SURVEILLANCE OF PENICILLIN-RESISTANT GONORRHOEA S in g a p o r e . — The number of genital gonococcal infections detected in Singapore in 1975 was 2 997. In 1976 due to better and more vigilant case-detection the figure increased to 7 668. There is no evidence to show that the morbidity rate had increased during that period but there were changes in the resistance pattern to penicillin among circulating gonococcal strains. Gonococci with both the following types of resistance are being encountered with greater frequency in recent years: (a) Total resistance in which certain strains of Neisseria gonarrhoeae produce an enzyme, ^-lactamase (penicillinase), which destroys the penicillin nucleus. These strains, also known as penicillin ase-producing N. gonarrhoeae (PPNG) are completely resistant to therapeutic doses of penicillin. (b) Partial or relative resistance in which there is no production of P-lactamase but a higher dose of penicillin is required to destroy these strains of gonococcus. From Table 1 it is evident that there has been a significant increase in the number of PPNG infections in 1978 as they comprise 4.8% of total isolates up till thp end of July as compared to 0.24% for the whole of 1977. Of the 259 cases which had been treated by the end of July, 192 (74.1%) were in prostitutes. O f the remaining 67 (25.9%), 88.1% acquired the infection locally, and mostly from prostitutes, and 11.9% contracted it outside Singapore. It was found that 60% of the usual clients o f the infected prostitutes were tourists and the r e m a in in g 40 % were relatively affluent local citizens who were likely to have travelled to other countries. The present findings suggest that the PPNG infections found in Singapore are mainly “imported” either by tourists o r locals who have recently returned from abroad. PPNG infections are known to be prevalent in a number of countries.1
SURVEILLANCE DE LA GONOCOCCIE PÉN ICIIJ .1NORÉSISTANTE S in g a p o u r . — En 1975, on avait détecté à Singapour 2 997 cas d ’infections gonococciques génitales. Le chiffre est passé à 7 668 en 1976 grâce à une vigilance accrue et à un meilleur dépistage. Rien n ’indique que le taux de morbidité ait augmenté mais le tableau de la résistance à la pénicilline parmi les souches circulantes de gonocoques s’est modifié. Les deux types de résistance ci-après se rencontrent plus fréquemment depuis quelques années:
a) Résistance totale — Certaines souches de Neisseria gonarrhoeae produisent une enzyme, la (3-lactamase (pénicillinase) qui détruit le noyau de la pénicilline. Ces souches, dites aussi «N . gonarrhoeae productrices de pénicillinase» (NGPP) résistent complètement aux doses thérapeutiques de pénicilline. b) Résistance partielle ou relative — Il n’y a pas production de (3-lactamase, mais une plus forte dose de pénicilline est nécessaire pour éliminer les souches en cause. U ressort du Tableau 1 que le nombre des infections à NGPP a notablement augmenté en 1978, les isolements correspondants repré sentant 4,8% du total à la fin de juillet contre 0,24% pour toute l’année 1977. Sur 259 cas de ce type traités au 31 juillet, 192 (74,1 %) concernaient des prostituées ; pour les 67 autres (25,9 %), on comptait 88,1 % de sujets infectés localement, surtout par des prostituées, et ll,9 % d e sujets infectés en dehors de Singapour. R a été établi que la clientèle habituelle des prostituées contaminées se composait pour 60 % de touristes, pour 40 % de Singapouriens relativement riches qui avaient probablement voyagé à l’étranger. Ces constatations laissent penser que les infections à NGPP découvertes à Singapour sont en majorité «im portées» par des touristes ou par des Singapouriens revenus récemment de l ’étranger. On sait que les infections à NGPP sont répandues dans un certain nombre de pays.1

The laboratory surveillance of non-PPNG strains revealed an increasing proportion of strains which were resistant to penicillin at a minimum inhibitory concentration (MIC) of 0.06 [ig/ml. (Table 2 ). This trend is reflected among patients with non-PPNG infections for whom the treatment failure rate to the standard dosage of three mega units of procaine penicillin plus one gramme of probenecid rose from 2.5% in 1975 to 10.2% in 1977 (Table 2 ).
L ’étude en laboratoire des gonocoques non NGPP a révélé une proportion croissante de souches résistant à la pénicilline à la concen tration minimale inhibitrice (CMI) de 0,06 (xg/ml (Tableau 2 ). Une tendance analogue s’observe chez les malades dont l’infection n’est pas à NGPP; le taux d'échecs thérapeutiques avec la dose standard de trois millions d ’unités de procalne-pénicilline plus un gramme de probénécide est passé de 2,5 % en 1975 à 10,2 % en 1977 (Tableau 2).
