EXPANDED PROGRAMME ON IMMUNIZATION (EPI) Reported trends of EPI diseases in Europe T he WHO Regional Committee for Europe at its 1984 annual m eeting adopted as one of the targets (Target 5) for achieving health for all by the year 2000, the elim ination of indigenous measles, poliomyelitis, neonatal tetanus, congenital rubella and diphtheria from the Region. A conference of European countries which took place in December 1984 in Karlovy Vary, Czecho­ slovakia, form ulated th e following recom mendations related to the attainm ent of this target: “By 1990, diphtheria, indigenous poliom yelitis, and neonatal tetanus should be elim inated from all countries of the Region; By 1990, indigenous measles should be elim inated in countries where measles im m unization is already effective; in the rem ainder of the countries o f the Region, indigenous measles should be elim inated by 1995; By 2000, congenital rubella should be eliminated from all countries of the Region.” 1 T he first m eeting of the European Advisory G roup on EPI held in Copenhagen in Septem ber 1986 urged im plem entation of the elim ination targets specified in the regional health-for-all strategy and elaborated at th e Karlovy Vary Conference. T he following is a short review ofth e trends in 5 E PI diseases in the Region. T he incidence rates have been calculated from data provided by the WHO Regional O ffice for Europe as on November 1987. Fig. 1 shows incidence rates per 100 000 population due to tetanus, pertussis and diphtheria in some of th e European coun­ tries. T he left sections o f the figures show countries w ith high or increasing rates, w hile the right sections show countries reporting a d ear downward trend in th e disease inddence w ith single or no cases reported in recent years. ‘ T he reported diphtheria incidence increased in recent years in the USSR and Turkey. T he absolute num bers of diphtheria cases reported annually over the last 3 years exceed 1000 and 100 in those 2 countries, respectively. In Portugal, Italy and the Federal Republic of Germany the incidence of diphtheria decreased slowly. T he num ber of reported cases ranged from a few to several tens over the last 6 years.

In 13 countries, no single diphtheria case was reported for at least 3 years (Table 1). In some of them , diphtheria has not been reported for over 10 years. T he num ber of countries -reportingzero incidence was increasing at th e beginning of the 1980s, but is now constant, between 45% and 50% (Fig. 2). Pertussis incidence rem ains high in several countries, including those w hich do not use pertussis vaccine routinely in their infant im m unization programmes (Italy, Sweden), as well as in those w here such programmes are im plem ented (Denmark, USSR, U nited Kingdom). In countries w ith low incidence rates (Hungary, Poland), th e reported im m unization coverage w ith 3 doses of D PT vaccine was at least 90%.' H ie reported increase of pertussis incidence in the N etherlands may have been at least partly due to a change in the diagnosis of the disease. About 50% of countries reported pertussis incidence rates of 1 or less per 100 000 population (Fig. 2). N eonatal tetanus is not reported separately by many Member States. T he downward trend in tetanus incidence is very slow in all countries, mainly because the disease occurs among non-im m une elderly people. T he incidence rates are higher in southern coun­ tries than in those of central and northern Europe. Poliom yelitis incidence ranges from 0.05-0.1 per 100 000 (Turkey, Israel, USSR), to less than 0.01 per 100 000 (Spain, France, Poland) (Fig. 3). T hirteen countries have no t reported a single case of poliomyelitis for at least 3 years. Some o f them (Czechoslovakia, th e G erm an Dem ocratic Republic, Iceland, Luxembourg, M alta, and San M arino) have not reported cases of poliomyelitis for at least 10 years (Table 1). For measles also, significant differences in th e reported inci­ dence are observed between countries. In some countries the reported incidence is about or higher than 100 per 100000 (Denmark, Spain, U nited Kingdom, and USSR) w hile in some others (Czechoslovakia, N etherlands and Hungary) the reported incidence rate is below 1 per 100 000 population (Fig. 3). About 40% of countries reported measles incidence rates o f 10 or less per 100 000 (Fig. 2). T he data point out the need to accelerate the regional EPI and to intensify national programmes on im m unization in order to im plem ent the above-mentioned recom mendations of th e second conference on im m unization policies in Europe. 1 See No 23, 1986, pp. 174-176.

Le nom bre de pays faisant état d’une incidence zéro augm entait au début des années 80, mais il est m ainte­ nant constant, entre 45 et 50% (Fig. 2). L ’incidence de la coqueluche Teste élevée dans plusieurs pays, dans ceux qui n’em ploient pas systématiquem ent 1e vaccin anticoquelucheux dans leurs programmes de vaccination des nourrissons (Italie, Suède), comme dans ceux qui pratiquent de telles vaccinations (Danemark, Royaume-Uni, URSS). Dans les pays où l’incidence est peu élevée (H ongrie, Pologne), le taux de couverture vaccinale au moyen de 3 doses de vaccin D TC est d’au moins 90%.1 II se peut que l’augm entation de l’incidence de la coqueluche signalée aux Pays-Bas ait tenu en partie à un changem ent dans le diagnostic de cette m aladie.-Environ 50% des pays ont signalé des taux d’incidence pour la coqueluche d’un cas au plus pour 100000 habitants (Fig. 2). Dans de nombreux Etats Membres, le tétanos néonatal ne fait pas l’objet d’un rapport séparé. La tendance à la dim inution de l’incidence du tétanos est très faible dans tous les pays, principalem ent parce que cette m aladie se déclare chez les personnes âgées non immunisées. Les taux d’incidence sont plus élevés dans les pays du sud que dans les pays du centre et du nord de l’Europe. L’incidence de la poliom yélite se situe ente 0,05-0,1 pour 100 000 (Turquie, Israël, URSS), et moins de 0,01 pour 100 000 (Espagne, France, Pologne) (Fig. 3). T reize pays n’ont signalé aucun cas de polio­ m yélite depuis au moins 3 ans. Certains d’entre eux (M ande, Luxem­ bourg, M alte, République dém ocratique allem ande, Saint-M arin et Tchécoslovaquie) n’ont pas signalé de cas de poliomyélite depuis au moins 10 ans (Tableau 1). En ce qui concerne la rougeole, on observe égalem ent des différences im portantes des taux d’incidence notifiés selon les pays. Darts certains, ces taux sont d’environ 100 pour 100 000 habitants, ou supérieurs (Danemark, Espagne, Royaume-Uni et URSS), alors que dans d’autres (Hongrie, Pays-Bas et Tchécoslovaquie) l’incidence notifiée est infé­ rieure à 1 pour 100 000 habitants (Fig. 3). Environ 40% des pays ont signalé un taux d’incidence de 10 pour 100 000 au plus pour la rougeole (Fig. 2). Ces données font bien ressortir la nécessité d’accélérer le programme régional du PEV et d’intensifier les programmes nationaux de vaccina­ tion afin de m ettre en oeuvre les recom mandations m entionnées précé­ dem m ent et formulées lors de la deuxièm e Conférence européenne sur les politiques de vaccination. 1Voir N° 23, 1986, pp. 174-176.

In the week ending 5 M arch, 29 states reported regional or widespread activity of influenza-like illness. Influenza A(H3N2) virus had then been isolated in 46 states and the D istria of Columbia, influenza A(H1N1) in 12 states and influenza B in IS states. Only influenza A(H3N2) has been associated w ith more than sporadic activity and has been confirm ed in outbreaks among elderly residents in nursing homes throughout the country. >See No 9, 1988, p 63.
