1WHO guideline on health workforce development, attraction, recruitment and retention in rural and remote areas: a summary With nearly half of the world’s population living in a rural or remote area (1), meeting the health needs of rural populations, where over 80% of the world’s extremely poor live, is imperative in achieving universal health coverage. Leaving no one behind means ensuring that health workers are available in rural and remote areas. Health, social and economic inequities remain cross-cutting challenges for rural populations. Rural populations tend to be poorer, have worse health outcomes, and experience higher rates of unemployment, underemployment and informal employment (2, 3). It is estimated that about 51–67% of rural populations are without adequate access to essential health services (4), translating to about 2 billion people being left behind. In some countries, rural populations have access to numbers of health workers that are 10 times less than the numbers available to urban populations. The deficiency in numbers and mix of trained motivated health workers to provide the needed health services is a critical health system issue. This inequitable access to health workers and health services impacts health outcomes and increases socioeconomic disadvantages. Higher under-5, maternal and preventable mortality rates, increased morbidity, decreased life expectancy, and more costs to access distant care are seen across rural areas (5–8). A persistent health system challenge There is a mismatch between the need for, demand for and supply of health workers in rural areas. While needs are high, underinvestment has weakened demand for health workers in rural and remote areas and resulted in insufficient numbers of health workers available for rural practice. WHO guideline on health workforce development, attraction, recruitment and retention in rural and remote areas: a summary 2 A need for action and an opportunity for inclusive economic development The Sustainable Development Goals and universal health coverage cannot be achieved without giving due consideration to rural and remote areas and populations. The disruptive health, health system and socioeconomic impacts of the COVID-19 pandemic further threaten vulnerable rural populations. Unequal access to health services jeopardizes national and global health security, inclusive socioeconomic recovery and development. On the road to recovery there is an opportunity to invest in rural health jobs and empower rural communities, in particular youths and women. What the guideline presents This guideline is an update of the 2010 WHO guideline Increasing access to health workers in remote and rural areas through improved retention: global policy recommendations. It presents 17 evidence- based interventions to improve access to a skilled and motivated rural health workforce. The guideline presents a pathway for reversing both the current and predicted worsening shortage of health workers in rural and remote areas through: • protecting the existing rural health workforce, which will aid their retention; • investing in the development and training of multidisciplinary fit-for- purpose health teams and in the attraction and recruitment of health workers. To achieve gains, a whole-of-government, whole-of-society approach involving different sectors and stakeholders, along with community engagement, will be necessary in selecting, monitoring and evaluating the impact of a contextually relevant bundle of interventions. Through proactive evidence-based interventions, policy-makers can ensure that equitable access to health workers in rural areas is achieved and that no one is left behind. 