Global health security – challenges and opportunities with special emphasis on the International Health Regulations (2005) Executive Summary 1. Significant public health threats exist in many countries. The vast majority of threats to public health security are posed by emerging and re-emerging outbreak-prone infectious diseases. However, threats related to accidental or deliberate release of chemical, radiological and nuclear agents are of increasing concern. Public health threats in one country are just a few hours away from another country and the opportunities for these threats to spread globally are increasing all the time. Gains in many areas of infectious disease control are seriously jeopardized by the spread of antimicrobial resistance. Mass gatherings also pose unique risks to health security. 2. The importance of collective action to manage global health emergencies has long been recognized. Many initiatives, agreements, frameworks and WHO resolutions have been agreed upon to support the building of national capacities in specific areas of work and enhance international coordination and collaboration in responding to related events. The International Health Regulations (2005) impose obligations on States Parties to develop and maintain core public health capacities for surveillance and response, including at points of entry, in order to early detect, assess, notify, report to WHO events covered by their provisions, and to make a tailored response to local situations on the ground and with the advice of the emergency committee. Fulfilment of these obligations benefits the pursuit of global health security. 3. The five-year target date for implementing the Regulations following their entry into force in 2007 passed in 2012. Of the 21 States Parties in the WHO Eastern Mediterranean Region, 20 fell short of the implementation goals. The June 2014 deadline to ensure that certain functional capacities required by the Regulations are in place throughout the territories of States Parties has also now passed and 13 have requested a second extension. The regional implementation level of core capacity requirements was 70% in 2013, compared with 64% in 2012 and only slightly higher than the global implementation level of 68%. States Parties are making good progress in surveillance, laboratory, risk communication, legislation, coordination and food safety. However, a number of challenges exist that not only affect the building of the required capacities but also the maintaining of those capacities. WHO also faces challenges to its ability to continue meeting its mandate as the Secretariat responsible for overseeing implementation of the Regulations. 4. There is an urgent need for investment in all-hazard surveillance and response capabilities in the Region. Without such investment, compliance with the obligations of the Regulations will be poor, creating dangerous holes in global health security. Combined national and international efforts are needed to ensure that all countries have the necessary capacity for early detection, investigation, reporting and responding to public health events. With these collective national and international efforts, implementation of the Regulations and of other initiatives, frameworks and resolutions will result in a greater level of global health security, and the opportunity for diseases to cross borders will be much less as more countries become capable of maintaining their own health security. The only way to guarantee global, and regional, health security is to implement the International Health Regulations (2005). 5. Meeting the obligations of the International Health Regulations (2005) is a decision made by State Parties alone. WHO may advise Member States, at their request, on their implementation of the

Regulations. Thus, declarations by Member States with regard to having met the obligations do not necessarily mean that all the required capacities are available on the ground for early detection, assessment and notification of and response to public health events of potential international concern. The Regional Committee may wish to consider the option of strengthening and sustaining the ability to review progress in implementation of the Regulations. This might require, for example, establishing a regional structure for the assessment and independent verification and certification of the capacities in States Parties to manage public health events. Introduction 6. Over the past 60 years, the world has made significant progress in improving human health. However, global health challenges remain and ways to address them are being studied. Although locally relevant solutions are needed to address most health problems, some health issues can only be solved using a global approach. The experiences of recent years with severe acute respiratory syndrome (SARS) and avian influenza H5N1, in particular, have brought attention to the various issues surrounding health security. Global health issues are of concern to all Member States because they transcend national boundaries and need different actors to work together to address them. 7. Significant public health threats exist in many countries. Increased population movement, whether due to tourism, migration or the result of disaster, growth in international trade, and social and environmental changes have all reaffirmed that infectious disease in one country is only a few hours away from becoming a threat to another country and potentially a concern for the entire world. These threats can devastate countries that are not well prepared to deal with them. An outbreak of disease, for example, can rapidly become a global health threat as the result of a combination of factors. These include: lack of information; insufficient capacity at country level to rapidly detect public health events, and to contain them; fear of costly repercussions if these events are notified; and lack of appropriate overarching international response mechanisms, both legal and technical. 8. The concept of global health security has expanded over time from an infectious disease-only perspective to one that incorporates any serious risk to public health with international implications. Many public health emergencies could have been prevented or better controlled if the health systems concerned had been stronger and better prepared. Global health security ultimately depends on the quality of national public health systems and needs international cooperation and governance. 9. Many global steps have been taken to protect countries from threats to public health. The most important of these in recent years has been the International Health Regulations (IHR 2005) which govern the response to public health emergencies that pose an international threat and which became binding on States Parties on 15 June, 2007. The Regulations provide the legal framework for collective responsibility in global health security, imposing obligations on States Parties to develop and maintain core public health capacities. Fulfilment of these obligations benefits the pursuit of global health security, and will protect all countries from significant public health threats, whether these originate from biological, chemical, radiological or nuclear sources. 10. The purpose of this paper is to highlight the importance of the International Health Regulations (2005) for global health security and to provide the Regional Committee with an update on where the Region stands in relation to implementation of the Regulations. Threats to global health security 11. The vast majority of threats to public health security are posed by emerging pathogens and reemerging outbreak-prone infectious diseases. However, threats related to accidental or deliberate release of chemical, radiological and nuclear agents are of increasing concern. As the world becomes increasingly more interconnected, so there is increasing opportunity for these public health threats to have global impact.

Epidemic-prone diseases 12. Outbreaks of acute infectious diseases, such as cholera, meningitis, measles, yellow fever, haemorrhagic fevers, influenzas and Nipah virus infection, among others, pose a threat to global public health security and require containment at their source to prevent illness and mortality. In 2013–2014, a number of countries in the Eastern Mediterranean Region reported major outbreaks of a range of acute infectious diseases to WHO. These included Crimean–Congo haemorrhagic fever, cholera, dengue fever, severe forms of pandemic influenza (H1N1) 2009, measles, avian influenza (H5N1) and yellow fever. 13. The current outbreak of Ebola virus disease, caused by a strain of ebolavirus with very close homology (98%) to the Zaire ebolavirus, is the first time this disease has been detected in west Africa. It evolved rapidly in Guinea, spreading to neighbouring countries Liberia and Sierra Leone, and with one imported case reported from Nigeria, making this the largest geographical spread of an Ebola outbreak ever reported since the disease first emerged in 1976 in human populations in Zaire (currently Democratic Republic of Congo) and southern Sudan. As of 27 July 2014, the cumulative number of cases attributed to the disease in the four countries stands at 1323 including 729 deaths. The frequency of human travel between the affected countries and some countries in the Eastern Mediterranean Region is high, which poses a risk for transmission of the virus to the Region. 14. Within the Region, two major global health events are currently ongoing with potential for major impact on countries outside the Region. The first, polio, is one of four internationally notifiable diseases specifically listed in the International Health Regulations (2005). An extensive infrastructure exists to ensure surveillance, immediate notification of confirmed polio cases, and investigation of potential cases. At end of 2013, 60% of polio cases were the result of international spread of wild poliovirus, and there was increasing evidence that adult travellers were contributing to this spread. During the 2014 low transmission season there was international spread of wild poliovirus from 3 of the 10 countries currently infected. The consequences of further international spread are particularly acute today given the large number of polio-free but conflict-torn and fragile States. These countries have severely compromised routine immunization services and are at high risk of re-infection. In May 2014 WHO declared polio a public health emergency of international concern (PHEIC) based on the unanimous advice of the International Health Regulations Emergency Committee concerning the international spread of the wild poliovirus. The Director-General endorsed the Committee’s advice for ‘States currently exporting wild polioviruses’ (Pakistan, Cameroon and Syrian Arab Republic) and for ‘States infected with wild poliovirus but not currently exporting’ (Afghanistan, Equatorial Guinea, Ethiopia, Iraq, Israel, Somalia and Nigeria) and issued Temporary Recommendations to reduce the international spread of wild poliovirus, effective 5 May 2014. 15. The second public health event of concern is the emergence in the Region in 2012 of the Middle East respiratory syndrome coronavirus (MERS-CoV), which has affected 20 countries within and outside the Region to date. As of end July 2014, 837 laboratory-confirmed cases of infection with MERS-CoV, including at least 292 related deaths, have officially been reported to WHO. The virus appears to be circulating widely throughout the Arabian Peninsula, with most cases reported by Saudi Arabia. While most cases have occurred among residents, some have occurred among travellers. The number of cases increased sharply from March until end of May 2014, essentially in Saudi Arabia and United Arab Emirates with much of the increase apparently driven by hospital outbreaks. The number of cases that acquired the infection in the community has also increased since mid-March. Some of these cases have reported contacts with animals. Other factors adding to the substantial increase in numbers include the possibility that Spring may be the high season for transmission, and the fact that affected countries are finding more cases because they are testing more people. There are no solid data on how many people may be catching the virus but showing no or mild symptoms, if asymptomatic cases can spread the virus, or how to successfully treat patients. Although camels are suspected to be the primary source of infection for humans, the exact routes of direct or indirect exposure remain

unknown. So far, the conditions for declaring MERS-CoV a PHEIC under the International Health Regulations (2005) have not been met. Foodborne disease 16. The safety of food in the Region has dramatically improved in recent years. However, progress in countries is uneven and foodborne outbreaks due to microbial, parasitic and zoonotic pathogens and contamination with chemicals and toxins are common in many countries. The trading of contaminated food between countries increases the potential that outbreaks will spread. In addition, the emergence of new foodborne diseases is cause for considerable concern. Examples of interregional spread of foodborne diseases from within the Region include a hepatitis A virus infection outbreak between 1 November 2012 and 30 April 2013 and an enterohaemorrhagic E. coli outbreak in 2011. Accidental and deliberate public health events 17. Since 2011 there have been several disturbing health events resulting from chemical, radiological or nuclear accident, deliberate release of infectious, chemical, radiological or nuclear agents, and sudden environmental change. The nuclear meltdown at a nuclear power plant in Fukushima, Japan in March 2011 resulted in the release of radioactive material into the environment, raising concerns about the possible long-term public health consequences of external and internal radiation exposure. WHO has published its assessment of the health risks associated with the disaster. 18. The use of chemical weapons in the Syrian Arab Republic in 2013 and 2014, affecting several hundred people, drew global attention to the need to enforce related national and international laws and to enhance national capacities to deal with such incidents. The aftermath of flooding in Afghanistan in March–May 2014, affecting thousands of people in 27 provinces, had major public health consequences, including potential for spread of infectious and vector-borne diseases as a result of damage to infrastructure, contamination of water sources and disruption in solid waste collection and disposal. Antimicrobial resistance 19. The rise of microorganisms that are resistant to the most powerful antibiotics has emerged as one of the biggest threats to public health. Antimicrobial resistance has been recognized as an important threat to global public health security by many global forums, including the G8 group of industrialized countries. The crisis stems from the irresponsible use of antimicrobials by health care providers, communities and industries across the globe, including in food production, particularly as growth promoters in animal husbandry. The gains achieved in many areas of infectious disease control are considered to be under serious threat from the spread of antimicrobial resistance. The WHO report Antimicrobial resistance: global report on surveillance 2014 revealed widespread resistance patterns for several bacteria of public health importance, including E. coli, Klebsiella pneumonia, Staphylococcus aureus, Streptococcus pneumonia, non-typhoid Salmonella, Shigella and Neisseria gonorrhoeae. There are now examples of drug-resistant strains in all types of micro-organisms, including bacteria, viruses, fungi and parasites. No new class of antibacterial has been discovered since 1987, partly because companies can no longer make enough money out of antimicrobials to justify investing in the research needed. The potential for serious medical, social and economic consequences, within just a few years, led WHO to call for urgent, coordinated action from the many global stakeholders. Action is needed at all levels and from all countries of the world. Mass gatherings 20. Large numbers of people in a small space can aid the spread of infectious diseases, lead to accidents, provoke terror attacks, and exceed the capacity of routine public health measures. Mass

