Recommended composition of infioenzo virus Yuccines fo r use io4he 19 9 4 -1 9 9 5 season Infioenzo activity, October 1993’ February. 1.9941 Between October 1993 and February,,^994, influenza A(H3N2) viruses predominated in Europe and N orth America; influenza A(H3N2) and influenza B viruses were reported in Asia; influenza A(H1N1) viruses , were un­ common. ,,, r Influenza activity started in August and Septem ber in the northern hemisphere with local outbreaks in Louisiana, United States of America, and in Scotland. T he outbreaks in Scotland were fallowed by an epidemic in the U nited King­ dom during October and November. Other parts o f western and northern Europe experienced widespread: influenza activity during November and December and outbreaks were reported in southern and eastern Europe in January. Influenza spread in N orth America during December and January, with a moderately severe epidemic in,the United States and sporadic cases in Canada. Sporadic cases were also reported in Hong Kong and Thailand in October, November and December and in Japan in December and January., Outbreaks, were reported in China jo December and January. L ’activité grippale a débuté en août et septem bre dans l’hémi­ sphère N ord p ar des flambées locales--en Louisiane, Etats-Unis d’Amérique, et en Ecosse. T he outbreak in Scotland in August and September was caused by influenza A(H3N2) viruses as were die epidemics which occurred in the U nited Kingdom and in western and northern Europe (Austria, Belgium, Denmark, .F in lan d ,. the Czech Republic, Greece, Romania, the Russian Federa­ tion, Spain and Yugoslavia by mid-Februaxy. The outbreaks of influenza A(H3N2) in Louisiana, United States in August were followed in November by outbreaks in a num ber of other states and an epidemic in December and January. Influenza A(H3N2) viruses were also isolated sporadi­ cally in China, Japan, Madagascar and Zambia. Influenza AtHINl] Since October, influenza A(H1N1) activity has been re­ ported from Hungary, H ong Kong, the Netherlands and the U nited States. Influenza B Influenza B viruses caused outbreaks in China in December and January and were isolated sporadically in Hong Kong and Thailand. Outbreaks occurred in Slovakia in January and a few cases were reported from Canada, Finland, Japan, the Netherlands, Portugal, the Russian Federation, Spain, Sweden, Switzerland, the U nited Kingdom and die United States. Aitigenic characteristics of recent isolates Influenza A(H3N2) virus Many o f the recent influenza A(H3N2) isolates were antigenically distinguishable from A/Beijingf32/92, the vaccine strain recommended in 1993.1T he characteristics o f a num­ ber of them are shown in Table 1 which includes A/Shangdong/9/93 as a current reference virus. Grippe A(HINI) Depuis octobre, une activité grippale A(H1N1) est signalée aux Etats-Unis, en Hongrie, à Hong Kong et aux Pays-Bas.

Grippe 6 Les virus B ont été à l’origine de flambées en Chine en décembre et janvier et ont été isolés sporadiquement à Hong Kong et en Thaïlande. Des flambées se sont également produites en Slovaquie en janvier avec quelques cas signalés au Canada, en Espagne, 'aux Etats-Unis, en Finlande, en Fédération de Russie, au Japon, aux Pays-Bas, au Portugal, au Royaume-Uni, en Suède et en Suisse.

