MENINGOCOCCAL DISEASE C anada (Province o f Ontario). — Meningococcal disease occurs throughout the year in Ontario, with increased activity seen during the winter and early spring. No epidemics have occurred jn Canada since 1941, During that year, the highest rates of disease were reported in Prince Edward Island and Nova Scotia, with close to SO cases per 100 000 population in each province. Between 1968 and 1988 in Ontario, the annual incidence rate per 100 000 for meningococcal disease has ranged from a low of 0.3 in 1968 to a high of 1.99 in 1973 (Fig. 1). An upward trend in incidence has been occurring since 1983. In 1987 and 1988, the annual incidence was 1.4 per 100 000. Between 1972 and 1988, the annual case-fatality rate (CFR) of reported cases has ranged from a low of 2% in 1983 to a high of 25% in 1972. In the past 5 years (1984-1988) the CFR has aver aged 11.6%.
INFECTIONS À MÉNINGOCOQUES C a n a d a (province d’Ontario). — En Ontario, la maladie méningococcique sévit tout au long de l’année, mais l’activité maximale est en hiver et au début du printemps. Le Canada n ’a pas connu d’épidémie depuis 1941, année où les taux d’atteinte les plus élevés ont été signalés dans l’île du Prince-Edouard et en Nouvelle-Ecosse : près de 50 cas pour 100 000 habitants dans l’une et l’autre provinces.
De 1968 à 1988, l’incidence annuelle de la maladie méningococcique pour 100 000 habitants a oscillé en Ontario entre un minimum de 0,3 cas en 1968 et un maximum de 1,99 cas en 1973 (Fig. J), Depuis 1983, la tendance est à la hausse. En 1987 et en 1988, l’incidence a été de 1,4 cas pour 100 000 habitants. Entre 1972 et 1988, le taux annuel de létalité pour les cas signalés a atteint un minimum (2%) en 1983 et un maximum (25%) en 1972. Au cours des 5 dernières années (1984-1988), on a enregistré un taux de létalité moyen de 11,6%.

The Ontario experience in the winter of 1988-1989 Examination of the number of reported cases in 1988 by month of onset reveals that prior to December the experience was similar to that of the previous 2 years. In December, however, 21 cases (including 4 deaths) were reported (Table l) . In the first 2 months pf 1989, numbers of cases continued to be higher than expected and 2 major clusters (described below) have occurred. In the 3 months from December 1988 to February 1989,29 of the 43 Ontario health units from all 5 health regions have reported cases. Other parts of the country do not appear to be experiencing increased prevalence. The age distribution o f the 85 cases of meningococcal disease reported in Ontario from December 1988 tp February 1989shows that only 16,5% p f the cases occurred in infants under 1 year of age and 34% in children under 5. T he majority of cases (79%) were in persons aged 20 years or less. Although die latter finding is in keeping with the 25-year Canadian experience from 1960 to 1984, in the 3 months from December 1988 to February 1989 in Ontario 45% of cases have occurred in the 5-20-year age group compared jo the national average of 20%. Eleven (12.9%) of the 85 cases have died. The distribution of cases by serogroup of Neisseria meningitidis isolated shows that group B accounted for 43.4% of the 69 typed cases. Of the remaining typed cases, all but 1 were group C. During the 4 years 1985-1988 in Ontario, group B has accounted for a similar proportion of typed cases (40.8%).
Expérience de l’Ontario en hiver 1988-1989 Il ressort de l’étude des nombres de cas signalés en 1988, d’après le mois d’apparition de la maladie, que jusqu’en décembre la situation était analogue à celle des 2 années précédentes. En décembre, 21 cas (dont 4 décès) ont toutefois été recensés (Tableau 1). Au coun des 2 premiers mois d e 1989, les cas ont continué d’être plus nombreux que prévu et 2 groupes importants (décrits plus bas) ont été observés. Au cours de la période de 3 mois allant de décembre 1988 à février 1989, des cas ont été signalés par 29 des 43 services de santé des 5 régions ontariennes. Dans le reste du pays, la prévalence ne semble pas accrue. La répaniuon par âge des 85 cas de maladie méningococcique enre gistrés en Ontario de décembre 1988 à février 1989 révèle que 16,5% seulement concernaient des enfants de moins d’un an et 34%, des moins de 5 ans.L a majorité des cas (79%) sont survenus chez des sujets de 20 ans ou moins. Si cette dernière observauon est conforme à ce que l’on a observé au Canada au cours des 25 ans qui vont de 1960 à 1984, pendant les 3 mois allant de décembre 1988 à février 1989, le groupe des 5 à 20 ans a représenté 45% des cas ontariens, contre 20% pour la moyenne nationale. Onze (12,9%) des 85 malades sont décédés. La répartition des cas d’après le sérogroupe de Neisseria meningitidis isolé montre que 43,4% des 69 cas ayant fait l’objet d’un typage appar tiennent au groupe B. A une exception près, tous les autres apparte naient au groupe C. En Ontario, le groupe B a représenté une proportion analogue des cas ayant fait l’objet d’un typage (40,8%) au cours des 4 ans allant de 1985 à 1988.

