ACUTE VIRAL HEPATITIS B U n it e d K in g d o m . — During the period July 1975-June 1976 inclusive, 1 061 reports of patients with acute hepatitis associated with hepatitis B surface antigen (HBsAg) were received from labor atories in England, Wales and Ireland. Since 1972, when HBsAgassociated hepatitis reporting began, the number of reports from England and Wales for corresponding yearly periods has increased steadily from 565 in 1972-1973 to 1 032 in 1975-1976. The increase probably reflects a growing clinical awareness of the possibility of type B infection and improved sensitivity of the available laboratory tests. Clinical details were given for 899 (85%) patients, of whom 868 were jaundiced: only 57 were reported to have one or more of the following: arthralgia, pruritus, or urticaria. These manifestations were observed more often among anicteric than icteric cases: five of 31 (16%) compared with 52 of 868 (6%). Despite the sustained upward trend in the number of cases reported over the years, the age and sex distribution of patients remained constant; males out numbered females in all age groups and they accounted for more than two-thirds of the total number of reports. The largest number of patients—438 (41 %)—was in the 15-24 year group, then 279 (26%) in the 25-34 year group. There was a sharp decrease, to 83 (8 %) in the 35-44 year group, and the numbers declined progressively as age increased. There were no infants and only 18 children aged under 15 years. O f the 1 061 patients, 227 (21%) were drug abusers, 46 (4%) had been treated with blood or blood products, and 19 (2%) had been tattooed within six months of the onset of illness, Drugabuse and tattoo-associated cases formed a large number and a greater proportion of the total than in any previous period, whereas there was no increase in the number of cases associated with blood or blood products. A further 51 (5 %) patients may have been infected in the course of their work. There were nine medical practitioners, 32 nurses, three laboratory workers, six ancillary medical staff, and one dentist. Ten nurses and four ancillary staff worked in hospitals for the mentally subnormal. One nurse worked in a renal unit but no other hepatitis B infections, either clinical or symptomless, were found among patients and staff either before or after the nurse’s illness. Of the 51, 13 reported probable sources of their infections. Four had accidentally inoculated themselves with material that contained HBsAg; three subsequently received anti-HBs immunoglobulin but the fourth, who was working abroad at the time, did not. Three

Depuis qu’on a commencé, en 1972, à signaler les cas d ’hépatite associée à l’HBsAG, le nombre des notifications d ’Angle terre et de Galles pour des périodes annuelles similaires a régulière ment augmenté, passant de 565 en 1972-1973 à 1 032 en 1975-1976. Le fait est probablement dû à la prise de conscience croissante par les cliniciens de la possibilité d ’une infection de type B et à l’amélio ration de la sensibilité des épreuves de laboratoire. Des détails cliniques ont été fournis pour 899 malades (85%), dont 868 ictériques; 57 seulement présentaient un ou plusieurs des symptômes suivants: arthralgie, prurit ou urticaire. Ces manifesta tions ontété plus fréquentes chez les sujets non ictériques que chez les autres ; cinq sur 31 (16 %) contre 52 sur 868 (6 %). Malgré l’augmenta tion continue du nombre des cas notifiés au cours des années, la répartition par âge et par sexe n ’avait pas changé; les hommes étaient plus nombreux dans tous les groupes d ’âge et représentaient plus des deux tiers des notifications totales. Le groupe 15-24 ans était le plus touché (438 (41 %)), suivi par le groupe 25-34 ans (279 (26%)). Un net fléchissement s’observait pour 35-44 ans (83 (8 %)) et le nom bre diminuait ensuite avec l’âge. H n ’y avait aucun nourrisson et 18 enfants seulement de moins de 15 ans.
Sur les 1 061 malades, 227 (21 %) étaient des toxicomanes, 46 (4 %) avaient reçu du sang ou des dérivés sanguins et 19 (2 %) avaient été tatoués dans les six mois précédents. Les toxicomanes et les tatoués formaient un groupe nombreux constituant une proportion plus grande du total qu’en aucune des périodes antérieures, alors que le nombre des cas associés à l’administration de sang ou de dérivés sanguins n ’avait pas augmenté. Dans 51 cas (5 %) — neuf médecins, 32 infirmières, trois agents de laboratoire, six auxiliaires médicaux et un dentiste — l’infection pouvait être liée au contexte professionnel. Dix infirmières et quatre auxiliaires travaillaient dans des hôpitaux pour déficients mentaux. Une infirmière travaillait dans une unité pour maladies rénales où il n ’y a eu aucun autre cas d ’hépatite B clinique ou asymptomatique chez les malades ou le personnel avant ou après la maladie de l’infirmière. Sur ces 51 personnes, 13 ont indiqué une source pro bable d’infection. Quatre s’étaient accidentellement inoculé du matériel contenant l’HBsAg; des immunoglobulines anti-HBs avaient été administrées à trois d ’entre elles mais non à la quatrième

