1Pandemic Influenza Severity Assessment 1.1 Background and development history The events of the 2009 H1N1 pandemic revealed that WHO and national organizations did not have a robust and standardized method for making a timely assessment of the severity of pandemic influenza. They also showed that Member States were not fully prepared to rapidly assess the severity of a pandemic, or to implement the necessary risk management and communications plans. In 2011, the World Health Assembly adopted a report by the Review Committee on the Functioning of the International Health Regulations (2005) and on Pandemic Influenza (H1N1) 2009(1). The committee recommended that WHO should develop and apply measures that can be used to assess the severity of every influenza epidemic, whether seasonal or pandemic. A severity assessment provides the information needed to determine the timing, scale, emphasis, intensity and urgency of pandemic response actions. The report stated that, “by applying, evaluating and refining tools to measure severity every year, WHO and Member States can be better prepared to assess severity in a timely manner in the next pandemic” (1). Severity assessments should be conducted in the early phase of a pandemic and regularly thereafter as the pandemic evolves. Since the World Health Assembly highlighted this need, WHO has made great progress on developing a framework on pandemic influenza severity assessment (PISA)(2). The framework was developed through several meetings, expert consultations, collaborative WHO projects and the establishment of a technical working group (TWG) on pandemic influenza severity assessment. The framework defines influenza severity in terms of three indicators: transmission, seriousness of disease and impact. In April 2014, WHO convened a meeting of the TWG to review the scope of surveillance data currently collected by Member States. The TWG reviewed surveillance data in the selected Member States to identify strengths and gaps, and areas for improvement, all of which varied dramatically among Member States. Interim guidance describing a structured way of assessing influenza severity was developed, based on the results of previous meetings, work that WHO did with the Robert Koch Institute (Germany), a pilot study performed by Member States in the southern Hemisphere (Australia, Chile, New Zealand and South Africa) in 2014, and ongoing conference calls and expert consultations. Development of this framework formed the basis of discussion for the second and third meetings of the TWG, held in November 2014 and June 2015, respectively. Scope of the document1 2 Pandemic Influenza Severity Assessment From 2014 to 2016, pilot testing of the interim guidance continued in selected Member States: Australia, Bangladesh, Canada, Chile, China, Egypt, France, Germany, India, Iran, Ireland, Japan, Madagascar, New Zealand, Norway, Portugal, Singapore, South Africa, Spain, Thailand, the United Kingdom of Great Britain and Northern Ireland, and the United States of America. The guidance was further refined based on the findings and experience from the pilot testing. In March 2017, the PISA approach was launched and Member States were encouraged to start implementation. The immediate use of the tool to assess seasonal epidemic severity will highlight the need for strong baseline data; in addition, it will encourage Member States to refine their available data and familiarize themselves with the severity assessment tool, which would also be used during a pandemic. 1.2 Purpose of the guidance document This guidance has been developed by WHO to be used by Member States and WHO as part of the pandemic influenza risk management guidance (PIRM)(3) to assess the severity of influenza in seasonal epidemics and pandemics, when sustained human-to-human transmission occurs. The assessments will take place continuously during seasonal epidemics or a pandemic, with a conclusive assessment being undertaken once transmission has subsided. During a pandemic, time and resources are limited. Therefore, countries need to become familiar with the tool as soon as possible (i.e. during seasonal influenza epidemics), and operationalize the methods and outputs in their routine seasonal influenza situation assessments and reports. This will allow countries to assess severity more easily and efficiently during a pandemic. This guidance does not apply to the alert phase situation(3); that is, before sustained human-to- human transmission of zoonotic influenza viruses, when the aim is to define the risk of pandemic spread. Other tools are available from WHO for this purpose; for example, the Rapid risk assessment of acute public health events(4) or the tool for influenza pandemic risk assessment (TIPRA)(5). Information to assess severity early and throughout the course of a pandemic will also be provided through special studies and modelling. However, this guidance focuses mainly on the information collected during routine influenza surveillance. 