gatherings therefore pose unique risks to health security. The Region is host to a number of such gatherings, including annual events, such as the Hajj in Saudi Arabia and the Arba’een and Muharram in Iraq, regular sporting events and the recent political gatherings that drew hundreds of thousands of people in some countries of the Region. Public health preparedness efforts at mass gatherings in the majority of countries of the Region are insufficient and need enhancement. The public health risk associated with mass gatherings is not only the number of people who will fall in sick in the host country and the resultant burden on the health system, but also the potential for spread of infectious diseases when those people travel back to their countries. Frameworks and initiatives to enhance global health security 21. The International Health Regulations (2005) impose obligations on States Parties to develop and maintain core public health capacities including at points of entry in order to early detect, assess, notify and report events covered by their provisions. As such, the major differences with previous versions include: shift from containment at the border to containment at the source of the event; shift from a small list of notifiable diseases to focus on all public health risks, including chemical, radiological and nuclear threats; and shift from pre-set measures to tailored responses, with more flexibility to deal with local situations on the ground, and advice from the emergency committee. The Regulations comprise the principal legal tool governing the response to public health emergencies that pose an international threat. Collaboration with other mandated organizations is absolutely essential and mandatory under these regulations, and fulfilment of these obligations benefits the pursuit of global health security. 22. A number of other frameworks and resolutions exist to protect global health security, to enhance national capacities for alert and response to different public health emergencies, and to bring together the different partners to coordinate response. In the area of food safety the Codex Alimentarius Commission (CAC) established by FAO and WHO has, since the early 1960s, harmonized regulations for dietary supplements worldwide and set international safety standards to support trade in food. The Statutes of the CAC were amended (WHA 59.16) in 2006 with the aim of protecting the health of consumers; promoting coordination between the different stakeholders; and determining priorities and initiating and guiding the preparation of food standards. 23. World Health Assembly resolutions of note include WHA54.14 on global health security, WHA55.16 on global public health response to natural occurrence, accidental release or deliberate use of biological, chemical agents or radiological and nuclear material that affect health and WHA59.15 on the Strategic Approach to International Chemicals Management. In the area of emergencies and disasters resolutions WHA58.1 on health action in relation to crises and disasters, WHA59.22 on emergency preparedness and response and WHA64.10 on strengthening national health emergency and disaster management capacities and the resilience of health systems are pertinent, while WHA51.29 and WHA61.19 focus on the protection against the risks to health from climate change. In WHA64.5 the Health Assembly adopted the Pandemic Influenza Preparedness (PIP) Framework aimed at implementing a global approach to pandemic influenza preparedness and response. 24. In other forums, governments around the world have committed to action to reduce disaster risk. United Nations General Assembly resolution 60/195 endorsed the Hyogo Declaration and the Hyogo Framework for Action (HFA) 2005–2015, a 10-year plan to reduce vulnerabilities to natural hazards. Consultations are under way across the world to shape the recommendations for the development of a post-2015 framework. The global health security agenda launched by the United States of America in 2014, with its nine objectives related to preventing avoidable epidemics, detecting threats early and responding rapidly and effectively, supports the implementation of the International Health Regulations in promoting global health security. The United States of America has committed to working with at least 30 countries to prevent, detect and effectively respond to infectious disease threats, whether naturally occurring or caused by accidental or intentional releases

of dangerous pathogens. Other initiatives include the G8, G20 and the global partnership against the spread of weapons and materials of mass destruction, all aimed at building national capacities in specific areas of work and enhancing the international coordination and collaboration to respond to related events. Progress in implementation of the International Health Regulations (2005) 25. The five-year target date for implementing the International Health Regulations (2005) following their entry into force in 2007 passed in 2012. Of the 21 States Parties in the WHO Eastern Mediterranean Region, 1 (Islamic Republic of Iran) declared its readiness to meet the obligations by June 2012 and the rest fell short of the implementation goals. Nineteen (19) States Parties obtained a two-year extension to fully achieve the core capacity requirements by 15 June 2014 and 1 State Party did not comply with the requirements for extension. 26. The 2014 deadline to ensure that certain functional capacities required by the Regulations are in place throughout the territories of States Parties has also now passed. Seven (7) States Parties indicated their readiness to meet the obligations by the deadline of 15 June 2014 (Bahrain, Jordan, Morocco, Oman, Qatar, Saudi Arabia and United Arab Emirates). Thirteen (13) States Parties in the Region have requested a second extension (Afghanistan, Djibouti, Egypt, Iraq, Kuwait, Libya, Lebanon, Pakistan, Somalia, Sudan, Syrian Arab Republic, Tunisia and Yemen). All the States Parties that requested further extension submitted a new implementation plan to WHO. 27. Analysis of the requests for extension shows that the 13 States Parties concerned all identified laboratory capacity as an area that needs further support. This was followed by the capacity of coordination and national IHR focal point communication and capacities at points of entry by 12 States Parties, and surveillance and response capacities by 10 States Parties. Few States Parties identified human resources capacity in itself as a priority for requiring extension. However, building the capacity of human resources to implement the different core capacities was highlighted by each State Party. Therefore, the capacity of human resources is considered as needing further support by the 13 States Parties. Zoonotic capacity was the one least identified by States Parties as needing further support. Fig. 1 shows the capacities requiring further support.

28. WHO monitors implementation of the Regulations and reports the results annually to the World Health Assembly using the IHR monitoring tool. Results generated from this tool indicate that the regional implementation level of core capacity requirements was 70% in 2013, slightly higher than the 64% level achieved in 2012. The regional implementation level in 2013 was slightly higher than the global implementation level of 68% (Fig. 2).

29. Overall, States Parties are making good progress in surveillance, laboratory, risk communication, legislation, coordination and food safety. However, capacities for points of entry and for handling chemical and radionuclear events remain low (Fig. 3). 30. In addition to the global monitoring tool, in-country missions provided further information on the achievements and gaps in the Region. (Country specific data and information in the form of country profiles will be disseminated to the Regional Committee). 31. Several countries have reviewed national legislation and taken relevant action to facilitate the implementation of the Regulations. The National IHR Focal Point is designated with annual confirmation in all countries, except Somalia. Multisectoral multidisciplinary IHR committees have been established in almost all the countries and meet with regular frequency. Considerable efforts are being invested in enhancing indicator-based surveillance in all countries. This includes: the availability of designated units with adequate human resources; a defined list of notifiable diseases with case definitions; national policy/guidance on surveillance; and systems for reporting, analysing and interpreting the surveillance data. Disease early warning systems have been established in some countries and are being expanded. The use of electronic versions of these systems is also expanding. Some components of event-based surveillance are available and functioning, such as media monitoring, rumour verification and school absenteeism. Familiarity and use was observed within the health sector of the decision instrument in Annex 2 of the International Health Regulations (2005) for the assessment and notification of events that may constitute a public health emergency of international concern. 32. All countries reported having established mechanisms to respond to public health events including the availability of rapid response teams. Guidelines and standard operating procedures on the management and response are available to address few public health events. Fifteen countries reported having national policies/programmes for infection prevention and control. Guidelines and standard operating procedures on infection prevention and control are available in most countries. Fourteen countries reported having the capacity to mobilize internal resources, when available, to respond to public health events; the time period required to mobilize internal resources differs between countries. Some countries have the capacity to mobilize external resources to respond to public health events, although these tend to be those that are already receiving support from donors. 33. All countries have a defined structure and mechanism, including plans for specific sectors, for emergency preparedness and response. Half of the countries reported having conducted risk assessment and mapping of hazards either nationally or of some parts of the country. 34. Almost all countries reported having identified risk communication partners in their countries. The capacity to communicate health risks is strong in the health sector for disease outbreaks. Around half of countries reported having sector-specific risk communication plans. Although 14 countries reported having policies or guidelines on the clearance and release of information, the rest of the countries are doing this based on undocumented practice. Communicating the risk related to new public health events is a shared responsibility between the different concerned units/departments. However, the level of coordination and collaboration between the relevant sectors in the different countries varies. The release of information related to new events and the type of the information to be released has to be decided by senior officials. A formal institutionalized mechanism for expediting the release of information during emergencies seems to be lacking. However, in some countries, some people, in their individual capacities, can take action and expedite the release of such information. Based on the availability of funds, countries regularly update information, education and communications materials and make them accessible to the public. 35. Eighteen (18) countries reported having a network of national and international laboratories in place to meet diagnostic and confirmatory requirements and support outbreak investigations for

events and 15 countries reported having an up-to-date inventory of public laboratories with relevant diagnostic capacity. Almost all countries reported having national regulations for packaging and transport of clinical specimens, and a functional system for collection, packaging and transport of clinical specimens. Almost all countries reported having pre-positioned collection and transportation kits at appropriate levels, and the ability to deliver clinical specimens to appropriate laboratories within the appropriate time-frame for investigation of regular events. However, this is not the case for investigation of urgent public health events. Assessment of the laboratory quality management system was conducted by WHO in half the countries and results showed that laboratory biorisk management systems still require considerable investment. 36. Almost all countries have identified their designated points of entry for implementing the International Health Regulations (2005) and half of the countries have identified the list of ports authorized to issue ship sanitation certificates and have shared the list with WHO. The core capacities required at designated points of entry are insufficiently available. Public health services related to health care services for ill travellers and crew, inspection of imported food and animal goods, food and water safety, waste management, vector control and application of public health control measures related to certain public heath events are available in some countries and are usually delivered by different providers. For some services, private companies are contracted to do the job. 37. Considerable progress has been made in zoonotic capacity, particularly in countries where zoonotic diseases are considered a major public health threat. Surveillance for zoonotic events is established within the ministry of health in most countries. A list of animal diseases, including zoonotic diseases, for which surveillance is required has been established within the sectors responsible for animals in most countries. Sharing of information between health and sectors dealing with animals is usually ad hoc. Some countries have the capacity to conduct laboratory tests to confirm priority zoonotic events. Access to international laboratory capacity, through established procedures, to confirm priority zoonotic events is maintained in the majority of countries. All countries reported having an established mechanism to respond to outbreaks of known zoonotic disease, but not to emerging (unknown) zoonotic disease. 38. Countries vary in their food safety capacity. Some countries are progressing in this area and working on existing gaps to improve capacity. Others lack many of the requirements. Food safety is a shared responsibility between different sectors in each country. The roles and responsibilities of each have been identified but are mostly not documented. Almost all countries reported having national or international food safety standards and national laws, regulations or policies in place to facilitate food safety control. However, in the majority of countries, they are not up to date. Fifteen (15) countries reported having established a list of priority food safety risks. Fourteen (14) countries reported having risk-based food inspection services in place. Rumours about food safety events are investigated by relevant units when captured. Established systems for the regular sharing of information related to food safety events among the different sectors are rarely available. Guidelines and standard operating procedures for the case management of specific food safety events are available in 15 countries. Relevant professionals in the food safety sectors in the majority of the countries are trained on response and control to food safety events but not regularly. 39. The regulation and use of chemicals is a joint responsibility between different sectors in each country. Most countries have a national body to regulate all activities involving chemicals and hazardous substances, with a legal framework. An inventory of sites or facilities that could potentially be a source for chemical emergency and an assessment of chemical risks at these sites have been developed in a few countries. Policies for surveillance of and response to chemical events, and guidelines for response to and management of chemical events, are available in half of the countries but are not accessible to all concerned sectors. Half of the countries reported having plans for preparedness and response for chemical events but not as part of a national plan. In most countries, exchange of information between the relevant units is ad hoc.

