VIRAL HEPATITIS C anada. — Over the years, the incidence o f viral hepatitis has followed a cyclical pattern. F o r Canada as a whole, there was one peak in 1954 and a second, higher peak in 1961-1962. The peak o f the current cycle cannot yet be identified. The incidence rate o f viral hepatitis per 100 000 population declined from 67.5 in 1961 and 1962 to 29.4 in 1966, and then increased to 40.0 m 1967 and 54.0 m 1968. The rate decreased to 511 in 1969 and rose to 57.1 in 1970, based on the provisional total num ber o f cases for th at year, The rates are calculated from the num ber o f cases of both serum and infectious hepatitis, which, since 1 January 1 9 0 , have been reported separately. Only 3 and 12 cases o f serum hepatitis were recorded in 1969 and 1970, respectively. The incidence o f viral hepatitis in Canada during the period 1966-1970 followed a general upward trend from the low level in 1966; there was an annual rise in the peaks and a corresponding rise in the low levels. The pattern o f case rate trends was consistent for Canada as a whole, but rather erratic and variable for the individual provinces. The incidence rates for British Columbia, Alberta, Saskatchewan, Prince Edward Island and Newfoundland were higher than those for Canada as a whole, whereas those for Ontario and Quebec were generally lower and those fo r the other provinces fluctuated In 1968 the m ain contributors to the epidemic pattern were Newfoundland, Prince Edward Island, New Brunswick, Saskatche­ wan, A lberta and B ritish Columbia. In 1969 the rates fell m Newfoundland, New Brunswick and Saskatchewan but increased in the other provinces mentioned above In 1970 the annual case rate fo r British Colum bia unproved (89 4), as did those for New­ foundland (139.4) and New Brunswick (21.2), while the rates for A lberta (97.4) and Saskatchewan (1113) were higher than in the previous year. The rate fo r Prince Edward Island (1 144 5) was the highest ever recorded for a f-anarfun province. In M anitoba, the rates increased from 40 5 in 1968 to 105.2 in 1969 and 216.5 in 1970. Throughout 1970, for Canada as a whole, the num ber o f hepatitis cases per week were consistently above the num ber for the corres­ ponding week m 1 9 0 . The situation fo r the provinces varied. F or Newfoundland, weekly cases were considerably lower m the first half o f 1970 than in th e corresponding period in 1 9 0 , but higher during th e rest o f the year. Prince Edward Island had weekly totals throughout 1970 th at were higher than m 1 9 0 .

M ore hepatitis cases were reported in N ova Scotia during the first few weeks of 1970 than in the corresponding period in 1969, sub­ sequently, the numbers did not vary greatly from those in 1969. Except for a short period near the end of 1970, weekly totals in Quebec were usually below those for the previous year. For A lberta, during 1970, weekly totals were m ore often above than below those for 1969, while for New Brunswick the 1970 weekly totals were more often below then above those for the preceding year In O ntario, weekly totals for the two years followed a similar pattern, w ith the cumulative to tal for 1970 slightly above th at for 1969. D uring the first 37 weeks o f 1970, the num ber o f viral hepatitis cases in M anitoba greatly exceeded those fo r the corre­ sponding weeks in 1969, thereafter, the two years were quite sim ilar In Saskatchewan, an outbreak o f viral hepatitis occurred during the first 12 weeks of 1970, after which the situation was generally better than during the previous year. British Colum bia recorded fewer cases for the first half of th e year m 1970 as com pared with 1969, but this situation was reversed during the second half o f the year. In m ost provinces the incidence o f viral hepatitis was high during the w inter m onths and fell to its lowest levels m the summer and early autum n. Newfoundland, New Brunswick, M anitoba, Saskatchewan, A lberta, B ritish Colum bia and Quebec report viral hepatitis cases by age and sex; the figures fo r 1970 are not yet available. In most o f these provinces the largest num ber o f cases during 1966-1969 occurred in the 20-39 year age-group However, in Newfoundland and New Brunswick in 1967-1969, and m Quebec in 1969, there were m ore cases in both the 5-9 and 10-14 year age-groups than in the 20-39 year age-group. Pertussis: It is estim ated th at at least 80 % o f th e infants receive a primary senes o f three injections o f D TP vaccine in the first year of life, and about 70% receive a booster during their second year. The annual num ber o f notifications, which represent about one third of the cases m edically attended, dropped from thousands in the 1950’s to hundreds in the early 1960*s and to a few dozens in the middle o f the decade In 1967, however, th e figure rose and over one thousand cases were notified. This was related, as in some other countries, to differences between the antigenic composi­ tion of the current strains and those used in the preparation o f the vaccine. Since the introduction o f a new vaccine the annual number o f cases has dropped again and fluctuates around a hundred. In 1970 and during the first m onths o f 1971, small outbreaks have been reported in different parts o f the country. M ost o f the Children affected had been vaccinated but before the use of the modified vaccine. The bacteriological investigations earned out m a fiw of these outbreaks have shown that they were usually due to