1.3 Target audience This document is intended for use primarily by public health professionals at the national level, who perform or plan to perform national influenza severity assessments, and who can contribute to global influenza severity assessments. It is a living document that will be further updated as needed, and will serve as guidance for seasonal and pandemic influenza severity assessments. 3Pandemic Influenza Severity Assessment 2 Aim of the influenza severity assessment The aim of the influenza severity assessment at the national level is to (3): The global influenza severity assessment will be used by WHO to monitor and understand the global situation, and to inform and support global decisions and recommendations on public health interventions. • describe the epidemiological situation and assess the severity of an influenza epidemic or pandemic based on all available information; • inform national and global risk assessments; and • inform public health preparedness, response and recovery measures, as well as resource allocation. 4 Pandemic Influenza Severity Assessment Influenza severity assessment concept & definitions 3 3.1 Indicators Based on consultations between external experts and WHO staff between 2011 and 2015, influenza (pandemic) severity is defined in terms of three indicators: transmissibility of an influenza virus, seriousness of influenza disease and impact. These terms are defined in detail below and in Table 1. 3.1.1 Transmissibility The transmissibility indicator reflects the ease of movement of the influenza virus between individuals and communities. Thus, a virus with high transmissibility will spread rapidly from one person to another. Several factors affect transmissibility: the ability of the virus to spread from person to person, the dynamics of the spread and the susceptibility of the exposed population. Transmissibility will be influenced by social and climatic factors. During seasonal influenza epidemics, transmissibility is usually measured by routine surveillance systems using a proxy (e.g. how many people are seeking health care for influenza-like illness). The actual dynamics of the spread (the reproductive number) and the susceptibility of the exposed population would be measured by ad hoc special studies during a pandemic. 3.1.2 Seriousness of disease The seriousness of disease (also referred to as “severity of infection”) indicator describes the extent to which individual people get sick when infected with the influenza virus. It describes the frequency of clinical symptoms, complications of influenza illness and outcomes following influenza infection. The seriousness of disease depends on the virus; for example, an influenza virus with a high level of clinical severity can result in a disproportionate number of people with serious illnesses, some of whom will be hospitalized and some of whom will die. Seriousness of disease also depends on the host; for example, the presence of underlying medical conditions that predispose individuals to develop severe illness, a history of vaccinations that may be protective (e.g. for influenza and pneumococcal disease), the person’s age and the availability of health care. An infection is likely to be much more severe for some segments of a population than for others, and descriptions of the groups that are at increased risk will be an important part of this indicator. During seasonal influenza, seriousness of disease is measured with routine, hospital- based surveillance; for example, the case fatality ratio among people hospitalized or admitted to intensive care units (ICU) due to influenza. 5Pandemic Influenza Severity Assessment 3.1.3 Impact The impact indicator generally describes how the influenza epidemic or pandemic affects society. It represents the impact on the health-care sector; for example, the impact on health-care use (hospitalization and ICU admissions), on the health-care workforce and on society (including excess mortality). The impact will be affected by the implementation of public health measures, public concern and the behaviour of the affected population. If the health-care sector impact is high, there may be stress on health-care resources. The public health impact may also result in societal and economic consequences, such as absenteeism from workplaces and schools, loss of critical infrastructure, and decreases in trade and tourism. 3.2 Parameters Each indicator described above is derived from various parameters that come from virological and epidemiological surveillance and clinical sources. These parameters are often collected on a routine basis by a country’s public health surveillance systems(6); alternatively, they may be measured in specific investigations (special studies). This guidance document focuses on the information collected through routine surveillance; information from special studies would only be obtained during influenza pandemics or other unusual events. This guidance is intended to be flexible and should be adapted to the country implementing it. Each country has unique surveillance systems and has to choose the parameters (based on their experience) from their surveillance systems that could feed into the three indicators. For assessment of influenza severity, countries are encouraged to identify those parameters that are collected by established routine surveillance systems. Table 1. Steps for assessing influenza severity There are four key steps to assessing influenza severity, as shown in Figure 1. 