SURVEILLANCE O F NEISSERIA GONORRHOEAE PRODUCING [3-LACTAMASE U n it e d S ta tes o f A m e r ic a . — Cases of penicillinase-producing Neisseria gonorrhoeae (PPNG) which can be linked epidemiologically to infection acquired in eastern Asia continue to be detected.

SURVEILLANCE DE NEISSERIA GONORRHOEAE PRODUCTRICES DE (3-LACTAMASE E t a ts - U n is d ’A m é r iq u e . — On continue à observer des cas de Neisseria gonorrhoeae (NGPP) productrices de penicillinase qui, du point de vue épidémiologique, peuvent être rattachées à des infec­ tions contractées en Asie du Sud-Est. Du 21 avril au 16 mai 1980, on a signalé à Albuquerque, NouveauMexique, cinq cas d ’infection par des NGPP. Le premier cas cli­ nique, un jeune homme de 20 ans qui avait contracté une urétrite gonococcique en Asie orientale, a été traité dans un hôpital par l’administration de 4,8 millions d ’unités de pénicilline procaine aqueuse G et d’1 g de probénécide par voie buccale, sans amélio­ ration. Deux jours plus tard, un médecin privé lui a prescrit 500 mg de chlorhydrate de tétracycline deux fois par jour pendant 21 jours. Les symptômes du malade ont diminué et alors qu’il prenait encore la tétracycline il a eu des rapports sexuels avec trois femmes. Chacune de ces femmes a contracté une infection endocervicale par les NGPP; l’une d ’elles a également présenté une salpingite aigue et une autre une infection asymptomatique du pharynx. Un autre partenaire sexuel de l’une des femmes a été atteint d ’une infection à la fois de l’urètre et du pharynx par les NGPP. Une fois ces infections reconnues comme résistantes à la péni­ cilline, les quatre malades sans complications ont été traités par des injections intra-musculaires (IM) de 2 g de spectinomycine. La malade atteinte de salpingite a été améliorée après les IM de 2 g de spectinomycine deux fois par jour pendant trois jours, puis une fois par jour pendant sept jours. Les cultures après traitement étaient négatives aux épreuves de détection des NGPP pour tous les malades. A San Diego, Californie, on a compté de juillet 1979 à mars 1980 quatre cas de salpingite liés à des infections endocervicales avec NGPP. Dans chaque cas, l’infection était consécutive à des rapports sexuels avec un partenaire qui avait contracté une gonococcie en Asie orientale. Bien qu’on n’ait pratiqué ni culdocentèse ni laparo­ scopie sur aucune de ces patientes, toutes présentaient des signes et symptômes typiques de salpingite aiguë. Chaque malade a reçu un traitement différent: 1) 500 mg de chlorhydrate de tétracycline par voie buccale, quatre fois par jour pendant 14 jours; 2) 2 g de specti­ nomycine par voie intraveineuse (IV) trois fois par jour pendant cinq jours; 3) 1 g de céfoxitine en IV toutes les six heures pendant sept jours; et 4) 2 g de céfoxitine en IM, avec 1 g de probénécide en une dose par voie buccale. Chez toutes les malades, les symptômes ont disparu complètement et et il n ’y avait pas de N. gonorrhoeae dans les cultures de prélèvement endocervical fait après traitement. N o t e d e l a R é d a c t io n : L ’administration précoce d ’un traite­ ment efficace limite la propagation et les risques de complication des infections NGPP. Le Center fo r Disease Control (CDC) recom­ mande maintenant expressément l’administration de 2 g de specti­ nomycine pour le traitement initial des blennorragies anogémtales sans complication chez les malades provenant d ’un pays tels que les Philippines, Singapour et Thaïlande, où existent des zones de forte prévalence des infections à NGPP. Cette posologie est également recommandée pour: 1) le traitement initial des infections prouvées par les NGPP, 2) le traitement des partenaires sexuels de ces malades, et 3) le retraitement des malades chez qui l’infection persiste après un premier traitement par un autre antibiotique. Les isolats de N. gonorrhoea provenant de malades traités par la spectinomycine doivent être soumis à une épreuve de détection de la production de pénicillinase. Il convient de faire pour tous les cas de blennorragie une culture de prélèvement pratiqué de trois à sept jours après la fin du traitement.

In Albuquerque, New Mexico, from 21 April through 16 May 1980, there were five cases of infection caused by PPNG. The index patient, a 20-year old man who acquired gonococcal urethritis in eastern Asia, was treated at a hospital with 4.8 million units of aqueous procaine penicillin G and probenecid 1 g orally; he failed to improve, however. Two days later, a private physician prescribed tetracycline hydrochloride 500 mg twice daily for 21 days. The patient’s symptoms diminished, and, while still taking tetracycline, he had sexual contact with three women. Each of these women developed endocervical PPNG infection; one also developed acute salpingitis, and another acquired asymptomatic pharyngeal infec­ tion. Another male sexual partner o f one o f the women acquired both urethral and pharyngeal PPNG infection. Once these infections were recognized as being penicillin-resistant, the four patients with uncomplicated infections were treated with spectinomycin 2 g intramuscularly (IM). The patient with salpin­ gitis improved after receiving spectinomycin 2 g IM twice daily for three days, then once daily for seven days. Post-treatment cultures from all patients were negative for PPNG. In San Diego, California, from July 1979 through March 1980, there were four cases of salpingitis associated with endocervical PPNG infection. In each case, infection resulted from sexual contact with someone who had acquired gonorrhoea in eastern Asia. Although neither culdocentesis nor laparoscopy was performed in any of these patients, all had typical signs and symptoms of acute salpingitis. Each patient received a different treatment regimen: (1) tetracycline hydrochloride 500 mg orally, four times daily for 14 days ; (2) spectinomycin 2 g intravenously (IV) three times daily for five days; (3) cefoxitin 1 g IV every six hours for seven days; and (4) cefoxitin 2 g IM plus probenecid 1 g orally as a single dose. Each patient had complete resolution of symptoms and a negative post­ treatment endocervical culture for N . gonorrhoeae.

E d it o r ia l N o t e : Early use of effective treatment will minimize the spread of and complications from PPNG infections. The Center for Disease Control (CDC) is now specifically recommending spectinomycin 2 g for the initial treatment of uncomplicated ano­ genital gonorrhoea in patients who have recently returned from countries, such as the Philippines, Singapore, and Thailand, that have areas of high prevalence of PPNG infections. The same dosage is also recommended for (1) the initial treatment of patients with proven PPNG infections, (2) treatment of the sexual partners of these patients, and (3) retreatment of patients who have persistent infections after initial therapy with another antibiotic. Isolates of N . gonorrhoeae obtained from patients treated with spectinomycin should be tested for penicillinase production. All patients treated for gonorrhoea should have a post-treatment culture taken three to seven days after treatment.