Bordetella pertussis. Trichinosis: This disease which had not been reported locally in the past, has been observed recently in a C hristian village m lower Galilee. The outbreak was traced back to wild hogs which had been bunted and eaten by the villagers.

Coqueluche: On estime qu ’environ 80% des nourrissons reçoivent une prem ière série de trois injections de vaccin D TC au cours de leur première année de vie, et que quelque 70% sont présentés pour une injection de rappel au cours de leur deuxième année. Les recherches bactériologiques effectuées lors de quelques-unes de ces poussées ont généralement mis en évidence Bordetella pertussis. Trichinose: Cette affection, qui n ’avait encore jam ais été notifiée en Israël, a été récemment observée dans un village chrétien de la basse Galilée. LEPTOSPIROSIS U n it ed K in g d o m . — The numbers of cases of leptospirosis reported by laboratories in England and Wales during the last ten years are shown m Table 1 F o r the last seven years the number has been about 30 to 40 cases a year M ore than half the cases were due to leptospira belonging to the Icterohaem orrhagiae serogroup and m ost o f the rem ainder to the Canicola serogroup. D uring 1970 there was a to tal o f 27 cases reported in England and Wales and another 13 m Scotland and Ireland. A m ore detailed analysis of these42 cases is shown in Tables 2 and 3. As in previous years, the great m ajority were in males, mostly adults of working age.

LEPTOSPIROSE R oyaume-U nl — Les cas de leptospirose signalés p ar les labo­ ratoires d’Angleterre-Galles au cours des dix dernières années sont indiqués au Tableau 1 Au cours des sept dernières années, le nomine annuel de cas a oscillé entre 30 et 40. Plus de la m oitié des cas était due à d e leptospires du sérogroupe Icterohaem orrhagiae et la plupart des aubes au sérogroupe Canicola. En 1970,27 cas on t été notifiés en A ngleterre-Galles et 13 autres en Ecosse et Irlande, des 42 cas sont analysés en détail dans les Tableaux 2 e t 3. The source o f infection is rarely known with certainty. However, o f those infected with the Icterohaem orrhagiae serogroup, eight were farm workers, whose occupation probably exposes them to various risks o f infection. F or example, one m an, who was also a horse tram er, had a history of having fallen off a horse into a stream that was probably rat-infested; another was a student who had been vaccinating sheep against o rf and attending pigs on a farm where there was a large increase o f rats A nother patient was a pest control officer, and one, a 64-year-old woman who died, was a fish cleaner. Three other patients were stated to have been swimming in rivers or ponds, and another, who worked as a builder, fished fo r eels in rat-infested w aters. Five o f these patients had jaundice, two had meningitis, one encephalitis, two mcningism, one pericarditis and for the rem ainder there was no clinical inform ation.

