HUMAN BRUCELLOSIS U n it e d S tates o f A m er ic a . — In 1969, a total of 231 cases of human brucellosis was reported as compared with 251 cases in 1968.1 Surveillance reports were submitted to the Office of Veterinary Public Health Services of the National Communicable Disease Center (NCDC) on 195 (84%) of the cases reported in. 1969. Thirty states as compared to 35 in 1968 reported one or more cases, with three states, California, Iowa, and Virginia, recording 55% of the cases. The greatest increase in number of cases as compared with 1968 occurred in Iowa although there has been a general downward trend in that State since 1960. The largest decrease was in Georgia from 16 cases in 1968 to 4 in 1969.

In over one-third of the 194 cases for which the date of onset was known, clinical signs were first noted in the spring months. Of the total cases, 90% were males, predominantly between 20 and 55 years of age. 1 See No. 18, 1969, p. 305.

Trente états, contre 35 en 1968, ont eu à enregistrer un ou plu­ sieurs cas en 1969; trois d’entre eux — la Californie, l’Iowa et la Virginie — comptent ensemble pour 55% du total. Le plus fort accroissement d’incidence par rapport à 1968 s’est produit dans l’Iowa où la tendance générale était pourtant décroissante depuis 1960. La plus forte diminution a été constatée en Géorgie, où les cas sont passés de 16 en 1968 à 4 en 1969. Dans plus du tiers des 194 cas pour lesquels on connaît la date du début de la maladie, les premiers signes cliniques sont apparus au cours du printemps. Of the 195 cases on which epidemiological information was sent to the NCDC, 114 (58%) recorded swine as the most probable source of infection: 20 cases (10%) were associated with cattle only; 16 cases (8%) with both cattle and swine; and 5 (3%) were attributed to dairy products. In 31 cases (16%), the source was unknown (Table 1). In 1969 brucellosis continued to be a problem in meat processing plants and was diagnosed and reported in 139 employees in about 40 different establishments. The morbidity rate in meat processing plant workers has risen from 28% of 396 case reports reviewed in 1958 to 71 % of the 195 cases reviewed in 1969. The incidence of brucellosis in man reached a peak in 1947 with 6 321 reported cases (4.4 cases per 100 000 population). The cooperative bovine brucellosis eradication programme and asso­ ciated control measures instituted since then, such as widespread pasteurization of milk, have resulted in a steady reduction in human disease. Since 1965 the annual incidence of reported cases has leveled off at around 250 cases.

Dans 114 (58 %) des 195 rapports épidémiologiques reçus par le NCDC, le porc était mentionné comme la source d’infection la plus probable: dans 20 rapports (10%), le bœuf était seul incriminé; dans 16 (8 %), le porc et le bœuf et dans 5 (3 %), des produits laitiers. Pour 31 des cas (16%), la source d’infection était inconnue (Tableau 1). En 1969, la brucellose a continué de poser un problème dans les conserveries de viande: elle a atteint au total 139 employés d’une quarantaine d’établissements. La morbidité chez les travailleurs de ce secteur est passée de 28% des 396 cas étudiés en 1958 à 71 % des 195 cas étudiés en 1969. L’incidence de la brucellose humaine avait atteint un maximum en 1947, avec 6 321 cas notifiés, soit 4,4 pour 100 000 habitants. Depuis, le programme coopératif d ’éradication de la brucellose bovine et les mesures complémentaires de lutte — par exemple la généralisation de la pasteurisation du lait — ont eu pour effet de faire reculer régulièrement la maladie chez l’homme. A partir de 1965, l’incidence annuelle s’est maintenue aux environs de 250 cas.