7Pandemic Influenza Severity Assessment 2 3 4 1 Choose the parameters that will beused to assess severity indicators Set the thresholds for each parameter, using historical data Apply the thresholds to assess severity Report the severity assessment findings Step 1: Choose parameters Certain criteria should be considered when selecting the parameters that will be used to assess the three severity indicators: • Parameters must be: – indicative of influenza activity; therefore, at least a subset of samples should be laboratory tested for influenza viruses; – reliable and come from a surveillance system that is stable over time (or from a system in which changes that occur are well-documented); – timely; • Historical data must be available (on historical seasonal epidemics or pandemics) for the parameters; and • Where possible, denominators should be available to calculate (representative) proportions or rates. The parameters considered to be most useful as defined by the WHO TWG are shown in Table 2. Table 2. These non-health-care-related parameters might not be reported to the disease surveillance units in a country; rather, they might be captured by the ministry of education or social security systems. Such parameters were not tested during the pilot period. The minimum age groups are: under 15 years, 15–64 years and 65 years or over. If more age groups are used (e.g. >2 years or <5 years), it is important to ensure that they can aggregate to previously proposed age groups. Data might be insufficient at the beginning of the season to stratify by age groups, but it is important to collect age-specific information from the start. For the seriousness of disease and impact indicators, the parameters should, where possible, take into account the presence or absence of underlying chronic diseases or conditions known to be linked with severe outcomes for influenza (asthma, HIV/AIDS, pregnancy, heart or lung disease, for example). Refer to appendix 3 of the WHO global epidemiological surveillance standards for influenza for further information on pre-existing conditions.(6) In addition to these proposed parameters, Member States can include other parameters based on their own surveillance system, provided they have confidence in the data. • At the beginning of the assessment, use the criteria included in this guidance to select the parameters you will use. • Document the following information on the selected parameters: - data source (e.g. outpatient sentinel or hospital surveillance); - number of years of data available; - strengths of this parameter; - limitations of this parameter; and - proportion of data that is laboratory confirmed. INFLUENZA SEVERITY ASSESSMENT: Step 1: Key points 9Pandemic Influenza Severity Assessment Step 2: Set thresholds Surveillance systems differ widely from one country to another; hence, the absolute values of the parameters cannot be used to make meaningful comparisons between countries. However, within a country, it is possible to see how a parameter compares with previous seasons and, more specifically, how the parameter’s value compares with the peak value of previous seasons. Thus, using historical data, each country can describe an epidemic’s (or pandemic’s) activity compared with previous seasons, using qualitative descriptions such as: • no activity or below seasonal threshold • low • moderate • high • extraordinary. These qualitative assessments can then be compared both within a country over time and between countries. It is useful to set thresholds or define ranges (using historical data) for the parameters that allow for qualitative categorization by describing the levels of activity. This involves defining the start of the epidemic (or pandemic) and then setting thresholds for each parameter to define the different levels of activity, as outlined below. Defining the start of an epidemic To define the beginning of the influenza epidemic (seasonal or pandemic), the following methods can be used: • calculating the epidemic threshold using the moving epidemic method (MEM) (7, 8); or • using the weekly positivity rate (ratio of influenza-confirmed samples to all collected samples) to determine the virological threshold. 