TETANUS B elgium. — Since 1954 there has been a general downward trend in the number of reported tetanus cases and deaths1 Morbidity and mortality reached the lowest figure in 1970, when 16 cases with seven deaths were reported, giving incidence and mortality rates of 0 16 and 0 07 per 100 000 population However, the actual figures were probably much higher, as it is estimated that each year an appreciable number of tetanus cases and deaths are not reported. In 1970, the disease was more frequent and more severe in the higher age-groups 11 o f the 16 cases (68 8%) and five of the seven deaths (71 4%) occurred among persons aged 40 years and over, the remainder being among children between 5 and 14 years of age. Males (13 cases with six deaths) were affected more than fe m ales (three cases with one death) Most of the patients had suffered only a minor injury, frequently of the hand or knee All the patients with one exception were Belgian nationals The majority of the cases and deaths occurred in predominantly rural communities (10 cases with four deaths) Of the five cases occur ring m cities, two were fatal and the remaining case, also fatal, was reported from a predominantly industrial community. In Belgium, only servicemen, the police and certain categories o f workers are immunized against tetanus as a routine measure; m 1970, of the 16 patients, only one had been vaccinated against tetanus However, the number of doses of tetanus toxoid distrib uted is increasing each year, and there is a constant rise in the number o f young children vaccinated In 1970, 88% of infants between three months and one year of age received a first dose of diphthena-tetanus-pertussis vaccine, 85% a second dose, and 82% a third dose 1 See No. 45, 1970, p 501

INFLUENZA A ustria (information dated 11 December 1971) — In Vienna, since the 1 December, there has been an increase in the incidence of influenza-like illness (about 10 000 cases reported for week ended 11 December) The disease, which is generally very mild, affects mostly adults. Two strains of virus A2/Hong Kong/68 have been isolated. B ulgaria (information dated 10 December 1971). — 1 The epi demic of influenza-like illness has been decreasing since 25 October m Sofia and m the rest of the country A total of 390 604 cases were registered in Bulgaria during the penod from 1 October to 30 November During the same penod, 250 strains of virus A2/Hong Kong/68 were isolated in the National Influenza Centre and various laboratones, and serological evidence of infection with virus A2/Hong Kong/68 was obtained in 184 of 271 paired sera tested at the National Influenza Centre (beside a small number of infections with viruses B andQ . F ederal R epublic of G ermany (information dated 22 December 1971). — * In Lower Saxony, there are several outbreaks of influenzalike disease spreading southward Twelve strains of virus A2/Hong Kong/68 have been isolated In Nordrhwe-Westfalia, RhtnelandPalatinate and Saarland local increases m the incidence of influenzalike illness have been reported. F inland (information for week ended 11 December 1971) — * Extensive influenza epidemics have occurred m the southern parts of the country, and smaller epidemics in the northern parts Ab senteeism in Helsinki was 6 6% for the week. F rance (information dated 13 December 1971) — In Pans, there are a number of sporadic cases of influenza-like illness. Two strains antigemcally related to virus A2/Hong Kong/68 have been isolated. H ungary (information for week ending 10 December 1971) — s The epidemic associated with virus A2/Hong Kong/68 is declining in most areas of the country, except in the southern and western counties, where it only started about 20 November and where the attack rate has been 5 to 10% so far In the towns of Budapest and Miskolc, and in the Borsod County (in the north), 350000 cases of influenza-like illness were notified during the week ended 4 De cember (attack rate 20% since 1 November) During the week ended 10 December, 300 new fatal cases were reported in the country, and complicated cases represented 5 7 % of notified cases. As shown in the table which appeared in No 50, a proportion o f the isolated strains have presented certain antigemcal differences from the A2/Hong Kong/1/68 prototype. S pain (information dated 18 December 1971) — * In the whole of the Barcelona Province, the epidemic associated with virus A2/Hong Kong/68 is now declining (peak in Barcelona: 25 Novem ber; outside Barcelona: 30 November) U ntied K ingdom (information dated 17 December 1971) — The first evidence o f the appearence of influenza A this winter was the isolation of a strain similar to the A2/Hong Kong/68 virus from an infant which died on 19 November m the north of England A t the same time an epidemic o f influenza began in a girl’s school in the south of the country, where 100 cases occurred out of a total of 227 pupils. Strains of virus antigemcally similar to the A2/Hong Kong/68 variant were isolated. Strains have since been isolated from sporadic cases m several areas in the south of England, those strains examined so far are antigemcally similar to the A2/Hong Kong/68 variant except for one which is more like A2/England/878/69.

