EXPANDED PROGRAMME ON IMMUNIZATION Investigation and Control of a Measles Outbreak in a Community N ig e r ia . — Between January and M ardi 1977, a measles epi demic occurred in a local government area (154 km2; population 34 000) within Bendel State, Nigeria. The age distribution o f 176 cases diagnosed in the health centre (Table 1) revealed that 73 % of the cases were under the age o f 36 months.

La répartition par âge des 176 cas diagnostiqués au centre de santé (Tableau 1) indique que 73% des malades avaient moins de 36 mois.

As soon as the presence of this epidemic had been recognized by the health authorities, a request to institute a mass measles vaccina tion campaign was made to the physician in charge of a pilot project of the Nigerian National Youth Services Corps Rural Mobile Health Service which was operating in the area. Within a period of six days the campaign was begun, and within a period of only 11 days, teams using three vehicles and three jet injectors succeeded in vaccinating 4 205 children, who represented virtually 100 % of the target population. Among the factors contributing to this success were the following:

The physician, using school teachers and literate Chiefs in the villages, had compiled a census so that the names, ages and other pertinent data concerning the inhabitants were known. A medical field unit was made available to supplement the staff1 of the pilot project. Measles vaccine was maintained at the correct temperature in the refrigerator used for storing fish in the community. Good health education methods were employed. An already existing system was used to disseminate information to the elders and mothers in the community, and a loudspeaker was used by the physician to reinforce the information. Transport was available from the outskirts of villages to the central village collecting points used by the vaccinating teams. Children, particularly contacts of known cases, were actively sought and transported to one of the vaccination sites.

Studies are now underway to determine the antibody responses to vaccination in a representative sample of immunized children.
Le médecin, par l’intermédiaire des instituteurs et des chefs de village sachant lire et écrire, avait procédé à un recensement, si bien qu’il connaissait les noms, âges et autres caractéristiques pertinentes des habitants. • Une unité médicale mobile a été adjointe au personnel du projet pilote. • Le vaccin antirougeoleux a été conservé à la température requise dans le réfrigérateur où la collectivité stocke son poisson. • De bonnes méthodes d ’éducation sanitaire ont été employées. On a recouru à u n système déjà existant pour toucher les nota bles et les mères, e t le médecin s’est servi d ’un haut-parleur pour renforcer l’action d ’information. • On disposait de moyens de transport entre la périphérie des villages et les points centraux où opéraient les équipes de vaccina teurs. The health authorities and the communities they served participated effectively together to fight a common enemy. The fact that the measles epidemic had persisted for several weeks before being noted, however, reemphasizes the need to establish continuing surveillance of diseases, like measles, which are the target of control efforts. Although vaccination campaigns, such as the one described above, may certainly succeed in stopping an epidemic in progress, they also entail the vaccination of many children who have already had the disease. The ideal, of course, is to plan continuing programmes in such a way as to prevent such epidemics entirely. ARBOVIRUS SURVEILLANCE St. Louis Encephalitis (SLE) — In June, the first two laboratoryconfirmed cases of human infection with St. Louis encephalitis (SLE) virus for 1977 were reported in Dallas, Texas. SURVEILLANCE DES ARBOVIRUS Encéphalite de Saint-Louis (ESL) E t a ts - U n is d ’A m é r i q u e . — C’est en juin que, pour l’année 1977, étaient notifiés à D allas (Texas) les deux premiers cas d ’infection humaine confirmés a u laboratoire dus au virus de l’encéphalite de Saint-Louis (ESL). E n juillet, deux autres cas étudiés au laboratoire, présumés atteints d ’une infection à virus ESL, étaient signalés à Memphis (Tennessee). Le premier malade, âgé de 89 ans et demeurant à Dallas, a éprouvé les premiers symptômes le 10 juin: désorientation, état fébrile et tremblement. Des ponctions lombaires pratiquées à deux reprises ont révélé de la pléocytose dans le liquide céphalo-rachidien, le malade présentant à l’épreuve d ’inhibition de l’hémagglutination IH un titre d ’anticorps de 1: 640 dans du sérum prélevé le 15 juin. Son état s’étant régulièrement amélioré, il a pu être renvoyé guéri à la fin juin. Dans Je second cas, il s’agissait d’une femme de 24 ans tombée malade le 15 juin; o n notait entre autres symptômes de la raideur de la nuque, de la myalgie, des nausées et une température de 39 °C. Elle devait être hospitalisée le 22 juin. Son sérum était d ’abord négatif à l’épreuve IH quant aux anticorps de l’ESL, mais un autre échantillon de sérum prélevé le 28 juin présentait un titre ffl de 1:180, apportant ainsi la preuve de l’infection. En mai, deux des dix groupes de poulets « sentinelles » que l’on conserve à Dallas po u r la surveillance de l’ESL présentaient des signes d ’infection p a r le virus ESL. A Dallas, le programme régulier de larvîcide a été complété par un renforcement des mesures d’adulticide en volume ultra-faible, tandis que l’on intensifiait les mesures destinées à éliminer les récipients pouvant servir de gîtes larvaires autour des habitations. Des études ont été entreprises pour isoler le virus ESL sur les moustiques et mettre des anticorps en évidence chez les oiseaux sauvages. Au Mississippi, les autorités sanitaires ont signalé des niveaux d ’activité d ’ESL faibles, mais très répandus. En juin, de jeunes passereaux saignés près de Clarksdale, Greenwood, Hattiesburg et Jackson présentaient des anticorps du virus.
The first patient, an 89-year old man living in the city of Dallas, had onset of illness on 10 June, becoming disoriented, febrile, and tremulous. Pleocytosis was shown in cerebrospinal fluid obtained from lumbar punctures on two occasions and he had a haemagglutination-inhibition (HI) antibody titre of 1:640 in serum drawn on 15 June. His condition improved, and he was gradually subsequent ly discharged at the end o f June. The second patient was a 24-year-old woman who had onset of an illness on 15 June; her symptoms included a stiff neck, myalgia, nausea, and a temperature of 39 °C. She was hospitalized on 22 June. Initially, her serum was negative for HI antibodies to SLE; another serum sample drawn on 28 June had an H I titre of 1:180, however, documenting her infection. In May, two of the ten sentinel chicken flocks maintained for SLE surveillance in Dallas showed evidence of infection with SLE virus. The regular mosquito larviciding programme in Dallas was sup plemented by increased ultra-low-volume adulticiding, and efforts to eliminate breeding containers around homes were intensified. Studies were undertaken to isolate SLE virus from mosquitoes and to demonstrate antibodies in wild birds.
In Mississippi, the Mississippi State Board of Health also reported evidence of low, but widespread, levels of SLE activity. In June, juvenile sparrows bled near Clarksdale, Greenwood, Hattiesburg, and Jackson were found to have antibodies to the virus.

