SUMMARY
This second Informal Consultation on the Development of the Western Pacific Region Index Medicus (WPRIM) was convened as a follow-up to the first Informal Consultation on the Development of the Western Pacific Region Index Medicus, held in Manila on 30 - 31 March 2006 and the Meeting on the Development of the Western Pacific Region Index Medicus and the Global Health Library with China Collaboration, held in Beijing on 11 - 12 December 2006, to assess the progress and development of the WPRIM project in China, Japan, the Republic of Korea, and the Philippines and to prepare the agenda for the WPRIM meeting in October 2007. The status of WPRIM at the WHO Regional Office for the Western Pacific and in China, Japan, the Republic of Korea, and the Philippines were discussed. Several discussion points and issues were taken up which resulted in decisions and agreements, especially the proposed agenda for the next WPRIM meeting in Seoul on 21 - 23 November 2007, and list of recommendations. The expectations in the informal consultation were achieved and it can be considered as having successfully attained its objectives.

1. INTRODUCTION
The Western Pacific Region Index Medicus (WPRIM) is a project of the WHO Regional Office for the Western Pacific which has the primary objective to create an online regional medical index of selected medical/health journals published in countries covered by the WHO Western Pacific Region. The WPRIM database is currently being hosted at the Western Pacific Region Office server and initially populated with records of journal articles from the Republic of Korea and the Philippines. Other Member States are expected to send their journal records for uploading into the WPRIM database. The WPRIM will soon be deployed on the Internet, along with the regional index medici of other WHO Regions, in the Global Health Library (GHL) portal. An Informal Consultation on the Development of WPRIM was held from 30 to 31 March 2006 in Manila and the Meeting on the Development of WPRIM and the GHL with China Collaboration was held from 11 to 12 December 2006 in Beijing which resulted in the adoption of important decisions and identification, as well as implementation, of activities for the development and maintenance of WPRIM. This second informal consultation is convened as a follow-up to the consultation and meeting, to assess the progress and development of the WPRIM project in China, Japan, the Republic of Korea, and the Philippines and to prepare the agenda for the WPRIM-GHL meeting in October 2007. 1.1 Objectives The objectives of the consultation are: (1) to update participants on the various activities done in each country to implement the WPRIM project, discuss various issues and make some decisions relative to the development of the WPRIM database; and to identify relevant activities to be included in the agenda of the larger WPRIM meeting in October 2007.

Participants
The participants were medical librarians, medical journal editors and information professionals from selected countries in the Western Pacific Region who were designated as temporary advisers and observers.
7 1.3 Opening ceremony
On behalf of the National Selection Committee (NSC) for WPRIM Japan, Dr Kiichiro Tsutani welcomed the participants and asked each one to introduce himself/herself briefly. After the introduction, the officers of the meeting were elected. 1.4 Appointment of Chairperson, Vice-Chairperson and Rapporteurs The following appointments were unanimously approved: Chairperson: Dr Kiichiro Tsutani (Japan)

2.1
Status Report on the WPRIM and the GHL (Charles Raby)
Mr. Raby reported that a more powerful server will soon be acquired in the WHO Western Pacific Regional Office for the needs of the WPRIM and the Global Health Library (GHL). A new staff member has been recruited to work full-time on the WPRIM initiative and 146 journals from the Republic of Korea and 11 from the Philippines have been received and uploaded into the server. Communications between Western Pacific Region Office and Mongolia, as well as Australia, have also been established. The countries that have already established their National Journal Selection Committees have been identified – namely China, Japan, the Republic of Korea, Mongolia, the Philippines, and Viet Nam. The WPRIM portal website and the GHL site were shown and some of the links demonstrated. The importance of a GHL regional workshop was emphasized. Furthermore, the modules most relevant to the Region and the country should be identified and selected. A good mix of trainees from participating countries, including librarians and information technology people, must also be identified. An issue that was raised by Mr Raby was the agreement on a common platform because each individual country has its own special concerns. 2.2 Status Report: China (Mr Qian Qing)