Influenza A(H1N1) itirui -, As a result of the low level of H1N1 influenza activity internationally, no isolates of this subtype were character­ ized in the W HO Collaborating Centres for Reference and Research on Influenza between October and February. Influenza Bvirus The influenza B viruses received for analysts, including the m ost recent isolate from China, were antigenically similar to B/Panama/45/90 and B/Quingdan/102/91. A/Beijing/32/92(H3N2)-like, A/Singapore/6/86(H lN l)like, and B/Panama/45/90-like viruses adm inistered in doses of 15 rmcrograms of each Fiawnagglnrinin. In general, post-immunization haem agglutbation-inhibition (HI) antibodies at litres of >40 against the H3N2 vaccine virus were detected in 55-70% of immunized chil­ dren, 65-90% of adults, and 50-80% of the elderly. Using representative recent isolates such as A/Shangdong/9/93, however, post-immunization H I antibodies at dues of >40 were found in only 40% of children, 35-80% of adults, and 45-80% of the elderly. T he post-immunization geometric mean titres of H I antibodies to A/Shangdong/9/93 were 2070% lower than those to the vaccine virus. Post-immunization H I antibodies at litres of >40 to the influenza A(H1N1) vaccine viruses were detected in the sera of 70-90% of children, 95-97% of adults, and 60-85% of elderly vacdnees. F or the-m ost recent isolates of A(H1N1) virus, 70-80% of children, 92-97% of adults and 65-70% of elderly vaccinées had H I titres >40. For influenza B viruses post-immunization H I anti­ bodies at titres of >40, or equivalent titres obtained using single radial haemolysis assays,-were detected for the vaccine strains and representative recent isolates in the sera of 100% of children, 95-100% of adults, and 93-100% of the elderly. Recommendations for thé composition of influenza virus vaccines During thé 1993-1994 season, influenza A(H3N2), A(H1N1) and influenza B viruses all continued to circulate. Influenza A(H3N2) viruses predominated, many of which were antigenieaUy distinguishable from the vaccine strain AI Beijing/32/92, They were similar to the more recent refer­ ence isolate A/Shangdong/9/93 (Table I). Vaccines containing- A/Beijing/32/92-like viruses in­ duced antibody responses to A/Shangdong/9/9 3-like viruses at lower frequency and titre than to the vaccine strain. Fewer influenza B viruses were isolated. They were antigenically similar to B/Panaraa/45/90 and B/Quingdao/ 102/91 and vaccines containing B/Panama/45/9Q-like viruses induced antibodies with similar frequency and titre both to th e vaccine virus and to representative recent isolates. N o evidence has been obtained for variation in the anti­ genic properties o f recently circulating influenza A(H1N1) viruses. . ■Consequently, it is recommended that trivalent vafones be used in tfae.I99.4rl995 season, and that they contain:the following; /igdon j ::,/93 >,H3N2) « ■ . . v. ,;apon. ■ ■■6{H lN l> li.

Recommandations pour la composition des vaccins antigrippaux Au cours de la saison 1993-1994, les virus , grippaux A(H3N2), A(H1N1) e t B jonî continué à circuler. Ce sont les virus A(H3N2) qui prédom inaient, un grand nombre d’entre euxétant antigénique­ m ent distincts de la souche vaccinale A/Beijing/32/92. Us étaient similaires à l’isolement de référence plus récent A/Shangdong/9/93 (Tableau 2). >' ■ • Les vaccins contenant des virus analogues à A/Beijing/32/92 ont suscité des réponses en anticorps de fréquence e t de titre plus faibles aux virus analogues à A/Shangdong/9/93 qu’à lasouche vaccinale:, Les isolements de virus grippamc R ont été m oins nombreux. Us étaient anrigéniqnpmi»nr analogues à . B/Panama/45/90 et B/Quingdao/102/91, et la réponse en .anticorps suscitée par les vaccins contenant des virus analogues à B/Panama/45/90 > était dé fréquence et de titre similaires, q u ’il s’agisse du virus vaccinal ou d’isolements représentatifs récents. Rien n ’indique qu’il y ah eu variation dans les propriétés anti­ gèniques des virus A(H1N1) récemment en circulation. E n conséquence, il est recommandé d’utiliser pour, la saison 1994-1995 des vaccins trivalents contenant les-souchcs. suivantes: • une souche analogue à A/Shangdong/9/93 (H3N2), • une souche analogue à A/Singapore/b/86(H lN l); • une souche analogue à R/Panama/45/90. Comme les années précédentes, les virus utilisés dans chaque pays devront être approuvés par les autorités nationales de contrôle. La majorité de la population a probablem ent été infectée-au cours de ces dernières années par les virus A(H3N2), A(H 1N 1) et B. Il s’ensuit qu’une dose de vaccin inactivé devrait être immunogène quel que soit l’âge des sujets, sauf chez le jeune enfant. Les enfants non encore immunisés auront besoin de 2 doses de vaccin, admi­ nistrées à 4 semaines d’intervalle au moins.