40. With regard to national capacity for preparedness and response to radiation emergencies, several activities are ongoing in countries, while others are planned in order to strengthen capacity further. Responsibilities are shared across different sectors. Plans for the detection and assessment of and response to radiation emergencies are available in 16 countries. Systems for monitoring radiation emergencies are available in 15 countries. Guidelines and standard operating procedures for risk assessment, reporting, event verification and notification, investigation and management of radiation emergencies are available in the majority of countries but are not accessible to all concerned sectors. Systems for export and disposal of radioactive materials exist and are monitored in most of the countries, and the capacity for inspection of goods contaminated with radioactive material exists in the majority of the countries, including at points of entry. Sector-specific plans for preparedness and response to radiological and nuclear events exist in most countries but are not accessible to other sectors. Strong collaboration with countries was observed from the International Atomic Energy Authority (IAEA) to further strengthen existing capacity. Gaps in implementation 41. Lack of understanding of the process for review of national legislation in the context of the International Health Regulations (2005) and enactment of national legislation to facilitate implementation of the Regulations are considered a major challenge for some States Parties. The modification of legislation can take many years to be endorsed. The option of issuing a ministerial decree to support implementation of the Regulations, as an alternative, is not widely used. 42. The majority of responsible persons within the national IHR focal points are equipped with neither sufficient understanding of the Regulations nor the necessary equipment to facilitate roundthe-clock (24/7) communications with the different sectors and with WHO. The functions and obligations of the national focal points and of the multisectoral committees are not clear in most countries, and where they are clear, they are not fully implemented. Furthermore, insufficient understanding of the Regulations among the different sectors, and their role in implementing them, and inadequate political commitment to the Regulations among the non-health sectors is observed in the majority of the countries. 43. Under routine circumstances coordination and information sharing between sectors are not structured. This is attributed in part to a culture of “not sharing information” and in part to lack of proper guidance on how to strengthen coordination, how to share information and what kind of information should be shared. The roles and responsibilities of the various stakeholders in implementing the Regulations are is not clearly defined. 44. The completeness and timeliness of reported surveillance data is an issue, particularly with regard to data coming from health facilities located in insecure areas in some countries. The methods and software used to manage surveillance data are not up to date. Feedback on surveillance results is not provided regularly to all levels and to other relevant stakeholders. Although a considerable number of countries reported the availability of event-based surveillance, a formal system has not been not established in any country. Furthermore, the existence of a major gap in understanding of the concept of event-based surveillance was observed in the countries, even among senior surveillance officers. The decision instrument in Annex 2 of the Regulations is not being used among the nonhealth sectors. 45. Multidisciplinary investigation of, and response to, public health events is insufficient. Guidance on the management of and response to some priority public health events is not available. Also, infection control and patient safety measure are inadequate in most countries. The current outbreak of MERS-CoV highlights the breaches/gaps in the infection prevention and control programmes. The capacity of isolation units in tertiary hospitals in the Region needs to be strengthened and decontamination units need to be established in selected hospitals to deal with cases

with chemical, radiological and nuclear contamination. A comprehensive system for monitoring antimicrobial resistance is lacking in most countries. 46. National public health plans for preparedness and response to public health hazards, including those at points of entry are lacking in most countries. Mapping of potential hazards and hazardous sites needs to be carried out in most countries and national public health plans for preparedness and response to these potential hazards need to be developed/updated accordingly. Prepositioned stockpiles of medicines and supplies for public heath emergencies, and plans for the management of these stockpiles, are not sufficiently available. 47. Risk communications capacity in the non-health sectors, particularly those dealing with chemical and radiation emergencies, requires significant development. There is a need for an integrated approach to this. This includes development, with the involvement of all stakeholders, of integrated risk communication plans to address potential hazards in the country. Other specific plans, addressing specific hazards, are also needed. Joint risk communications training is needed for major stakeholders, in order to strengthen coordination and collaboration, and especially in order to be able reduce the time-lapse in moving into an emergency response mode as integrated teams for risk communications. Strategies need to be developed that enable civil society, the private sector and international organizations to work together effectively in this regard, while the risk communications needs at the different administrative levels also need to be explored so governments are better prepared to communicate in health emergencies. 48. While considerable work has been carried out to build the capacity of the national laboratory biorisk management systems, this is still a major challenge in some countries. The reasons include lack of awareness and political commitment and the lack of a comprehensive plan that addresses infrastructure, equipment, collaboration between the different sectors and trained human resources. 49. Lack of coordination and communication between the different service providers at the designated points of entry, together with lack of awareness of the Regulations and the role of the competent authorities in implementing them is a major gap. The health documents used at points of entry are not the updated ones required by the Regulations and the capacities of the personnel responsible for issuing such documents are not sufficient. Most designated points of entry do not have public health contingency plans. There is a lack of guidance on how to strengthen the capacities required at ground crossings, particularly in countries where these points are porous and the control over the passage of travellers is insufficient. There is also a lack of guidance on how to integrate surveillance data at points of entry with national surveillance data. Efforts need to be made to enhance coordination with neighbouring countries in order to strengthen cross-border surveillance and response. 50. Most countries do not have national plans for preparedness and response to zoonotic events. Some countries do not have case definitions and guidance for the investigation, control and management of animal diseases, including zoonotic diseases. Coordination between sectors responsible for animals and other sectors, and timely and systematic collection, collation and exchange of information concerning zoonotic diseases between animal surveillance units, laboratories, human health surveillance units and other relevant sectors are insufficient. Moreover, the detection of animal disease is based on passive surveillance and rarely on active surveillance, which is needed. The use of effective isolation and infection control measures in health care, surge capacity to adequately respond to events, supply of prophylaxis or countermeasures and access to experts who can support in the rapid response to zoonotic events are also not sufficient in most countries. 51. The majority of the countries have not sufficiently addressed the following requirements for food safety capacity: an established mechanism of coordination and information sharing among the different stakeholders and with the National IHR Focal Point; an established surveillance system with

a defined list of food safety events, enhanced traceability and a recall mechanism for contaminated products; improved inspection and licensing capacity for both locally produced food and at points of entry for imported food products; laboratory testing capacity for foodborne disease and food contamination including with chemicals; national plans for preparedness and response to food safety events; and a fully functioning food safety control management system. 52. There is insufficient coordination and sharing of information among the different chemicalrelated sectors and with the National IHR Focal Point. Gaps in surveillance systems and public health responses to chemical events are common in several countries. Major gaps exist among the least developed countries and countries under conflict. These include: mapping of potential chemical hazards; a national multisectoral plan for preparedness and response to chemical events; enhanced surveillance of chemical events; laboratory capacity for the testing of chemical events; sufficient public health capacity to respond to chemical events, including infrastructure, equipment, material and fully equipped human resources; and availability of functioning chemical poisons centres or chemicals information centres. 53. Coordination and collaboration between radiation-related sectors and the public health sector is not sufficient. National surveillance for radiological and nuclear events or sources of events is lacking in most countries. Treatment and clinical management of cases exposed to radiation are mainly assigned to the health sector. However, there is a need for further strengthening of the existing public health capacity in areas related to decontamination, personal protective equipment, stockpiles of necessary medication and supplies and training of medical staff. Sector-specific plans for preparedness and response to radiological and nuclear events need to be updated in the form of national plans and be made available to all concerned sectors. Existing challenges to meeting the obligations of the International Health Regulations (2005) 54. Considerable progress has been made in implementing the Regulations across States Parties in the Region. Nevertheless, a number of challenges exist that not only affect the building of the required capacities but also the maintaining of those capacities. The following are the most significant of these challenges. • A considerable number of countries are going through different types of political and geopolitical transition. This has resulted in, and continues to cause, destruction or weakening of health systems. It has reduced access to health care, reduced capacity to detect, prevent and respond to public health events, and has interrupted efforts to build and further enhance national public health capacities. There is insufficient understanding of the Regulations at national level, even within the national IHR focal points. This has resulted in lack of political commitment from the non-health sectors towards implementing the Regulations and insufficient efforts to enhance coordination among the different sectors, including the non-health sectors, and to review national legislation to facilitate implementation. There is insufficient coordination between neighbouring countries to enhance capacities at points of entry and to share data and information from epidemiological surveillance to ensure rapid and effective cross-border response to public health events. The high turnover among professionals makes it difficult to keep sufficiently equipped human resources in place to maintain and further develop the required capacities. Activities to build the capacity of human resources are being carried out extensively by the different partners and in the different areas. However, coordination needs to be strengthened between the different partners to ensure that training of human resources in all the necessary areas of work is being addressed. A lack of financial resources in some countries has prevented allocation of the necessary budget for implementing the Regulations and for further enhancing infrastructure and medical 12

equipment. These funding constraints are mainly due to competing national priorities. However, the policy of donors with regard to support is also a factor as such support often does not match the actual needs of the recipient countries or does not go to the countries that actually need the support. In addition, although States Parties are mandated under the Regulations to support each other, up to now this has not been maintained. 55. WHO has been, and is, working extensively to support countries to meet the obligations, providing technical support, mobilizing technical and financial resources to enable countries to build national capacities for surveillance and response, assessing the risk associated with public health events of potential international concern, and coordinating the response to public health events of potential international concern. An internal restructuring exercise was conducted in order to streamline its efforts and the support given to countries. However, WHO also faces challenges that are affecting its ability to continue meeting its mandate as the Secretariat responsible for overseeing implementation of the Regulations. 56. The first of these challenges concerns the obligatory notification and reporting by countries to WHO of public health events of potential international concern that occur in their territory. The recent MERS-CoV outbreak was an opportunity to test and assure the functionality of the communication system for such events under the Regulations and progress was seen in the rapid notification of this event to WHO by the national IHR focal points. However, depending on the policy of each State Party, the approval of senior nationals is often required before events are notified to WHO and this has potential to delay the early notification required by the Regulations. National IHR focal points are required to have sufficient authority to notify WHO without further recourse. Also, the comprehensive reporting necessary to allow WHO to assess the risk of events is not being maintained and needs to be addressed by national focal points. 57. The second challenge concerns a lack of sufficient human resources at the Regional Office to be able to monitor the implementation of the Regulations closely and to follow up with nationals on actions needed in order to gear up implementation. Similarly, there is insufficient capacity at WHO country offices to support implementation at country level. Conclusion 58. Every day, the world and the Region face a variety of public health threats. The potential for such threats to have negative implications for countries of the Region is huge, particularly given the current political instability in many countries. More than 50% of the Member States in the Region have requested a second extension of the deadline for implementation of the International Health Regulations (2005). For these countries, many of the capacities required still need to be developed. At the same time, some of the capacities are already available but their availability is not known to all the concerned sectors, or to the national IHR focal points, due to insufficient coordination. Countries that have announced their readiness to meet the obligations by June 2012 and June 2014 also need to put in place mechanisms to continue to develop the required capacities, particularly those related to preparedness, risk communication, points of entry and enhancing the public health sector to manage chemical and radiation emergencies. 59. Some countries are receiving support of various kinds from partners. However, the lack of coordination between partners has resulted in some areas of work being over supported while others areas are not receiving any attention, and some countries in need of support have received no support from partners at all. Several initiatives are going on at the global level between different international organizations to enhance national capacities. These are yet not fully implemented at country level. 60. There is an urgent need for investment in all-hazard surveillance and response capabilities in the Region. Without such investment, compliance with the obligations of the Regulations will be poor, creating dangerous holes in global health security. Combined national and international efforts are 13

needed to ensure that all countries have the necessary capacity for early detection, investigation, reporting and responding to public health events. With these collective national and international efforts, implementation of the Regulations and of other initiatives, frameworks and resolutions will result in a greater level of global health security, and the opportunity for diseases to cross borders will be much less as more countries become capable of maintaining their own health security. The only way to guarantee global, and regional, health security is to implement the International Health Regulations (2005). 61. Meeting the obligations of the International Health Regulations (2005) is a decision made by State Parties alone. WHO may advise Member States, at their request, on their implementation of the Regulations. Thus, declarations by Member States with regard to having met the obligations does not necessarily mean that all the required capacities are available on the ground for early detection, assessment and notification of and response to public health events of potential international concern. The Regional Committee may wish to consider the option of strengthening and sustaining the ability to review progress in implementation of the Regulations. This might require, for example, establishing a regional structure for the assessment and independent verification and certification of the capacities in States Parties to manage public health events. Next steps for Member States 1) Conduct advocacy and outreach activities to increase the level of understanding of the International Health Regulations (2005) (IHR) and the role of the different sectors in implementing them, and to obtain the political commitment of minsters other than minsters of health. 2) Establish a committee of legal advisers representing the different sectors to review national legislation and facilitate the implementation of the Regulations. Consider issuing ministerial decrees as a short-term action to facilitate implementation until the revision and enactment of the necessary national legislation is complete. 3) Designate appropriate and different responsible persons to national IHR focal points to ensure the full implementation of their functions. Responsible persons with national focal points need to be properly equipped facilitate round-the-clock (24/7) communications with the different national sectors and with WHO, and have a direct line of communication with senior officials of all other sectors to facilitate coordination. 4) Establish an IHR intersectoral committee with high-level representation from the different sectors to allow timely decision-making and active involvement of each sector in the development, implementation and monitoring of the national IHR plan of action. 5) Establish a mechanism of information sharing between the different national stakeholders, particularly with the non-health sectors and with the National IHR Focal Point, with clear terms of reference for this mechanism on what information must be shared, how it is to be shared and whom to involve. 6) Enhance the use of the decision instrument in Annex 2 of the International Health Regulations (2005) for the assessment and notification of events that may constitute a public health emergency of international concern, particularly by the non-health sector. Further improve the rapid notification of public health events of potential international concern to WHO and ensure comprehensive reporting of cases associated with these events. 7) Allocate the necessary budget to implement the national IHR plan of action in order to be able to meet and maintain the obligations as a top priority and to implement the other existing frameworks and agreements. Strengthen networking with partners, including WHO collaborating centres, to support closing of the gaps.

