Yellow fever in Présidente Prudente, Sâo Paulo B r a z i l . - The city of Présidente Prudente, a centre of intensive agricultural activity in the western region o f the state o f Sâo Paulo, is situated on the route that connects the city of Sâo Paulo with Cuiaba and Campo Grande in the State of Mato Grosso, The infestation of this city by Aedes aegyplt, detected m April 1985, and the considerable movement of people toward areas where jungle yellow fever is endemic increase the vulnerability o f Présidente Prudente to urban yellow fever. Two truck drivers who had been travelling together in the State of Mato Grosso since 4 May 1985 returned sick to Présidente Prudente on 20 May. Their illness with symptoms of fever, lum­ bago, and arthralgia had started on 18 May. One of them recovered within days of his arrival, but the other was hospitalized on 21 May with a tentative diagnosis of hepatitis or malaria. In the hospital the patient presented jaundice, vomiting (sometimes of blood), tarry stools, and acute renal and hepatic insufficiency. He died on 24 May. The same day another truck driver, from the same region of Mato Grosso, presented fever, headache, nausea, vomiting, and muscle aches. His condition worsened in the next few days, and on 27 May he was hospitalized in Présidente Prudente, The patient was later transferred to the hospital in Sâo Paulo, where he died on 1 June of yellow fever, the diagnosis of which was then con­ firmed.1 The presence of yellow fever patients m the viraemic phase m an Aedes aegypti-iaSested city, most of whose population was not immunized against the disease, posed a major threat o f urban yellow fever which has not occurred in Brazil for the past 43 years. The health authorities considered the situation an emergency, and, as soon as they had the confirmed diagnosis of the first case, initiated a campaign against Ae. aegypu that included the appli­ cation of organophosphorus insecticides and mass vaccination of the populauon, in 5 days 165 830 persons were vaccinated—the whole population of Présidente Prudente along with thousands of persons in neighbouring cities. To date, no indigenous cases have been reported. With the spread oiAe. aegypu to different areas of the country and the great movement of people in the Amazon region toward some of these areas, it is possible that other incidents similar to the one in Présidente Prudente will occur. ‘ See No. 25, 1985, p 196.

FIÈVRE JAUNE Fièvre jaune à Présidente Prudente, Sâo Paulo B r é s il . - L’agglomération de Présidente Prudente, centre d ’une intense activité agricole dans l’ouest de l’Etat de Sâo Paulo, est située sur la voie qui relie la ville de Sâo Paulo à celles de Cuiabâ et Campo Grande dans l’Etat du Mato Grosso. L’mfestation de Présidente Prudente par Aedes aegypu, décelée en avril 1985, ajoutée aux importants mouvements de population vers des secteurs où la fièvre jaune de brousse est endémique, augmente le risque d’exposition de cette ville à la fièvre jaune urbaine. Deux chauffeurs de camion qui voyageaient ensemble dans l’Etat du Mato Grosso depuis le 4 mai 1985 sont rentrés malades le 20 mai à Présidente Prudente. La maladie avait commencé le 18 mai par de la fièvre, des lombalgies aiguës et des arthralgies. L’un s’est rétabli dans les jours qui ont suivi son arrivée mais l’autre a été hospitalisé le 21 mai avec le diagnostic provisoire d’hépatite ou de paludisme. Pendant son hospi­ talisation le malade a présenté un ictère, des vomissements (contenant parfois du sang), des selles noirâtres et une insuffisance rénale et hépa­ tique aiguë. Il est décédé le 24 mai. Le même jour, un autre chauffeur de camion, revenant de la même région du Mato Grosso, présente de la fièvre, des céphalées, des vomissements et des myalgies. Son état s’aggrave au cours des jours suivants et le 27 mai il est hospitalisé à Présidente Prudente. Le patient a par la suite été dirigé sur le centre hospitalier de la ville de Sâo Paulo où il est m on le 1erjuin de fièvre jaune, diagnosuc qui a été alors confirmé.1 La présence de malades atteints de fièvre jaune en phase virémique dans une agglomération infestée par Aedes aegypu où la plus grande partie de la population n’était pas vaccinée contre la fièvre jaune, a fait peser une très lourde menace de fièvre jaune urbaine, que Ton n’avait pas vue au Brésil depuis 43 ans. Les autorités sanitaires ont estimé que la situation était grave et dès qu’elles ont eu confirmation du diagnostic du premier cas elles ont entrepris une campagne antivectorielle contre Ae. aegypu avec notamment épandage d’insecticides oiganophosphoiés et vaccina­ tion massive de la populatioa En 5jours, on a vacciné 165 830 personnes, la totalité de la population de Présidente Prudente, ainsi que plusieurs milliers de personnes des agglomérations voisines. Jusqu’à ce jour aucun cas indigène n’a été notifié. Avec la propagauon d 'Ae. aegypu dans différentes régions du pays et l’importance dies mouvements de population dans la région amazonienne vers certains de ces secteurs, il n’est pas exclu que d’autres incidents semblables à celui de Présidente Prudente se produisent.
