MALARM (IMPORTED CASES) V e n e z u e l a . — On 8 June 1976, two sailors from a Chilean cargo ship were admitted to a private hospital in La Guaira. The ship had sailed from Valparaiso (Chile) on 1 May, with stops at various ports in Peru, Colombia, Panama and Venezuela. Both sailors had similar symptoms which began on 4 and 6 June respectively. The symptoms included unremitting high fever, general malaise, vomiting, jaundice, pronounced anaemia and hepatosplenomegaly. On 13 and 14 June, both patients developed acute renal insufficiency and one died on 15 June.

PALUDISME (CAS IMPORTÉS) Venezuela. — Le 8 juin 1976, deux marins d ’un cargo chilien ont été admis dans un hôpital privé de L a Guaira. Le navire, parti de Valparaiso (Chili) le 1er mai, avait fait escale dans divers ports du Pérou, de la Colombie, du Panama et du Venezuela. Les deux hommes présentaient des symptômes similaires (appa­ rus chez l’un le 4 juin et chez l’autre le 6): fièvre élevée en plateau, malaise général, vomissements, ictère, anémie prononcée et hépa­ tosplénomégalie. Une insuffisance rénale aiguë s’est déclarée chez l’un des malades le 13 juin et chez l’autre le 14. L ’un d ’eux est mort le 15. Les malades avaient d ’abord été traités contre la leptospirose, mais juste avant la mort de l’un d ’eux la présence de Plasmodium falciparum a été confirmée. On a donc alors appliqué au survivant un traitement antipaludique et le malade a guéri. Les autorités sanitaires vénézuéliennes ont examiné l ’équipage afin de dépister tout cas éventuel de leptospirose ou de paludisme et ont ordonné la dératisation du cargo. Tous les membres de l’équi­ page possédaient des certificats valables de vaccination contre la fièvre jaune et, dans aucun des prélèvements examinés, on a décou­ vert de parasites du paludisme. Un traitement préventif contre le paludisme a cependant été administré à tout l’équipage.

Both patients were at first treated for leptospirosis, but just before the death occurred the presence of Plasmodium falciparum was confirmed. The surviving patient was then treated for malaria and recovered. The Venezuelan health authorities examined the ship’s crew for leptospirosis and malaria and ordered the deratization of the ship. All crew members had valid certificates against yellow fever and all samples for malaria were negative. Nevertheless, all crew members were given preventive malaria treatment.