Three patients infected with the Canicola serogroup were farm workers and three were schoolchildren, one of whom bred rabbits. O f the other three patients one, a labourer, was a keen fisherman whose case history is given below; one was a m iner and the third a technician. Seven of the patients had meningitis, one had jaundice, one pyrexia and in two no clinical details were reported. A ll but two of the eleven patients with Hebdomadis serogroup infections were farm workers; one o f the others was a butcher and the o ther a schoolboy. N early all these patients had febrile mfluenza-like illnesses. M ore detailed case reports have been received for two o f the patients included above. The fiist was a 16-year-old boy who was adm itted to hospital w ith headache and vomiting for two days, thought to be due to meningitis. H e had bean swimming in rivers on several occasions in the previous three weeks. On examina­ tion he had slight jaundice, some conjunctival inflammation and spider naevi below the left eye and on both forearms. H is spleen

T rois autres sujets s'étaient baignés dans des rivières ou des étangs et un autre, qui travaillait dans le bâtim ent, avait pêché des anguilles dans des eaux infestées p ar les rats. Cinq des malades présentaient un ictère, deux uni méningite, un une encéphalite, deux du méningisme, un une péri­ cardite. On ne possède pas de renseignements cliniques sur les autres. Parm i les sujets infectés par le sérogroupe C anicola, il y avait trois ouvriers agricoles et trois écoliers, dont un élevait des lapins. Les trois autres étaient, l’un journalier, grand am ateur de péchi, dont le cas est décrit ci-dessous, le second m ineur et le troisième technicien. Sept des m alades ont eu une méningite, un un ictère, un une pyrexie et pour deux d’entre eux on n ’a pas reçu de renseigne­ m ents cliniques. N euf des onze sujets infectes par le sérogroupe Hebdomad» étaient des ouvriers agricoles; les deux autres étaient un boucher et un écolier. Presque tous ces malades ont présenté une affection fébrile analogue à la gnppe. D es renseignements plus détaillés sont parvenus au sujet de deux des malades. Le prem ier était un garçon de 16 ans, hospitalisé alan qu’il souffrait de céphalées e t de vomissements depuis deux jours, ce que l’on avait attribué i une méningite. O s’etait baigné dans des rivières à plusieurs reprises au cours des trois sem aines précédantes. Ceiebro-spmal fluid (CSF) pressure was 200 m m ; the CSF cell count, protein and glucose content were norm al and cultures were sterile, A white blood cell count was 7 700/c.mm (92% neutrophil polymorphs), serum bilirubin 1 5 mgm/10Q m l, SGPT14SF umts/ml, alkaline phosphatase 8.8 K A units O ther relevant investigations were negative and a provisional diagnosis o f leptospirosis was made. O n admission, the leptospira CF antibody titre was less than 1/4, b u t eight days later was 1/1024. Leptospires belonging to the Jcterohaem orrhagiae serogroup were isolated from the blood. T h e patient made a rapid and uneventful recovery w ithout anti­ biotic treatm ent.

La form ule leucocytaire était de 7 700/mm1 (92% de polynucléaires neutrophiles), la bili­ rubine sérique s’élevait à 1,5 mg/100 ml, la SGFT à 14 unités SF/m l et la phosphatase alcaline à 8,8 unités KA. Les autres épreuves effectuées ont donné un résultat négatif et on a posé un diagnostic provisoire de leptospirose Lors de l’hospitalisation, le titre d’anti­ corps FC était de moins de 1/4, mais huit jours plus tard ce titre était de 1/1024 Des leptospires du sérogroupe Icterohaem orrhagiae ont été isolés dans le sang. Le m alade a guén rapidem ent et sans complication après un traitem ent sans antibiotique.

The o ther case was in a m an aged 26 who was adm itted to hospital w ith a history o f interm ittent headaches, occasional dyspnoea and m uscular aches and pains fo r th.ee weeks. On admission he had a fever o f 40°C, b u t n o other abnorm ality was found on clinical « am in atio n. A specimen of his serum examined 16 days after his «dm»™»" showed leptospira antibody titres o f 1/640 to the Icteroh a m o rrb asu e serogroup and 1/80 to the Canicola serogroup. E ight days later th e titre to th e Icterohaem orrhagiae serogroup was alm ost tiie same (1/320) but th a t to th e Canicola serogroup had increased to 1/10240.