had treated hepatitis B cases or carriers and a further three were aware that the infection was endemic in the institutions in which they worked, but none of the six reported an accidental inoculation or contamination. Three may not have acquired their infections at work but from the following sources at home: a spouse with acute hepatitis B, a carrier spouse, a household member with acute hepatitis B. The endemic pattern in institutions for the mentally subnormal persists. Though symptomless carrier patients continued to be identified, relatively few clinical infections were reported—21 (2%) among patients in 19 institutions. Some 76 patients (7 %) vere reported to have been in close contact with hepatitis or HBsAg carrier within six months of the onset of illness: of the probable sources of infection, there were 12 spouses, 27 sexual partners, six household members, and 31 unspecified. In one instance an infant was infected: in this small family outbreak a woman who gave birth three months after the onset of acute hepat itis B in her husband, herself developed an acute attack one month after the birth and the infant was found to be a symptomless HBsAg carrier. There were 45 (4%) patients who had received prophylactic or therapeutic injections but some of them had other histories which may have been equally or more relevant, such as visits abroad and acupuncture treatment. Five patients had no relevant histories other than having been abroad within six months of onset. Various histories reported for a further 28 patients may have been relevant—for example, surgical operation but no blood transfusion, venereal disease, homosexuality or patients with Down’s syndrome living at home. The reports for 82 patients (8%) indicated that thorough history taking had disclosed no known source of infection. In 461 patients (43 %) no epidemiological details were supplied. Ten patients died and hepatitis was stated as the cause of death in seven.

Lors d ’une petite poussée familiale, un nouveau-né a été infecté: sa mère, qui a accouché trois mois après l ’apparition de l’hépatite B aiguë chez son mari, a eu elle-même une atteinte aiguë un mois après l’accouchement et l’enfant s’est révélé porteur aysmptomatique d’HBsAg. Dans 45 cas (4%), les malades avaient reçu des injections prophy lactiques ou thérapeutiques, mais certains d’entre eux ont signalé des faits qui pourraient avoir eu autant d ’importance, voire davantage, par exemple des voyages à l’étranger ou des traitements par acu puncture. Cinq malades n ’avaient d ’autre antécédent qu’un voyage à l’étranger dans les six mois précédant la maladie. Pour 28 autres, diverses origines sont possibles: opération chirurgicale sans trans fusion sanguine, maladie vénérienne, homosexualité, présence au foyer d ’une personne présentant le syndrome de Down, etc. Dans 82 cas (8%), l’anamnèse n ’a révélé aucune source connue d ’infection. Aucune donnée épidémiologique n ’a été fournie pour 461 cas (43%). Dix malades sont morts et, pour sept d ’entre eux, l’hépatite a été indiquée comme cause de décès.

SURVEILLANCE OF FOOD POISONING F r a n c e . — Two outbreaks of food poisoning due to deep-frozen fish (globe-fish and burbot) imported from the Pacific region occurred in March and June 1977 in two departments in southern France. In the first outbreak (two cases with paraesthesia and vomiting) investigation led to the discovery in imported batches of fish of a toxin with activity of the same type as that of ciguatoxin. The enquiry concerning the second outbreak is still in progress (also two cases, with loss of balance and ataxia in addition).
SURVEILLANCE DES INTOXICATIONS ALIMENTAIRES F r a n c e . — Deux poussées d ’intoxications alimentaires dues à du poisson surgelé (tétrodon et lotte) importé de la région du Pacifique ont été observées en mars et en juin 1977 dans deux départements du sud de la France. Dans la première poussée (deux cas avec paresthé sies et vomissements), l ’enquête a mis en évidence une toxine pré sentant une activité du type ciguatoxine dans des lots de poissons importés examinés; l’enquête concernant la deuxième poussée est encore en cours (deux cas également, présentant en outre des pertes de l’équilibre et de l’ataxie).