to the Center for Disease Control Eight o f these cases occurred in persons working with imported goat hair, four m those working with imported wool, and one each m persons who had contact with goat skins, bone chips and an infected cow For one patient, the source of infection was unknown Fourteen of the patients had cutaneous anthrax, and two had inhalation anthrax. The sources o f infection for these latter two patients who died were imported goat hair and wool. As indicated above only one case, m a man who buried anthrax-infected cattle, was associated with domestic annual husbandry. All four of the human cases reported in 1971, however, were associated with domestic anim al husbandry: two fanners from Mississippi and two Louisiana veterinarians who necropsied anthrax-infected cattle Animal anthrax is endemic m several regions, but has occurred sporadically in almost every state Most outbreaks m recent years involved few animals. Occasionally, however, larger out breaks are reported, such as the recent 1971 Louisiana epizootic in which over 700 animals died.
Outbreaks m grazing animals are usually associated with periods o f marked environmental change, such as drought, heavy rainfall, and flooding which apparently induce proliferation o f B anthracis in sod. The sporadic occurrence of the necessary environmental factors may allow many years to pass between recognized out breaks even in known endemic areas, serving to emphasize the need to maintain the vaccination of herds

Quatorze malades présentaient la forme cutanée de la maladie, deux la forme pulmonaire; ces deux derniers, qui sont décédés, avaient été contamines par des fourrures de chèvre et de la laine importées. Ainsi, un seul des 16 cas, survenu chez un homme qui enterrait des bovins infectés, était associé à l’élevage d ’animaux domestiques. Par contre, les quatre cas signalés chez l’homme en 1971 ont tous été en relation avec l’élevage: il s’est agi de deux agriculteurs du Mississippi et de deux vétérinaires de Louisiane qui autopsiaient des bovins infectés. Chez l’animal, l’infection est endémique dans plusieurs régions mais s ’est manifestée de façon sporadique dans presque tous les états. La plupart des flambées qui ont été enregistrées ces dernières années ne portaient que sur un petit nombre de bêtes. H arrive cependant que l'on signale des flambées plus importantes, telle l’epizootie qui a éclate en 1971 en Louisiane et au cours de laquelle plus de 700 animaux sont morts. Les poussées qm touchent les herbivores sont généralement asso ciées a de grands changements météorologiques, qu’il s’agisse de sécheresse ou au contraire de pluies torrentielles et d'inondations qui favorisent apparemment la prolifération de B. anthracis dans le sol Etant donné le caractère sporadique des phénomènes écolo giques favorables à l’appantion de la maladie, de nombreuses années peuvent s’écouler sans que l’on décèle une poussée même dans des zones d ’endémie connues, ce qui montre bien la nécessité de vacciner régulièrement les troupeaux.

RUBELLA U nited K ingdom — German measles in infancy and childhood is mild and the clinical picture well established and often charac teristic Consequently, laboratory tests to establish the diagnosis, either by virus isolation or by serological methods, are usually undertaken only when complications of the disease are encountered, or when the possibility of rubella m pregnancy is being considered. The two recognized complications of rubella, apart from the congenital abnormalities associated with foetal infection, are ence phalitis and arthritis Encephalitis was the presentmg symptom in five (0 8%) of the 631 cases reported in 1971 and all five patients survived. The incidence of rubella encephalitis is believed to be about 0 02%, so that the relatively high number of cases reported must refiect the tact that diagnostic investigations are more likely to be done in such patients No cases of arthritis have been re ported this year, but the incidence of this complication is believed to vary from outbreak to outbreak Moreover, arthritis is rarely a serious complication and its clinical diagnosis usually not difficult, so that laboratory investigations are seldom needed