Western Equine Encephalitis (WEE) The virus was isolated from approximately one of every 50 Culex tarsalis mosquitoes collected from the Ft. Collins, Colorado, area in July by the Vector-borne Diseases Division Laboratory, Center for Disease Control. The sera from a horse with encephalitis and from several sparrows from the Ft. Collins area demonstrated evidence of WEE infection. The California State Health Department has also isolated WEE virus from C. tarsalis mosquitoes collected near Needles, California, and from the Califomia-Arizona border near Mojave, Arizona. No equine cases were confirmed in California. By mid-August widespread WEE activity in horses from 14 states had been reported by the Veterinary Services Laboratories, US Department of Agriculture. By the beginning of September, five human infections, four serologically confirmed and one presumptive had been detected from North and South Dakota and Colorado. Three of the cases had onset of clinical encephalitis in July. A number of suspected clinical cases are being studied. The state health departments concerned have publicized the necessary pre ventive measures to be taken.
Encéphalite équine de l'ouest (EEO) Le virus a été isolé chez environ un sur 50 moustiques Culex tarsalis recueillis en juillet dans la région de Fort-Collins (Colorado) par le laboratoire des maladies à vecteurs du Center fo r Disease Control. Le sérum d’un cheval atteint d’encéphalite et ceux de plu sieurs passereaux du secteur de Fort-Collins se révélèrent infectés. Les services de santé de Californie ont également isolé le virus sur des moustiques C. tarsalis recueillis près de Needles, en Californie, et dans la région frontière séparant la Californie de l’Arizona, près de Mojave (Anzona). Aucun cas équin n ’a été confirmé en Californie. A la mi-août, les laboratoires des services vétérinaires du Ministère de l'Agriculture des Etats-Unis signalaient que l’encéphalite équine de l’ouest était très répandue chez les chevaux de 14 Etats. Au début de septembre, cinq infections humaines, dont quatre sérologîquement confirmées et un cas présumé, étaient dépistées dans le Dakota du Nord, le Dakota du Sud et le Colorado. Dans trois de ces cas, l’encéphalite clinique s’était déclarée en juillet. On est en train de suivre un certain nombre de cas cliniques suspects. Les autorités sanitaires des Etats intéressés ont averti le public des précautions à prendre.