SYNOPSIS OF THE WORLD MALARIA SITUATION IN 1976 A narrative summary of the global malaria situation was published in the Weekly Epidemiological Record No. 41 of 14 October 1977. No. 42 of 21 October contained in tabular form the malaria situation by large epidemiological areas for the period from January to December 1976. No. 43 of 28 October 1977 included a map of the world indicating the status as of December 1976, as well as informa tion on those originally malarious countries which have reported malaria-free areas during 1976 and the official register of areas where malaria eradication has been achieved. No. 44 of 4 November 1977 contained data on the semestrial follow-up of registration of malaria eradication, and No. 45 of 11 November 1977 information on countries reporting induced and imported cases by country of origin and by species of plasmodium. This issue includes a table on the status of susceptibility/resistance of P. falciparum to 4-aminoquinolines. Those readers particularly interested in the subject may obtain reprints consolidating the above-mentioned malaria information from the Division of Malaria and Parasitic Diseases.
SOMMAIRE DE LA SITUATION DU PALUDISME DANS LE MONDE EN 1976 Il a été publié dans le Relevé épidémiologique hebdomadaire N» 41 du 14 octobre 1977 un texte résumant la situation mondiale du palu disme et dans le N° 42 un tableau décrivant la situation du palu disme par grandes régions épidémiologiques pour la période de janvier à décembre 1976. Le N° 43 du 28 octobre 1977 contenait une carte du monde indiquant la situation en décembre 1976, des rensei gnements sur les pays primitivement impaludés qui ont notifié des zones sans paludisme en 1976, et le registre officiel des zones où l’éra dication a été réalisée. Le N° 44 du 4 novembre 1977 comportait des données sur le contrôle semestriel de l’enregistrement de l’éra dication du paludisme, et le N° 45 du 11 novembre 1977 des infor mations sur les pays ayant notifié des cas de paludisme provoqués et des cas importés, des données indiquant le pays d ’origine de l'infec tion et l’espèce de plasmodium responsable. Ce numéro contient un tableau comportant des renseignements sur l’état de sensibilité/résistance de P. falciparum aux amino-4 qui noléines. PERTUSSIS — Two recent outbreaks of pertussis, one in Maine, the other in Georgia, have been reported to the Center for Disease Control. Pertussis was diagnosed in a two-year-old girl from Bridgton, Maine, in April 1977 after a six-week history of cough. The child had been seen on several occasions as an outpatient and in the hos pital, where diagnoses of asthma, bronchitis, and cystic fibrosis were entertained before the diagnosis of pertussis was considered. Direct fluorescent antibody (FA) stain of a nasopharyngeal smear from the patient and from an ill sibling confirmed the diagnosis o f pertussis in both. Two other siblings, the parents, and a neigh bour’s child had also had a clinical illness compatible with per tussis. The children had all received the recommended number of immunizations for diphtheria and tetanus toxoids and pertussis vaccine (DPT) for their age. The cases were treated with ery thromycin, and an immunization clinic was held in the community. U nited S tates of A m erica .

Deux autres membres de la même fratrie, les parents et un enfant du voisinage présentaient d ’ailleurs des signes cliniques pouvant être dus à la coqueluche. Tous les enfants avaient reçu le nombre recommandé pour leur âge d ’injections de vaccin triple antidiphtérique/antitétanique/anticoquelucheux. Les cas ont été traités à l’érythromcycine et des séances de vaccination ont été organisées dans la localité.

Another outbreak of pertussis occurred among students of a Decatur, Georgia, elementary school over a five-week period in May and June 1977. Twenty-six of the school’s 580 students had a clinical syndrome of fever and catarrhal symptoms followed by prolonged cough, as did four preschool siblings o f sick children. None developed clinical pneumonia or required hospitalization, and most had a relatively mild cough. Twenty-six of the 30 cases were in students in the third grade or in their contacts. Nasopharyngeal swabs were obtained for culture and FA staining from 28 ill school children and their siblings. Bordetella pertussis was isolated from six children; B. pertussis was identified by FA staining in one culture-positive child and in three other children.

Chile. — One strain from Chile reacted mostly with antiserum A/Texas/1/77.1 Canal Zone o f Panama. — Four strains from the Canal Zone reacted mostly with antiserum A/Texas/1/77 (isolation of strains started during the last week of May, reached a maximum frequency in June and ended in mid-July). Keeping in mind the results already published in the WER on the characterizations carried out in Atlanta for strains isolated in other countries in 1977, it may be said that strains reacting mostly with antiserum A/Texas/1/77 have been isolated in most regions of the world, and in particular in the following countries: Australia; Marshall Islands (Majuro Atoll); Thailand, the Philippines, Sin gapore, China (Taiwan Province), and Japan; Argentina and Chile; Canal Zone of Panama, Trinidad and Tobago, and Puerto Rico; Canada and the United States of America (including one recent isolate, obtained from Oregon in September). No other variant was isolated among the strains sent to Atlanta from Marshall Islands, Japan, Chile, Canal Zone of Panama and Trinidad and Tobago, whereas elsewhere evidence of infection with virus A/ Victoria/3/75 was also obtained. 1 This strain was related but not similar to A/Victoria/3/75 as was indicated by error in No. 38. p. 305.