2 3 4 1 Choose the parameters that will beused to assess severity indicators Set the thresholds for each parameter, using historical data Apply the thresholds to assess severity Report the severity assessment findings 10 Pandemic Influenza Severity Assessment Thresholds for transmissibility and impact parameters To set the thresholds for the transmissibility and impact parameters, the following methods can be used: • MEM (7, 8); • WHO method (6); and • country-specific statistical or empirical approaches, depending on the intrinsic properties of the systems. The pilot testing indicated that MEM works well in most countries with the transmissibility and impact parameters, but more work is needed in tropical settings. Country-specific approaches to threshold setting might have to be derived locally using national historical data, and should be comparable to the thresholds derived by MEM and the WHO method. The thresholds have been set in such a way that about 50–60% of the season peaks should be above the moderate threshold, ±10% above the high threshold and ±2.5% above the extraordinary threshold. Table 3 provides proposed cut-off points for transmissibility and impact parameters used by WHO and in MEM. Table 3. From season to season, peak values are fairly stable, with differences by age group depending on circulating influenza subtypes. However, the values might be very different during a pandemic. Therefore, country-specific means and standard deviations should be calculated using end-of-season values of previous seasons and thresholds should be refined using historical data including pandemic data, as shown in Table 4. Age-specific thresholds should be set when data are available. • Define the beginning of the epidemic or pandemic and document the cut-off values. • Set thresholds for qualitative levels of activity for each parameter, and document the threshold values for each parameter. • Describe the methods and the historical data used. INFLUENZA SEVERITY ASSESSMENT: Step 2: Key points 12 Pandemic Influenza Severity Assessment Step 3: Apply the thresholds The severity assessment at the national level is mainly driven by quantitative data, expertise and experience. A country will come to a qualitative assessment for each indicator by looking at all parameters for a specific indicator; categorizing the values for the current situation in relation to their own historical data (ideally five seasons), or otherwise available comparable data; and taking into account additional available information. When performing the qualitative assessment, additional information needs to be considered; for example, timeliness, reporting biases and relative reliability of information from different systems. Ideally, on a weekly basis, after the beginning of the epidemic or pandemic, the thresholds should be applied to the contemporary parameter data for transmissibility and impact indicators. As discussed earlier, assessing the seriousness of disease indicator should be done once influenza activity has peaked. At a minimum, the three indicators should be assessed at the middle of the epidemic and at the conclusion of the epidemic. Where data allow, it is important to also check whether there are differences in specific age groups or risk groups. The assessments, including confidence levels, should be used for national risk assessments and to inform public health decisions. • At a minimum, assess the contemporary data for each indicator at the midpoint and end of the epidemic. Transmissibility and impact indicator data can be assessed on a weekly basis. • Examine data for any differences between age groups or risk groups. • Use the findings from the assessments, including confidence levels, to inform national risk assessment. • Document the parameters and thresholds used, the parameters that were most reliable and gave most weight in the overall assessment, how the interpretation was done and how you combined the information of the different parameters to give a judgement on each indicator. Where there are differences in the estimates by age groups or groups with underlying conditions, the final assessment should be based on the aggregated data (all age groups). Include the following: - weekly qualitative assessments for transmissibility and impact indicators; - at the peak of the season and at the end of the season, a qualitative assessment for the seriousness of disease indicator; and - any comment you have in the comment box (e.g. whether certain age groups are more affected or whether a high proportion of cases did not have underlying chronic conditions that put them at risk for severe disease). INFLUENZA SEVERITY ASSESSMENT: Step 4: Key points 15Pandemic Influenza Severity Assessment 5 Presentation of influenza severity assessments at the global level The severity assessment results from countries will be visible to participants reporting data (i.e. there will be user-restricted access). Outputs will include heat charts for each indicator by week and by country, with comments fields visible when the user scrolls over the chart. Maps with results for each indicator will be included, showing assessments per country at a certain time point. WHO will further refine the display of the information received by Member States, but some of the potential outputs are shown in Figure 3 and Figure 4. WHO will monitor the three indicators on a regular basis and will provide mid-season and end- of-season reports describing the severity in terms of transmission, seriousness of disease and impact, for the temperate northern and southern hemisphere influenza seasons. WHO will also provide situation updates whenever an unusual event is happening or during a pandemic. تقدير وخامة الأنفلونزا الجائحة 1 1-1 المعلومات الأساسية