Decentralization of case management training was pur­ sued through wide use of the Guidelines for conducting clinical training courses at health centres and small hospitals1 and tire ' Notes on training o f trainers.2* More than 40 courses for clinical trainers were conducted in 32 countries. efforts have been devoted to learning more about and con­ trolling the spread of a new strain, Vibrio cholerae 0139, which has caused major, epidemics in South Asia.. Attention has also been given to the control of epidemic dysentery due to SMgeUa dyserueriae type 1 .in Southern Africa. In relation to evaluation, the Programme promoted the use of the guides for household and health facility surveys. During 1992, 18 household surveys of diarrhoea case man­ agement were conducted in 15 countries. This brought the total number of surveys conducted to at least 88 in 36 countries since 1989, when the guide became available. In the meantime, a new household survey guide, integrating questions on diarrfaoeal diseases, acute respiratory infec­ tions, and breast-feeding has been developed and fieldtested in the Philippines, Sri Lanka and the United Republic of Tanzania. During 1992, health facility surveys were con­ ducted in 12 countries, bringing the total number of surveys supported by the Programme to at least 21 in 20 countries since the method became available in 1990. In addition to these two survey guides, the Programme completed a new method of reviewing national programme progress — the Focused Programme Review — which is a problem-solving approach to assessing all areas of the CDD Programme, identifying achievements and constraints, and developing recommendations and plans to improve programme perfor­ mance. Vers la réalisation des objectifs mondiaux du programme

The global progress in ORS access, and ORS and/or recommended home fluids (RHF) use rates is presented in Fig. 1. ORS access rate means the proportion of the popula­ tion with a regular supply of oral rehydration salts in their community. At the end of 1992, this rate was estimated to be 73%. ORS and/or RHF use rate represents the percentage of diarrhoea episodes in children under 3 years of age who have received oral rehydration salts and/or a recommended home fluid. This rate was estimated to be 46% at the end of 1992, an increase of 8 percentage points over the rate in 1991. F ig. 1 Global ORS access, and ORS and/or RH F use rates,a 1984-1992

methods to measure fluid-giving behaviour and fluid intake during diarrhoea episodes; and care-seeking practices for children with diarrhoea. The Programme gave pnority to the development of a Focused Ethnographic Study (FES) protocol to gather de­ scriptive data oh local terms, beliefs and practices, and local conditions and constraints, which should be used for die adaptation of home care advice and the identification of factors influencing the correct home treatment of diarrhoea. Activities in disease prevention research focused on a few pnority areas: development and evaluation of feeding interventions, particularly those related to promotion, pro­ tection and support of breast-feeding; the effect of breast­ feeding on die prevention of shigellosis among young and malnourished children; the identification of hygiene be­ haviours which influence in a significant way the incidence of diarrhoea in children (sanitary disposal of faeces, hand­ washing, maintaining drinking-water free from faecal con­ tamination); the role of'vitam in A and micronutrients in prevention of diarrhoea (and acute respiratory infections); and the evaluation of candidate vaccines against infections by rotavirus, cholera, shigella and enterotoxigenic Es­ cherichia coli. An evaluation of the efficacy of tetravalent rhesushuman rotavirus vaccine was completed in Brazil and a large trial involving a higher dose of the vaccine was initiated in Venezuela. A trial of the live oral cholera vaccine CVD-103-HgR was initiated in Indonesia. The Programme sponsored some activities addressed to enhancing the capacity of national institutions to conduct high quality research in priority topics. In a workshop in Turkey, researchers from 20 developing countries designed 14 proposals for intervention-related research projects. Re­ search strengthening support was provided to 6 institutes in Brazil, India, Peru and the Philippines with the aim of improving their facilities and their capacity to conduct re­ search on the treatment and prevention of diarrhoea.

