Dracunculiasis Certification of eradication The International Commission for the Certification of Dracunculiasis Eradication (ICCDE) held its second meeting at WHO headquarters on 23 and 24 January 1997.1 This independent commission, established by WHO in 1995, is made up of 12 public health experts representing the 6 WHO Regions and the principal organizations participating directly in the global dracunculiasis eradication campaign: the World Bank, the United Nations Development Programme (UNDP), the United Nations Childrens Fund (UNICEF) and Global 2000. The ICCDE reviewed requests for certification from 22 countries. On the basis of its certification criteria, and of its analysis of information submitted by these countries and by the International Certification Teams (ICTs), the ICCDE proposed to the Director-General of WHO that 21 candidate countries be certified as being free of dracunculiasis transmission.
Dracunculose Certification de léradication La Commission internationale pour la certification de léradication de la dracunculose (CICED) sest réunie pour la deuxième fois les 23 et 24 janvier 1997 au Siège de lOMS.1 Cette Commission indépendante, créée en 1995 par lOMS, est composée de 12 experts en santé publique représentant les 6 Régions OMS ainsi que les principaux organismes participant directement à la campagne mondiale déradication de la dracunculose: la Banque mondiale, le Programme des Nations Unies pour le Développement (PNUD), le Fonds des Nations Unies pour lEnfance (UNICEF) et Global 2000. La CICED a examiné les demandes de certification de 22 pays. Sur la base des critères de certification et de lanalyse des renseignements fournis par les pays et par les équipes internationales de certification (EIC), la CICED a proposé au Directeur général de lOMS la certification de 21 pays candidats comme étant «libres de transmission de la dracunculose».
African Region Africas most important objective is to interrupt dracunculiasis transmission as quickly as possible. This continent alone accounts for 99% of the cases notified in 1996 (occurring in Benin, Burkina Faso, Cameroon, Chad, Côte dIvoire, Ethiopia, Ghana, Mali, Mauritania, Niger, Nigeria, Senegal, Sudan,2 Togo and Uganda). The endemic foci are all to be found in the countries south of the Sahara. The countries of the North, along with those of southern Africa, could accordingly be candidates for certification at the next meeting of the Commission.

Region of the Americas The history of dracunculiasis in the WHO Region of the Americas shows that in past centuries the disease was well known, at least in the tropical area of the Atlantic coast and in the neighbouring islands. Nordeste region of Brazil, there were small indigenous foci of transmission. However, the vast majority of cases were imported, mainly from the western African coast. At the first meeting of the Commission in March 1996, the report presented by Cuba was proposed as a model for other countries in the Region. The report stated that 12 cases imported from Africa had been identified between 1979 and 1992. The Commission had requested further information concerning the risk of indigenous foci developing from those cases. In December 1996, WHO sent a consultant to assess this risk. The data collected showed that it was almost zero. The Commission was fully satisfied with the evidence and proposed Cuba for certification, congratulating that country on the health information network set up by its authorities. Complete applications had been submitted by another 5 countries. These requests were judged to be satisfactory, and the Commission proposed the following countries for certification: Barbados, Brazil, Colombia, the Dominican Republic and Trinidad and Tobago.