8) Develop retention plans for human resources, with both short-term and long-term solutions that address the continuous turnover among the personnel needed to implement the requirements of the Regulations. Introduce the International Health Regulations into relevant education curriculums, including the curriculum of the existing field epidemiology training programmes as a start. 9) Start dialogue with neighbouring countries and find the proper mechanism to enhance crossborder collaboration for surveillance and response to public health events. Ensure the involvement of relevant international organizations at the country level to facilitate finding the proper mechanism of collaboration. 10) Establish a mechanism to provide financial, technical and logistics support to other countries to gear up the development and maintenance of the core capacity requirements of the Regulations. 11) Identify national experts in the different areas of work and designate them for inclusion in the global roster of IHR experts. 12) Review and update the national IHR plan of action to address all identified gaps, within a clearly identified time-frame and budget.

Sécurité sanitaire mondiale – enjeux et opportunités – axée plus particulièrement sur le Règlement sanitaire international (2005) Résumé d’orientation 1. De nombreux pays sont confrontés à de sérieuses menaces pour la santé publique. Si les principales menaces pour la sécurité sanitaire sont dues aux maladies infectieuses à potentiel endémique émergentes et réémergentes, celles liées à la dissémination délibérée ou accidentelle d'agents chimiques, radiologiques et nucléaires sont de plus en plus préoccupantes. Il ne faut que quelques heures pour que les menaces pour la santé publique qui pèsent sur un pays touchent un autre pays et elles ont de plus en plus d'occasions de se propager au niveau international à tout moment. En outre, le développement de la résistance aux antimicrobiens compromet sérieusement les bénéfices obtenus dans de nombreux domaines de la lutte contre les maladies infectieuses. Les rassemblements de masse constituent eux aussi des risques uniques pour la sécurité sanitaire. 2. On connaît depuis longtemps l’importance de l’action collective dans la gestion des urgences sanitaires de portée internationale. Un grand nombre d’initiatives, d’accords, de cadres et de résolutions de l’OMS ont été adoptés pour soutenir le renforcement des capacités nationales dans des domaines d'activité spécifiques et renforcer la coordination et la collaboration internationales pour faire face à des événements liés. Le Règlement sanitaire international impose l’obligation pour les États Parties de développer et maintenir les principales capacités de santé publique pour la surveillance et l’action, y compris aux points d'entrée, afin de détecter, d’évaluer, de notifier et de déclarer rapidement à l'OMS les événements couverts par ses dispositions, et de prendre des mesures adaptées aux situations locales sur les conseils du Comité d'urgence. Le respect de ces obligations favorise la sauvegarde de la sécurité sanitaire mondiale. 3. Le délai de cinq ans prévu pour la mise en œuvre du Règlement après son entrée en vigueur en 2007 est arrivé à échéance en 2012. Sur les 21 États Parties dans la Région OMS de la Méditerranée orientale, 20 n’ont pas atteint les objectifs de mise en œuvre. La date limite de juin 2014 pour faire en sorte que certaines capacités fonctionnelles requises par le Règlement soient mises en place dans l’ensemble des territoires des États Parties est également passée et 13 pays ont demandé une seconde prorogation. Le niveau de mise en œuvre au niveau régional des principales capacités requises était de 70 % en 2013, contre 64 % en 2012, et seulement légèrement supérieur au niveau de mise en œuvre international de 68 %. Les États Parties progressent de façon satisfaisante en matière de surveillance, de laboratoires, de communication sur les risques, de législation, de coordination et de sécurité sanitaire des aliments. Toutefois, un certain nombre de problèmes entravent le renforcement des capacités requises ainsi que le maintien de ces capacités. L’OMS rencontre également des difficultés pour continuer à s'acquitter de sa mission de Secrétariat chargé de superviser la mise en œuvre du Règlement. 4. Il est urgent d’investir dans la surveillance de tous les risques et des capacités d'intervention dans la Région. Sans cet investissement, les obligations du Règlement seront peu respectées, entraînant certaines failles dans la sécurité sanitaire mondiale. Des efforts conjoints aux niveaux national et international s’imposent pour veiller à ce que tous les pays disposent des capacités nécessaires pour détecter, examiner et notifier rapidement les événements de santé publique et réagir dans les plus brefs délais. Avec ces efforts collectifs nationaux et internationaux, la mise en œuvre du Règlement et d’autres initiatives, cadres et résolutions permettra d’augmenter le niveau de sécurité sanitaire mondiale et la possibilité pour les maladies de traverser les frontières sera bien moindre

car les pays seront plus nombreux à pouvoir assurer leur propre sécurité sanitaire. Le seul moyen de garantir la sécurité sanitaire aux niveaux international et régional est de mettre en œuvre le Règlement sanitaire international (2005). 5. Respecter les obligations du Règlement sanitaire international (2005) est une décision prise par les États Parties seuls. L’OMS ne peut que conseiller les États Membres, à leur demande, sur la mise en œuvre du Règlement. Ainsi, le fait pour un État Membre de déclarer respecter les obligations ne signifie pas nécessairement qu’il dispose de toutes les capacités requises sur le terrain pour détecter, évaluer et notifier rapidement les événements susceptibles de constituer une urgence de santé publique de portée internationale et y faire face. Le Comité régional pourrait envisager la possibilité de renforcer et maintenir les capacités à examiner les progrès dans la mise en œuvre du Règlement. Cela peut nécessiter, par exemple, la création d'une structure régionale pour l'évaluation ainsi que la vérification et la certification indépendantes des capacités des États Parties à gérer les événements de santé publique. Introduction 6. Au cours des 60 dernières années, le monde a fait des progrès considérables pour améliorer la santé humaine. Toutefois, des problèmes de santé mondiaux persistent et des moyens pour y remédier sont à l'étude. Si des solutions adaptées à la situation locale sont nécessaires pour lutter contre la plupart des problèmes sanitaires, certains ne peuvent être résolus que dans le cadre d’une approche mondiale. Les expériences de ces dernières années, avec notamment le syndrome respiratoire aigu sévère (SRAS) et la grippe aviaire H5N1, ont attiré l’attention sur différentes questions relatives à la sécurité sanitaire. Les problèmes sanitaires mondiaux préoccupent tous les États Membres car ils dépassent les frontières nationales et différents acteurs doivent œuvrer ensemble pour les combattre. 7. De nombreux pays sont confrontés à de sérieuses menaces de santé publique. L’augmentation des mouvements de populations, dus au tourisme, aux migrations ou à des catastrophes, le développement du commerce international ainsi que les changements sociaux et environnementaux confirment qu’il ne faut que quelques heures pour qu'une maladie infectieuse survenant dans un pays devienne une menace dans un autre pays, voire dans le monde entier. Ces menaces peuvent dévaster des pays qui ne sont pas bien préparés à y faire face. Une flambée de maladie par exemple peut rapidement devenir une menace sanitaire à l'échelle mondiale suite à une combinaison de facteurs. Ces facteurs sont notamment les suivants : manque d'informations ; capacités insuffisantes au niveau des pays pour détecter rapidement les événements de santé publique et les endiguer ; crainte de répercussions coûteuses si ces événements sont notifiés ; et absence de mécanismes de riposte internationaux, à la fois juridiques et techniques. 8. Le concept de la sécurité sanitaire mondiale s’est élargi au fil du temps : portant à l'origine uniquement sur les maladies infectieuses, il englobe désormais tout risque sérieux pour la santé publique ayant des implications internationales. De nombreuses urgences de santé publique auraient pu être évitées ou mieux maîtrisées si les systèmes de santé concernés avaient été plus solides et mieux préparés. La sécurité sanitaire mondiale dépend en fin de compte de la qualité des systèmes nationaux de santé publique et nécessite une coopération et une gouvernance internationales. 9. De nombreuses mesures ont été prises au niveau mondial pour protéger les pays des menaces pour la santé publique. La plus importante de ces mesures ces dernières années a été l'établissement du Règlement sanitaire international (RSI 2005) qui régit la réponse apportée aux urgences de santé publique constituant une menace de portée internationale et qui a un caractère obligatoire pour les États Parties depuis le 15 juin 2007. Le Règlement fournit le cadre juridique pour la responsabilité collective en matière de sécurité sanitaire mondiale, imposant l’obligation aux États Parties de développer et maintenir des capacités essentielles en santé publique. Le respect de ces obligations favorise la sauvegarde de la sécurité sanitaire mondiale et permet de protéger tous les pays des sérieuses menaces pour la santé publique, qu’elles soient d’origine biologique, chimique, radiologique ou nucléaire. 2