PARALYTIC SHELLFISH POISONING1 V e n e z u e l a . — On 26 July, 15 cases of food poisoning from El M ono de Puerto Santo were admitted to the health center at Rio Caribe, Sucre State. Two of the patients were under 14 years of age, and 13 were 15 years or older. The symptoms were mild, and only one required hospitalization due to ataxia. During the afternoon of 31 July, 27 cases, with seven dead on arrival, from the area of La Llanada de Cangua were admitted with severe food poisoning characterized by numbness of the mouth, hands and feet, very severe signs of ataxia and respiratory difficulty in some. During die night of 31 July and on 1 August, a total of 166 patients arrived of whom two died with symptoms of respiratory paralysis. The most affected localities were La Llanada de Cangua with 32 cases and seven deaths, Santa Elena with 25 cases and one death, Popui (Puipui) with seven cases, and Rio Caribe, 54 cases and one death. These areas are not far from one another on the north coast of the Arismendi district in the State of Sucre between Rio Caribe and San Juan de las Galdonas. Case fatality was 4.2% with 8.3% for those under 15 years of age and 1.5 % in the older group. All of the patients had eaten mussels at noon either boiled or in a stew with rice. Most of them had consumed no other food. A survey of 240 persons who ate mussels in the different places showed that 177 (73.7%) became ill.
INTOXICATION PARALYSANTE PAR LES COQUILLAGES1 Venezuela . — Le 26 juillet, 15 habitants d ’El Morro de Puerto Santo souffrant d ’intoxication alimentaire se sont présentés au centre de santé de Rio Caribe (Etat de Sucre). Deux avaient moins de 14 ans, les autres 15 ans ou plus. Les symptômes étaient bénins et un seul malade a dû être hospitalisé pour ataxie. Dans l’après-midi du 31 juillet, on a reçu 27 personnes, dont sept mortes à l’arrivée, venant de la région de La Llanada de Cangua; elles étaient atteintes d’intoxication alimentaire grave, avec paresthé sie de la bouche et des extrémités, ataxie prononcée et, dans quelques cas, difficulté respiratoire. Pendant la nuit suivante et la journée du 1er août, 166 malades en tout ont été admis. Deux sont morts; ils présentaient des symptômes de paralysie respiratoire. Les localités les plus touchées ont été La Llanada de Cangua (32 cas; sept décès), Santa Elena (25 cas; un décès), Popui (Puipui) (sept cas) et Rio Caribe (54 cas; un décès). Elles sont proches l’une de l’autre sur la côte septentrionale du district d ’Arismendi (Etat de Sucre) entre Rio Caribe et San Juan de las Galdonas, Le taux de létalité a été de 4,2% en moyenne, de 8,3 % chez les moins de 15 ans, de 1,5 % pour les âges supérieurs. Tous les malades avaient mangé au repas de midi des moules cuites, parfois avec du riz. La plupart d ’entre eux n ’avaient con sommé aucun autre aliment. Il est ressorti d ’une enquête sur 240 per sonnes ayant mangé des moules dans ces différentes localités que 177 (73,7 %) avaient été malades. Le 2 août, on a effectivement constaté l’existence d ’une « marée rouge » dans la région et prélevé des échantillons de moules aux endroits suspects. A l’Institut national de la Santé de Caracas, des épreuves sur des souris ont révélé une toxicité telle qu’une dilution à 1:40 de la macération obtenue provoquait une paralysie respiratoire et la mort en sept minutes, avec des symptômes analogues à ceux observés chez des être humains. On n ’a trouvé ni toxine botulique, ni insecticides, ni métaux lourds. De toute évidence, les poissons n’étaient pas en cause. Les données disponibles font penser à une poussée d’intoxication paralysante par les coquillages, mais des recherches se poursuivent en vue de déter miner la toxine et le flagellé responsables.1 1 Voir N" 42.1972, pp. 397-398, N“ 34, 1976, p. 275. On 2 August, “red tide” was verified in the region and samples of mussels were taken from the suspected places. In the National Health Institute at Caracas tests on mice revealed toxicity to the extent that a 1:40 dilution of the obtained maceration caused res piratory paralysis and death in seven minutes, with symptoms similar to those in humans. Efforts to find botulism, insecticides and heavy metals were neg ative. Fish evidently were not involved. E d it o r ia l N o t e : Presuming the diagnosis is confirmed this con dition, also known as mussel poisoning, gonyaulax poisoning and mytilointoxication is caused by a heat-stable alkaloid, saxitoxin, elaborated by Gonyaulax spp. and other dinoflagellates. Potentially toxic species of these pigmented protozoans are often present in sea water in small numbers but occasionally they multiply to such an extent that the water becomes coloured, causing the so-called “red tide” . Shellfish feeding on these organisms may concentrate the saxitoxin in their tissues and, when ingested by humans, may give rise to a characteristic syndrome. and the onset of symptoms varies but is usually rapid and may be as little as 20 minutes. Paiaesthesiae of the mouth and the extremities may be followed by weakness of the limbs, vomiting and a peculiar floating sensation. The paralytic effect of the poisonous alkaloid is mediated by toxicity to both nerve and muscle and may progress to respiratory arrest in fatal cases. The major determinant of casefatality rates is the dose of toxin ingested. Treatment includes removal of unabsorbed toxin by emesis and cathartics and, in severe cases, supportive respiratory care.