1Orientações da OMS para o desenvolvimento, atração, recrutamento e retenção da força de trabalho da saúde em zonas rurais e remotas: resumo Com quase metade da população mundial vivendo em zonas rurais e remotas (1), é inevitável satisfazer as necessidades de saúde das populações rurais, onde vivem mais de 80% das pessoas em situação de pobreza extrema, para que se possa atingir a cobertura universal de saúde. Não deixar ninguém para atrás significa garantir a existência de profissionais de saúde nas zonas rurais e remotas. As desigualdades na saúde, sociais e económicas continuam a ser desafios transversais para as populações rurais. As populações rurais tendem a ser mais pobres, ter piores resultados na saúde e ser afetadas por taxas de desemprego mais elevadas, subemprego e emprego informal (2, 3). Estima-se que cerca de 51–67% das populações rurais não tenham acesso adequado a serviços de saúde essenciais (4), o que significa que, aproximadamente, 2 mil milhões de pessoas são deixadas para trás. Em alguns países, as populações rurais têm acesso a um número de profissionais de saúde que é 10 vez menor do que o número disponível par as populações urbanas. A insuficiência do número e a combinação de profissionais de saúde devidamente formados e motivados para prestar os serviços de saúde necessários é um problema crítico dos sistemas de saúde. Este acesso desigual a profissionais e serviços de saúde tem impacto sobre os resultados na saúde e agrava as desvantagens socioeconómicas. Um desafio persistente aos sistemas de saúde Existe um desequilíbrio entre a necessidade, a procura e a disponibilidade de profissionais de saúde nas zonas rurais. Embora as necessidades sejam elevadas, o sub-investimento enfraqueceu a procura de profissionais de saúde nas zonas rurais e remotas, o que resulta num número insuficiente de profissionais de saúde disponíveis para a prática clínica nas zonas rurais. 2Orientações da OMS para o desenvolvimento, atração, recrutamento e retenção da força de trabalho da saúde em zonas rurais e remotas: resumo Necessidade de ação e oportunidade para o desenvolvimento económico inclusivo Os Objetivos do Desenvolvimento Sustentável e a cobertura universal de saúde não podem ser atingidos, sem atribuir a devida consideração às zonas e populações rurais e remotas. Os impactos disruptivos sobre a saúde, os sistemas de saúde e socioeconómicos da pandemia de COVID-19 ameaçam ainda mais as populações rurais vulneráveis. O aceso desigual aos serviços de saúde coloca em risco a segurança da saúde a nível nacional e mundial, inclusive a recuperação socioeconómica e o desenvolvimento. No caminho para a recuperação existe uma oportunidade para investir em empregos na saúde em zonas rurais e para capacitar as comunidades rurais, em particular os jovens e as mulheres. O que apresentam as orientações Estas orientações são uma atualização das orientações da OMS de 2010 sobre o Aumento do acesso a profissionais de saúde nas zonas rurais e remotas através de uma melhor retenção: recomendações para políticas globais. Apresentam 17 intervenções baseadas em evidências para melhorar o acesso a uma força de trabalho qualificada e motivada nas zonas rurais. موجز المبادئ التوجيهية لمنظمة الصحة العالمية بشأن تنمية 1 القوى العاملة في مجال الصحة واجتذابها وتوظيفها والاحتفاظ بها في المناطق الريفية والنائية: ملخـص يعيش ما يقرب من نصف سكان العالم في إحدى المناطق الريفية أو النائية (1)، لذا فإن تلبية الاحتياجات الصحية لسكان الريف، حيث يعيش أكثر من 08 في المائة من سكان العالم الذين يعانون من الفقر المدقع، أمر حتمي لتحقيق التغطية الصحية الشاملة. ويعني عدم ترك أي أحد خلف الركب لضمان توافر العاملين الصحيين في المناطق الريفية والنائية. لا تزال أوجه الإجحاف في المجالات الصحية والاجتماعية والاقتصادية تشكل تحديات شاملة لسكان الريف. فسكان المناطق الريفية بالأمراض، وانخفاض متوسط العمر المتوقع، وزيادة تكاليف الحصول على الرعاية عن بُعد عبر أنحاء المناطق الريفية (5-8). تحدٍّ مستمر ّللنظم الصحية هناك عدم تطابق بين الحاجة إلى العاملين الصحيين في المناطق الريفية، والطلب عليهم والأعداد المتاحة منهم. رغم ارتفاع الاحتياجات، فإن نقص الاستثمار أضعف الطلب على العاملين الصحيين في المناطق الريفية والنائية، وأدى إلى عدم كفاية أعداد العاملين في المجال الصحي 1Руководство ВОЗ по развитию, привлечению, набору и удержанию медицинских кадров в сельских и отдаленных районах: резюме Поскольку почти половина населения мира проживает в сельских или отдаленных районах (1), удовлетворение потребностей в услугах здравоохранении в сельской местности, где проживает более 80% крайне бедного населения мира, является обязательным условием достижения всеобщего охвата услугами здравоохранения. Призыв «Не оставить никого без внимания» означает необходимость обеспечить наличие медицинских