10. Le présent document vise à souligner l'importance du Règlement sanitaire international (2005) pour la sécurité sanitaire mondiale et à fournir au Comité régional un compte rendu sur la situation de la Région en matière de mise en œuvre de ce Règlement. Menaces pour la sécurité sanitaire mondiale 11. Les principales menaces pour la sécurité sanitaire sont dues aux agents pathogènes émergents et aux maladies infectieuses réémergentes à potentiel endémique. Toutefois, les menaces liées à la dissémination délibérée ou accidentelle d'agents chimiques, radiologiques et nucléaires sont de plus en plus préoccupantes. Le monde étant de plus en plus interconnecté, les possibilités pour ces menaces à la santé publique d'avoir une portée internationale sont de plus en plus nombreuses. Maladies à potentiel épidémique 12. Les flambées de maladies infectieuses aiguës, comme entre autres le choléra, la méningite, la rougeole, la fièvre jaune, les fièvres hémorragiques, les grippes et l’infection au virus Nipah, constituent une menace pour la sécurité sanitaire mondiale et doivent être confinées à la source pour prévenir la morbidité et la mortalité. En 2013-2014, plusieurs pays de la Région de la Méditerranée orientale ont signalé à l'OMS d'importantes flambées de maladies infectieuses aiguës. Il s’agissait notamment de la fièvre hémorragique Crimée-Congo, du choléra, de la dengue, de formes sévères de grippe pandémique (H1N1) 2009, de la rougeole, de la grippe aviaire (H5N1) et de la fièvre jaune. 13. L’actuelle flambée de maladie à virus Ebola, due à une souche d'Ebolavirus présentant une très grande homologie (98 %) avec Ebolavirus Zaïre, est une première en Afrique occidentale. Du fait de son évolution rapide en Guinée, de sa propagation aux pays voisins, le Libéria et la Sierra Leone, et d’un cas importé signalé par le Nigéria, il s’agit de la plus grande propagation géographique d’une flambée de virus Ebola jamais signalée depuis l’apparition de la maladie en 1976 dans les populations humaines au Zaïre (l’actuel République démocratique du Congo) et dans la partie sud du Soudan. Au 27 juillet 2014, le nombre total de cas attribués à la maladie dans les quatre pays s’élevait à 1323, dont 729 décès. Compte tenu de la fréquence des déplacements de personnes entre les pays touchés et certains pays de la Région de la Méditerranée orientale, cette dernière est exposée à un risque de transmission du virus. 14. Dans la Région, deux grands événements de santé mondiaux présentent actuellement un risque d’impact majeur sur des pays situés hors de la Région. Le premier, la poliomyélite, est l’une des quatre maladies à déclaration obligatoire au niveau international figurant sur la liste du Règlement sanitaire international (2005). Une infrastructure importante permet d’assurer la surveillance, la notification immédiate des cas confirmés de poliomyélite ainsi que l’investigation des cas potentiels. Fin 2013, 60 % des cas de poliomyélite étaient dus à la propagation internationale du poliovirus sauvage, et de plus en plus d'éléments laissent à penser que les voyageurs adultes contribuent à cette propagation. Pendant la saison de faible transmission de 2014, le poliovirus sauvage s’est propagé au niveau international à partir de trois pays sur les 10 pays qui étaient alors infectés. Les conséquences d'une propagation internationale plus importante sont particulièrement graves aujourd'hui compte tenu du grand nombre d’États exempts de poliomyélite mais ravagés par des conflits et fragiles. Ces pays disposent de services de vaccination systématique gravement affaiblis et présentent un risque élevé de réinfection. En mai 2014, l'OMS a déclaré la poliomyélite « urgence de santé publique de portée internationale » (USPPI) suite aux conseils unanimes du Comité d’urgence du Règlement sanitaire international sur la propagation internationale du poliovirus sauvage. Le Directeur général a approuvé les recommandations du Comité à l’intention des « États d’où le poliovirus sauvage est actuellement exporté » (Pakistan, Cameroun et République arabe syrienne) et des « États infectés par le poliovirus sauvage mais d’où il n’est pas actuellement exporté » (Afghanistan, Guinée équatoriale, Éthiopie, Iraq, Israël, Somalie et Nigéria) et les a publiées à titre de recommandations temporaires afin de réduire la propagation internationale du poliovirus sauvage, avec effet au 5 mai 2014.

15. Le second événement de santé publique préoccupant est l’émergence dans la Région en 2012 du coronavirus du syndrome respiratoire du Moyen-Orient (MERS-CoV), qui touche à ce jour 20 pays au sein et à l’extérieur de la Région. Fin juillet 2014, 837 cas d’infection à MERS-CoV confirmés en laboratoire, dont au moins 292 décès associés, ont été officiellement notifiés à l’OMS. Le virus semble circuler largement dans la Péninsule arabique, la plupart des cas étant signalés par l’Arabie saoudite. La majorité des cas concernaient des résidents, mais certains ont touché des voyageurs. Le nombre de cas a considérablement augmenté entre mars et fin mai 2014, principalement en Arabie saoudite et aux Émirats arabes unis, l’augmentation étant apparemment due à des flambées dans les hôpitaux. Le nombre de cas ayant contracté l'infection dans la communauté a également augmenté depuis la mi-mars. Certains personnes ayant été infectées ont indiqué avoir eu des contacts avec des animaux. Parmi les autres facteurs expliquant la forte augmentation des cas, il est possible que le printemps soit la haute saison pour la transmission, et que les pays affectés trouvent davantage de cas car ils soumettent un plus grand nombre de personnes à des tests. Il n’existe pas de données sérieuses sur le nombre de personnes ayant pu être infectées par le virus mais présentant peu ou pas de symptômes, sur la possibilité pour les cas asymptomatiques de propager le virus, ou sur la façon de traiter efficacement les patients. Bien que les chameaux soient suspectés d’être la principale source d’infection pour les humains, les modes exacts d’exposition directe ou indirecte restent inconnus. À ce jour, les conditions pour déclarer le MERS-CoV urgence de santé publique de portée internationale aux termes du Règlement sanitaire international (2005) ne sont pas remplies. Maladies d'origine alimentaire 16. La sécurité sanitaire des aliments dans la Région s’est considérablement améliorée ces dernières années. Toutefois, les progrès dans les pays sont inégaux et les flambées d’origine alimentaire dues à des agents pathogènes microbiens, parasitaires et zoonotiques ainsi que la contamination par des produits chimiques et des toxines sont courantes dans de nombreux pays. Le commerce d’aliments contaminés entre les pays augmente le risque de propagation des flambées. En outre, l’émergence des nouvelles maladies d’origine alimentaire est très préoccupante. Parmi les exemples de propagation interrégionale de maladies d’origine alimentaire à partir de la Région, l’on peut citer la flambée d’infection par le virus de l’hépatite A entre le 1er novembre 2012 et le 30 avril 2013 et une flambée de E.coli entérohémorragique en 2011. Événements de santé publique accidentels et délibérés 17. Depuis 2011, il y a eu plusieurs événements de santé alarmants dus à un accident chimique, radiologique ou nucléaire, à la dissémination délibérée d’agents infectieux, chimiques, radiologiques ou nucléaires et à des changements climatiques soudains. La fusion du cœur d’un réacteur à la centrale de Fukushima au Japon en mars 2011 a entraîné la dissémination de matière radioactive dans l’atmosphère, suscitant des inquiétudes quant à d’éventuelles conséquences à long terme sur la santé publique d’une exposition externe et interne aux rayonnements. L’OMS a publié une évaluation des risques sanitaires associés à la catastrophe. 18. L’utilisation d’armes chimiques en République arabe syrienne en 2013 et 2014, touchant des centaines de personnes, a attiré l'attention du monde entier sur la nécessité d'appliquer des lois nationales et internationales et de renforcer les capacités nationales pour faire face à ces incidents. Les inondations en Afghanistan en mars-mai 2014, touchant des milliers de personnes dans 27 provinces, ont eu des répercussions importantes sur la santé publique, notamment le risque de propagation de maladies infectieuses et à transmission vectorielle en raison des dégâts infligés aux infrastructures, de la contamination des sources d'eau et de l'interruption de la collecte et de l'élimination des déchets solides.

Résistance aux antimicrobiens 19. L’augmentation du nombre de micro-organismes résistants aux plus puissants antibiotiques constitue l'une des plus grandes menaces pour la santé publique. La résistance aux antimicrobiens est reconnue comme une grave menace pour la sécurité sanitaire mondiale par de nombreux forums mondiaux, dont le groupe des huit pays les plus industrialisés (G8). La crise est due à l’utilisation irresponsable des antimicrobiens par les prestataires de santé, les communautés et les industries à travers le monde, notamment dans la production d'aliments, en particulier comme facteurs de croissance dans l’élevage d’animaux. On considère que les progrès accomplis dans de nombreux domaines de la lutte contre les maladies infectieuses sont gravement menacés par la propagation de la résistance aux antimicrobiens. Le rapport de l’OMS Antimicrobial resistance : global report on surveillance 2014 a révélé d’importantes formes de résistance pour plusieurs bactéries d’importance de santé publique, dont E. coli, Klebsiella pneumonia, Staphylococcus aureus, Streptococcus pneumonia, Salmonella non-typhoïde, Shigella et Neisseria gonorrhoeae. Il existe désormais des exemples de souches pharmacorésistantes pour tous les types de micro-organismes, y compris les bactéries, virus, champignons et parasites. Aucune nouvelle classe d’antibactériens n’a été découverte depuis 1987, notamment parce que les sociétés ne peuvent plus faire suffisamment de bénéfices sur les antimicrobiens pour justifier l’investissement dans la recherche nécessaire. Le risque de conséquences médicales, sociales et économiques graves, en seulement quelques années, a incité l’OMS à appeler à une action coordonnée d'urgence de la part des nombreuses parties prenantes internationales. Une action est nécessaire à tous les niveaux et de la part de tous les pays du monde. Rassemblements de masse 20. Un grand nombre de personnes dans un espace restreint peut contribuer à la propagation de maladies infectieuses, entraîner des accidents, provoquer des attaques terroristes et dépasser les capacités des mesures de santé publique habituelles. Les rassemblements de masse constituent donc des risques uniques pour la sécurité sanitaire. La Région accueille plusieurs rassemblements de ce genre, dont certains annuels, comme le pèlerinage (Hadj) en Arabie saoudite et les fêtes de l’Arba’een et de Muharram in Iraq, des événements sportifs réguliers et récemment des rassemblements politiques qui ont attiré des centaines de milliers de personnes dans certains pays. Les mesures de préparation en matière de santé publique lors des rassemblements de masse dans la majorité des pays de la Région sont insuffisantes et doivent être renforcées. Le risque pour la santé publique associé aux rassemblements de masse réside non seulement dans le nombre de personnes qui tombent malades dans le pays hôte et la charge qui en résulte pour le système de santé, mais également dans la possibilité de propager des maladies infectieuses quand ces personnes rentrent dans leur pays. Cadres et initiatives pour renforcer la sécurité sanitaire mondiale 21. Le Règlement sanitaire international (2005) impose aux États Parties l’obligation d'acquérir et de maintenir des capacités essentielles en santé publique, y compris aux points d’entrée, afin de détecter, d’évaluer, de notifier et de déclarer rapidement les événements couverts par ses dispositions. Dans ce domaine, les principales différences avec les versions précédentes sont notamment les suivantes : passage du confinement à la frontière au confinement à la source de l’événement ; passage d’une petite liste de maladies à déclaration obligatoire à l’accent mis sur tous les risques de santé publique, y compris les menaces chimiques, radiologiques et nucléaires ; et passage de mesures préétablies à des interventions sur mesure, avec plus de flexibilité pour traiter les situations locales sur le terrain, et recommandation du Comité d'urgence. Le Règlement comprend le principal outil juridique régissant la réponse aux urgences de santé publique qui constituent une menace de portée internationale. La collaboration avec d’autres organisations mandatées est absolument essentielle et obligatoire aux termes de ce Règlement et le respect de ces obligations favorise la sauvegarde de la sécurité sanitaire mondiale.