Le taux de létalité depend principale ment de la dose de toxine ingérée. Le traitement consiste à évacuer la toxine non absorbée en administrant un émétique ou une purge et, dans les cas graves, à soutenir la fonction respiratoire.

Between 1961 and 1972, there was a clear pattern with a year of markedly increased incidence followed by two years with a relatively low number of cases. Following the largest recorded epidemic in 1971-1972 and the initiation of a national poliomyelitis immuniza tion programme, the number of cases fell to less than 30 for a three-year period. There was, however, a trend towards a rising number of cases in some states in the period 1973-1976 and the possibility of another epidemic was foreseen. In 1977, there has been a marked increase of cases in some states and epidemiological investigations have shown that over two-thirds of the paralytic cases were children under three years of age, and nearly all of them had not been immunized. Most of the cases were from small estates and new villages. A survey of the immunization status revealed that only four states had a coverage of >75% of infants. The more developed states with a coverage less than 75 % were those having the highest number of cases. To bring the present epidemic under control, all states have been requested to locate isolated estates and new villages and undertake a vaccination programme aimed at preschool children and those under three years of age in particular. The cooperation of community leaders and appropriate organizations has been sought.
De 1961 à 1972, l’incidence a suivi un cycle bien défini: nettement supérieure une année, elle était relativement faible les deux suivantes. Après l ’épidémie de 1971-1972, la plus importante jamais enregistrée, et l’institution d ’un programme national de vaccination antipolio myélitique, le nombre des cas a été inférieur à 30 pendant trois ans. Il a toutefois tendu à monter dans quelques Etats entre 1973 et 1976, ce qui a laissé entrevoir la possibilité d’une nouvelle épidémie. En 1977, l’incidence a sensiblement augmenté dans certains Etats; les investigations épidémiologiques ont révélé que plus des deux tiers des malades atteints de la forme paralytique étaient des enfants de moins de trois ans, presque tous non vaccinés, la plupart des cas se produisant dans de petites plantations ou des villages nouveaux. H est ressorti d’une enquête qu’une couverture vaccinale >75% pour les nourrissons n ’était assurée que dans quatre Etats. C ’est dans t e plus développés des Etats à couverture inférieure à 75% qu’ont été notés les plus grands nombres de cas. Pour enrayer l’épidémie en cours, tous les Etats ont été invités! localiser les plantations isolées et les villages nouveaux ainsi qu’à entreprendre un programme de vaccination des enfants d ’âge pré scolaire et en particulier de ceux de moins de trois ans. On s’est efforcé d’obtenir la coopération des notables locaux et d ’organisa tions appropriées.

