Of 2 576 cases of bacteraemia reported in weeks 11-52, 1972 (11 March to 29 December), 76 patients (3%) were infected with members of the genus Bacteriodes— the anaerobic, Gram-negative bacilli which, often difficult to grow in laboratory culture, form a large proportion of the flora of the large intestine. In recent years their importance as potential human pathogens has become more widely appreciated, as improved knowledge of their cul­ tural requirements has led to more frequent isolations in the di­ agnostic laboratory. Many bacteroides give rise to a character­ istic offensive odour when they grow in laboratory media, but cultures often die before the species can be identified. Six of the isolates were reported to be B fragilis, three were B. necrophorus, one was B. serpens, and the remainder were reported as Bacteroides with no species identification.

The age and sex breakdown of the 76 reported cases is given in Table 1. Bactereamia caused by bacteroides was most commonly reported in elderly patients, most of whom were known to have had an underlying illness. Forty-five of the patients (59%) were reported as having either disease of the large intestine or an ab­ dominal wound—frequently an operation wound made for surgery involving the large intestine. The operations were often compli­ cated by post-operative collapse or the development of peritonitis. In ten of the 16 women aged from 15 to 49 years, the bacteroides bacteraemia was known to be associated with a complication of pregnancy; post-partum pyrexia in seven patients, caesarian section in one and septic abortion in two women.

Depuis un certain temps, l’importance de ces bacilles en tant qu’organismes potentiellement pathogènes pour l’homme est déplus en plus largement admise, une meilleure connaissance de leurs exi­ gences culturales ayant conduit à des isolements plus fréquents dans les laboratoires de diagnostic. La croissance de beaucoup de ces orga­ nismes sur les milieux employés en laboratoire donne naissance à une odeur déplaisante caractéristique, mais les cultures meurent souvent avant que l’on puisse identifier les espèces. Six des isolats étudiés ont été identifiés comme étant B. fragilis, trois comme B. necrophorus, un comme B. serpens et le reste comme appartenant au genre Bacteroides, sans mdication d ’espèce. Le Tableau 1 illustre la répartition par âge et par sexe des 76 cas notifiés. Les bactériémies à Bactéroides ont été surtout observées chez des personnes âgées dont la plupart souffraient d ’une maladie préexistante. Quarante-cinq des 76 malades (59%) étaient atteints d'une affection du gros intestin ou porteurs d ’une blessure abdomi­ nale — le plus souvent une cicatrice d ’opération du gros intestin. Les interventions chirurgicales avaient été dans bien des cas compli­ quées de collapsus post-opératoire et de péritonite. Chez dix femmes âgées de 15 à 49 ans sur 16, la bactériémie à Bacteroides était associée à une complication de la grossesse: pyrexie post-partum (7), césarienne (1), avortement septique (2).

la another eight of the patients, the bacteraemia was not attribu­ table to an abdominal condition but was associated with illness which might be regarded as predisposing to infection—malignant disease in four patients, renal failure in two and diabetes in two. One of the two diabetic patients was a woman of 48 years who developed what was clinically diagnosed as gas gangrene following accidental amputation o f her toe. She became seriously ill with icnal failure and pulmonary oedema, developed bacteroides sep­ ticaemia and died.

Chez huit autres malades, la bactérienne n ’était pas imputable à une infection abdominale, mais associée à une maladie que l’on pourrait considérer comme prédisposant à l’infection: maladie maligne (4), insuffisance rénale (2) et diabète (2). L ’un des deux diabétiques était une femme de 48 ans chez laquelle le diagnostic clinique de gangrène gazeuse avait été posé à la suite de l ’amputa­ tion accidentelle d ’un orteil. Elle mourut après avoir présenté une grave insuffisance rénale et un œdème pulmonaire, puis une septi­ cémie Bacteroides.

Thirteen of the cases (17%) are known to have died (Table J). Although most of the fatalities were in patients who had a serious underlying illness of one sort or another, the bacteroides infection must presumably have contributed to the fatal outcome.

Sur les 76 malades, 13 (17%) sont décédés (Tableau 1). Bien que la plupart des victimes aient été des sujets souffrant déjà d ’une mala­ die grave, l’infection à Bactéroides a vraisemblablement contribué à l’issue fatale.