China reported that the GHL-China Board was created during a meeting in March 2007. The list of the members was presented and it was also reported that the China Journal Selection Committee for the WPRIM was formed. The charters for both had also been drafted during those meetings. There was also a plan to expand the members of the GHL China Board, making it more nationwide as, at present, all of the participating agencies and individuals come from Beijing. There was a recommendation that the status of the association of medical or health journal editors from China be presented at the next meeting. 2.3 Status Report: Japan (Dr Kiichiro Tsutani) The members and officers of the Japanese National Committe for WPRIM (WPRIMJ), which
8 was established in November 2006, have been updated. The website of WPRIMJ (http://wprimj.umin.jp) was opened at the end of March 2007 for advocacy purpose in Japan. The criteria for journal selection (including minimal and additional criteria) were presented. A comment was made that one of the additional criteria, i.e. “journal should not be indexed in PubMed/Medline”, may have to be reconsidered for maximum exposure of the journal in the GHL and WPRIM portals. For other countries, this probably would not be productive. It was also mentioned that for clinical trials, there is now a WHO website where trials can be registered online. The current status of three clinical trial registry (CTR) systems in Japan and the international CTR platform (http://www.who.int/ictrp/) established by WHO Headquarters aimed at the global portal site, were discussed. Fifteen Japanese journals have already been selected for the WPRIM as of 17 May 2007. Two test files, including Japanese characters, were sent to WHO-WPRIM for possible inclusion in the WPRIM and technical problems with regards to Japanese characters are being solved. The Japanese model of Memorandum of Agreement, patterned after the Philippine model with some differences (in deference to Japanese customs/traditions), was presented although it was stated that the Japanese were hesitant to such documents. 2.4 Status Report: the Republic of Korea (Dr Jeong-Wook Seo)
The history and highlights of the development of the Korean Association of Medical Journal Editors (KAMJE) were briefly presented, as well as its structure and objectives since its establishment in 1996. The philosophy of KAMJE, with regards to journal evaluation (criteria), was presented in detail. The increasing numbers of articles in KoreaMed, the medical citation index in Korea, as well as its increasing number of site visitors in the past six years, were also presented. Future perspectives of KAMJE were also presented in detail and the desire to share its experience with editors at the national and international levels was also manifested. The overlapping and complex interactions and relationships between the online full text services of Korean journals were also presented. 2.5 Status Report: the Philippines (Dr Jose Avila)
It was reported that 13 journals have been uploaded to the WPRIM server and that the editors of these journals have met and organized themselves into a “technical panel” with officers elected and agenda/objectives stated. Initial meetings centered on the identification of general problems and the evaluation of the International Committee of Medical Journal Editors criteria (i.e. to adopt and revise some of the criteria for Philippine usage). It was also reiterated that the Philippine Council for Health Research and Development (PCHRD), an agency of the Department of Science and Technology, does the monitoring and indexing, and maintains a database for medical journal articles and health. 2.6 Expansion of the roles and functions of the WPRIM national journal selection boards/committees for the Global Health Library (1.) Each Member State has its own specific needs and concerns as far as the WPRIM and GHL are concerned. For example, a developing country, like the Lao People's Democratic Republic, probably needs the GHL more than the WPRIM. On the other hand, China, for example, may opt to expand the role of its national journal selection board for the GHL, combining elements of the WPRIM into it and vice-versa. For some countries, like the Philippines, the WPRIM could either be a subcommittee of the GHL or a separate one altogether. (2.) Each Member State should probably establish its own national portal. Sometimes, basic information is needed for a specific area. Also, it is up to the country to “adapt” this portal to what it wants – “share what you want to be shared.” For example, KoreaMed is open to everyone; some sectors may want to block some people from accessing certain information.