22. Il existe un certain nombre d’autres cadres et résolutions pour protéger la sécurité sanitaire mondiale, renforcer les capacités nationales pour l’alerte et la riposte aux différentes urgences de santé publique et regrouper les différents partenaires pour coordonner la riposte. Dans le domaine de la sécurité sanitaire des aliments, la Commission du Codex Alimentarius (CCA) créée par la FAO et l’OMS a, depuis le début des années 1960, harmonisé les réglementations pour les compléments alimentaires dans le monde et établi des normes de sécurité internationales pour le commerce des aliments. Les statuts de la Commission du Codex Alimentarius ont été amendés (WHA59.16) en 2006 afin de protéger la santé des consommateurs, de promouvoir la coordination entre les différentes parties prenantes et d’établir des priorités, prendre l’initiative et guider la conduite du travail de préparation des normes alimentaires. 23. Parmi les résolutions importantes de l’Assemblée mondiale de la Santé, l’on peut citer les résolutions WHA54.14 sur la sécurité sanitaire mondiale, WHA55.16 intitulée Présence naturelle, dissémination accidentelle ou usage délibéré de matériel chimique, biologique ou radionucléaire affectant la santé : l’action de santé publique internationale et WHA59.15 sur l’approche stratégique de la gestion internationale des produits chimiques. Dans le domaines des situations d’urgence et des catastrophes, l'on peut mentionner les résolutions WHA58.1 sur les interventions sanitaires en cas de crises et de catastrophes, WHA59.22 sur la préparation aux situations d’urgence et l’organisation des secours et WHA64.10 sur le renforcement au niveau national des capacités de gestion des urgences sanitaires et des catastrophes et de la résilience des systèmes de santé, tandis que les résolutions WHA51.29 et WHA61.19 portent sur la protection de la santé contre les risques liés aux changements climatiques. Dans la résolution WHA64.5, l’Assemblée de la Santé a adopté le cadre de préparation en cas de grippe pandémique visant à mettre en œuvre une approche mondiale de la préparation et la riposte à la grippe pandémique. 24. Dans d’autres forums, les gouvernements à travers le monde se sont engagés à agir pour réduire le risque de catastrophe. La résolution 60/195 de l’Assemblée générale des Nations Unies approuve la Déclaration de Hyogo et le Cadre de Hyogo pour 2005-2015, plan sur 10 ans pour réduire les vulnérabilités face aux catastrophes naturelles. Des consultations sont en cours dans le monde entier afin de formuler les recommandations pour l'élaboration d'un cadre après 2015. Le programme mondial de sécurité sanitaire lancé par les États-Unis d’Amérique en 2014, avec ses neuf objectifs liés à la prévention des épidémies évitables, la détection précoce des menaces et la réponse rapide et efficace, soutient la mise en œuvre du Règlement sanitaire international en favorisant la sécurité sanitaire mondiale. Les États-Unis d’Amérique se sont engagés à travailler avec au moins 30 pays pour prévenir et détecter les menaces de maladies infectieuses, et y réagir efficacement, qu’elles soient d’origine naturelle ou dues à des disséminations intentionnelles ou accidentelles d’agents pathogènes dangereux. Parmi les autres initiatives, l’on peut citer le G8, le G20 et le partenariat mondial contre la diffusion d’armes et de matériels de destruction massive, visant toutes à renforcer les capacités nationales dans des domaines d’action spécifiques ainsi que la coordination et la collaboration internationales pour faire face aux événements connexes. Progrès accomplis dans la mise en œuvre du Règlement sanitaire international (2005) 25. La date butoir à cinq ans pour la mise en œuvre du Règlement sanitaire international (2005) après son entrée en vigueur en 2007 est passée en 2012. Sur les 21 États Parties dans la Région OMS de la Méditerranée orientale, un seul (la République islamique d’Iran) a déclaré être prêt à satisfaire aux obligations d’ici juin 2012 ; les autres n’ont pas atteint les objectifs de mise en œuvre. Dixneuf (19) États Parties ont obtenu une prorogation de deux ans pour atteindre les principales capacités requises d'ici au 15 juin 2014 et un État Partie n’a pas satisfait aux exigences pour la prorogation. 26. Le délai fixé à 2014 pour la mise en place de certaines capacités fonctionnelles requises par le Règlement dans l’ensemble des territoires des États Parties est également écoulé. Sept (7) États Parties ont indiqué être prêts à satisfaire aux obligations pour la date limite du 15 juin 2014 (Arabie saoudite, Bahreïn, Émirat arabes unis, Jordanie, Maroc, Oman et Qatar). Treize (13) États Parties dans la Région ont demandé une seconde prorogation (Afghanistan, Djibouti, Égypte, Iraq, Koweït, Liban, 6

Libye, Pakistan, République arabe syrienne, Somalie, Soudan, Tunisie et Yémen). Tous les États Parties ayant demandé une prorogation supplémentaire ont soumis un nouveau plan de mise en œuvre à l'OMS. 27. L’analyse des demandes de prorogation révèle que les 13 États Parties concernés ont indiqué que les capacités des laboratoires devaient être davantage renforcées. Venaient ensuite les capacités de coordination et de communication des points focaux nationaux RSI et les capacités aux points d’entrée par 12 États Parties, ainsi que les capacités de surveillance et de riposte par 10 États Parties. Quelques États Parties ont fait des capacités en ressources humaines une priorité pour demander une prorogation. Cependant, le renforcement des capacités en matière de ressources humaines pour mettre en œuvre les différentes capacités essentielles a été souligné par chaque État Partie. Les 13 États Parties considèrent donc que ces capacités nécessitent un soutien supplémentaire. Les capacités zoonotiques sont celles qui ont été les moins citées par les États Parties comme nécessitant un soutien supplémentaire. La Figure 1 montre les capacités nécessitant un soutien supplémentaire.

28. L’OMS surveille la mise en œuvre du Règlement et présente chaque année les résultats à l’Assemblée mondiale de la Santé à l’aide de l’outil de surveillance du RSI. Les résultats obtenus par le biais de cet outil indiquent que le niveau régional de mise en œuvre des principales capacités requises était de 70 % en 2013, légèrement supérieur aux 64 % de 2012. Le niveau de mise en œuvre régional de 2013 était légèrement supérieur au niveau de mise en œuvre mondial de 68 % (Figure 2). 29. Dans l’ensemble, les États Parties progressent de façon satisfaisante en matière de surveillance, de laboratoires, de communication sur les risques, de législation, de coordination et de sécurité sanitaire des aliments. Toutefois, les capacités pour les points d’entrée et la prise en charge des événements d’origine chimique et radionucléaire restent insuffisantes (Figure 3). 30. En plus de l’outil mondial de surveillance, des missions dans les pays ont fourni d’autres informations sur les réalisations et les lacunes dans la Région (les données et informations spécifiques à chaque pays seront communiquées au Comité régional sous la forme de profils de pays). 31. Plusieurs pays ont révisé leur législation nationale et pris des mesures pour faciliter la mise en œuvre du Règlement. Le point focal national RSI est désigné et confirmé chaque année dans tous les pays, sauf en Somalie. Des comités RSI multisectoriels et multidisciplinaires ont été créés dans la quasi-totalité des pays et se réunissent régulièrement. Des efforts considérables ont été réalisés pour renforcer la surveillance basée sur les indicateurs dans tous les pays. Cela comprend les points suivants : l’existence d’unités spéciales avec des ressources humaines adaptées ; une liste établie de maladies à déclaration obligatoire avec définition des cas ; une politique/orientation nationale sur la surveillance ; et des systèmes pour la notification, l’analyse et l’interprétation des données de surveillance. Des systèmes d'alerte précoce pour les maladies ont été créés dans certains pays et sont en cours d’expansion. L’utilisation de versions électroniques de ces systèmes se développe également. Certains composants de la surveillance basée sur les événements sont fonctionnels, comme la surveillance par les médias, la vérification des rumeurs et l'absentéisme scolaire. On observe dans le secteur de la santé une bonne connaissance et une utilisation de l'instrument de décision de l'Annexe 2 du Règlement sanitaire international (2005) pour l'évaluation et la notification des événements susceptibles de constituer une urgence de santé publique de portée internationale. 32. Tous les pays ont indiqué avoir mis en place des mécanismes pour réagir aux événements de santé publique, y compris des équipes d’intervention rapide. Des lignes directrices et procédures normalisées sur la prise en charge et la riposte ont été formulées pour faire face à quelques événements de santé publique. Quinze pays ont indiqué disposer de politiques/programmes nationaux pour la prévention et la maîtrise des infections. La plupart des pays disposent de lignes directrices et procédures normalisées en la matière. Quatorze pays ont indiqué avoir les capacités pour mobiliser des ressources internes, le cas échéant, pour faire face aux événements de santé publique ; le délai nécessaire à la mobilisation des ressources internes diffère selon les pays. Certains pays ont les capacités de mobiliser les ressources externes pour réagir aux événements de santé publique, mais ce sont généralement ceux qui reçoivent déjà une aide de donateurs. 33. Tous les pays ont une structure et un mécanisme définis, y compris des plans pour des secteurs spécifiques, pour la préparation aux situations d’urgence et l'organisation des secours. La moitié des pays ont indiqué avoir réalisé une évaluation des risques et une cartographie des dangers au niveau national ou dans certaines parties du pays. 34. La quasi-totalité des pays a indiqué avoir désigné des partenaires pour la communication sur les risques dans leurs pays. Le secteur de la santé dispose de solides capacités pour communiquer sur les risques sanitaires pour les flambées de maladies. Environ la moitié des pays ont mentionné disposer de plans de communication sur les risques spécifiques aux secteurs. Si 14 pays ont déclaré avoir des politiques ou lignes directrices sur la diffusion d’informations, les autres procèdent sans l’appui de documents. La communication sur les risques liés aux nouveaux événements de santé publique relève de la responsabilité commune des divers services/départements concernés. Toutefois, le niveau de coordination et de collaboration entre les secteurs concerné(e)s varie selon les pays. La diffusion d'informations sur les nouveaux événements et le type d'informations à diffuser relèvent de la décision des hauts responsables. Il semble qu’un mécanisme officiel pour accélérer la diffusion d'informations pendant les situations d'urgence fasse défaut. Toutefois, dans certains pays, certaines personnes, à titre individuel, peuvent prendre des mesures et accélérer la diffusion de ces informations. En fonction de la disponibilité des fonds, les pays actualisent régulièrement les matériels d’information, d’éducation et de communication et les mettent à la disposition du public. 35. Dix-huit (18) pays ont indiqué disposer d’un réseau de laboratoires nationaux et internationaux pour répondre aux exigences en matière de diagnostic et de confirmation et fournir un appui aux investigations des flambées pour les événements et 15 pays ont déclaré avoir un inventaire actualisé des laboratoires publics ayant des capacités diagnostiques appropriées. La quasi-totalité des pays a 9

indiqué disposer de réglementations nationales pour le conditionnement et le transport des échantillons cliniques et un système fonctionnel pour la collecte, l'emballage et le transport de ces échantillons. La quasi-totalité des pays ont affirmé avoir des kits de collecte et transport en quantités appropriées et la capacité de livrer les échantillons cliniques aux laboratoires compétents dans le délai nécessaire pour l’investigation d’événements ordinaires. Toutefois, ceci n’est pas le cas pour l’investigation des urgences de santé publique. L’OMS a procédé à l’évaluation du système de gestion de la qualité des laboratoires dans la moitié des pays et les résultats ont révélé que les systèmes de gestion des risques biologiques nécessitaient encore un investissement important. 36. Presque tous les pays ont identifié leurs points d’entrée désignés pour la mise en œuvre du Règlement sanitaire international (2005) tandis que la moitié des pays ont établi la liste des ports autorisés à délivrer des certificats de contrôle sanitaire de navire qu’ils ont communiquée à l’OMS. Les principales capacités requises aux points d'entrée sont insuffisantes. Les services de santé publique liés aux services de soins de santé pour les voyageurs et membres du personnel malades, l’inspection des denrées alimentaires et marchandises pour animaux importées, la sécurité sanitaire des aliments et de l’eau, la gestion des déchets, la lutte antivectorielle et l’application des mesures de contrôle de santé publique liées à certains événements de santé publique sont disponibles dans certains pays et sont généralement dispensés par des prestataires différents. La prestation de certains services est confiée à des sociétés privées. 37. Les capacités en matière de lutte contre les zoonoses ont été très largement améliorées, en particulier dans les pays où les maladies zoonotiques sont considérées comme une importante menace de santé publique. La surveillance des événements d’origine zoonotique s’effectue au sein du ministère de la Santé dans la plupart des pays. Une liste des maladies animales, y compris les maladies zoonotiques nécessitant une surveillance, a été établie dans les secteurs responsables des animaux dans la plupart des pays. L'échange d’informations entre le secteur de la santé et celui en charge des animaux se fait généralement de manière ponctuelle. Certains pays disposent des capacités pour procéder à des analyses en laboratoire visant à confirmer les événements zoonotiques prioritaires. L’accès aux capacités de laboratoires internationales, par des procédures établies, pour confirmer les événements zoonotiques prioritaires, est maintenu dans la majorité des pays. Tous les pays ont indiqué disposer d’un mécanisme pour réagir aux flambées de maladies zoonotiques connues, mais non aux maladies zoonotiques émergentes (inconnues). 38. Les capacités en matière de sécurité sanitaire des aliments diffèrent selon les pays. Certains pays progressent dans ce domaine et s’emploient à améliorer les capacités dans les domaines où des lacunes persistent. Nombre des capacités requises font défaut dans d'autres pays. La sécurité sanitaire des aliments relève de la responsabilité commune de différents secteurs dans chaque pays. Les rôles et responsabilités de chacun ont été définis mais dans la plupart des cas n’ont pas été consignés par écrit. La quasi-totalité des pays a indiqué disposer de normes nationales et internationales en matière de sécurité sanitaire des aliments ainsi que de lois, réglementations ou politiques pour faciliter le contrôle de la sécurité sanitaire des aliments. Toutefois, elles ne sont pas actualisées dans la majorité des pays. Quinze (15) pays ont indiqué avoir établi une liste des principaux risques en matière de sécurité sanitaire des aliments. Quatorze (14) pays ont déclaré disposer de services d'inspection des aliments fondée sur les risques. Des unités spécialisées procèdent à l’investigation des rumeurs collectées sur les événements de sécurité sanitaire des aliments. Les systèmes pour l’échange régulier d’informations sur les événements de sécurité sanitaire des aliments entre les différents secteurs sont rares. Quinze pays disposent de lignes directrices et procédures normalisées pour la gestion des cas d’événements spécifiques de sécurité sanitaire des aliments. Dans la majorité des pays, des professionnels des secteurs de la sécurité sanitaire des aliments sont formés à la riposte et la maîtrise des événements de sécurité sanitaire des aliments ; cette formation n’a toutefois pas lieu régulièrement. 39. La réglementation et l'utilisation des produits chimiques relèvent de la responsabilité commune de différents secteurs dans chaque pays. La plupart des pays sont dotés d’un organisme national pour réglementer toutes les activités impliquant les produits chimiques et les substances dangereuses, avec 10

