POLIOMYELITIS SURVEILLANCE U n it ed K in g d o m . — In Scotland during the years 1961-1970, cases of poliomyelitis were few in number and occurred sporad­ ically, with the exception of the two years, 1962 and, to a much lesser extent, 1961 (Table 1). During these two years more than three-quarters of all the confirmed cases were paralytic, the ostensible reason for this being that many non-paralytic cases may not have been diagnosed as poliomyelitis, but mistaken for influenza, etc. Oral poliomyelitis vaccine has been available in Scotland for routine application since 1962. It is a vaccine made from Sabin’s strains of living attenuated polio viruses of types 1,2 and 3. There seems little reason to doubt that the adoption of oral vacccine has had a marked impact on the incidence of notifications of new cases of poliomyelitis since the date of its introduction.

Plus des trois-quarts de tous les cas confirmés de ces deux années étaient paralytiques, apparemment parce que de nombreux cas non paralytiques avaient fait l’objet d ’un diagnostic erroné de grippe, etc.

Depuis 1962, on procède en Ecosse à la vaccination systématique par le vaccin buccal de Sabin. 11 s’agit d ’un vaccin vivant atténué préparé à partir de souches de virus des types 1, 2 et 3. Il semble peu douteux que la vaccination buccale ait eu, depuis la date de son introduction, une influence marquée sur l’incidence des notifications de cas nouveaux de poliomyélite.

In Table 2 the figures relate to the numbers of children com­ pleting a primary course of polio immunisation between 1961 and 1970. It should be noted that the recommended regime involves two doses administered at intervals of six to eight weeks commencing at the age of three to six months, to be followed by a third dose approximately six months later.

Le nombre d ’enfants ayant reçu la primo-vaccination complète mitre 1961 et 1970 est indiqué dans le Tableau 2. From Table 3 (which expresses the absolute numbers of Table 2 in percentage form), it can be seen that the proportion of children completing a primary course of polio immunization in the year of birth rose from 10.8% in 1961 to 23.9% in 1967, after which point it fell abruptly to 7.3 % in 1969 and 6.1 % in 1970. By the end of the second year of age, an average of two-thirds of children had completed a primary course of vaccination, though this showed some fluctuation over the decade. In 1962, for example, the figure was as low as 56.9%, compared with a figure of 74.9% three years later, although this high level was not subsequently maintained. The proportion of children ultimately completing a primary course has appeared to stabilize at or around the 85 % level. The fact that only two cases of paralytic poliomyelitis occurred in 1970 and one in 1969 suggests that little wild poliomyelitis virus was in circulation. This is confirmed by laboratory reports, emphasizing that immunity and protection against this disease are dependent almost wholly on artificial stimulus by vaccination, and underlines the need for the continuation of a comprehensive vaccination programme to secure the protection o f young children against this disease.

Il ressort du Tableau 3 (qui exprime en pourcentages les nombres absolus du Tableau 2) que la proportion d ’enfants ayant reçu les trois doses recommandées au cours de leur première année est passé de 10,8% en 1961 à 23,9% en 1967, pour tomber ensuite approxi­ mativement à 7,3% en 1969 et 6,1% en 1970. L a proportion moyenne d ’enfants ayant reçu les trois doses avant la fin de leur deuxième année est de 2/3, mais elle a été variable d ’une année à l’autre. En 1962, par exemple, on a observé un minimum de 56,9% contre un maximum de 74,9% trois années plus tard, taux qui ne s’est pas maintenu par la suite. La proportion totale d ’enfants ayant finalement reçu la primo-vaccination complète semble s'être stabilisée au voisinage de 85%. Etant donné que deux cas seulement de poliomyélite paralytique se sont produits en 1970 et un cas seulement en 1969, on peut penser que les virus « sauvages » n ’ont guère circulé.