Public health response to 2 clusters • Between 7 January 1988 and 2 February 1989, a geo graphical cluster of 10 cases of meningococcal disease occurred in die Windsor-Essex County Health Unit area. The first 8 occurred among residents of Windsor where two-thirds of the health unii area populauon reside. T he 10 cases (6 females, 4 males) ranged in age from 1 to 65 years (median 13.5 years).Three cases ranged between 1 and 5 years, 5 were teenagers and 2 were adults. No deaths occurred. All school-aged cases attended diiferent schools. Clustering within the health unit area by residence or place of work did not occur, and no common exposure could be identified. Rifampycin prophylaxis was offered to close contacts of each case. Large scale prophylaxis, e.g. schools and workplaces, was not undertaken. No further cases have occurred. N. meningitidis group B was isolated in all 10 cases. Nearby Detrok did not experience increased prevalence. • A cluster of 9 cases occurred among residents of 3 adjacent health unit areas. Seven of the 9 cases were residents of 3 town ships of south Victoria County in one of the health unit areas. Eight cases (4 males, 4 females) occurred between 27 January 1988 and 3 February 1989. Seven of these cases were aged 8-14 years, and 1 was 26 years old. A 10-year-old female died. Three cases (including the one who died) were students in 3 different classrooms of the same school. With the diagnosis of the thkd case on 3 February, rifampycin prophylaxis was administered to approximately 740 of the 750 students of the school at a clinic held on 4 February. No further cases occurred until a 6-monthold infant who had received rifampycin prophylaxis in early February became ill on 17 February. The organism isolated from this case was sensitive to rifampycin. Seven of the 9 cases in this cluster had identifiable contact with another case or cases from whom infection may have been transmitted. N. meningitidis group C was isolated from blood and/or CSF in 7 of the 9 cases. Following the occurrence of the case of 17 February demon strating ongoing transmission of infection in the community, the decision was made to provide Ministry of Health funded quadri valent meningococcal vaccine in this community. The criteria for selection of a population at risk included the following: 1. geographical clustering; 2. very high disease rates, i.e. > 1 0 0 times expected; 3. evidence o f recent cases; and 4. an identifiable target group at most risk.
Mesures de santé publique prises à l’égard de 2 groupes de cas • Entre le 7 janvier 1988 et le 2 février 1989, un groupement géo graphique de 10 cas de maladie méningococcique a été observée dans la circonscription sanitake du comté de Windsor-Essex. Les 8 premiers cas sont survenus chez des habitants de Windsor, ville qui représente les deux-tiers de la population de la ckconscription. Les 10 cas (6 sujets féminins, 4 sujets masculins) étaient âgés de 1 à 65 ans (médiane: 13,5 ans); 3 avaient de 1 à 5 ans, 5 étaient des adolescents, et 2, des adultes. Aucun décès n’a été enregistré. Tous les sujets d’âge scolabe fréquen taient des écoles différentes. D n’y a pas eu de concentration de cas dans la ckconscription sanitake en rapport avec le domicile ou le lieu de travail, et aucune exposinon commune n’a pu être identifiée. Un trai tement prophylactique de rifampyeine a été administré aux proches contacts de chaque cas, mais on n ’a pas eu recours à une prophylaxie de masse (p.ex., dans les écoles et les lieux de travail). Aucun autre cas n ’a été recensé. N. meningitidis du groupe B a été isolé dans les 10 cas. Tout près, à Détroit, la prévalence n ’a pas augmenté. • Un groupe de 9 cas a été observé parmi les habitants de 3 circons criptions sanitaires adjacentes. Sept des 9 cas concernaient des habitants de 3 municipalités du comté de Victoria sud, dans l’une des ckconscriptions sanitaires. Entrele 27 janvier 1988 et le 3 février 1989,8 cas (4 sujets masculins et 4 sujets féminins) ont été enregistrés. Sept étaient âgés de 8 à 14 ans, et 1 avait 26 ans. Une fillette de 10 ans est décédée. Dans 3 cas (dont le décès), il s’agissait d’étudiants de 3 classes différentes de la même école. Le troisième cas ayant été diagnostiqué le 3 février, on a administré de la rifampyeine à titre prophylactique à envkon 740 des 750 étudiants de l’école, dans le cadre d’une consultation organisée le 4 février. Aucun autre cas n’a été signalé jusqu’au 17 février; ce jour-là, un nourrisson de 6 mois ayant reçu la prophylaxie au début de février est tombé malade. Le micro-organisme isolé chez ce cas s’est révélé sensible à la rifampy eine. Sept des 9 cas de ce groupe avaient eu des contacts avec au moins un autre cas susceptible d’avok transmis l’infection. Dans 7 des 9 cas, N. mentngitidis du groupe C a été isolé à partir du sang et/ou du LCR. A la suite du cas du 17 février qui démontrait une transmission continue de l’infecuon dans la collectivité, il a été décidé de fournir à cette dernière le vaccin méningococcique quadrivalent financé par le Ministère de la Santé. Une population à risque est identifiée sur la base des critères suivants: 1. concentration géographique de cas; 2. taux d’atteinte très élevé, c’est-à-dke plus de 100 fois les taux prévus; 3. signe de cas récents; et 4. groupe cible le plus à risque pouvant être identifié.