un cadre juridique. Un inventaire des sites ou établissements susceptibles d’être des sources d’urgence chimique ainsi qu’une évaluation des risques chimiques sur ces sites ont été effectués dans quelques pays. La moitié des pays disposent de politiques pour la surveillance des incidents chimiques et l’intervention, ainsi que de lignes directrices pour la riposte et la prise en charge, mais elles ne sont pas accessibles à tous les secteurs concernés. La moitié des pays ont mentionné l’existence de plans pour la préparation et la riposte aux incidents chimiques mais pas dans le cadre d’un plan national. Dans la plupart des pays, l’échange d'informations entre les unités compétentes s’effectue de manière ponctuelle. 40. En ce qui concerne les capacités nationales pour la préparation aux situations d'urgence radiologique, plusieurs activités sont menées dans les pays, tandis que d’autres sont prévues pour renforcer davantage les capacités. Les différents secteurs se partagent les responsabilités. Seize (16) pays disposent de plans pour la détection et l’évaluation des situations d’urgence radiologique ainsi que pour l’intervention. Il existe des systèmes pour surveiller les situations d’urgence radiologique dans 15 pays. Des lignes directrices et procédures normalisées pour l’évaluation des risques, la notification, la vérification et la notification des événements ainsi que l'investigation et la gestion des situations d'urgence radiologique sont disponibles dans la majorité des pays mais ne sont pas accessibles à tous les secteurs concernés. Il existe des systèmes pour l’exportation et l’élimination de matières radioactives faisant l’objet d’une surveillance dans la plupart des pays, ainsi que des capacités pour l'inspection des biens contaminés par des matières radioactives, y compris aux points d'entrée. Il y a des plans spécifiques aux secteurs pour la préparation et la riposte aux événements radiologiques et nucléaires dans la plupart des pays mais ils ne sont pas accessibles aux autres secteurs. L’Agence internationale de l’énergie atomique (AIEA) a largement collaboré avec les pays pour renforcer les capacités existantes. Lacunes dans la mise en œuvre 41. Le manque de compréhension du processus permettant de réviser la législation nationale dans le contexte du Règlement sanitaire international (2005) et d’adopter une législation nationale pour faciliter la mise en œuvre du Règlement constitue un défi majeur pour certains États Parties. L’approbation de la modification de la législation peut prendre de nombreuses années. L’option qui consiste à édicter un décret ministériel pour favoriser la mise en œuvre du Règlement, comme alternative, n'est pas très répandue. 42. Dans la majorité des cas, les personnes responsables auprès des points focaux nationaux RSI ne connaissent pas suffisamment le Règlement ou n’ont pas l’équipement nécessaire pour faciliter les communications en permanence (24h/24, sept jours sur sept) avec les différents secteurs et l’OMS. Les fonctions et obligations des points focaux nationaux et des comités multisectoriels ne sont pas clairement définies dans la plupart des pays, et lorsqu’elles le sont, elles ne sont pas pleinement appliquées. En outre, on observe dans la majorité des pays une compréhension incomplète du Règlement de la part des différents secteurs ainsi que de leur rôle pour sa mise en œuvre ; l’engagement politique à l’égard du Règlement est également insuffisant parmi les secteurs autres que celui de la santé. 43. En temps normal, la coordination et l'échange d'informations entre les secteurs ne sont pas structurés. Cela est dû en partie à une culture de « non partage des informations » et en partie à l’absence de conseils appropriés sur la façon de renforcer la coordination et d’échanger des informations ainsi que sur le type d’informations à échanger. Les rôles et responsabilités des diverses parties prenantes dans la mise en œuvre du Règlement ne sont pas clairement définis. 44. L’exhaustivité et l’actualité des données de surveillance communiquées constituent un problème, en particulier en ce qui concerne les données venant d'établissements de santé situés dans des zones d'insécurité de certains pays. Les méthodes et le logiciel utilisés pour gérer les données de surveillance ne sont pas à jour. Les résultats de la surveillance ne sont pas communiqués régulièrement à tous les niveaux et aux autres parties prenantes concernées. Bien que de nombreux 11

pays aient mentionné l'existence d’une surveillance basée sur les événements, aucun pays n’a mis en place de système officiel. En outre, on observe d'importantes lacunes dans les pays en matière de connaissance du concept de surveillance basée sur les événements, même parmi les hauts responsables de la surveillance. L’instrument de décision figurant à l’Annexe 2 du Règlement n’est pas utilisé dans les secteurs autres que celui de la santé. 45. L’investigation multidisciplinaire des événements de santé publique ainsi que la riposte à ces événements sont insuffisantes. Il n’existe aucune orientation sur la prise en charge de certains événements de santé publique prioritaires et la riposte à ces événements. De même, les mesures de lutte contre les infections et de sécurité des patients sont inadaptées dans la plupart des pays. La flambée actuelle de MERS-CoV met en évidence les lacunes dans les programmes de prévention et de maîtrise des infections. Il convient de renforcer les capacités des services d’isolement dans les hôpitaux tertiaires de la Région et de mettre en place des unités de décontamination dans certains hôpitaux pour traiter les cas de contamination chimique, radiologique et nucléaire. La plupart des pays ne disposent pas d’un système complet pour surveiller la résistance aux antimicrobiens. 46. Dans la majorité des cas, les pays n’ont pas de plan national de santé publique pour la préparation et la riposte aux risques de santé publique, y compris aux points d’entrée. La plupart d’entre eux doivent établir une cartographie des risques potentiels et des sites dangereux et élaborer/mettre à jour des plans nationaux de santé publique pour la préparation et la riposte à ces risques potentiels. Les stocks de médicaments et de matériel prépositionnés pour les urgences de santé publique, ainsi que les plans pour la gestion de ces stocks, sont insuffisants. 47. Il convient de renforcer les capacités de communication sur les risques dans les secteurs autres que celui de la santé, en particulier ceux qui traitent les situations d’urgence chimique et radiologique, et d’adopter une approche intégrée pour ce faire. Cela inclut l’élaboration, avec la participation de toutes les parties prenantes, de plans intégrés de communication sur les risques pour faire face aux dangers potentiels dans le pays. D’autres plans spécifiques, portant sur les risques particuliers, sont également nécessaires. Les principales parties prenantes doivent suivre une formation commune en matière de communication sur les risques pour renforcer la coordination et la collaboration et surtout réduire le temps nécessaire au passage à un mode d’intervention d’urgence en tant qu’équipes intégrées pour la communication sur les risques. Il est nécessaire d’élaborer des stratégies pour permettre à la société civile, au secteur privé et aux organisations internationales d’agir efficacement dans ce sens et d’examiner les besoins en matière de communication sur les risques afin que les gouvernements soient mieux préparés à communiquer en cas de situation d’urgence sanitaire. 48. Des efforts considérables ont été accomplis pour renforcer les capacités des systèmes nationaux de gestion des risques biologiques ; toutefois, d'importantes lacunes persistent dans certains pays. Cela s'explique notamment par une prise de conscience et un engagement politique insuffisants ainsi que par l’absence de plan complet pour les infrastructures, l’équipement, la collaboration entre les différents secteurs et les ressources humaines qualifiées. 49. Le manque de coordination et de communication entre les différents prestataires de services aux points d’entrée désignés, ainsi que la méconnaissance du Règlement et du rôle des autorités compétentes dans sa mise en œuvre constituent des lacunes importantes. Les documents sanitaires utilisés aux points d’entrée ne sont pas mis à jour selon les exigences du Règlement et les capacités du personnel chargé d'émettre ces documents sont insuffisantes. La plupart des points d’entrée désignés ne disposent pas de plan d’urgence de santé publique. Les directives sur la façon de renforcer les capacités requises aux postes-frontières sont insuffisantes, en particulier dans les pays où ces points sont poreux, tout comme le contrôle du passage des voyageurs. On observe également un manque de recommandations sur la façon de procéder pour intégrer les données de surveillance aux points d’entrée aux données nationales y afférentes. Des efforts doivent être accomplis pour renforcer la coordination avec les pays voisins afin d’améliorer la surveillance et la riposte aux frontières.

