POLIOMYELITIS SURVEILLANCE H onduras. — The week of 6 November 1978 was the apparent onset of an outbreak of poliomyelitis in Honduras, with a total of 40 cases occurring up to the ena o f 1978 and 173 cases up to the 26th week of 1979. The following is a description of the evolution of the outbreak in 1979 and a discussion of the current situation.
SURVEILLANCE DE LA POLIOMYÉLITE H onduras. — C ’est dans la semaine du 6 novembre 1978 qu’on a décelé les premiers signes d ’une poussée de poliomyélite au Honduras, avec un total de 40 cas jusqu’à la fin de 1978 et de 173 cas dans les 26 premières semaines de 1979. On trouvera ci-après une description de l'évolution de cette poussée en 1978, puis le point de la situation actuelle. La Figure 1 indique la répartition hebdomadaire des 173 cas signalés pendant les 26 premières semaines de 1979.
The weekly distribution of the 173 cases reported during the first 26 weeks of 1979 is shown in Figure 1.

Distribution of the cases by age is presented in Table 1. The youngest case occurred in a child of two months and the oldest in a ten-year old. Seventeen cases occurred in children younger than six months.
Le Tableau 1 donne la répartition des cas par groupe d ’âge. Le plus jeune enfant atteint avait deux mois et le plus âgé dix ans. Dix-sept malades avaient moins de six mois.

Between January and June 1979 a total of 600 000 doses o f polio vaccine were distributed to the Health Regions; provisional data indicate that 122 435 doses were administered, with the age dis tribution shown in Table 3. E d it o r ia l N o t h : Based on the figures given in Table 3 for the number o f persons vaccinated in each age group, it can be calculated that 12.3% (15 083 persons) of the total number of persons who received three doses of polio vaccine (122 435) were under one year of age; 22.7 % (27 874) were between one and two years of age; 46.8% (57 203) were between two and four years of age; and 18.2% (22 275) were over five years of age. Comparing these figures with those given in Table 1, it can be seen that whereas 68% of the polio cases have occurred in children younger than two, only 35% o f the total number o f persons vaccinated were under two years o f age.

N o i e d e l a R é d a c t io n : D ’après les chiffres du Tableau 3 (nombre de personnes vaccinées, par groupe d ’âge), on peut calculer que 12,3% (15 083 personnes) de l’ensemble des personnes ayant reçu trois doses de vaccin antipoliomyélitique (122 435) avaient moins d ’un an, 22,7% (27 874) entre un et deux ans, 46,8% (57 203) entre deux et quatre ans, et 18,2% (22 275) plus de cinq ans. En comparant ces chiffres à ceux du Tableau I , on peut voir que si 68 % des cas de poliomyélite se sont produits chez des enfants de moins de deux ans, par contre 35% seulement de l’ensemble des personnes vaccinées avaient moins de deux ans.

As shown in Table 3, the 15 083 vaccinations given to children under one year old represent little more than 10% of this group, although it is the age group at highest risk. A change of vaccina tion strategy would be advisable, with the aim of giving greater priority to the under-two age group, in accordance with the recom mendations o f the Expanded Programme on Immunization in the Americas.
Comme le montre le Tableau J, les 15 083 enfants de moins d'un an vaccinés représentent à peine plus de 10% de ce groupe d ’âge, bien que ce soit le plus exposé. 1 1 serait souhaitable d ’adopter une nou velle stratégie de vaccination, en donnant une plus grande priorité aux moins de deux ans, conformément aux recommandations du Programme élargi de vaccination dans les Amériques.