والتسلسل الزمني لتطّور الأحداث كشفت أحداث الأنفلونزا الجائحة ¬¬1N1H المندلعة في عام 9002 النقاب عن أن منظمة الصحة العالمية (المنظمة) والمنظمات الوطنية لا تتّ بع طريقة رصينة وموّحدة في إجراء تقديرات مناسبة التوقيت لمدى وخامة الأنفلونزا الجائحة، كما أثبتت تلك الأحداث أن الدول الأعضاء غير مستعد¬ة تماما ًلإجراء تقديرات سريعة لمدى وخامتها، أو لتنفيذ ما يلزم من خطط لإدارة مخاطرها وإجراء الاتصالات بشأنها. وكانت جمعية الصحة العالمية قد اعتمدت في عام 1102 تقريرا ًمن إعداد لجنة المراجعة المعنية بكيفية تنفيذ اللوائح الصحية الدولية (5002) وبالأنفلونزا الجائحة (1) 9002 (1N1H)، أوصت فيه المنظمة بضرورة اتخاذ وتطبيق تدابير يمكن استخدامها لتقدير وخامة كل واحد من أوبئة الأنفلونزا، سواء كانت موسمية أم جائحة. ويوفر تقدير الوخامة هذا المعلومات العلمية اللازمة لتحديد وقت اتخاذ إجراءات الاستجابة للجوائح ونطاقها وشّدتها وكثافتها ومدى عجالتها. وذكر التقرير ما يلي: “بإمكان المنظمة والدول الأعضاء أن تحّسن مستوى استعدادها لتقدير وخامة الجوائح المقبلة في الوقت المناسب عن طريق تطبيق أدوات لتقدير وخامتها سنويا ًوتقييم تلك الأدوات وتحسينها” (1). وينبغي أن تُجرى تقديرات وخامة الجائحة في المرحلة المبكرة من اندلاعها وأن يُواظب على إجرائها عقب ذلك بعد تفاقم اندلاعها. وقد حرصت المنظمة منذ أن أبرزت جمعية الصحة العالمية هذه الحاجة على إحراز تقدم كبير بشأن وضع إطار معني بتقدير وخامة الأنفلونزا الجائحة (2)، وهو إطار ُوِضع بفضل عقد العديد من الاجتماعات والمشاورات مع الخبراء وتنفيذ مشاريع تعاونية للمنظمة وإنشاء فريق عامل تقني معني بتقدير وخامة الأنفلونزا الجائحة. ويحدد الإطار وخامة الانفلونزا على أساس المؤشرات الثلاثة التالية: سريان المرض وخطورته ووقعه. ودعت المنظمة في نيسان/ أبريل 4102 إلى عقد اجتماع للفريق العامل التقني المعني بتقدير وخامة الأنفلونزا الجائحة لاستعراض نطاق ما تجمعه حاليا ًالدول الأعضاء من بيانات الترّصد. واستعرض الفريق العامل بيانات الترّصد المجّمعة في عدد مختار من الدول الأعضاء لتحديد مكامن قوتها والثغرات التي تتخلّ لها والمجالات التي يلزم تحسينها منها، والتي تباينت جميعها بشّدة فيما بين الدول الأعضاء. وأُِعّدت إرشادات مؤقتة تبّين طريقة منّسقة لتقدير وخامة الأنفلونزا بالاستناد إلى نتائج ُمستمدة من اجتماعات ُعِقدت سابقا،ً وأعمال أنجزتها المنظمة بالتعاون مع معهد روبرت كوخ (ألمانيا)، ودراسة رائدة أجرتها في عام 4102 الدول الأعضاء في نصف الكرة الجنوبي (أستراليا وشيلي ونيوزيلندا وجنوب أفريقيا)، وإلى ما يجري توجيهه من نداءات بشأن عقد المؤتمرات والمشاورات مع الخبراء. وقد شّكل وضع هذا الإطار الأساس اللازم لإجراء مناقشات الفريق العامل التقني في اجتماعيه الثاني والثالث المعقودين بشهري تشرين الثاني/ نوفمبر 4102 وحزيران/ يونيو 5102، على التوالي. وتواَصَل في الفترة المُمتدة من عام 4102 إلى عام 6102 اختبار الإرشادات على أساس تجريبي في عدد مختار من الدول الأعضاء، وهي: أستراليا وبنغلاديش وكندا وشيلي والصين ومصر وفرنسا وألمانيا والهند وإيران وإيرلندا واليابان ومدغشقر ونيوزيلندا والنرويج والبرتغال وسنغافورة وجنوب أفريقيا وأسبانيا وتايلند والمملكة المتحدة لبريطانيا العظمى وأيرلندا الشمالية والولايات المتحدة الأمريكية. واستمر تنقيح الإرشادات بناًء على النتائج المُحّققة والخبرات المُكتسبة من اختبارها على أساس تجريبي. وأُطلِق في آذار/ مارس 7102 نهج تقدير وخامة الأنفلونزا الجائحة وُشجِّ عت الدول الأعضاء على استهلال تطبيقه. وسيسلّط التطبيق الفوري لأداة تقدير وخامة الأوبئة الموسمية الضوء على ضرورة توفير بيانات أساسية متينة؛ وسيشّجع كذلك الدول الأعضاء على تنقيح البيانات المتاحة لها والإلمام بأداة التقدير هذه التي من شأنها أن تُستخدم أيضا ًأثناء اندلاع الجوائح. 1– نطاق الوثيقة تقدير وخامة الأنفلونزا الجائحة2 1-2 الغرض من إعداد وثيقة الإرشادات لقد وضعت المنظمة هذه الإرشادات لكي تستخدمها مع الدول الأعضاء في إطار تطبيق الإرشادات المتعلقة بإدارة مخاطر الأنفلونزا الجائحة (3) لأغراض تقدير وخامتها أثناء اندلاع أوبئتها وجوائحها الموسمية وعند التثبّت من انتقال فيروساتها من إنسان إلى آخر. وسوف يُواظب على إجراء التقديرات أثناء اندلاع أوبئة الأنفلونزا أو جوائحها الموسمية، بالتلازم مع إجراء تقدير نهائي لها بمجرد انحسار انتقالها. ويكون الوقت ضيقا ًوالموارد محدودة أثناء اندلاع الجوائح، لذا يلزم أن تلّم البلدان بالأداة في أقرب وقت ممكن (أي أثناء اندلاع جوائح الأنفلونزا الموسمية)، وأن تنّفذ الطرائق والمخرجات في سياق ما تجريه روتينيا ًمن تقديرات وتعّده من تقارير فيما يتعلق بوضع الأنفلونزا الموسمية، مّما سيتيح المجال أمام البلدان لتقدير وخامة الجوائح بمزيد من السهولة والكفاءة أثناء اندلاعها. ولا تنطبق هذه الإرشادات على الوضع السائد أثناء مرحلة توجيه الإنذارات (3)؛ أي قبل التثبّت من انتقال فيروسات الأنفلونزا الحيوانية المنشأ من إنسان إلى آخر، وعندما يكون الهدف منها تحديد مدى خطورة انتشار الجائحة، لأن هناك أدوات أخرى تُتيحها المنظمة لهذا الغرض؛ من قبيل التقدير السريع لمخاطر أحداث الصحة العمومية الحادة (4) أو أداة تقدير مخاطر الأنفلونزا الجائحة (5). وستُ وفّ ر أيضا،ً بفضل إجراء الدراسات الخاصة وعمليات النمذجة، معلومات لتقدير وخامة الجوائح في مراحلها المبكرة وطوال مدة اندلاعها، على أن هذه الإرشادات تركّ ز أساسا ًعلى ما يُجمع من معلومات أثناء ترّصد الأنفلونزا روتينيا.ً 1-3 الجمهور المستهدف هذه الوثيقة معّدة لكي يستخدمها في المقام الأول مهنيو الصحة العمومية على الصعيد الوطني مّمن يتكّفلون بإجراء تقديرات وطنية لوخامة الأنفلونزا أو يزمعون إجراءها، ويتمّكنون من الإسهام في التقديرات العالمية المتعلقة بوخامة الأنفلونزا. وهذه الوثيقة هي وثيقة حية سيتواصل تحديثها حسب اللزوم، وستقوم مقام وثيقة إرشادية لإجراء تقديرات لوخامة الأنفلونزا الموسمية وتلك الجائحة. 