50. La plupart des pays ne disposent pas de plan national de préparation et de riposte aux événements zoonotiques. Dans certains pays, il n’existe ni définition de cas ni directives pour l'investigation, la maîtrise et la prise en charge des maladies animales, y compris les zoonoses. Des lacunes persistent dans la coordination entre les secteurs chargés des animaux et les autres secteurs, ainsi que la collecte et l'échange rapides et systématiques d'informations concernant les zoonoses entre les unités de surveillance animale, les laboratoires, les unités de surveillance de la santé humaine et d'autres secteurs concernés. En outre, la détection des maladies animales repose sur une surveillance passive et rarement sur une surveillance active, qui est nécessaire. L’utilisation de mesures efficaces d’isolement et de lutte contre les infections en milieux de soins, les capacités à réagir rapidement et correctement aux événements, la fourniture d’une prophylaxie ou de contremesures et l'accès à des experts pouvant contribuer à la réponse rapide aux événements zoonotiques sont également insuffisants dans la plupart des pays. 51. La majorité des pays ne respectent pas suffisamment les exigences suivantes pour les capacités en matière de sécurité sanitaire des aliments : mise en place d’un mécanisme de coordination et d’échange d’informations entre les différentes parties prenantes et avec le point focal national RSI ; mise en place d’un système de surveillance avec une liste des événements de sécurité sanitaire des aliments, une traçabilité renforcée et un mécanisme de rappel des produits contaminés ; amélioration des capacités d'inspection et d'homologation à la fois pour les aliments produits localement et pour les produits alimentaires importés aux points d'entrée ; des capacités en matière d'analyses en laboratoire pour les maladies d’origine alimentaire et la contamination alimentaire, y compris avec des produits chimiques ; des plans nationaux pour la préparation et la riposte aux événements de sécurité sanitaire des aliments ; et un système de gestion des contrôles de sécurité sanitaire des aliments entièrement fonctionnel. 52. La coordination et l’échange d’informations entre les différents secteurs liés aux produits chimiques et avec le point focal national RSI sont insuffisants. Les défaillances dans les systèmes de surveillance et les ripostes de santé publique aux événements chimiques sont courantes dans plusieurs pays. Des lacunes majeures sont observées dans les pays les moins avancés et en situation de conflit. Cela concerne notamment : une cartographie des risques chimiques potentiels ; un plan multisectoriel national pour la préparation et la riposte aux événements chimiques ; la surveillance accrue des événements chimiques ; des capacités de laboratoire pour analyser les événements chimiques ; des capacités de santé publique suffisantes pour réagir aux événements chimiques, notamment infrastructures, équipement, matériel et ressources humaines entièrement équipées ; et la présence de centres antipoisons chimiques fonctionnels ou de centres d'information sur les produits chimiques. 53. La coordination et la collaboration entre les secteurs liés aux rayonnements et le secteur de la santé publique sont insuffisantes. La plupart des pays ne disposent d’aucun système de surveillance nationale pour les événements ou sources d’événements radiologiques et nucléaires. Le traitement et la prise en charge clinique des cas exposés aux rayonnements sont principalement confiés au secteur de la santé. Toutefois, il est nécessaire de renforcer les capacités de santé publique existantes dans les domaines liés à la décontamination, à l’équipement de protection individuelle, aux stocks de médicaments et aux fournitures nécessaires et à la formation du personnel médical. Les plans spécifiques aux secteurs pour la préparation et la riposte aux événements radiologiques et nucléaires doivent être actualisés sous la forme de plans nationaux et être mis à la disposition de tous les secteurs concernés. Difficultés rencontrées pour satisfaire aux obligations du Règlement sanitaire international (2005) 54. Les États Parties de la Région ont accompli des progrès considérables dans la mise en œuvre du Règlement. Toutefois, un certain nombre de problèmes entravent le renforcement des capacités requises ainsi que le maintien de ces capacités. Les principales difficultés sont les suivantes : • De nombreux pays sont en phase de transition politique et géopolitique de divers genres, ce qui entraîne, jusqu’à ce jour, une destruction ou un affaiblissement des systèmes de santé. situation limite l’accès aux soins de santé, réduit les capacités à détecter, prévenir et réagir aux événements de santé publique, et interrompt les efforts pour renforcer les capacités nationales de santé publique. La compréhension du Règlement est insuffisante au niveau national, même de la part des points focaux nationaux RSI. Cela se traduit par un manque d’engagement politique des secteurs autres que ceux de la santé pour la mise en œuvre du Règlement ainsi que par des efforts insuffisants pour renforcer la coordination entre les différents secteurs, y compris les secteurs non liés à la santé, et pour réviser la législation nationale afin de faciliter la mise en œuvre. Il n'y a pas assez de coordination entre les pays voisins pour renforcer les capacités aux points d'entrée et échanger les données et informations de la surveillance épidémiologique afin de garantir une réaction aux frontières rapide et efficace aux événements de santé publique. En raison du taux de rotation élevé parmi les professionnels, il est difficile de garder des ressources humaines suffisamment équipées pour maintenir et développer les capacités requises. Les différents partenaires agissent à grande échelle pour renforcer les capacités des ressources humaines dans les différents domaines. Toutefois, la coordination doit être renforcée entre les différents partenaires pour garantir la formation des ressources humaines dans tous les domaines d'activités nécessaires. Par manque de ressources financières, certains pays ne peuvent allouer le budget nécessaire à la mise en œuvre du Règlement et au renforcement de l’infrastructure et de l’équipement médical. Ces contraintes budgétaires sont dues principalement à d’autres priorités nationales. Toutefois, la politique des bailleurs de fonds à l'égard du financement est également un facteur car souvent ce financement ne correspond pas aux besoins réels des pays bénéficiaires ou ne va pas aux pays qui en ont vraiment besoin. En outre, si le Règlement exige des États Parties qu’ils se soutiennent mutuellement, ce n’est pas le cas à ce jour.

55. L’OMS œuvre activement pour aider les pays à satisfaire aux obligations, en apportant un appui technique, en mobilisant les ressources techniques et financières afin de permettre aux pays de renforcer les capacités nationales pour la surveillance et l'action, en évaluant le risque associé aux événements de santé publique pouvant avoir une portée internationale et en coordonnant la riposte à ces événements. L’Organisation a procédé à une restructuration interne afin de rationaliser ses efforts et le soutien apporté aux pays. Toutefois, l’OMS rencontre également des difficultés pour continuer à s'acquitter de sa mission de Secrétariat chargé de superviser la mise en œuvre du Règlement. 56. La première de ces difficultés concerne la déclaration obligatoire à l’OMS par les pays des événements de santé publique pouvant avoir une portée internationale qui surviennent sur leur territoire. La récente flambée de MERS-CoV a été l’occasion de tester et garantir la fonctionnalité du système de communication pour ces événements dans le cadre du Règlement et des progrès ont été observés pour la notification rapide de ces événements à l'OMS par les points focaux nationaux RSI. Toutefois, selon la politique de chaque État Partie, l’approbation des hauts fonctionnaires est souvent requise avant de notifier les événements à l’OMS, ce qui peut retarder la notification rapide requise par le Règlement. Les points focaux nationaux RSI doivent avoir suffisamment d’autorité pour informer l’OMS sans aucun autre recours. De même, l’établissement de rapports complets permettant à l’OMS d’évaluer le risque lié aux évènements n’est pas respecté et les points focaux nationaux doivent remédier à cela. 57. La seconde difficulté concerne un manque de ressources humaines au Bureau régional pour surveiller de près la mise en œuvre du Règlement et suivre les homologues nationaux sur les actions nécessaires à la mise en œuvre. De même, les capacités sont insuffisantes dans les bureaux de pays de l'OMS pour soutenir la mise en œuvre au niveau national. Conclusion 58. Chaque jour, le monde et la Région sont confrontés à différentes menaces pour la santé publique. Ces menaces peuvent avoir de graves répercussions pour les pays de la Région, en particulier compte tenu de l’instabilité politique actuelle dans nombre d’entre eux. Plus de 50 % des États Membres de la Région ont demandé une seconde prorogation du délai pour la mise en œuvre du 14

Règlement sanitaire international (2005). Pour ces pays, nombre des capacités requises restent encore à développer. Parallèlement, certaines des capacités sont déjà disponibles mais, du fait d’une mauvaise coordination, les secteurs concernés ou les points focaux nationaux n'en sont pas tous informés. Les pays ayant déclaré être prêts à satisfaire aux obligations pour juin 2012 et juin 2014 doivent également mettre en place des mécanismes pour continuer à développer les capacités requises, en particulier celles liées à la préparation, à la communication sur les risques, aux points d'entrée et au renforcement du secteur de la santé publique pour gérer les situations d'urgence chimique et radiologique. 59. Certains pays reçoivent un soutien sous différentes formes de la part de partenaires. Toutefois, en raison du manque de coordination entre les partenaires, certains domaines d’activités font l’objet d’une trop grande attention tandis que d’autres sont négligés et certains pays ayant besoin de soutien n’en reçoivent aucun. Il existe plusieurs initiatives au niveau mondial entre différentes organisations internationales pour renforcer les capacités nationales. Elles ne sont toutefois pas encore mises en œuvre au niveau des pays. 60. Il est urgent d’investir dans la surveillance de tous les risques et les capacités d'intervention dans la Région. Sans cet investissement, les obligations du Règlement seront peu respectées, entraînant certaines failles dans la sécurité sanitaire mondiale. Des efforts conjoints au niveau national et international s’imposent pour veiller à ce que tous les pays disposent des capacités nécessaires pour détecter, étudier et notifier rapidement les événements de santé publique et réagir dans les plus brefs délais. Avec ces efforts collectifs nationaux et internationaux, la mise en œuvre du Règlement et d’autres initiatives, cadres et résolutions permettra d’augmenter le niveau de sécurité sanitaire mondiale et les maladies auront moins d’occasions de traverser les frontières car les pays seront plus nombreux à pouvoir préserver leur propre sécurité sanitaire. Le seul moyen de garantir la sécurité sanitaire au niveau international et régional est de mettre en œuvre le Règlement sanitaire international (2005). 61. Respecter les obligations du Règlement sanitaire international (2005) est une décision prise par les États Parties seuls. L’OMS peut conseiller les États Membres, à leur demande, sur la mise en œuvre du Règlement. Ainsi, les déclarations des États Membres sur le fait de respecter les obligations ne signifient pas nécessairement que toutes les capacités requises sont disponibles sur le terrain pour la détection précoce, l'évaluation et la notification des événements de santé publique de portée internationale ainsi que la riposte à ces événements. Le Comité régional pourrait envisager l’option de renforcer les capacités à examiner les progrès dans la mise en œuvre du Règlement. Cela peut nécessiter, par exemple, la création d'une structure régionale pour l'évaluation ainsi que la vérification et la certification indépendantes des capacités des États Parties à gérer les événements de santé publique. Prochaines étapes pour les États Membres 1) Mener des actions de sensibilisation et d’information pour faire mieux comprendre le Règlement sanitaire international (2005) et le rôle des différents secteurs dans leur mise en œuvre et pour obtenir l’engagement politique des ministères autres que ceux de la santé. 2) Créer un comité de conseillers juridiques représentant les différents secteurs pour réviser les législations nationales et faciliter la mise en œuvre du Règlement. Envisager d'édicter des décrets ministériels comme action à court terme pour faciliter la mise en œuvre jusqu'à la révision et l'adoption de la législation nationale nécessaire. 3) Désigner différentes personnes responsables pour les points focaux nationaux RSI afin de garantir la mise en œuvre complète de leurs fonctions. Équiper correctement les responsables des points focaux nationaux pour faciliter les communications en permanence (24h/24, 7 jours sur 7) avec les différents secteurs nationaux et l’OMS, et avoir une ligne directe de communication avec les hauts fonctionnaires de tous les autres secteurs pour faciliter la coordination.

4) Établir un comité intersectoriel RSI avec une représentation à haut niveau des différents secteurs afin de permettre une prise de décision rapide et une implication active de chaque secteur dans l'élaboration, la mise en œuvre et la surveillance du plan d'action national RSI. 5) Créer un mécanisme d'échange d'informations entre les différentes parties prenantes nationales, en particulier avec les secteurs autres que celui de la santé et avec le point focal national RSI, définissant clairement les informations à partager, la façon de les partager et les personnes à impliquer. 6) Renforcer l’utilisation de l'instrument de décision figurant à l'Annexe 2 du Règlement sanitaire international (2005) pour l'évaluation et la notification des événements pouvant constituer une urgence de santé publique de portée internationale, en particulier par les secteurs autres que celui de la santé. Améliorer davantage la notification rapide à l’OMS des événements de santé publique pouvant avoir une portée internationale et garantir un compte rendu détaillé des cas associés à ces événements. 7) Allouer le budget nécessaire à la mise en œuvre du plan d’action national RSI afin de pouvoir maintenir comme priorité le respect des obligations et appliquer les autres cadres et accords existants. Renforcer le travail en réseau avec les partenaires, y compris les centres collaborateurs de l’OMS, afin de mieux réduire les écarts. 8) Élaborer des plans de fidélisation des ressources humaines, avec des solutions à court terme et à long terme portant sur la rotation continue du personnel nécessaire à la mise en œuvre des exigences du Règlement. Présenter le Règlement sanitaire international dans les programmes d’enseignement adaptés, y compris les programmes de formation à l’épidémiologie de terrain existants pour commencer. 9) Entamer un dialogue avec les pays voisins et trouver le bon mécanisme pour renforcer la collaboration transfrontalière pour la surveillance et la riposte aux événements de santé publique. Veiller à l’implication des organisations internationales concernées au niveau national pour faciliter le bon mécanisme de collaboration. 10) Établir un mécanisme pour apporter un soutien financier, technique et logistique à d’autres pays afin de préparer le développement et le maintien des principales capacités requises du Règlement. 11) Sélectionner des experts nationaux dans les différents domaines d’activité et les désigner pour être inclus dans la liste mondiale d’experts du RSI. 12) Réviser et mettre à jour le plan d’action national RSI pour remédier à toutes les lacunes identifiées, dans un délai et un budget clairement définis.