The most common reason for laboratory diagnostic work in rubella is in relation to infection in pregnancy In this connexion recovery of rubella virus has been reported from 1 572 patients since January 1967, and of these patients approximately 15% were infants surviving with congenital abnormalities, and 22% were women ot child-bearing years In about one-third of the latter, the virus was recovered from products of conception obtained by therapeutic abortion However, these figures give little idea of the size of the problem that is presented in the United Kingdom by rubella infection m early pregnancy, because m many cases laboratory tests are not undertaken. A better idea is given by the number of therapeutic abortions which are earned out in order to prevent congenital rubella Notification of therapeutic abortion is required by the 1967 Abortion Act, and 907 of these operations were ascribed to rubella in the Registrar-General’s report of 1969 A further indication of the number of pregnancies in which the possibility of rubella infection causes alarm can be obtained from the issues of immunoglobulin for prophylaxis. This is given, al most entirely, when a pregnant woman who is not known to have had rubella previously, encounters the disease and is not prepared to contemplate a therapeutic abortion. Such an event is not un common since m the United Kingdom about 15% o f women of child-bearing years are still susceptible to the infection. It is doubt ful if the procedure has a significant protective effect, and the patient and her doctor therefore remain uneasy until the outcome of the pregnancy is known In 1970 the amount of normal immuno globulin issued in England and Wales for rubella prophylaxis was approximately 23 000 doses It is evident that congenital rubella, and the fear of acquiring the infection in the early months of preg nancy, still causes much anxiety, which the widespread use in girls of the currently-available vaccine should do much to relieve
R oyaume-U ni — Chez les enfants en bas âge et les jeunes enfants, la rubéole est bénigne et son tableau clinique, bien connu, est sou vent caractéristique Aussi ne fait-on d’épreuves de laboratoire pour poser le diagnostic, soit par isolement du virus, soir par des méthodes sérologiques, q u ’en cas de complications ou quand on craint la présence de l’infection chez une femme enceinte A part les anomalies congénitaies associées a l’infection fœtale, l’enccphalite et l’arthnte sont les deux complications classiques de la rubéole En 1971 il a été signale 631 cas de rubeole, l’encephalite étant le symptôme qui a provoque la consultation pour cinq d ’entre eux (0,8%) Ces cinq sujets ont survécu Comme on estime l’mcidencc de la rubeole compliquée d ’encephahte a environ 0,02%, le nombre relativement elevé de cas signales doit résulter de ce qu’en presence d’une telle complication on a davantage tendance i etayer le diagnostic par des recherches Aucun cas d ’arthnte n ’a ete signale cette année, mais on pense que l’mcidence de cette complication varie d ’une poussée a l’autre. De plus, l’arthrite constitue rarement une complication grave et son diagnostic clinique n ’est généralement pas difficile, de sorte que des examens de laboratoire sont rarement necessaires. C ’est le plus souvent en cas d ’infection pendant une grossesse qu’on procède a des analyses diagnostiques. Depuis janvier 1967, on a trouve le virus de la rubeole chez 1 572 malades, dont 15% environ étaient des enfants en bas âge atteints d’anomalies congéni tales et qui ont survécu, alors que 22% étaient des femmes d’âge fertile Chez un tiers environ de ces femmes, le virus a été trouve sur le produit de conception obtenu par avortement thérapeutique. Cependant, comme dans beaucoup de cas on ne procédé pas a des épreuves dé laboratoire, ces chiffres ne donnent guère l ’idée de l’importance du problème que la rubeole en début de grossesse représente au Royaume-Uni. On peut en avoir une idée plus précise d ’apres le nombre des avortements thérapeutiques effectues pour empêcher une rubeole congénitale La loi de 1967 sur l’avortement exige la declaration des avortements thérapeutiques; 907 de ces avortements sont imputes a la rubeole dans le rapport du RegistrarGeneral pour 1969 Le nombre de prescriptions d'immunoglobulines à des fins pro phylactiques donne egalement une idée du nombre de grossesses pour lesquelles on craint un risque d ’infection rubeoleuse. Les immunoglobulines sont presque uniquement administrées aux femmes enceintes sans antecedents rubéoleux connus, mais qui, ayant contracte la maladie, n ’envisagent pas de se prêter a un avorte ment thérapeutique Une telle situation n ’est pas rare, puisqu’au Royaume-Uni 15 % environ des femmes d’âge fertile restent sensibles a l’infection On ne sait pas si cette mesure assure une réelle pro tection, et la malade ainsi que son médecin restent dans le doute jusqu’à ce qu’on connaisse l’issue de la grossesse. En 1970, U a été administre environ 23 000 doses d ’immunoglobuline normale pour la prophylaxie de la rubeole en Angleterre-Galles. La rubéole congé nitale et la crainte de contracter l’infection dans les premiers mois de la grossesse commuent manifestement a causer beaucoup d ’inquietude, on pense toutefois que l’emploi, très répandu chez les jeunes filles, du vaccin actuellement disponible devrait beaucoup contribuer a dissiper cette inquietude.

Sweden — During a period of 20 months (1969-1970) 58 patients investigated at the Department for Infectious Diseases, Danderyd Hospital, Danderyd, Sweden, were found to excrete Giardia lamblia in their stools Ten of these excretors were children below seven years of age from countries in Asia, Africa and South America They had recently arrived m Sweden to be adopted by Swedish families. S uede — En vingt mois (1969-1970), 58 malades examinés au service des maladies infectieuses de l’hôpital de Danderyd (Suède) se sont révélés excreter Giardia lamblia dans leurs selles Dix d ’entre eux étaient des enfants de moins de sept ans venus de pays d’Asie, d ’Afrique et d ’Amenque du Sud Ils étaient arrives depuis peu dans le pays pour être adoptes par des familles suédoises La plupart étaient des porteurs asymptomatiques Au contraire, les autres malades qui excrétaient Giardia lamblia dans leurs selles, présentaient des troubles intestinaux graves avec diarrhée et, souvent, un manque d ’assimilation, tous, a l’exception de quatre, avaient ete infestes à l’occasion de voyages dans divers pays Quatre malades (appartenant à trois familles) avaient été contaminés en Suede par des enfants etrangers récemment adoptés, qui eux-mêmes excrétaient Giardia lamblia Le traitement par le metronidazole s’est révélé efficace