9 (3.) A “conceptual” rather than a “technical” approach should be used – the WPRIM should probably be considered as one component of the GHL. Eventually, Japan and the Republic of Korea agreed that the GHL and WPRIM should be separate with WPRIM as a “country platform.” China stated that WPRIM should be retained as it is for now and that there is no need to expand it just yet. This should just be limited to journal articles. Everyone agreed after the discussion that priority should be given to WPRIM for the time being and that the GHL could come in much later. 2.7 Creation of the Western Pacific Region Association of Medical Journal Editors (WPRAMJE)
The proposal for the creation of the Western Pacific Region Association of Medical Journal Editors (WPRAMJE) needed to be discussed further and will be on the agenda of the WPRIM Meeting in November 2007 in Seoul, Republic of Korea. 2.8 Mandatory and flexible journal selection criteria
A review of the journal selection criteria agreed upon in the Manila meeting (first informal consultation, March 2006) was made. The need to develop a formal document for these criteria was discussed. Individual countries should decide if they want to include additional criteria. For example, Japan wanted additional requirements for clinical trials. Thus, it was agreed to finalize the minimum criteria (mandatory) with a set of flexible criteria for each country to be given as the case warrants. It was decided that English abstracts are essential. Mr Raby accepted the task of finalizing this and shall send an e-mail to the respective countries. For countries where there is a WHO country office, it was deemed important for a WHO staff member (not full-time) to serve as a link to WPRIM. This is important for countries like the Lao People's Democratic Republic. Dr Wang suggested that quality of journal content, originality, downloading rate, impact factors, and editorial board composition be included as additional criteria. Mr Raby stated that this is probably applicable to China but not other countries, like the Lao People's Democratic Republic and the Philippines. Mr Raby asked how to deal with countries without or with few journals and what to do with the grey literature in those countries. He recommended that a "footnote" stating, "additional health information may be loaded into WPRIM based on evaluation by the WHO country office and WHO Regional Office" be included in the additional criteria. 2.9 Copyright
After much discussion, it was concluded that the agreement terms must be individualized for each country. In the Philippines, the memorandum of understanding/agreement (MOU/A) is similar to a “moral” agreement among gentlemen and involved the publisher (university, dean), the editor-inchief, the WPRIM and the Department of Science and Technology, among others. In China and Japan, a legal “contract” may be more appropriate, with contracts between the Western Pacific Region Office and the publisher. The Philippine model is more of a “school” or “university” model. Mr Raby stated that the important thing is facilitating access. He offered the opinion that every journal can not be forced to grant full-text access, although this would be ideal. Dr Tsutani suggested that if the publisher feels there is no need for it, then no written agreement or contract would be necessary. Mr Raby recommended that an MOU between WPRIM-WHO and the WPRIM national team is necessary to show that the team has the authority to deal on WPRIM issues with national
10 publishers. The Japanese National Committee for WPRIM, for example, represents WPRIM in Japan and shall have the authority to represent WPRIM in Japan in negotiations with publishers as well as in finalizing contract papers. Contracts could be in Japanese for easy understanding, with the MOU attached. The participants all agreed with this two-step approach and a draft of the MOU is to be prepared. After final posting of journal articles from publishers is achieved, it was recommended that WHO Western Pacific Region Office send a "letter of appreciation" to each publisher. A draft of this is to be prepared as well. Mr Raby promised to send them to the countries very quickly for evaluation. 2.10 Advocacy and fund-raising It was suggested that this should probably be part of knowledge management and sharing. There is a broad vision on what WHO is trying to do in this area. It should also be packaged in a more attractive way – that of sharing knowledge. It was stated that if you hide something you can not get support: this also applies to health information. If you do not share information with other countries, you can not learn and improve your own policies. KAMJE stated that it has annual dues/fees for members, but still has a limited budget. It was suggested that an “advocacy document” with a global vision be presented to potential donors as this might be more persuasive. Mr Raby offered his presentation to be part of the document. WPRIM is but one component of knowledge management. China now pays attention to health and medical research so there is a good chance that China will receive financial support. It was suggested further that WPRIM GHL China should participate in the global health forum from 29 October to 2 November 2007 in Beijing on the theme – "Equitable Access – Research Challenges in Developing Countries". The website was shown. WPRIM is following the same objectives as the forums. What to do for this advocacy: (1.) Know what you are doing and explain this to potential donors. (2.) Target potential donors: identify countries and potential donors who are already talking about knowledge management and who think that this is an important issue, such as the Bill Gates Foundation. Also, how donors are approached is important. Most practical course for WHO is to approach Member States like China, Japan, and the Republic of Korea which traditionally have not been approached before. Possible funding from donors, like the Toyota Foundation, in a manner similar to what was done by Southeast Asian Medical Information Center for the production of national Index Medici in several Asian countries, the Philippines and Singapore in the 1970s – 1980s, was given as an example by Dr Tsutani. For governance: the suggestion was to open original membership in WPRIM with focal persons to be identified for each country. There was a suggestion that the focal person should be the Chairperson of the country team for WPRIM.

