In Pakistan, smallpox endemic areas, as in India, have become increasingly localized. This year, more than 70% of the cases have been detected in only three of the country’s 50 districts. Special programmes are in progress in each (Fig. 4). In Bangladesh similarly, problem areas have now largely been restricted to the districts of Rajshahi Division and Mymensingh District of Dacca Division. Smallpox incidence is 64% below the comparable period of last year and appears now to be decreasing at a time when a seasonal increase in smallpox cases normally occurs

Au Pakistan, comme en Inde, l ’endémicité de la variole est maintenant de plus en plus localisée. Cette année, plus de 70% des cas ont été enregistrés dans trois seulement des 50 districts du pays, et des programmes spéciaux y sont en cours (Fig. 4). De même, au Bangladesh, les zones difficiles se limitent surtout à la Division de Rajshahi et au district de Mymensingh dans la Division de Dacca. L ’incidence de la maladie a diminué de 64% par rapport à la période correspondante de l’an dernier et elle parait fléchir au moment même ou l’on enregistre normalement un ac croissement saisonnier (Fig. 5).

More than 80% of all cases in Ethiopia have so far this year been detected in only four o f the country’s 14 provinces and smallpox incidence has decreased for the third consecutive year (Fig. 6).

Estimation
The next few months are most critical in the strategy of the pro gramme. All programmes are now increasing the tempo of their activities in an effort to achieve effective control of known foci before the summer rains begin. In contrast to previo us years, the tempo of activity will be sustained to the extent possible during the summer months of normally low incidence. If these activities are successful, the endemic foci should be much fewer in number at the beginning of the next smallpox season thus permitting available resources to be concentrated in order to deal with them more rapidly and effectively.
Les prochains mois seront des étapes critiques dans la stratégie antivariolique. Dans tous les programmes, on accélère en ce moment les activités afin d ’arriver à maîtriser les foyers connus avant que n ’arrivent les pluies d ’été. Contrairement à ce qui s’est fait les années précédentes, ce rythme d ’activité sera maintenu autant que possible pendant les mois d ’été où l’incidence est normalement faible. Si cette action est fructueuse, les foyers d ’endémicité devraient être beaucoup moins nombreux quand débutera la prochaine saison de la variole, ce qui permettra de s’y attaquer plus rapidement et plus efficacement avec toutes les ressources disponibles.