INFLUENZA R ico (11 November 1977). — 11Influenza is widespread throughout the western and central areas of the island. Scattered cases of influenza-like illness were first observed in late August. The number of reports of patients visiting clinics for febrile illness (collected as a function of routine influenza surveillance) began to show an increase in September. These illnesses were generally pre sumed to be dengue. On 20 October, influenza A infections in Puerto Rico were confirmed serologically. The United States Environmental Protection Agency has recently approved the one-shot, break off tip aerosol containing the above formulation. In accordance with Annex VI of the Recommenda tions for the Disinsecting of Aircraft of the International Health Regulations (1969), Second Annotated Edition (1974), the formula tion is to be used a t the rate of 10 g/1000 cu. ft. (35 g/100 m3). In the notification, the US Environmental Protection Agency indicated the number of the above aerosol units required for various aircraft. V Environmental Protection Agency (EPAJ des Etats-Unis d ’Amérique vient d’approuver une préparation d ’aérosol du type ci-dessus qu’émet un diffuseur à pointe bnsable à utiliser en une fois. Conformément à l’Annexe VI « Recommandations pour la désinsec tisation des aéronefs » du Règlement sanitaire international (1969), Deuxième édition annotée (1974), la préparation sera employée à raison de 35 g/100 m3 (10 g/1 000 pieds cubes). CRITERIA USED IN COMPILING THE INFECTED AREA LIST Based on the International Health Regulations (1969) the following criteria are used in compiling and maintaining the infected area list (only official govern mental information is used): 1. An area is entered in the list on receiptofinformation of: (i) a declaration o f infection under Article 3 ; (ü) the first case o f plague, cholera, yellow fever or smallpox that is neither an imported case nor a transferred case; (iii) plague infection among domestic o r wild rodents; (iv) activity o f yellow-fever virus m vertebrates other than man using one of the following criteria: (a) the discovery o f the specific lesions o f yellow fever in the liver of vertebrates indigenous to the area; o r (b) the isolation o f yellow-fever virus from any indigenous vertebrates. II. An area is deleted from the list on receipt o f information as follows: (0 if the area was declared infected (Article 3), it is deleted from the list on receipt o f a declaration under Article 7 that the area is free from infection. If information is available which indicates that the area has not been free from infection during the time intervals stated in Article 7, the Article 7 declaration is not published, the area remains on the list and the health administration concerned is queried as to the true situation; (u) if the area entered the list for reasons other than a declaration under Article 3 (see I. (ii) to (iv) above), it is deleted from the list on receipt of negative weekly reports for the tune intervals stated in Article 7. In the absence o f such reports, the area is deleted from the list on receipt of a notification o f freedom from infection (Article 7) when at least the time period given in Article 7 has elapsed since the last notified case.
CRITÈRES APPLIQUÉS POUR LA COMPILATION DE LA LISTE DES ZONES INFECTÉES Conformément au Règlement sanitaire international (1969) les critères sui vants sont appliqués pour la compilation et la mise à jour de la liste des vo te infectées (seules sont utilisées les informations officielles émanant des gouvernes ments). I. Une zone est portée sur la liste lorsque l’Organisation a reçu: 0 une déclaration d’infection, aux termes de l’article 3; ü) notification d ’un premier cas de peste, de choléra, de fièvre jaune ou de variole qui n ’est m un cas importé ni un cas transfère ; ni) notification de la présence de la peste chez les rongeurs domestiques et chez les rongeurs sauvages; iv) notification de l'activité du virus amani chez des vertébrés autres que l’homme, déterminée par l'application de l'un des critères suivants; a) découverte des lésions spécifiques de la fièvre jaune dans le foie de vertébrés de la faune indigène du territoire ou de la circonscription; ou b) isolement du virus de la fièvre jaune chez n ’importe quel vertébré de la faune indigène. II. Les zones sont radiées de la liste Hans les conditions suivantes ; i) si la zone a été déclarée infectée (article 3), elle est radiée de la liste b ra que l'Organisation reçoit une notification faite en application de l’article 7, suivant laquelle la zone est indemne d’infection. Si l ’on dispose de ren seignements indiquant que la zone n’a pas clé indemne d'infection pen dant une période correspondant à la durée indiquée dans l'article 7, la notification prévue par l'article 7 n'est pas publiée, la zone reste sur la liste et l’administration sanitaire intéressée est priée de donner des éclair cissements quant à la situation exacte; ii) si la zone a été portée sur la liste pour des raisons autres que la réception de la notification prévue par l’article 3 (voir I, 0 0 à (iv) ci-dessus), elle est radiée de la liste lorsque des rapports hebdomadaires négatifs ont été reçus pendant une période dont la durée est indiquée à l’article 7. A défaut de tels rapports, la zone est radiée do la liste lotsque, au terme de la pénotjiç indiquée à l’article 7, l’Organisation reçoit une notification d ’exemption d’mfecuon (article 7).