Presentations
Dr Akihito Takano of National Institute of Informatics presented the work being done at the National Institute of Informatics (http://www.nii.ac.jp). The goal of research is to connect brain and cyberspace and the keyword of his research is "association". He presented this with a practical demonstration of website searches via association. Academic resources of the NII were presented via a DVD show. The infrastructure of the system was also shown. The Japan Science and Technology Agency (JST) was also presented in the mini-conference. A video was shown on JST activities. A speaker introduced the electronic journal system named J-STAGE (Japan Science and Technology Information Aggregator, Electronic) (http://www.jstage.jst.go.jp/browse/-char/ja) , a free platform for electronic journals of academic societies in Japan since October 1999, it currently hosts 375 journals. 2.13 Decisions and other matters agreed upon (1) Minimum (mandatory) criteria for journal selection for WPRIM were discussed and finalized as follows: 1. Journal has to be peer-reviewed, either internally or externally. 2. It must be regularly published, at least two issues per year. Initially, if the journal has only one issue per year, it will be accepted. However, it will be encouraged to increase its frequency to two issues in the succeeding year. 3. The quality of instructions to authors should be reviewed and accepted by the national WPRIM board/committee. 4. Journals should cover health and biomedical issues. 5. Only journals with English abstracts will be included. Note on country-specific criteria: The National Journal Selection Committee may decide to include country-specific criteria for journal selection for WPRIM. (2) Finalize National Journal Selection Committee (NSC) membership and report to Regional Journal Selection Committee (RSC): show list of members of the NSC on the website of WPRIM and contact addresses. (3) Country-specific selection criteria decided by the NSC will be reported to the RSC through Western Pacific Region Office.
13 (4) The task force for the selection of criteria for monographs and grey materials (to be led by China) will report recommendations during the WPRIM meeting in November 2007. (5) Dr Cho (KAMJE) will lead discussions on the creation of a regional editors' association. (6) Dr Tsutani will lead strategy and advocacy activities and act as adviser on issues such as ethics and clinical trials registration. (7) Library and IT issues will be managed by Western Pacific Region Office with the support of PCHRD (Philippines) and BIREME. (8) Dr Seo and Mr Raby will be working on the 2008-2009 workplan.
3. RECOMMENDATIONS
After the temporary advisers have thoroughly discussed and argued on the agenda and arrived at some important decisions and agreements on various matters relative to the development of WPRIM, the following recommendations are presented: (1) The next meeting will be convened in Seoul on 21 - 23 November 2007; (2) The status of the association of journal editors from China will be presented in the next meeting in Seoul; (3) Japan and the Republic of Korea will create a task force to look into the feasibility of establishing the Western Pacific Region Association of Medical Journal Editors (WPRAMJE); (4) A formal document on the finalized criteria for journal selection must be prepared; (5) A Memorandum of Understanding (MOU) between WHO-WPRIM and WPRIM national teams should be created to formalize the authority and credibility of the national teams in dealing with the publishers; (6) An advocacy document with a global vision should be prepared and presented to potential donors to heighten their support for WPRIM; (7) The focal person should be the Chairperson of the country team for WPRIM; (8) China will lead a task force to draft the selection criteria for monographs and grey literature.
