The existing data in the National Division of Epidemiology and Health Statistics (DNEES) of the Ministry of Health, sent in by the health departments or obtained with the participation of DNEES epidemiologists, have recorded the occurrence in other States, since 1972, of outbreaks of meningitis which is not always specified, so that in some areas too small a number or occasionally too high a number of confirmed meningococcal meningitis cases have been reported. This situation is due chiefly to the absence or limitations of laboratory facilities, including lack of adequately trained staff. Below we give a brief review of outbreaks of meningococcal meningitis and other forms of meningitis in Brazil during the last few years: 1972— Cearâ, 275 cases (54.4% of the total recorded in that year) occurred in the second quarter. In Goiâs, there was an outbreak as from October, with about 25 % of confirmed meningococcal menin gitis cases. This outbreak is still continuing. 1973- 1974— duly confirmed outbreaks occurred or are continuing in the following States: Acre—from January to April 1973, 56 meningococcal meningitis cases in Rio Branco (Capital) with 13 deaths (case-fatality rate 23.3% and morbidity rate 160/100000 inhabitants). Espirito Santo—Municipality of Colatina, from 12 March to 11 April 1973,20 cases with six deaths (case-fatality rate 30%). Alagoas—from January to August 1973, 60 cases of meningitis in general were reported in 15 municipalities. The largest concen trations were in Maceiô (29 cases), many of these probably coining from other municipalities, and in the Municipality of Mundaü (10 cases from May to August), probably of meningococcal menin gitis (there was no laboratory confirmation). Paranâ—from May 1973 onwards there was an increase in the meningitis cases reported, probably mostly of a meningococcal nature (infrequent laboratory confirmation) affecting 59 municipal ities. Up till 12 September there were 301 probable cases of menin gococcal meningitis. MÉNINGITE A MÉNINGOCOQUE Bré sil . — Depuis juin 1971, des épidémies répétées de ménin gite à méningocoque se sont produites dans la ville de Sâo Paulo, d'où elles ont gagné les municipalités de banlieue et d ’autres régions de l'E tat de Sâo Paulo, en augmentant d’intensité. D ’après les données que la Division nationale d’Epidémiologie et de Statistiques sanitaires (DNEES) du Ministère de la Santé a reçues des départements de la santé ou obtenues par les soins de ses propres épidémiologistes, des épidémies de méningite, dont la nature n’est pas toujours précisée, ont frappé d’autres Etats du Brésil depuis 1972; le nombre des cas confirmés de méningite à méningocoque signalés est parfois inférieur, et parfois aussi supé rieur à la réalité, ce qui tient essentiellement à l’absence de services de laboratoire ou aux insuffisances des services existants, et notam ment à la pénurie de personnel qualifié. On trouvera ci-après une brève récapitulation des poussées de méningite à méningocoque et d’autres formes de méningite enre gistrées ces dernières années au Brésil: 1972 — Cearâ, 275 cas pendant le deuxième trimestre (54,4% du total enregistré dans l’année). A Goiâs, une poussée a éclaté en octobre (environ 25% de cas confirmés de méningite à méningo coque) et se poursuit encore à l’heure actuelle. 1973-1974— Des poussées, dûment confirmées, ont en lieu ou sont en cours dans les Etats suivants: Acre — de janvier à avril 1973, 56 cas de méningite à méningo coque à Rio Branco (capitale), dont 13 cas mortels (taux de léta lité, 23,3 %; taux de morbidité, 160 pour 100 000 habitants). The highest concentration of cases occurred in the Municipality of Guaraniacu where, from May to 12 September, 92 cases were reported with nine deaths, a lethality rate of 9.8%, the morbidity and mortality rates being 664 and 65 per 100 000 inhabitants, respectively. Rio Grande do Sul—from January to September 1973 there were 412 cases of meningococcal meningitis in the State, 118 of them in the Municipality of Porto Alegre (morbidity rate, 12 per 100 000 inhabitants) and 64 in Caxias do Sul, up to August. Goiâs—from January to 15 October 1973 there were 392 cases of meningitis in 53 municipalities, of which 227 (57.9%) were con firmed meningococcal meningitis. The lethality rate of these cases was 17.6%, while the morbidity rate was 8 per 100 000 inhabitants. Sâo Paulo—the meningococcal meningitis outbreak is continuing with growing intensity in the State of Sâo Paulo, concentration of cases being greater in the second half of the various years (1971, 1972 and 1973). The number of confirmed