работников в сельских и отдаленных районах. Неравенство в области здравоохранения, а также в социальной и экономической сферах остается сквозной проблемой для сельского населения. Сельское население, как правило, беднее, имеет более низкие показатели здоровья и более высокий уровень безработицы, неполной занятости и неформальной занятости (2, 3). По оценкам, около 51-67% сельского населения не имеют адекватного доступа к основным услугам здравоохранения (4), что означает, что около 2 миллиардов человек остаются без внимания. В некоторых странах доступ сельского населения к медицинским работникам в 10 раз меньше, чем у городского населения. Нехватка обученных мотивированных медицинских работников и их неполный состав для оказания необходимых медицинских услуг является одной из важнейших проблем системы здравоохранения. Такой неравный доступ к медицинским работникам и медицинским услугам влияет на результаты здравоохранения и усиливает социально-экономическое неблагополучие. Более высокие показатели смертности детей в возрасте до 5 лет, материнской и предотвратимой смертности, повышенная заболеваемость, снижение продолжительности жизни и повышенные затраты на доступ к отдаленным услугам здравоохранения наблюдаются во всех сельских районах (5-8). Постоянные проблемы систем здравоохранения Существует несоответствие между потребностью в медицинских работниках, спросом на их услуги и предложением этих услуг в сельских районах. Хотя потребности высоки, из-за недостаточного инвестирования спрос на услуги медицинских работников в сельских и отдаленных районах снизился, и это привело к тому, что в сельской местности работает недостаточное количество медицинских работников. Руководство ВОЗ по развитию, привлечению, набору и удержанию кадров здравоохранения в сельских и отдаленных районах: резюме 2 Необходимость действий и возможность для всеобъемлющего экономического развития Цели устойчивого развития и задачи Всеобщего охвата услугами здравоохранения не могут быть достигнуты, если не уделять должного внимания сельским и отдаленным районам и сельскому населению. Разрушительные последствия пандемии COVID-19 для здоровья людей, систем здравоохранения и социально-экономической сферы еще больше угрожают уязвимому сельскому населению. Неравный доступ к услугам здравоохранения ставит под угрозу национальную и глобальную безопасность здравоохранения, а также усилия по всеобъемлющему восстановлению социально-экономического положения и развития. На пути к восстановлению есть возможность инвестировать в создание рабочих мест в сфере сельского здравоохранения и  расширять возможности сельских сообществ, в частности молодежи и женщин. Что предлагается в данном руководстве Данное руководство является обновленным изданием Руководства ВОЗ Расширение доступа к работникам здравоохранения в отдаленных и сельских районах посредством совершенствования системы сохранения кадров. Рекомендации по глобальной политике, опубликованного в 2010 г. В нем представлены описания 17 вмешательств с доказанной эффективностью, направленных на расширение доступа к подготовленным и мотивированным кадрам сельского здравоохранения. В руководстве предлагается путь для обращения вспять нынешней и прогнозируемой ухудшающейся ситуации с нехваткой медицинских работников в сельских и отдаленных районах посредством осуществления следующих мер: • защита существующих кадров сельского здравоохранения, что будет содействовать их удержанию на рабочем месте; • инвестиции в развитие и обучение многопрофильных медицинских бригад, отвечающих поставленным целям, а также в привлечение и набор медицинских работников. Для достижения положительных результатов при выборе, мониторинге и оценке воздействия комплекса мероприятий c учетом конкретных условий, потребуется подход, основанный на принципах общегосударственного подхода и вовлечения всего общества с участием различных секторов и заинтересованных сторон, а также вовлечение сообществ. Благодаря проведению инициативных мероприятий, основанных на фактических данных, лица, отвечающие за выработку политики, могут обеспечить равный доступ к услугам медицинских работников в сельской местности и гарантировать, что никто не будет оставлен без внимания.