3فيما يلي الهدف المنشود من تقدير وخامة الأنفلونزا على الصعيد الوطني (3): وستستفيد المنظمة من التقديرات العالمية لوخامة الأنفلونزا في رصد الوضع العالمي للأنفلونزا وفهمه والاسترشاد بها في اتخاذ القرارات العالمية ووضع التوصيات العالمية بشأن التدخلات المُنّفذة في مجال الصحة العمومية. بيان الوضع الوبائي لأوبئة الأنفلونزا أو جوائحها وتقدير وخامتها بناًء على جميع المعلومات المتاحة؛ الاسترشاد بها في إجراء تقديرات للمخاطر الوطنية والعالمية؛ الاستهداء بها في اتخاذ تدابير التأهب والاستجابة والإنعاش في مجال الصحة العمومية، فضًلا عن تخصيص ما يلزم من موارد. 2– الهدف من تقدير وخامة الأنفلونزا تقدير وخامة الأنفلونزا الجائحة تقدير وخامة الأنفلونزا الجائحة4 3- مفهوم تقدير وخامة الأنفلونزا وتعاريف تقديرها 3-1 المؤشرات فيما يلي ثلاثة مؤشرات تُعرّف على أساسها وخامة الأنفلونزا الجائحة بناًء على ما أُجِري من مشاورات بين خبراء خارجيين وموظفي المنظمة في الفترة الواقعة بين عامي 1102 و5102، ألا وهي: قدرة فيروسات الأنفلونزا على السريان وخطورة مرض الأنفلونزا ووقعه، وهي مصطلحات ترد أدناه تعاريفها بالتفصيل وكذلك في الجدول 1. 3-1-1 قدرة المرض على السريان يعكس مؤشر قدرة فيروسات الأنفلونزا على السريان مدى سهولة انتقالها بين الأفراد والمجتمعات، وبذا، فإن الفيروس الذي لديه قدرة عالية على السريان سينتقل بسرعة من شخص إلى آخر. وتتأثر هذه القدرة بعوامل عديدة هي كالتالي: قدرة الفيروس على الانتقال من شخص إلى آخر وديناميات انتقاله وقابلية السكان المعرضين له للتأثر به، كما تتأثر قدرته هذه بالعوامل الاجتماعية والمناخية. وتُقاس عادًة القدرة على السريان أثناء اندلاع أوبئة الأنفلونزا الموسمية بواسطة نظم الترّصد الروتيني باستخدام البدائل (من قبيل عدد من يسعون إلى الحصول على الرعاية الصحية من جراء الإصابة بأمراض شبيهة بالأنفلونزا). أما الديناميات الفعلية لانتقال الفيروسات (أعدادها عقب تكاثرها) وقابلية السكان المعرضين لها للتأثر بها، فإن من شأنها أن تُقاس بواسطة دراسات استثنائية مخّصصة تُجرى أثناء اندلاع الجوائح. 3-1-2 خطورة المرض يبّين مؤشر خطورة المرض (الذي يُعرف أيضا ًباسم “وخامة العدوى”) عدد من يمرضون من الأفراد جراء إصابتهم بعدوى فيروس الأنفلونزا وتواتر أعراضها السريرية ومضاعفاتها والحصائل اللاحقة للإصابة بعدواها. وتتوقف خطورة المرض على الفيروس؛ إذ يمكن مثلا ًأن يتسبّب فيروس الأنفلونزا المُصاحب لأعراض سريرية وخيمة للغاية في إصابة عدد غير متناسب من الأشخاص بأمراض خطيرة، ليُ نقل من جرائه بعضهم إلى المستشفى ويفارق بعضهم الآخر الحياة. وتتوقف أيضا ًخطورة المرض على المضيف؛ مثل معاناة الفرد من حالات صحية كامنة تهيئه للإصابة بمرض وخيم وتاريخ أخذه للقاحات التي قد تحميه من المرض (مثل تلك المضادة لمرض الأنفلونزا والمكورات الرئوية) وعمره ومدى توافر خدمات الرعاية الصحية. ومن المرّجح أن تكون إصابة بعض شرائح السكان بعدوى المرض أكثر وخامة بكثير من سواها، كما تشكل أوصاف الفئات المعرضة بشّدة لخطر المرض جزءا ًهاما ًمن هذا المؤشر. وتُقاس خطورة المرض أثناء اندلاع الأنفلونزا الموسمية بواسطة الترّصد الروتيني بالمستشفى لحالات المرضى الذين يُدخلون إليها أو إلى وحدات العناية المركزة من جراء إصابتهم بالأنفلونزا. 3-1-3 وقع المرض يبّين عموما ًالمؤشر الخاص بالوقع كيفية تأثر المجتمع بأوبئة الأنفلونزا أو جوائحها، وهو يمثل أثرها على قطاع الرعاية الصحية؛ مثل أثرها على معدلات استخدام مرافق الرعاية الصحية (حالات الاستشفاء والإدخال إلى وحدات العناية المركزة) وعلى القوى العاملة في مرافق الرعاية الصحية وعلى المجتمع (بما في ذلك الزيادة المفرطة في عدد الوفيات). ويتأثر مؤشر وقع المرض بتنفيذ تدابير الصحة العمومية وشواغل الجمهور وسلوكيات فئات السكان المتضررة بالمرض، وإذا كان الوقع كبيرا ًعلى قطاع الرعاية الصحية، فإن موارد الرعاية الصحية قد تتعرض لضغوط. وقد تخلّف أيضا ًالآثار المترتبة على الصحة العمومية عواقب اجتماعية واقتصادية، من قبيل التغيّب عن أماكن العمل والمدارس، وانهيار البنى التحتية الحيوية، وتراجع أنشطة التجارة والسياحة. 5 تقدير وخامة الأنفلونزا الجائحة الجدول 1– ملّخص المؤشرات المُستخدمة المعلمات المُطبقة يُستنبط كل مؤشر من المؤشرات المبيّ نة أعلاه من معلمات مختلفة ُمستمدة من مصادر معنية بترّصد الفيروسات والأوبئة ومن أخرى سريرية. وغالبا ًما تُجمع روتينيا ًتلك المعلمات بواسطة نظم ترّصد الصحة العمومية بالبلد (6)؛ وقد تُقاس، عوضا ًعن ذلك، في إطار إجراء بحوث ُمحّددة (دراسات خاصة). وتركّ ز وثيقة الإرشادات هذه على ما يُجمع من معلومات عن طريق أنشطة الترّصد الروتيني؛ لأن المعلومات المُستمدة من الدراسات الخاصة لا يمكن الحصول عليها إلا أثناء اندلاع جوائح الأنفلونزا أو وقوع غيرها من الأحداث غير العادية. وهذه الإرشادات معّدة بطريقة مرنة، وينبغي تكييفها وفقا ًلاحتياجات البلد الذي يطبقها، لأن كل بلد لديه نظم ترّصد تخّصه تحديدا،ً وعليه أن (يرتكز إلى خبرته) في اختيار المعلمات من نظم الترّصد التي قد تستند إلى المؤشرات الثلاثة. وسعيا ًإلى تقدير وخامة الأنفلونزا، تُشّجع البلدان على تحديد المعلمات التي تُجمع بواسطة النظم الراسخة للترّصد الروتيني. عدد من يمرضون من الأفراد من جماعة سكانية ما بسبب الإصابة بالأنفلونزا على أساس أسبوعي ما مدى وخامة المرض الذي ُيمرِض الفرد من جراء الناجمة عن جميع الأسباب (المُصّنفة من الناحية النموذجية إلى فئات بحسب العمر) • عدد الحالات المؤكدة للإصابة أسبوعيا ًبالأنفلونزا التي يلزم استشفاؤها في وحدة العناية المركزة، أو عدد تلك الحالات التي يلزمها استشفاء صورة: اختيار المعلمات المُطبقة في تقدير الوخامة على أساس المؤشرات. لابد من مراعاة بعض المعايير عند اختيار المعلمات المُستخدمة لتقدير الوخامة على أساس المؤشرات الثلاثة، وذلك كالتالي: يجب أن تكون المعلمات على النحو الآتي: أن تبّين نشاط الأنفلونزا، مّما يحتّ م بالتالي فحص مجموعة فرعية على الأقل من العينات في المختبر للكشف عن فيروسات الأنفلونزا فيها؛ أن تكون موثوقة وواردة من نظام ترّصد راسخ على مر الزمن (أو من نظام يُوثّق جيدا ًما يطرأ من تغييرات في هذا المضمار)؛ أن تكون مناسبة التوقيت. ويجب أن تُتاح البيانات المتوفرة سابقا ً(عن الأوبئة أو الجوائح الموسمية المندلعة بحسب التسلسل الزمني) فيما يخص المعلمات؛ وأن تُتاح، حيثما أمكن، القواسم المشتركة اللازمة لحساب النسب أو المعدلات (التي تمثلها). ويبّين الجدول 2 المعلمات التي يُنظر إليها على أنها مفيدة للغاية مثلما حّددها الفريق العامل التقني التابع للمنظمة والمعني بتقدير وخامة الأنفلونزا الجائحة. على أساس المؤشرات.