La recherche sur la prévention de la maladie s’est concentrée sur quelques secteurs prioritaires: mise au point et évaluation d’inter­ ventions nutritionnelles, en particulier celles qui sont en rapport avec la promotion, la protection et le soutien de l’allaitement au sein; l’effet de l’allaitement au sein sur la prévention de la shigellose chez les-enfants en bas âge et les enfants malnutris; l’identification des comportements en matière d’hygiène qui sont susceptibles d’influer de façon significative sur l’incidence de la dianhée chez l’enfant (évacuation hygiénique des matières fécales, lavage des mains, ab­ sence de contamination de l’eau destinée à la boisson par des matières fécales); rôle de la vitamine A et des micronutriments dans la prévention de la dianhée (et des infections respiratoires aiguës); évaluation de candidats vaccins contre les rotavirus, le choléra, la shigellose et les colibacilloses à Escherichia coli entérotoxigène. Une évaluation de l’efficacité d’un vaccin antirotavirose tétravalent rhésus-humain a été menée à bien au Brésil, et un vaste essai clinique portant sur une dose plus élevée du vaccin a commencé au Venezuela. En Indonésie a débuté l’essai clinique d’un vaccin anti­ cholérique buccal vivant, le CVD-103-HgR. Le programme a parrainé un certain nombre d’activités en vue d’améliorer la capacité des institutions nationales à réaliser des recherches de grande qualité dans des domaines prioritaires. Lors d’un atelier organisé en Turquie, les chercheurs de 20 pays en développement ont préparé 14 propositions de projets de recherche interventionnelle Le programme a aidé au renforcement de la re­ cherche dans 6 instituts du Brésil, de l’Inde, du Pérou et des Philip­ pines afin d’améliorer leurs installations et leur capacité à mener des recherches sur le traitement et la prévention de la diarrhée.

Veterinary public health Computer software package A computer software package, EPIZOO, has been devel­ oped specifically for analysis of information on animal health and diseases, including those transmissible to man. The software package includes indicators to analyse data on morbidity and mortality, geographical distribution, animal population characteristics, and dynamics of epizootics. It helps to prepare for investigations of animal population health and disease, to analyse diagnosis and consequences of illness, to design animal health programmes, to estimate costs and evaluate programmes. It also includes some sam­ pling and other simple and practical statistical techniques. It is written to.facilitate information management for problem-solving, modelling, simulation studies and training in veterinary epidemiology and for action-oriented epide­ miological analyses and decision-making. The EPIZOO version 2.4 fits onto one 1.4 megabyte diskette and can be used on IBM compatible personal computers with MSDOS operating system, • The software package, which was developed by Professor V. Kouba (B.P. 516, 17000 Prague 7, Czech Republic), is easy to use and does not require training or even a manual. Ce progiciel comporte des indicateurs permettant d’analyser les données relatives à la morbidité et à la moitalité, à la distribution géographique, aux caractéristiques de la population animale et à la dynamique des épizooties. Il permet la préparation d’enquêtes sur la santé des popu­ lations animales et leurs maladies, l’analyse du diagnostic et des conséquences des maladies, l’élaboration des programmes de santé animale et leur évaluation, notamment d’un point de vue financier. Il permet également de procéder à des échantillonnages et de mettre en œuvre d’autres techniques statistiques simples et pratiques. Il est écrit dans le but de faciliter la gestion de l’information en vue de la solution de problèmes, d ’études de modélisation et de simulation, et de la formation à l’épidémiologie vétérinaire; il permet également d ’effectuer des analyses épidémiologiques e t de prendre des décisions dans un but pratique. Influenza F inland (12 November 1993). Influenza A has been diag­ nosed by detection of the virus antigen in 2 children in the south-eastern part of the country. These cases were the first confirmed signs of in flu e n za activity this season, but local outbreaks and high absenteeism among young children in some schools have been reported as due to influenza-like illness since the last week of October. F rance (10 November 1993).* Additional cases of influ­ enza A have been confirmed by isolation or detection of the virus antigen across the country. A few have been further typed and found to be influenza A(H1N1). Sweden (9 November 1993).2 The number of cases of influenza-like illness started to increase in the Stockholm area in the last week of October. Since the beginning of November influenza A has been diagnosed in 19 cases, 2 of which were subtyped as influenza A(H3N2). Influenza B has been confirmed in 1 case. 1 See N o. 43,1993, p. 317. ! Grippe Finlande (12 novembre 1993). La grippe A a été diagnostiquée par détection de l’antigène du virus chez 2 enfants dans le sud-est du pays. Ces cas étaient les premiers signes confirmés d’activité grippale cette saison, mais des flambées locales et un absentéisme scolaire élevé parmi les jeunes enfants sont signalés depuis la dernière se­ maine d’octobre comme étant dus au syndrome grippal. France (10 novembre 1993).* D’autres cas de grippe A ont été confirmés par isolement ou détection de l’antigène du virus à travers le pays. Quelques-uns ont été caractérisés et se sont révélés être des virus A(H1N1).