cases per month in the Emilio Ribas Communicable Diseases Hospital, in the City of Sâo Paulo, became progressively greater in 1973 (from two-to six-fold) than in 1971 and 1972 and this rise has increased still more in 1974. Minas Gerais—between January and December 1973, 46 cases of meningitis with 12 deaths were recorded in the Municipality of Ipatinga, the majority being probably meningococcal (no labo- 1 Division d'Epidémiologie et de Statistiques sanitaires du Ministère de U Santé. Sâo Paulo — l’épidémie de méningite à méningocoque se pour suit et s’intensifie dans l’Etat de Sâo Paulo; en 1971, 1972 et 1973, la fréquence des cas a été plus forte pendant le second semestre. En 1973, le nombre mensuel des cas confirmés à l’Hôpital pour Conta gieux Emilio Ribas de la ville de Sâo Paulo a progressivement dépassé (de deux à six fois) ceux qui avaient été enregistrés en 1971 et 1972, et il a continué d’augmenter en 1974. Minas Gerais — entre janvier et décembre 1973,46 cas de ménin gite, dont 12 mortels, ont été enregistrés dans la municipalité d’Ipatinga; il s’agissait probablement, en majorité, de méningites h W klf tpldem. From January to June 1974 73 cases of meningitis with 13 deaths were registered in the same municipality, the majority also probably meningococcal (laboratory confirmation, including cultures and determination of serogroup, was commenced in March). As from the second week of August 1973, meningitis cases in Ipatinga, where there used to be isolated cases or, at most, two per week with periods of one to four weeks without any cases reported, commenced to be more frequent over a period of 13 weeks (August, September and October) during which there were 27 cases. After an interval of eight weeks in which there were only four cases the outbreak became more intensive as from the first week of 1974 and there were 52 cases up to the seventeenth week (last week o f April) with the following monthly distribution: January, 7 cases, February, 11, March, 16, April, 18. In May and June there were seven and 14 cases, respectively. (In 17 of the 1974 cases the presence of the meningococcus was confirmed (culture) and in 14 of these cases the serogroup was determined; it was always group C.) Vaccination of the population in Ipatinga (104 000 inhabitants) commenced on 7 April and some 70 000 persons have been vaccinated with antimeningococcal vaccine C. The totals for meningococcal meningitis and other forms of meningitis registered in Brazil in 1974 are shown in Table 1. SALMONELLA SURVEILLANCE S w it z e r l a n d . — 1 As a result of intensive systematic investiga tions carried out by the manufacturers of the baby food which caused an outbreak associated with S. tennessee and S. newington in various regions of the country around mid-July,1 the source of contamination has been traced. Salmonella were identified in a number o f samples of imported yeast. In order to preserve the nutritive value of the yeast, it is mixed cold into the finished pre cooked product. It is this process that allowed the growth of salmonella in the batch which was withdrawn from the market at the time of the outbreak. Effective immediately throughout Switzerland, cantonal chemists have prohibited the use of this yeast without pre-heating. As a precaution, all other packages of the incriminated baby food have been withdrawn from shops and households. 1 See N o. 30, p. 258. Toujours à Ipatinga, 73 cas de méningite, dont 13 mortels, ont été observés de janvier à juin 1974; la plupart étaient sans doute aussi des cas de méningite à méningocoque (la confirmation en laboratoire, par mises en culture et détermination des groupes sériques, a débuté en mars). A partir de la deuxième semaine d ’août 1973, à Ipatinga — où l’on observait auparavant des cas isolés, et, tout au plus, deux cas par semaine avec des interruptions de une à quatre semaines — les cas de méningite devinrent plus fréquents pendant une période de 13 semaines (août, septembre et octobre) au cours de laquelle on en enregistra 27. Après une accalmie de huit semaines pendant laquelle quatre cas seulement furent signalés, l ’épidémie s’intensifia à partir de la première semaine de 1974; à la dix-septième semaine (dernière semaine d’avril), on avait dénombré 52 cas se répartissant comme suit: janvier — 7 cas; février — 11 cas; mars — 16 cas; et avril — 18 cas. En mai et juin, il y eut respectivement sept et 14 cas (pour 17 des cas enregistrés en 1974, la présence du méningocoque a été confirmée par culture et pour 14 d’entre eux le groupe sérique a été déterminé: il s’agissait chaque fois