اختيار المعلمات المُطبقة في تقدير الوخامة 1 أ من المعلمات المُحتملة الأخرى التي قد تعكس وقع المرض على المجتمع، إغلاق المدارس وشغل أسرّة المستشفيات والتغيّب عن العمل وعن المدارس. وقد لا تُبلّغ وحدات ترّصد الأمراض الموجودة في بلد ما بهذه المعلمات غير المتعلقة بالرعاية الصحية؛ بل تجمعها بالأحرى وزارات التعليم أو الكافية، أن يُنظر إلى المعلمات بحسب الفئات العمرية، وفيما يلي الفئات الدنيا منها: أقل من 51 عاما ًوبما يتراوح بين 51 و46 عاما ًو56 عاما ًأو أكثر. وفي حال استخدام المزيد من تلك الفئات (مثل أكثر من عامين أو أقل من 5 أعوام)، فإن من الضروري التأكد من إمكانية تصنيفها إلى الفئات العمرية المقترحة سابقا.ً وقد تكون البيانات غير كافية في بداية الموسم لتصنيفها بحسب تلك الفئات، ولكن من الأهمية بمكان أن تُجمع معلومات عن كل فئة منها تحديدا ً منذ البداية. أما بالنسبة إلى المؤشرين الخاصين بخطورة المرض ووقعه، فينبغي، حيثما أمكن، أن تحسب المعلمات حساب معاناة الفرد من أمراض أو حالات مرضية مزمنة كامنة أو عدم معاناته من تلك الأمراض أو الحالات التي يُعرف عنها أنها تخلّف حصائل وخيمة عند إصابة الفرد بالأنفلونزا (من قبيل الربو أو الايدز والعدوى بفيروسه أو الحمل أو أمراض القلب أو الرئة). ويُرجى الرجوع إلى التذييل 3 من المعايير العالمية للمنظمة بشأن ترّصد أوبئة الأنفلونزا للحصول على مزيد من المعلومات عن الحالات المرضية الموجودة مسبقا.ً (6) وإضافة إلى هذه المعلمات المقترحة، فإن بإمكان الدول الأعضاء إدراج معلمات أخرى استنادا ًإلى نظم الترّصد الخاصة بها، وذلك شريطة أن تكون البيانات موثوقة. تقدير وخامة الأنفلونزا: النقاط الأساسية المطروحة بشأن الخطوة 1 يُرجى القيام في بداية التقدير بتطبيق المعايير الواردة في هذه الإرشادات لاختيار المعلمات التي ستستخدمونها. الرجاء توثيق المعلومات التالية بشأن المعلمات المُختارة: مصدر البيانات (مثل مراقبة المرضى المراجعين للعيادات الخارجية أو الترّصد في المستشفيات)؛ وعدد السنوات التي تُتاح فيها البيانات؛ ومكامن قوة هذه المعلمة؛ والقيود المفروضة عليها؛ ونسبة البيانات المؤكدة مختبريا.ً تقدير وخامة الأنفلونزا الجائحة 9 تختلف كثيرا ًنظم الترّصد بين بلد وآخر؛ وبذا، فإن من المتعّذر استخدام قيم المعلمات المطلقة لغرض إجراء مقارنات هادفة بين البلدان، ولكن يمكن الاطلاع داخل البلد على كيفية مقارنة المعلمة بمستواها في المواسم السابقة، وكيفية مقارنة قيمة هذه المعلمة بشكل أدق بقيمة ذروتها في تلك المواسم. وبالتالي، يمكن لأي بلد أن يستفيد من البيانات السابقة في بيان نشاط أحد الأوبئة (أو الجوائح) مقارنة بنشاطه في المواسم السابقة باستخدام مواصفات نوعية مثل ما يلي: لا يوجد أي نشاط أو هو دون العتبة الموسمية منخفض معتدل مرتفع استثنائي. ويمكن بعد ذك مقارنة تلك التقديرات النوعية داخل البلد على مر الزمن وبين البلدان على حد سواء. ومن المجدي (الاستفادة من البيانات السابقة) في تعيين عتبات أو تحديد مديات للمعلمات التي تفسح المجال أمام تحديد رتب نوعية عن طريق بيان مستويات نشاط الوباء (أو الجائحة)، وهو أمر ينطوي على تحديد بداية اندلاع هذا الوباء (أو الجائحة) ومن ثم تعيين عتبات لكل معلمة لتحديد مختلف مستويات نشاطه (نشاطها) على النحو المبين أدناه. فيما يلي أسلوبان يمكن اتباعهما في تحديد بداية اندلاع وباء الأنفلونزا (الموسمية أو الجائحة): حساب عتبة الجائحة بواسطة أسلوب ترّصد مستويات تنّقل الأوبئة (7، 8)؛ أو بواسطة معدل الإصابة أسبوعيا ًبحالات الأنفلونزا الفعلية (نسبة العينات التي يُؤكّد حملها لفيروسات الأنفلونزا مقارنة بجميع العينات المُجّمعة) لغرض تحديد عتبات الجائحة01 أ من الأفضل استخدام المتوسط الهندسي لقيم الذروة عوضا ًعن المتوسط الحسابي في حال وجود اختلافات جّد كبيرة في قيمها بين موسم وآخر. فيما يلي أسلوبان يمكن اتباعهما في تحديد بداية اندلاع وباء الأنفلونزا (الموسمية أو الجائحة): حساب عتبة الجائحة بواسطة أسلوب ترّصد مستويات تنّقل الأوبئة (7، 8)؛ أو أسلوب المنظمة (6)؛ أو اتباع نهوج إحصائية أو تجريبية تخص البلد تحديدا ًرهنا ًبالخصائص المتأصلة في نظمه. وأثبت الاختبار التجريبي أن أسلوب ترّصد مستويات تنّقل الأوبئة هو أسلوب ناجح تماما ًفي معظم البلدان التي تطبق المعلمتين المتعلقتين بقدرة المرض على السريان وبوقعه، ولكن يلزم إنجاز المزيد من العمل في المناطق المدارية. وقد يتعين محلّ يا ًاستنباط نهوج تخص البلد تحديدا ًبشأن تعيين العتبات بالاستفادة من البيانات الوطنية السابقة، ولابد من أن تكون هذه قابلة للمقارنة مع العتبات المُستمدة من أسلوب ترّصد مستويات تنّقل الأوبئة ومن أسلوب المنظمة. وتُعّين العتبات بطريقة ينبغي فيها أن تتجاوز قيم ذروتها الموسمية المُعيّ نة بنسبة تراوح بين 05 و٪06 العتبة المعتدلة، وأن تتجاوز قيمها المُعيّ نة بنسبة ± ٪01 العتبة المرتفعة وأن تتجاوز تلك التي تُعّين منها بنسبة ± ٪5.2 العتبة الاستثنائية. ويبّين الجدول 3 نقاط الفصل المقترحة والمُستخدمة من جانب المنظمة وفي أسلوب ترّصد مستويات تنّقل الأوبئة فيما يتعلق ولا تصبح مستقرة إلا عندما تبلغ الأنفلونزا ذروة نشاطها. وتتسم قيم الذروة بالاستقرار إلى حد ما بين موسم وآخر وتقترن باختلافات معينة بحسب الفئات العمرية ورهنا ًبدوران الأنماط الفرعية من الأنفلونزا. ولكن تلك القيم قد تكون مختلفة جدا ًأثناء اندلاع جائحة ما، وعليه، ينبغي أن تُحسب العوامل والانحرافات المعيارية الخاصة بكل بلد تحديدا ًباستخدام القيم المُحّددة في نهاية الموسم مقارنة بالمواسم السابقة، وأن تُنّقح العتبات بالاستفادة من البيانات السابقة، ومنها البيانات