The target group identified as a result of this cluster was the estimated 10 000 children aged 6 months, to 18 years residing in the affected municipalities of Victoria County. These areas expe rienced rates of disease 156 times that expected based on'the' Ontario experience for the previous 4 years and 52 times that expected based on the currently increased provincial incidence. Based on attendance at vaccination clinics operating since 22 Fe bruary, high coverage rates were anticipated. Information on adverse reactions and the prevalence of pharyngeal carriage of N. meningitidis was collected. Only 1 case of meningococcal disease occurred in south Vic toria County in the 4 weeks following 17 February. T he area has not been classified as a hyperendemic area for meningococcal disease and routine vaccination against meningococcal disease was not recommended for visitors.
Les 10 000 enfants de 6 mois à 18 ans des municipalités touchées du comté de Victoria représentaient le groupe cible identifié à la suite de ce groupement de cas. Les secteurs visés ont connu des taux d’atteinte 156 fois plus élevés que ceux qui avaient été prévus d’après l’expérience ontarienne des 4 années antérieures, et 52 fois plus élevés que les taux que laissait prévoir l’incidence provinciale accrue du moment. Etant donné la fréquentation des consultations de vaccination qui fonction naient depuis le 22 février, la couverture a dû être importante. On a recueilli des renseignements sur les réactions fâcheuses et la prévalence du portage pharyngé de N. meningitidis. Au cours des 4 semaines qui ont suivi le 17 février, le comté de Victoria sud n’a recensé qu’un seul cas de maladie méningococcique. La région n ’a pas été déclarée hyperendémique pour la maladie méningo coccique et la vaccination systématique n ’a pas été recommandée pour les visiteurs.

Immunization coverage in 1987 D PT and D T A total of 41 726 infants comprising 96% of the 1987 live births completed their primary course under 1 year of age. An estimated 83% of pre-school children were given a D PT booster dose at 18 months of age. About 38% of the infants and pre-school chil dren were immunized by general pracutioners. During the period 1983-1987, only 0.3% of primary school entrants had not been immunized and required primary immu nization. In 1987,84% of primary I school entrants (6 + years of age) and 93% of primary VI children (11+ years of age) were given a D T booster dose.

On estime à 83% le nombre d’enfants d’âge préscolaire ayant reçu un rappel de DTC à l’âge de 18 mois. Trente-huit pour cent environ des nourris sons et des enfants d’âge préscolaire ont été vaccinés par des médecins généralistes. Au cours de la période 1983-1987, 0,3% seulement des enfants qui sont entrés à l’école primaire n’avaient pas été vaccinés et ont dû recevoir une primo-vaccination. En 1987,84% des enfants entrant en première année d’école primaire (6 ans et plus) et 93% de ceux entrant en sixième année (11 ans et plus) ont reçu un rappel de DT.

The incidence of measles further declined from 218 cases in 1986 to 123 cases in 1987. Rubella incidence remained low with only 2 cases of congenital rubella reported. With the availability of laboratory facilities for the confirmation of pertussis by the immunofluorescence method, a total of 9 cases were reported in immunized children in 1987. It is still early to evaluate the efficacy of ihe hepatitis B immu nization programme. Six cases of acute hepautis B were reported in children below 15 years of age. Seroepidemiological surveys are periodically conducted to monitor the changing prevalence of HBV markers in the population. The immune response of immu nized children is also closely monitored todetermine the duration of protection. 1 See No 23, 1989, pp 176-178
L’incidence delà rougeole a encore baissé, passant de 218 cas en 1986 à 123 cas en 1987. Celle de la rubéole est restée faible avec seulement 2 cas de rubéole congénitale rapportés. Avec l’accès aux analyses de laboratoire pour confirmer un diagnostic de coqueluche par immuno fluorescence, 9 cas au total ont été rapportés en 1987 chez des enfants non vaccinés.
Il est encore trop tôt pour pouvoir évaluer l’efficacité du programme de vaccination anti-hépatite B. Six cas d’hépatite B aigue ont été rap portés chez des enfants de moins de 15 ans. On effectue périodiquement des enquêtes séro-épidémiologiques afin de surveiller les variations de la prévalence des marqueurs du virus de l’hépatite B dans la population. INFLUENZA (July 1989). — 1 Influenza virus has been isolated from 34 of 139 cases investigated during July. Sixteen isolates were identified as influenza A of H3N2 subtype, 4 of H1N1 subtype, and 14 isolates were influenza B. All were isolated from cases of upper respiratory tract infection.
