defer the review of relations with a few international NGOs until EB109. EB107 authorized the Chairman of the Executive Board, acting jointly with the Chairman of Standing Committee, to admit NGOs provisionally into official relations. The facility established by the present decision will apply to the NGOs that request for official relations solely or also for the purpose of participating in the work of the Intergovernmental Negotiating Body on the WHO FCTC subject to some conditions and requirements.

Review of NGO in official relations with WHO Measures to be taken for facilitating the participation of NGOs in the work of the Intergovernmental Negotiating Body on the WHO FCTC.

Two global NGOs viz. International Nongovernmental Coalition Against Tobacco (INGCAT) and Infact have been granted provisional affiliation with WHO for their participation in the FCTC process. NGOs at the regional level are involved in the FCTC discussions by countries but none has requested for official relationship with WHO so far. Countries would need to identify, screen and support major NGOs working in public health/tobacco control to request for affiliation.

The Fifty-fourth World Health Assembly appointed a Committee on Credentials consisting of delegates of 12 Member States, including Bangladesh. The Fifty-fourth World Health Assembly elected a Committee on Nominations consisting of delegates of 25 Member States, including DPR Korea and Nepal. The Fifty-fourth World Health Assembly, after considering the recommendations of the Committee on Nominations, elected President and Vice-Presidents. They included Mr Ri Tcheul (Democratic People’s Republic of Korea). The Fifty-fourth World Health Assembly, after considering the recommendations of the Committee on Nominations, elected officers of the main committees. They included Dr Pakdee Pothisiri (Thailand) as Committee B: ViceChairman. The Fifty-fourth World Health Assembly, after considering the recommendations of the Committee on Nominations, elected the delegates of 17 countries as members of the General Committee, including Bhutan. The Fifty-fourth World Health Assembly adopted the provisional agenda prepared by the Executive Board at its 107th session with the deletion of one item and the addition of a supplementary item. The Fifty-fourth World Health Assembly reappointed Dr J. Larivière (Canada) as member of the WHO Staff Pension Committee, and Dr Shyam P. Bhattarai (Nepal) as alternate member of the Committee, the appointments being for a period of three years. The Assembly also appointed Mr L. Rokovada (Fiji) as member of the Committee, and Mr M. Chakalisa (Botswana) as alternate member, for the remainder of the terms of office of Dr L. Malolo and Dr J.K.W. Mulwa respectively. The Fifty-fourth World Health Assembly further appointed Dr J.K. Gøtrik (Denmark) as alternate member of the Committee, for the remainder of the term of office of Dr E. Krag. The Executive Board appointed Dr Z. Wigzell (Sweden) to represent the Board at the Fifty-fifth World Health Assembly. The Executive Board decided that its 109th session should be convened from 14 to 21 January 2002, at WHO headquarters, Geneva, Switzerland.

PART II REVIEW OF THE DRAFT PROVISIONAL AGENDAS OF THE 107TH SESSION OF THE EXECUTIVE BOARD AND 55TH WORLD HEALTH ASSEMBLY In order to foster greater correlation of the work of the Regional Committee with that of the Executive Board and of the World Health Assembly, the Regional Committee has adopted the practice, since 1980, of reviewing the draft provisional agendas of the Executive Board and the World Health Assembly so that it could note important matters of regional and global interest. Regional Committees are urged to take an active part in the work of the Organization and to submit to the Executive Board their recommendations and concrete proposals on matters of regional and global interest. In its turn, the Executive Board routinely reviews the policy proposals of the Regional Committees concerning matters of worldwide interest, particularly for ensuing sessions of the Executive Board and the World Health Assembly. The draft provisional agendas of the 109th session of the Executive Board (14-21 January 2002) and the fifty-fifth World Health Assembly (13-18 May 2002) are awaited from WHO/HQ, and will be submitted to the Regional Committee for its review in due course.

REVIEW OF THE DRAFT PROVISIONAL AGENDAS OF THE 109TH SESSION OF THE EXECUTIVE BOARD AND THE FIFTY-FIFTH WORLD HEALTH ASSEMBLY In order to foster greater correlation of the work of the Regional Committee with that of the global governing bodies, the Regional Committee adopted the practice in 1980 of reviewing the draft provisional agendas of the Executive Board and the World Health Assembly so that it could note important matters of regional and global interest. Regional Committees are urged to take an active part in the work of the Organization and to submit to the Executive Board their recommendations and concrete proposals on matters of regional and global interest. In accordance with the Rules of Procedure of the Executive Board, the provisional agenda of each session is drawn up by the Director-General in consultation with the Chairman of the Executive Board. It is despatched with the notice of convocation six weeks before the commencement of a session. Attached is the draft provisional agenda for the 109th session of the Executive Board. It includes items requested by the Health Assembly or the Board at a previous session, proposed by members of the Board and proposed by the Director-General. It is submitted to the Regional Committee for information. In accordance with the Rules of Procedure of the Health Assembly, the Executive Board prepares the provisional agenda of each regular session of the Health Assembly after consideration of proposals submitted by the Director-General. The draft agenda for the Fiftyfifth World Health Assembly will be available after review by the Executive Board in January 2002.