Lors de la première réunion de la Commission en mars 1996, le rapport présenté par Cuba avait été proposé comme modèle aux autres pays de la Région. Il y était mentionné le dépistage de 12 cas importés dAfrique entre 1979 et 1992. La Commission avait demandé un supplément dinformation concernant le risque de développement de foyers autochtones à partir de ces cas. En décembre 1996, lOMS a envoyé un consultant pour évaluer ce risque. Les informations recueillies ont montré que ce risque était quasiment nul. Cela a pleinement convaincu la Commission, qui a proposé Cuba pour la certification et a exprimé ses félicitations pour le réseau dinformation sanitaire mis en place par les autorités cubaines. Cinq autres pays avaient soumis un dossier complet. Ces demandes ayant été jugées satisfaisantes, la Commission a proposé pour la certification les pays suivants: la Barbade, le Brésil, la Colombie, la République dominicaine et la Trinité-et-Tobago.
Eastern Mediterranean Region There were 3 candidate countries: Egypt, the Islamic Republic of Iran and Pakistan. In Pakistan, the last case was reported in October 1993. The risk of reintroducing the disease is negligible because India, the only endemic country bordering on Pakistan, has almost reached zero incidence. Moreover, a surveillance system reinforced by a considerable monetary award for any patient coming forward with an emerging guinea-worm has been set up and maintained to the present day by the Guinea-Worm Eradication Programme. The Commission was satisfied with the information submitted by the national authorities and by the ICT, and Pakistan was proposed for certification, thus becoming the first endemic country to have been certified as dracunculiasis-free in the course of the world eradication campaign which began in the 1980s. The Islamic Republic of Iran had not reported any cases of dracunculiasis since the mid-1970s. This was confirmed by the report of a WHO consultant working in conjunction with the national health authorities, and by an ICT visit. The analysis of the reports, the extreme reliability of the health information system and the existence of a reward for any confirmed cases all served to convince the Commission which proposed the Islamic Republic of Iran for certification. Egypt has made an official request accompanied by a historical study showing that no cases of dracunculiasis had been observed for at least 2 centuries. However, the Commission felt that Egypts shared border with Sudan, the country the most affected by dracunculiasis, entailed the risk of the disease being imported. It was proposed that an ICT be sent to assess this risk. The Eastern Mediterranean Region has 2 endemic countries: Sudan, which notified the highest number of cases in 1996 (107 693 cases from January to November, i.e. 77% of all cases), and Yemen with 62 cases in 1996. 1
Région de la Méditerranée orientale Trois pays étaient candidats: lEgypte, la République islamique dIran et le Pakistan. Au Pakistan, le dernier cas a été enregistré en octobre 1993. Les risques de réintroduction de la maladie sont négligeables car lInde, seul pays dendémie limitrophe, a presque atteint lincidence zéro. De plus, un système de surveillance renforcé par une forte récompense en argent pour tout malade se présentant avec un ver de Guinée en train de sortir a été instauré et maintenu jusquà ce jour par le Programme déradication du ver de Guinée. Lanalyse des renseignements fournis par les autorités nationales et par lEIC a donné satisfaction à la Commission, qui a proposé le Pakistan pour la certification. Le Pakistan devient ainsi le premier pays dendémie certifié libre de dracunculose au cours de la campagne mondiale déradication dont le début remonte aux années 80. La République islamique dIran navait plus signalé de cas de dracunculose depuis le milieu des années 70. Cela a été confirmé dans le rapport dun consultant de lOMS qui a travaillé en collaboration avec les autorités sanitaires nationales et par la visite dune EIC. Lanalyse des rapports, la grande fiabilité du système dinformation sanitaire et lexistence dune récompense pour tout cas confirmé ont convaincu la Commission, qui a proposé la République islamique dIran pour la certification. LEgypte a formulé une requête officielle accompagnée dune étude historique montrant quaucun cas de dracunculose navait été décrit depuis au moins 2 siècles. Cependant, la Commission a estimé que la frontière avec le Soudan, pays le plus touché par la dracunculose, présentait un risque dintroduction de la maladie. Aussi a-t-elle proposé lenvoi dune EIC qui devrait évaluer ce risque. La Région de la Méditerranée orientale compte 2 pays dendémie: le Soudan, qui a notifié le plus de cas en 1996 (107 693 cas de janvier à novembre, soit 77% de lensemble des cas) et le Yémen avec 62 cas en 1996.1

Of these replies, 6 fulfilled the criteria for certification, and the Commission therefore proposed Austria, Belgium, Bulgaria, Finland, Romania and Switzerland.

South-East Asia Region Of the 10 Member States belonging to this Region, only India is affected by dracunculiasis. The 9 cases notified and isolated in 3 villages in Rajasthan in 1996 show that India has almost interrupted the transmission. This result is very significant, since once India achieves zero incidence, the entire Indian subcontinent can be considered free of dracunculiasis. At the meeting, no country from the South-East Asia Region was proposed for certification because the applications were not complete.
Région de lAsie du Sud-Est Parmi les 10 Etats Membres que compte cette Région, seule lInde est touchée par la dracunculose. Les 9 cas notifiés et isolés dans 3 villages du Rajasthan en 1996 font que ce pays a quasiment interrompu la transmission. Ce résultat est très important car dès que lInde aura atteint lincidence zéro, on pourra considérer lensemble du sous-continent indien libre de dracunculose. Au cours de la réunion aucun pays de la région de lAsie du Sud-Est na été retenu pour être proposé à la certification car les dossiers étaient incomplets.
Western Pacific Region In this Region, indigenous dracunculiasis is unknown. Among the 20 requests put forward, 7 were selected by the Commission to be proposed for certification: the Cook Islands, Kiribati, Mongolia, Papua New Guinea, Singapore, the Solomon Islands and Vanuatu. Three cases of human dracunculiasis were described in Korea (1926), in Japan (1986) and in China (1995). These cases were diagnosed in individuals who had never left their country. They all occurred in an area where there is no Dracunculus medinensis transmission. The Commission proposed that an in-depth investigation of this phenomenon be undertaken and recommended that a study group be established for the purpose.
Région du Pacifique occidental Dans cette Région, la dracunculose autochtone est inconnue. Parmi les 20 requêtes formulées, 7 ont été retenues par la Commission, qui a proposé à la certification les Iles Cook, les Iles Salomon, Kiribati, la Mongolie, la Papouasie-Nouvelle-Guinée, Singapour et Vanuatu. Trois cas de dracunculose humaine ont été décrits en Corée (1926), au Japon (1986) et en Chine (1995). Ces cas ont été diagnostiqués chez des autochtones nayant jamais quitté leur pays. Ils sont tous survenus dans une zone où il nexiste pas de transmission de Dracunculus medinensis. La Commission a proposé dentreprendre une enquête approfondie de ce phénomène et a recommandé la création dun groupe détude pour mener cette investigation.