du groupe Q . La vaccination de la population d’Ipatinga (104000 habitants) a commencé le 7 avril; à ce jour, quelque 70 000 personnes ont reçu le vaccin antiméningococcique C. Le Tableau 1 indique le nombre des cas de méningite à méningo coque et d’autres formes de méningite enregistrés au Brésil en 1974. SURVEILLANCE DES SALMONELLA S uisse . — 1 Les examens systématiques et intensifs effectués par le fabricant, afin d’élucider la source de la contamination par S. ten- nessee et S. newington de l’aliment pour bébés qui causa vers mi- juillet une poussée associée à ces deux sérotypes dans diverses régions du pays,1 ont abouti à un résultat positif. Des salmonella ont été identifiées dans plusieurs échantillons d ’une levure provenant d’un fournisseur étranger. Afin de conserver la valeur nutritive de cette levure, on la mélange à froid au produit fini après la pré cuisson de ce dernier. C’est ce procédé qui a rendu possible la pré sence de salmonella dans le lot qui fut retiré en son temps du marché. Les chimistes cantonaux ont interdit dans toute la Suisse, avec effet immédiat, l’emploi de cette levure sans chauffage préalable. A titre préventif les boites du produit alimentaire incriminé qui se trou vaient encore dans le commerce et dans les ménages ont été retirées. ' Voir N® 30, p. 238. Salmonella virchow Infection in a District General Hospital and Local Community U n it e d K in g d o m . — A 52-year-old man who had been admitted to a surgical ward for gastrectomy on 4 April 1974, was transferred to the intensive care unit of a District General Hospital on 12 April. Within the next few days he was noted to have diarrhoea and a spe cimen sent to the laboratory on 24 April yielded an unidentified salmonella (Group Ç). Since the patient was too ill for transfer to an isolation hospital and because other patients in the unit had developed diarrhoea, it was decided to close the ward to further admissions. Of six patients at risk from the original patient four became infected, and o f 88 staff members visiting or working in the ward, three were found to be symptomless excreters. The first fatality was a 65-year-old male patient with respiratory and renal failure, who developed diarrhoea and became critically ill. Despite treatment with chloramphenicol he died within a few hours and an unidentified salmonella (Group C), isolated from his stool and blood culture, was found to be resistant to all antibiotics tested with the exception of trimethoprim, colistin and gentamycin. This isolate was subsequently confirmed by the Salmonella Reference Laboratory to be Salmonella virchow and by the Enteric Reference Laboratory to possess transferable drug resistance. Another male patient in the unit, suffering from multiple sclerosis, contracted severe diar rhoea and an attempt was made to treat him with gentamycin. After ten days treatment and some initial success, he died and S. virchow was isolated from his blood culture terminally. Dans les quelques jours qui suivirent, il fut atteint de diarrhée et, le 24 avril, l’analyse d ’un échantillon de ses selles révéla la présence d'une salmonella non identifiée (groupe Q . Comme le malade était en trop mauvais état pour être transféré dans un hôpital d’isolement, et comme les autres malades du service avaient également la diarrhée, on décida de ne plus admettre aucun nouveau malade dans ce service. Sur six malades exposés à des contacts avec le cas initial, quatre furent infectés, et sur 88 membres du personnel qui s’étaient rendus dans le service ou y travaillaient, on découvrit trois excréteurs asymptomatiques. Le premier cas mortel fut un homme de 65 ans, atteint d’insuffisance respiratoire et rénale, dont l’état devint critique après qu’une diarrhée se fut déclarée chez lui. Malgré un traitement au chloramphénicol, il succomba après quelques heures. Une salmonella non identifiée (groupe C) fut isolée de ses selles et d ’une culture sanguine, et l ’on constata qu’elle était résistante à tous les antibiotiques essayés, sauf à la triméthropime, à la colistine et à la gentamycine. Le Labo ratoire de référence pour les Salmonella confirma par la suite qu’il s’agissait de Salmonella virchow et le Laboratoire de référence pour les infections intestinales nota que cette souche possédait une pharmaco-résistance transférable. Un autre malade du service, atteint de sclérose en plaques, fut aussi atteint d ’une diarrhée grave. Le traitement à la gentamycine donna d’abord quelques résultats mais le malade finit par mourir dix jours plus tard, S. virchow étant alors isolée dans une culture sanguine.