المتعلقة بالجوائح، مثلما هو مبين في الجدول 4. وينبغي أيضا ًتعيين العتبات الخاصة بالسن تحديدا ًعند توفر البيانات. الجدول 4– أمثلة على تعيي المستمدة من التقديرات، بما فيها مستويات الثقة، لأغراض الاسترشاد بها في إجراء تقديرات وطنية للمخاطر. • توثيق المعلمات والعتبات المستخدمة والمعلمات الأكثر موثوقية والأهم في إجراء التقديرات إجمالا،ً وكيفية تفسيرها والسبل الكفيلة بالجمع بين المعلومات من مختلف المعلمات لغرض البت بشأن كل مؤشر. إن تقدير الوخامة على الصعيد الوطني مدفوع في المقام الأول بما يُجمع من بيانات كمية ويُكتسب من خبرات وتجارب. ويجري البلد تقديرا ًنوعيا ًلكل مؤشر عن طريق دراسة جميع المعلمات الخاصة بمؤشر معين؛ وتصنيف القيم المُحّددة في الوضع الراهن فيما يتعلق بالبيانات السابقة التي تخّصه تحديدا ً(يُفّضل جمعها عن خمسة مواسم)، أو بالبيانات المُتاحة بخلاف ذلك والقابلة للمقارنة؛ ومراعاة ما يُ تاح من معلومات أخرى. ويلزم عند إجراء التقديرات النوعية دراسة معلومات أخرى، مثل تلك الواردة من نظم مختلفة في الوقت المناسب وأوجه التحيّ ز في الإبلاغ عنها ومدى موثوقيتها النسبية. وينبغي القيام من الناحية المثالية وأسبوعيا ًوعقب بداية اندلاع الوباء أو الجائحة، بتطبيق العتبات على بيانات المعلمات الحديثة فيما يخص مؤشري قدرة المرض على السريان ووقعه. ومثلما نُوِقش سابقا،ً لابد من تقدير مؤشر خطورة المرض بمجرد بلوغ الأنفلونزا ذروة نشاطها. وينبغي، كحد أدنى، تقدير المؤشرات الثلاثة في المرحلة التي تتوسط اندلاع الوباء وفي مرحلة انتهائه. ومن الضروري التحّقق أيضا،ً في حال سمحت البيانات بذلك، مما إذا كانت هناك اختلافات في فئات عمرية معينة أو في فئات أخرى معرّضة للخطر. ولابد من استخدام التقديرات، بما فيها مستويات فواصل الثقة، في المنظمة بما تجريه البلدان من تقديرات نوعية فيما يخص كل مؤشر جنبا ًإلى جنب مع مستويات الثقة وأية تعليقات تُبدى بشأن المؤشرات تحديدا،ً ويُفضل إبلاغها بتلك التقديرات أسبوعيا،ً على أن تُبلّغ بها، على الأقل، في المرحلة التي تتوسط اندلاع الوباء وفي مرحلة انتهائه (الشكل 2). وتعكس نوعية مستويات الثقة مدى تأكد الباحث من نطاق اتساع وجودة ما يُستخدم من بيانات في تقدير المؤشرات. وينبغي في حال وجود اختلافات في التقديرات بحسب الفئات العمرية أو الفئات التي تعاني من حالات مرضية كامنة، أن تستند التقديرات النهائية إلى البيانات المُصّنفة (بشأن جميع الفئات العمرية)، ولكن في حال إجراء التقديرات لفئة 1大流行性流感的严重程度评估 1.1 背景及发展史 2009年H1N1流感大流行事件表明,世界卫生组织(WHO)和相关国家机构尚未掌握对 大流行性流感严重程度进行及时评估的稳健的、标准化的方法;同时揭示了联合国各成员国 尚未做好关于迅速评估流感大流行严重程度或实施必要风险管理和通信计划的足够准备。 2011年,世界卫生大会通过了审查委员会的报告《关于《国际卫生条例(2005)》实施 情况和2009年大流行性流感(H1N1)情况的报告》。委员会建议,WHO应当制定并实施可评 估所有流感流行严重性的方案,无论是针对季节性流感还是大流行性流感。严重性评估为确 定大流行性流感暴发的时间、规模、重点地区、强度和紧迫性提供了必备科学信息。报告指 出,“只有通过每年对测量流感严重程度的工具进行应用、评估和改进,世界卫生组织和成员 国才能在下一次大流行中做好充足准备以便更好、更及时地评估流感的严重性”(1)。 应在大流行的早期阶段实施严重性评估,并随着大流行的进一步发展进行定期评估。 自世界卫生大会强调这一需求以来,世界卫生组织已在制定大流行性流感严重程度评估 (PISA)框架方面取得了重大进展(2)。该框架是在历经数次会议、专家磋商、世界卫生组织 合作项目,并已组建大流行性流感严重性评估技术工作组(TWG)的前提下制定的。该框架 根据三个指标来定义流感严重性:传播能力、疾病严重性和影响程度。 2014年4月,世界卫生组织召开了一次TWG会议,目的是审查会员国目前所收集的监测 数据的范围。TWG审查了所选成员国的监测数据,以明确各成员国监测系统的优势、差距和 待改进的地方,审查结果显示各成员国之间的监测数据存在显著差异性。 以先前会议的结 果、世界卫生组织与罗伯特科赫研究所(德国)进行的一项由南半球成员国(澳大利亚、智利、 新西兰和南非)实施的试点研究、以及正在进行的电话会议和专家磋商结果为基础,制定了 描述流感严重程度结构化评估方法的临时指南。该框架的制定分别为2014年11月和2015年 6月举行的TWG第二次和第三次的讨论会议奠定了基础。 2014年至2016年,部分成员国陆续对临时指导进行试点测试,这些成员国包括:澳大利 亚、孟加拉国、加拿大、智利、中国、埃及、法国、德国、印度、伊朗、爱尔兰、日本、马达加斯 加、新西兰、挪威、葡萄牙、新加坡、南非、西班牙、泰国、英国和美国。 根据试点测试的结果和经验,本指导得以进一步完善。2017年3月,WHO启动了PISA办 法并鼓励成员国将之付诸于实践。使用该办法对季节性流感流行严重程度进行评估时非常依 赖于强大的基线数据;此外,它还鼓励成员国完善现有数据,熟悉评估办法,并且该评估办法 也会在流感大流行期间使用。 1.2 指导文件的目的 作为大流行性流感风险管理指南(PIRM)(3)的一部分,WHO制定本指南供成员国和 WHO在流感发生持续人际传播时对季节性流行和大流行的严重性进行评估。流感季节性流 行或大流行期间将进行持续评估,传播停止时则进行决定性评估。 在大流行期间,时间和资源有限。因此,各国需要尽快熟悉此评估办法(例如在季节性 流感流行期间),并在常规季节性流感期间实施本方法,评估其严重程度并报告。这将使各国 在大流行期间更容易、高效地评估流感大流行的严重程度。 文件范围1 2 大流行性流感的严重程度评估 本指导文件不适用于流感警戒阶段(3),即动物源性流感病毒在人际间持续传播之前,该 阶段的研究目的是确定大流行性流感传播的风险因素。可以从WHO处获取其他工具用以确 定大流行的危险因素;例如,急性公共卫生事件的快速风险评估(4)或流感大流行风险评估工 具(TIPRA)(5)。 有一些专门的研究和建模方法可用于估计早期和整个流行期间大流行的严重程度。然 而,本指南主要侧重于常规流感监测期间收集的信息。 1.3 目标读者 本文件的适用读者主要是国家级的公共卫生专业人员,这类专业人员会执行或计划对国 家流感严重程度进行评估,并且他们可以为全球流感严重程度评估做出贡献。本文会根据需 要持续更新,并将作为季节性和大流行性流感严重性评估的指南。 3大流行性流感的严重程度评估 2 评估流感严重程度的目的 在国家层面上,评估流感严重程度的目的主要有以下三点(3): 世界卫生组织将使用全球流感严重程度评估来监控、了解全球情况,并通报和辅助制定 关于公共卫生干预措施的全球决策和建议。 • 描述流行病学特征并在已有信息的基础上评估流感流行或大流行的严重 程度; o 可靠,且来自不随时间变化的稳定监测系统(或者来自每次变化都有详细记录的系 统); o 及时; • 可获得参数的历史数据(关于历史季节性流行或大流行);并且 • 在可能的情况下,分母应能够用来计算(有代表性的)比例或比率。 WHO 大流行性流感严重性评估技术工作组(TWG)定义的最具使用性的参数如表2所示。 表2. 评估严重程度的有用参数 ARI:急性呼吸道感染;ICU:加护病房;ILI:流感样疾病;MAARI:急性呼吸道疾病;P&I:肺炎和流 感;SARI:严重的急性呼吸道感染 a其他可以反映流感对社会影响的参数有:学校关闭情况、医院病床入住率、工作缺勤和学校缺课情况。这些非健康保健相关的参 数可能不会向国家的疾病监测单位报告,但会向教育部或社会保障机构报告。预试验期间未对这些参数进行观测。 2 3 4 1 8 大流行性流感的严重程度评估 在可实施且可以获得充足数据的情况下,应按年龄分层观察参数。年龄组至少分为3组:15 岁以下,15-64岁,65岁以上。如果使用更多的年龄组(例如大于2年或小于5年作为年龄组间 隔),很重要且需要确保的一点是新的年龄组可以合并为之前的年龄组。在流感流行季刚开始 时收集的数据可能不足以按年龄组进行分层,但从一开始就按年龄组别来收集信息很重要。 对于反映疾病严重性和影响力的指标,选取参数时应尽可能考虑到病人是否存在与流感严 重结局相关联的潜在慢性病或其他病症(如哮喘,艾滋病毒/艾滋病,怀孕,心脏或肺部疾病)。 请参阅附录3中的WHO全球流感流行病学监测标准,以了解已有条件下的更多信息(6)。 除了这些已提出的参数以外,各个成员国倘若对本国检测系统的数据有信心,可以基于自 身监测系统纳入其他参数。 9大流行性流感的严重程度评估 4.2 第二步:设置阈值 不同国家的监测系统差别很大,因此参数的绝对值不能在不同国家间进行有意义的比 较。然而,在一个国家内用当前观察的参数与之前流行季观察的参数做对比是可以的。更具体 来说,即将当前参数值与之前几个流行季节的峰值比较。因此,每个国家都可以使用之前几个 流行季的历史数据与当前数据的对比来描述流行(或大流行),定性描述诸如: • 没有活动或低于季节性阈值 • 低 • 中 • 高 • 非常明显的季节性活动 这些定性评估指标即可用于同一国家不同时间的比较或者不同国家之间的比较。 使用历史数据为参数设置阈值或定义范围非常有用,这些参数可被用来定性地对流感 活动强度进行等级分类,需要定义流行(或大流行)的开始,然后设置每个参数的阈值以定 义不同的活动水平,如下所述。 4.2.1 定义流感流行的开始 定义流感流行(季节性流行或大流行)的开始,可以运用以下几种方法: • 使用移动流行病学方法(MEM)计算流行阈值 (7, 8) ;
